Objective To explore the effect of insight education and self-confidence training on the quality of life and well-being rating of first episode schizophrenia. Methods 96 inpatients with first episode schizophrenia were randomly divided into the observed group (48 cases) in which patients were treated with risperidone plus insight education and self-confidence training and the control group (48 cases) in which patients were treated with risperidone only. These patients had been treated for 8 weeks and followed up for 0.5 year after discharged. The assessments included the Positive and Negative Symptoms Scale (PANSS),WHO QOL-100,General Well-Being Schedule (GWB) and the Self-Esteem Scale (SES) before the treatment and at the end of the following-up. Results The scores of PANSS, QOL-100, GWB and SES in both groups didn't show significant difference before the treatment (P>0.05). At the end of following-up, the total score, negative score and positive score of PANSS in the observed group were lower than that in the control group (P<0.01), while the total score of QOL-100, areas of mentality, dependence, social relation factor scores, the scores of GWB and SES in the observed group were higher than that in the control group(P<0.05 or P<0.01). Conclusion The insight education and self-confidence training can help to improve the symptom of first episode schizophrenia and enhance their quality of life and well-being rating.

Objective To compare the efficacy and safety of mirtazapine and buspirone on panic disorder. Methods 86 cases with panic disorder were divided into two groups: the mirtazapine(30~60 mg/d) group and the buspirone(15~30 mg/d) group.Hamilton Anxiety Scale(HAMA) was adopted to evaluate the efficacy.Safety was evaluated with Treatment Emergent Symptom Scale(TESS),laboratorial and physical examination. Results The effective rate of mirtazapine group and buspirone group was 90.7% and 83.7% respectively 8 weeks after treatment(χ2=1.17,P>0.05).The scores of HAMA of the mirtazapine group decreased more than that of the buspirone group 1 or 2 weeks after treatment(tt=2.94,P<0.01 and t=2.49,P<0.05 respectively). At the end of wk,But there was not significant difference between two groups 4 or 8 weeks after treatment(P>0.05).Some mild side-effects were observed in both groups. Conclusion Mirtazapine shows a similar effect to buspirone and takes effect earlier on panic disorder.

Objective:To investigate the specific method, efficacy and safety of intrathecal baclofen (ITB) in the treatment of moderate and severe spasticity. Methods:The clinical features, response to ITB trial, clinical outcomes and treatment-related complications after baclofen pump implantation surgery were retrospectively analyzed in patients with moderate to severe spasticity who received ITB treatment in Hong Kong University Shenzhen Hospital from August 2022 to August 2023.Results:The study included 22 patients with moderate to severe spasticity, of whom 18 patients (81.8%) were effective in ITB trial. For these patients with effective trial, the lowest mean Modified Ashworth Scale (MAS) score of lower extremity muscle groups after ITB bolus injection was decreased significantly compared with the baseline mean value before injection (1.29±0.81 vs 3.31±1.18 , t=11.364, P<0.001), and the M/H amplitude ratio of soleus H-reflex at 4 hours after ITB bolus injection was significantly higher than the baseline value (9.61±4.66 vs 3.83±2.69, t=7.780, P<0.001). Among the 18 patients with effective ITB trial, 10 patients received baclofen pump implantation surgery with the informed consent of the patients or their guardians. Compared with the 8 patients without surgery, the baseline mean MAS score of lower extremity muscle groups of the 10 patients who received surgery was higher (3.71±1.06 vs 2.83±1.16, F=5.645, P<0.05), the maximum reduction of mean MAS score of lower extremity muscle groups of the 10 patients who received surgery was greater (2.61±0.99 vs 1.30±0.64, F=20.696, P<0.001). In 10 patients who received baclofen pump implantation surgery, the mean MAS score of lower limb muscle groups was significantly decreased in the first and sixth months after surgery compared with the baseline mean value before surgery (1.64±0.91, 1.16±0.89 and 3.71±1.06, F=39.946, P<0.001). Among them, 2 patients were followed up to 12 months after surgery, 2 patients were followed up to 18 months after surgery, and the mean MAS score of their lower limb muscle groups showed continuous improvement, and 1 patient with cerebral palsy combined with lower urinary tract hyperactive syndrome also showed significant improvement of urgent urinary incontinence after surgery. Up to the last follow-up, the daily dose of baclofen in 10 patients receiving baclofen pump implantation was (143.60±96.24) μg, and no treatment-related complications occurred. Conclusions:ITB can effectively relieve spasticity symptoms and other related neurological dysfunction in patients with moderate and severe spasticity, and the efficacy can be maintained for a long time, and treatment-related complications are rare. Preoperative ITB trial is helpful to screen out patients more suitable for baclofen pump implantation. Patients with higher grade of spasticity at baseline and better efficacy of ITB trial are more suitable for baclofen pump implantation. The soleus H reflex can effectively reflect the change of stretch reflex of lower limb muscle groups in patients with spasticity treated by ITB, and can be used to evaluate the treatment effect of ITB.

Objective To evaluate the regulatory effect of Diwu Yanggan(DWYG)Decoction on lysoglycerophospholipids(Lyso-GPLs)in circulating exosomes in a mouse model of nonalcoholic fatty liver disease(NAFLD).Methods Circulating exosomes isolated from mouse serum by size exclusion chromatography were morphologically characterized using transmission electron microscope and examined for surface markers CD9,CD63 and TSG101 using Western blotting.Twenty-four male Kunming mice were randomized into 3 groups for normal feeding(control,n=8)or high-fat diet feeding for 1 week to induce NAFLD,after which the latter mice were given DWYG decoction(treatment group,n=8)or normal saline(model group,n=8)by gavage for 4 weeks. After the last treatment, blood samples were collected from the mice for testing serum TC,HDL-C,LDL-C,ALT and AST levels and isolating circulating exosomes.Using multivariate statistical analysis based on targeted metabolomics strategy, the potential biomarkers for Lyso-GPLs in the exosomes were screened. Results The isolated exosomes about 100 nm in size had a typical saucer-like structure with distinct double-layer membranes and a mean particle size of 137.5 nm and expressed the specific surface marker proteins CD9,CD63 and TSG101.The mouse models of NAFLD had significantly increased serum levels of TC,HDL-C,LDL-C and AST and lowered serum ALT level.A total of 43 Lyso-GPLs with significant reduction after DWYG Decoction treatment were identified in NAFLD mice. Conclusion DWYG Decoction can regulate Lyso-GPLs in circulating exosomes in NAFLD mice, which provides a new clue for studying the therapeutic mechanism of DWYG Decoction for liver disease.
