1.Observation of the efficacy of neoadjuvant chemotherapy combined with conserving-breast surgery in the treatment of patients with stage Ⅱ,Ⅲ breast cancer

Objective To observe the efficacy and safety of neoadjuvant chemotherapy combined with conserving-breast surgery in treatment of patients with stage Ⅱ,Ⅲ breast cancer.Methods 85 patients with stage Ⅱ,Ⅲ breast cancer were selected,42 patients in breast-conserving group and 43 patients in modified radical group.Neoadjuvant chemotherapy was used in breast-conserving,while was not used in modified radical group.Efficacy and follow-up results were observed in the two groups.Results After treatment,the CR and PR in the breast-conserving group was 30 cases and the effective rate was 71.4%,which of the modified radical group was 55.8% with 21 patients of CR and PR.After the comparative analysis,the difference was statistically significant(t=4.518,P=0.034).After followed for 12-36 months,the patients in the breast conserving group were all alive and one case of local recurrence.According to breast shape evaluation criteria,34 patients were excellent,accounting for 80.9%;good for the five cases,accounting for 11.9%;bad of two cases,accounting for 4.8% and the excellent rate was 92.9%.According to the patients' own judgment on the breast shape,37 cases were very satisfied,accounting for 88.1%;basic satisfaction for three cases,accounting for 7.1%;dissatisfied 2 cases,accounting for 4.8% and the satisfaction rate was 95.2%.Conclusion Patients with breast cancer of Ⅱ,Ⅲ period before breast-conserving surgery taking chemotherapy can reduce the clinical stage while the postoperative efficacy and breast appearance are good with important clinical value.
2.Mechanism by which interferon reduces the resistance of MGMT positive glioma stem cells to temozolomide

OBJECTIVE:To investigate the role of interferon to increase the sensibilization of MGMT positive glioma stem cel s to temozolomide in vitro. METHODS:Glioma cel lines, U251 and SKMG-4, were induced by suspended cloning bal formation method to harvest MGMT positive glioma stem cel s, U251G and SKMG-4G. Cel counting kit-8 assay was used to detect the kil ing effect of interferonα/βcombined with temozolomide on MGMT positive glioma stem cel s. RT-PCR and western blot assay were employed to determine the expression of MGMT and nuclear factorκB in MGMT positive glioma stem cel s. RESULTS AND CONCLUSION:Western blot results showed positive expression of MGMT in U251G and SKMG-4G cel s at protein levels. After intervention with interferonα/β, the mRNA expression of MGMT and nuclear factorκB in SKMG-4G and U251G cel s was reduced significantly, and then further decreased after temozolomide treatment. These findings indicate that interferonα/βcan remarkably strengthen the kil ing effect of temozolomide on MGMT positive glioma stem cel s.
3.Unroofed coronary sinus syndrome: A case report.

Unroofed coronary sinus syndrome (UCSS), also named coronary sinus septal defect, is a rare type of atrial septal defect with the incidence less than 1% of the total number of atrial septal defects. It is caused by incomplete formation of left atrial venous folds during embryonic development. Here we reported a patient with UCSS, who was treated in the Second Xiangya Hospital of Central South University. The patient was 50 years old and the main clinical manifestations were fatigue and shortness of breath after repeated exercise. Color Doppler echocardiography showed coronary sinus dilatation (17 mm×14 mm), indicating the possibility of permanent left superior vena cava. Pulmonary angiography showed that the left ventricle and coronary sinus were developed at the same time while the atrial septum was intact after the development of the left atrium, followed by the right atrium and right ventricle, indicating a partial anomalous pulmonary venous drainage (intracardiac type). Finally, the cardiac computed tomograhic angiography showed that 4 pulmonary veins and permanent left superior vena cava (PLSVC) went into the left atrium and the coronary sinus, respectively, while the coronary sinus septum was absent and the PLSVC was connected with the left atrium. The patient was later treated with the correction of non-parietal sinus syndrome in the Cardiovascular Surgery Department of our hospital.
