The blood pressure changes is very common in patients with acute ischaemic stroke.However,the optimal management of the blood pressure in acute ischaemic stroke remains unclear.There is a very important clinical significance on how to manage hypertension in acute ischaemic stroke.The review summarizes recent guidelines on blood pressure management after acute ischaemic stroke.The aim of the review is to expound the treated principle of hypertension in acute ischaemic stroke.
2.TRANSPORT OF GOSSYPOL BY RAT EPIDIDYMIS

Transport of gossypol by rat cauda epidldymis has been studied using microperfusion technique. It was found that gossypol entered ep-ididymal duct at very much limited rate in spite of its high plasma level. There was no difference between the aqueous & liposome formulations of gossypol to get into the epididymal duct from the blood. Since it was indicated in a previous paper that the transport rates of the 2 gossypol formulations via the blood-testis barrier were quite different. It is suggested that the transfer mechanism in test-is & epididymis may be different.

Acute aortic dissection is the most lethal one among the diseases involving the aorta.This article reviewed the operative indications,methods,curative effects and clinical prospect of Endovascular Graft Exclusion (EVGE) for aortic dissection .The indications for EVGE are DeBakeyⅢB type aortic dissections with the intimal tear. EVGE is a minus invasive method with a firmly curative effects, and its clinical prospect will be very bright.

5.Study on the Improvement of Processing Method of Carapax Trionycis

OBJECTIVE:To explore the optimal steaming time of Carapax trionycis during cleansing period,and to optimize and improve production technology of Carapax trionycis recorded by current Chinese Pharmacopoeia. METHODS:The mechanical processing replaced the artificial processing method in Chinese Pharmacopoeia. The content of protein and the appearance of Cara-pax trionycis were investigated after steaming for 30,60,90,120,180,240 min during cleansing period. The extract,decoction, ash content,appearance and property of Carapax trionycis decoction piece processed with vinegar were also investigated after cleansed Carapax trionycis decoction piece was processed by sand scalding and vinegar quenching method. RESULTS:The differ-ent steaming time obtained different quality of cleansed Carapax trionycis decoction piece and Carapax trionycis decoction piece pro-cessed with vinegar. Compared with decoction piece steamed for other duration,when the steaming time was 90 min,the content of protein in cleansed Carapax trionycis decoction piece was higher(31.16%),and its appearance was up to the requirement. Cara-pax trionycis decoction piece processed with vinegar had higher contents of extract and decoction(9.13%,11.39%)and lower con-tent of ash(66.29%),and its appearance was up to the requirement. CONCLUSIONS:Different steaming time have certain effect on the quality of cleansed Carapax trionycis and Carapax trionycis processed with vinegar,the optimal steaming time of Carapax tri-onycis is about 90 min during cleansing. The mechanical processing method maybe replace the artificial processing on Carapax tri-onycis for improving its production efficiency.
6.Effect of precursor on growth and accumulation of alkaloids of Lycoris radiata suspension cells.

In order to investigate the effects of phenylalanine, tyrosine and tyramine on the growth of Lycoris radiata suspension cells and the accumulation of alkaloids, the growth quantity of the cells as well as the content of alkaloids in cells were determined, which were treated with above three kinds of precursors alone and phenylalanine combined with tyrosine respectively. The results indicate that the addition of phenylalanine alone and addition of phenylalanine on the basis of tyrosine at high concentration (200 micromol/L) had no significant effect on the growth of Lycoris radiata suspension cells and the content of alkaloids in cells; whereas tyrosine and tyramine promoted the growth of the cells and alkaloids accumulation. Treated with tyrosine at high concentration (200 micromol/L), the content of alkaloids of the cells was 2.56-fold higher than that of the control group, the amounts of lycoramine (3.77 mg/g) and galanthamine (4.46 mg/g) were 6.61-fold and 6.97-fold higher than that of the control group, respectively. When treated with tyramine (200 micromol/L), the amount of alkaloids in Lycoris radiata suspension cells was 2.63-fold higher than that of the control group, and the amounts of lycoramine (4.45 mg/g) and galanthamine (5.14 mg/g) were 9.08-fold and 9.18-fold higher than that of the control group, respectively. The above results demonstrate that adding tyrosine and tyramine in the media significantly promoted the growth of the Lycoris radiata suspension cells and alkaloids accumulation in the cells.

7.Risk factors for unplanned readmission after transjugular intrahepatic portosystemic shunt in cirrhotic patients with esophagogastric variceal bleeding and construction of a nomogram model

ObjectiveTo investigate the risk factors for unplanned readmission within 30 days after discharge in cirrhotic patients with esophagogastric variceal bleeding undergoing transjugular intrahepatic portosystemic shunt (TIPS), and to construct a nomogram predictive model. MethodsA total of 241 cirrhotic patients who underwent TIPS due to esophagogastric variceal bleeding in Affiliated Drum Tower Hospital of Nanjing University Medical School from January 2020 to June 2023 were enrolled as subjects, and unplanned readmission within 30 days was analyzed. According to the presence or absence of unplanned readmission, they were divided into readmission group with 36 patients and non-readmission group with 198 patients, and related clinical data were collected from all patients. The independent-samples t test was used for comparison of normally distributed continuous data between two groups, and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups; the chi-square test was used for comparison of categorical data between two groups. A logistic regression analysis was used to identify independent risk factors for unplanned readmission. A nomogram prediction model was constructed, and the receiver operating characteristic (ROC) curve was plotted to assess its discriminatory ability for unplanned readmission; the calibration curve was plotted to evaluate the consistency of the nomogram model in predicting unplanned readmission; the ResourceSelection package of R language was used for the Hosmer-Lemeshow goodness-of-fit test to evaluate the degree of fitting of the mode; the decision curve analysis was used to investigate the practicality of the model. ResultsAge (odds ratio [OR]=2.664, 95% confidence interval [CI]: 1.139 — 6.233, P<0.05), CTP score (OR=1.655, 95%CI: 1.098 — 2.495, P<0.05), and blood ammonia (OR=1.032, 95%CI: 1.016 — 1.048, P<0.05) were independent risk factors for unplanned readmission within 30 days after discharge in the patients undergoing TIPS. The multivariate analysis showed that for the nomogram predictive model constructed in this study, repeated sampling for 1 000 times using the Bootstrap method was performed for internal validation, and the area under the ROC curve was 0.773, which was significantly higher than that of age (0.582), CTP score (0.675), and blood ammonia (0.641). The calibration curve showed good consistency between the probability of unplanned readmission predicted by the nomogram model and the actual probability, and the Hosmer-Lemeshow goodness-of-fit test showed good degree of fitting (c2=5.647 3, P=0.686 7). ConclusionAge, CTP score, and blood ammonia are independent risk factors for unplanned readmission within 30 days after TIPS, and the nomogram prediction model constructed based on these factors can help to predict the risk of unplanned readmission in TIPS patients and provide an accurate decision-making basis for early prevention.
