1.Dilemma and policy proposal for the ethical value of new health care reform:From the per-spective of Diagnosis Related Groups

Health care policy should follow principles of total efficiency, no-harm, fairness and justice as ethi-cal standards for public policy. Diagnosis Related Groups( DRGs) as a sign of new health care reform ( NHCR) is implemented as a pilot program across the country, with the obvious function of suppressing excessive growth in medi-cal costs and over-treatment. From the dimension of ethical values, the DRGs has avoided unethical individuals, fol-lowed the principle of total efficiency and achieved the equalization of social security. However it has also increased ethical risks and been adverse to the development of medical research. To realize the reasonable development of DRGS, the construction of DRGs must be improved, and security policies must be reasonable and feasible; orderly measures must be established to ensure promotion and implementation;the government’s financial investment must be increased to protect the social welfare of public hospitals;a sound communication mechanism must be implemented, and interaction between medical institutions and insurance institutions must be strengthened.
2.Correlation study of enterprise employee's emotion management ability and work ability

Objective To understand the related situations of employee's emotional management ability and work ability. Methods 180 employees from an automobile manufacturing company in Hefei were selected and surveyed. Results The mean score of employee's emotional management was 102.50. Mean score of employees work ability was 39.64, of all the work ability index, the physical and mental ability to work got the highest score; the work ability index about the psychological state of work and life was lower. Employee's emotion management ability and work ability was not significantly different between different genders. Conclusions There is significant correlation between employee's emotion management and work ability.
3.Effects of L-carnitine on electrophysiological changes induced by atrial ischemia in dogs

Objective To investigate the effects of L-carnitine(L-CN)on electrophysiological changes induced by atrial ischemia in dogs.Methods Twelve dogs were divided into normal saline(NS)group and L-CN group.Atrial ischemia was induced by ligation of an atrial arterial branch of right coronary artery.Atrial effective refractory period(AERP)and conduction velocity(CV)of right atrium were measured before and after 3 hour's coronary artery ligation.Atrial wavelength(WL)and inducibility of atrial fibrillation(AF)were calculated.Histological examination was performed.Results (1)AERP and WL shortened dramatically after ligation of coronary artery in NS group;they showed no remarkable change in L-CN group.(2)There was no significant change of CV in the two groups.(3)AF was induced in 0 of 12 dogs under basic condition.(3)After ligation of the right coronary artery,AF was induced in all of the 6 dogs in NS group and 0 of 6 dogs in L-CN group.(5)Histological examination revealed that the ischemic changes occurred only in NS group.Conclusion Electrophysiological changes induced by atrial ischemia could be prevented by infusion of L-CN.
4.Safety analysis of intravenous amiodarone in wolff parkinson white syndrome with atrial fibrillation

Objectives To evaluate the safety of intravenous amiodarone in patients with atrial fibrillation and rapid ventricular response via an accessory pathway. Methods Sixteen patients with Wolff Parkinson White syndrome (W P W group) and 16 patients with concealed accessory pathway (control group) were included. The anterograde effective refractory period of accessory pathway (ERP AP ) in W P W group and the effective refractory period of atrioventricular node (ERP AVN ) in control group were measured. 6.Arachnoidal fibrosis after subarachnoid hemorrhage

Objective To study the mechanism of arachnoidal fibrosis after subarachnoid hemor- rhage. Methods Rats were divided into control group, experiment group and treatment group. Radioim- munoassay (RIA) was employed to detect the levels of hyaluronic acid, laminin,type Ⅲ precollagen and type Ⅳ collagen in the arachnoid membrane. In the meantime, arachnoid cell's morphology and collagen distribution in the subarachnoid space were investigated by electron microscope. Results Results of RIA detection showed increase of Type Ⅲ precollagen level (peak at the second week), obvious higher levels of LN and HA but insignificant change of type IV collagen after subarachnoid hemorrhage. However, dexam- ethasone treatment decreased type Ⅲ precollagen level. Electron microscope found that arachnoid cells pres- ented accentruated bioactivity after subarachnoid hemorrhage, with significant increase of arachnoidal colla- gen fibers from one week after suharachnoid hemorrhage, continuing for 3 weeks. Dexamethasone treatment resulted in low density of mitochondria and sparsed arachnoidal collagen fibers. Conclusions Extracellu- lar matrix (ECM) increases in arachnoid membrane after subarachnoid hemorrhage and participates in a- rachnoid fibrosis. Dexamethasoue can relieve arachnoidal fibrosis after subarachnoid hemorrhage, as pro- rides fresh way for prevention and treatment of post hemorrhagic hydrocephalus.
7.The study of application of step-wise diversified teaching mode in cardiac electrophysiology

Objective To investigate the effect of a step-wise diversified teaching mode on cardiac electrophysiology (EP) education. Methods 64 young doctors (male:36, female:28) who studied in EP sub-specialty were enrolled. The atrioventricular nodal reentrant tachycardia (AVNRT) and atrioventricular reentrant tachycardia (AVRT) were selected as teaching content. All the doctors were randomly divided into traditional teaching group (TT) and step-wise diversified teaching group (DT) according to cross-over design protocol. In TT group, only teaching and answering the questions by teachers were given to students, for the DT group, in different stages of teaching, multiple teach-ing strategy were combined to complete teaching. T test and Chi square test were used to compare the effect of two teaching methods. Results There was no difference of basic features and time of self-studying between the two groups. In examinations, the students in DT group got higher score than TT group[DT:(92.3±9.8) and (93.1±7.8),TT:(88.3±8.6) and (87.1±10.0),P<0.05], and had better perfor-mance of learning interest, initiative, ability of finding and resolving problems, teamwork spirit and so on. Conclusion The step-wise diversified teaching mode contributes improving the quality of instruction of cardiac EP, moreover, arousing learning interest and initiative, enhancing ability of find-ing and resolving problems, teamwork spirit and so on, which is worthy of wide-popularizing in cardiac EP training.
8.Technology of extracting gypenoside from crude Herba Gynostemmae Pentaphylli solution by foaming separation

Objective To study the method of extracting the gypenoside from crude Herba Gynostemmae Pentaphylli solution.Methods The gypenoside was extracted from crude Herba Gynostemmae Pentaphylli solution by foaming separation.The optimum conditions were obtained through calculating the recovery rate and enrichment ratio.Results Under the optimum conditions:the concentration of the gypenoside in crude Herba Gynostemmae Pentaphylli solution was 3.21 ?g/mL,the gas flow rate was 30 mL/min,the volume of the solution was 110 mL,and pH value was 9.0 as well,the recovery rate of gypenoside was 49.20% and the enrichment ratio could reach 4.511.Conclusion Foam separation is a simple and effective method of extracting gypenoside from crude Herba Gynostemmae Pentaphylli solution.
9.Application of pulse-indicated continuous cardiac output in patients with acute inferior wall myocardial infarction

Objective: To explore the guiding significance of transpulmonary thermodilution pulse-indicated continuous cardiac output (PiCCO) in treatment for patients with acute inferior wall myocardial infarction (AIMI). Methods: A total of 26 AIMI patients in intensive care unit of cardiology department in our hospital from Jul 2012 to Jan 2014 received PiCCO and ultrasonic cardiography (UCG) to monitor cardiac output (CO) and cardiac index (CI), and their correlation analysis. Results: When PiCCO placement and after placement 72h, PiCCO monitoring CI were (2.77±0.77)L•min-1•m-2, (3.17±0.39) L•min-1•m-2 respectively, there was significant difference(P<0.01), UCG measured CI were (2.49±0.64)L•min-1•m-2, (2.63±0.24) L•min-1•m-2, there was no significant difference(P>0.05); PiCCO monitoring CO were(4.78±1.06)L/min, (5.08±1.53) L/min respectively, there was significant difference(P<0.05), UCG measured CO were(4.51± 0.86)L/min, (4.57±0.91) L/min, there was no significant difference(P>0.05); and CI,CO measured by PiCCO were significantly higher than those of UCG group (P<0.01 both) after PiCCO placement 72h. Conclusion: Pulse-indicated continuous cardiac output can offer more sensitive hemodynamic indexes compared with UCG, which possesses important treatment guiding significance in patients with acute inferior wall myocardial infarction and unstable hemodynamics.
10.Shooting and assessing the portfilm in radiation therapy

Objective To discuss how to practise and consummate to shoot and assess the portfilm for radiotherapy. Methods Double radiography technique was used to take a portfilm. The reference mark points (plumbum points) were added into the field at the first exposure and the cross lines drawn based on the plumbum point images were definited as the reference coordinate to the actual field. Then, the portfilm was compared and analyzed with the simulator planning film. Results The images of both plumbum points and block boundaries were distinct in the portfilm, but the resolution and contrast of the tissue structure image was inferior to the ordinary film from simulator. However, all different structures and positions could be identified in order to assess if the actual field coincided with the radiation planning. Conclusions Although the real-time property of the portfilm is inferior to the electric portal image device (EPID), the double radiography portfilm is feasible as an important quality control method in radiation therapy due to its lower price and easier performance. The design of reference points is the key point in this technique.
