砄bjective: To explore an effective method for Electroneurography(ENoG) of facial nerve. Methods: ENoG of 114 normal subjets and 69 patients with traumatic facial nerve injuries were investigated. Latency time(LT), duration time(DT), amplitude(A) and square under the curve(S) were measured in facial muscles when facial nerve was stimulated postauricularly or anteroauricularly. Results: In most facial musles except square muscle of lower lip postauricular stimulation gave longer LT, shorter DT, lower A and smaller S. Conclusion: The postauricular stimulation is effective prior to anteroauricular stimulation for electroneurography of the facial nerve.

Objective:To evaluate diagnostic sensitivity of electroneurography(ENoG) for facial neurotmesis. Methods:The data of 24 patients suffering from facial neuratmesis on one side, having non-emergency operation, and having preoperative records of ENoG were collected.Among the patients,18 were men and 6 were women, aged from 3 to 55 years(median 28).The duration of injury before ENoG examination was from 1 to 56 days(mean 23.6 days) for them. The electroneurographic examination was carried out for facial nerves on both sides preoperatively.Percentages of evoked potential amplitude reduction on affected sides were calculated according to the value of normal side. The 90% reduction of amplitude was used as a cutting point. The facial neuratmesis was observed during operation. The sensitivity and false negative proportions of ENoG were analyzed. Results:Among the 24 patients with facial neurotmesis,22 had ENoG amplitude reduction over 90%,and 2 having this reduction less than 90%.The sensitivity and false negative proportions of ENoG in patients with facial neurotmesis were 91.7% and 8.3% respectively. Conclusion:The preoperative ENoG examination is high sensitivity for facial neurotmesis. The patients whose ENoG reduction over 90% should be advised to have a facial neurorrhaphy as soon as possible.

Objective To observe the influence of short-term continuous subcutaneous insulin infusion(CSII)on ?-cell function in newly diagnosed type 2 diabetes(T2DM).Methods A total of 32 inpatients with newly diagnosed T2DM who had a 14-day course of CSII were randomly assigned to two groups of OGTT and arginine stimulating test(AST).Before and after 3 days and 14 days CSII treatments,OGTT and AST were performed respectively. The changes of the values of plasma glucose,HOMA-IR,HOMA-? and △C30/△G30,△C120/△G120 or arginine-induced acute C-peptide reaction(ACRARG)in two groups were compared. Results In OGTT,the values of plasma glucose and HOMA-IR were decreased,HOMA-? and △C120/△G120 were increased significantly after CSII therapy, both in day 3 and day 14;and △C30/△G30 were unchanged after 3 days but increased markedly after 14 days. However,in ACRARG,the above mentioned parameters almost remained unchanged after CSII treatment in 3 days or 14 days. Conclusions In newly diagnosed T2DM with elevated fasting glucose levels, intensive insulin therapy can partly improve ?-cell function showed by OGTT,not by AST.

Journal of Chinese Physician 2016;18(11):1744-1747
Vitamin D deficiency is highly prevalent in the general population including rheumatoid arthritis (RA) patients.Autocrine regulation of vitamin D modulates important immune response.Vitamin D deficiency may be involved in the pathogenesis of many autoimmune diseases including RA.Vitamin D regulates both the innate and adaptive immune responses.Vitamin D's effects on the innate immune system are mainly through the toll-like receptor and on the adaptive immune system through T cell differentiation, particularly the Helper T cell (Th) 17 response. As Thl7 cells are critical in the pathogenesis in RA,what effect will vitamin D deficiency lead to on the RA? Many data have indicated the relationship between vitamin D deficiency and RA disease activities, but not all the results are consistent. Here,we review the immune-modulatory roles of vitamin D and its effects on disease activity, osteoporosis and cardiovascular disease risk in RA.

Objective To study the efficacy of radical surgery for the invasion of chest wall by lung cancers and the different methods of chest wall reconstruction. Methods A retrospective analysis of clinical data was conducted in 27 cases of lung cancer with chest wall invasion in this hospital. 24 cases received lobectomy, 3 cases received pneumonectomy, and all cases received wall reconstruction. The size of chest wall defect after resection ranged from 6.5 cm × 5.4 cm × 5.0 cm to 15.5 cm × 12.5 cm ×10.0 cm. The number of rib resection for each patient ranged from 1 to 4. The reconstructive methods were as follows: using polyester fabric to reconstruct costal pleura, using plexiglass, stainless steel wire and grid-like titanium plate to reconstruct bone defect, and using nearby muscle flap and pedunculated muscle flap to reconstruct muscle defect. Results Pathologic diagnosis was non-small cell lung cancer, including 22 cases of squamous cell carcinomas and 5 adenocarcinomas. There were pulmonary infections in 6 cases, chest hemorrhage in 1 case and chest purulence in 1 case. One-year survival was 77. 9 %,3-yearsurvival was 58.3 %, and 5-year survival was 29. 1 %. Conclusion The lung cancer with chest wall invasion is advisable to make active use of radical surgery. The different methods and repair materials are chosen to reconstruct the chest wall defect, supplemented by comprehensive treatments.
