1.Clinical report of fracture complicated with brain-type fat embolism syndrome

Objective To study on diagnosis and treatment of brain type fat embolism syndrome complicated with fracture. Method The diagnosis and treatment of 7 cases of brain type fat embolism syndrome complicated with fracture were reviewed. All of 7 cases did not have any brain injury, 3 cases were complicated with femural fracture, 3 cases with fractures of tibia and fibula, and 1 cases with limbs multi fracture. They presented somnolence or coma complicated with rise in temperature (37.7℃~39℃) ,increase in breath and heart rate,anemia,small bleeding spots at chest. It was discussed how to distinguish brain type fat embolism syndrome from brain injury. Synthetical measure was adopted mainly using dexamethasone (20~80mg/d). Results 7 cases of brain type fat embolism syndrome complicated with fracture were cured in 1~7 days. Conclusions Sudden somnolence or coma that does not result from brain injure after limb fracturees in early stage can be diagnosed as brain type fat embolism syndrome. A synthetical measure should be adopted by mainly using dexamethasone (40~80mg/d) .
2.Analysis of volatile oil from Rubia tinctorum L.cultured in Xinjiang by GC-MS

AIM: To study the chemical components and contents of the volatile oil from the Rubia tinctorum L.. METHODS: The volatile oil was extracted by steam distillation-extract method,the chemical constituents of the volatile oil from the herb were identified by gas chromatography-mass spectrometry(GC-MS). RESULTS: Thirty compounds were identified,which accounted for over 86.32% of the volatile oil. CONCLUSION: The major components were theaspirane(17.37%),2-furaldehyde(11.10%),benzeneacetaldehyde(6.01%),eugenol(5.47%),ionone(3.87%),and so on.
3.Posterior single segment fusion or non-fusion in treatment of lumbar spinal disease:a comparative study

[Objective]To compare the treatment effects between posterior single segment fusion or non-fusion in lumbar spinal disease,and to evaluate their influence on adjacent segments.[Method]Thirty-two cases of degenerative lumbar spinal disorders were treated with Coflex(group non-fusion) and PLIF(group fusion),and followed up for over 2 years.The clinical effect was evaluated by visual analogue scale(VAS) and Oswestry disability index(ODI).Range of motion(ROM) of lumbar segments was measured by X-ray film.[Result]In group non-fusion,VAS decreased from 7.8?1.2 pre-operatively to 2.1?0.6 at final follow-up,ODI from 30.8?3.2 to 4.6?1.2,ROM improved significantly.In group fusion,VAS decreased from 7.2?1.1 to 2.0?0.6,ODI from 29.9?3.0 to 4.5?0.9,and ROM of L3、4 improved significantly.No significant difference of clinical effect was found between 2 groups.[Conclusion]Both fusion and non-fusion fixation have satisfying effects in treatment of degenerative lumbar spinal disorder.Non-fusion fixation has less influence on ROM of lumbar segments and could reduce adjacent segment degeneration.
4.The efficacy of folic acid combined with mecobalamine for the treatment of mild cognitive impairment in patients with cerebral small vessel disease and hyperhomocysteinemia

Objective To observe the efficacy of folic acid combined with mecobalamine for the treatment of vascular mild cogni-tive impairment (VMCI) in patients with cerebral small vessel disease (SVD) and hyperhomocysteinemia(Hcy) .Methods A total of 84 VMCI patients with cerebral small vessel disease and Hcy were randomly divided into combination group and control group . Two groups received conventional therapy for 6 months .Besides ,the combination group received folic acid combined with mecobal-amin .The level of plasma Hcy and ADAS-cog score were observed before and after 3 months and 6 months treatment .Results Af-ter treatment ,plasma Hcy significantly lower in the combination group (P<0 .01) ,whereas it did not show any improvement in the control group .ADAS-cog scores in combination group decreased compared with that of before treatment ,but there was no statisti-cally significant difference after 3 months treatment(P>0 .05) .However ,after 6 months treatment ,the ADAS-cog scores decreased obviously than that in the before treatment group (P<0 .05) .ADAS-cog scores in control group increased compared with before treatment ,but there was no statistically significant difference in after treatment (P>0 .05) .After 3 months treatment ,there was no significant difference on ADAS-cog score between combination group and control group (P>0 .05) ,however ,there had significant difference between the two groups after 6 months treatment(P<0 .05) .Conclusion The level of plasma Hcy could be reduced by adding folic acid and mecobalamin .Treatment of hyperhomocysteinemia may delay the progression of vascular cognitive impairment w hich caused by cerebral small vessel disease .
5.PRO effectiveness assessment of acupuncture for cervical spondylosis and associativity analysis

Objective: Patient-reported outcome (PRO) method was used to evaluate clinical effects of acupuncture for cervical spondysosis. Methods: With RCT method used, 162 cases were adopted, and 150 cases finished the whole course (treatment group: 76 cases, control group: 74 cases). Treatment group cases were treated by real-acupuncture while control group cases treated by sham-acupuncture. Northwick Park Pain Questionnaire(NPQ)was used as main outcome index, while SF-36 as secondary index. Both groups’ patients were evaluated before and after treatment course, plus one month and 3 months after the end of the treatment course. Results:Treatment group cases got lower NPQ Scores and higher mental health dimensions scores than the control group cases at any evaluation time-point. At the point of one month after the end of the treatment course, the treatment group cases got better social function scores than the control group cases. At the point of three months after the end of the treatment course, the treatment group cases got better body pain and energy scores than the control group cases. Multiple regression analysis showed that three dimensions of SF-36 (social function scores body pain and energy scores) had close relationship with NPQ scores. Conclusion: Acupuncture had satisfactory long-term effect for cervical spondylosis.
6.Effects of penehyclidine hydrochloride on expression of Toll like receptor-4 and nuclear factor kappa B in a rat model of sepsis-induced acute lung injury

Objective To evaluate the effects of penehyclidine hydrochloride (PHC) on the expression of Toll like receptor-4 (TLR4) and nuclear factor kappa B (NF-κcB) in a rat model of sepsis-induced acute lung injury (ALI).Methods Seventy-two adult male Sprague-Dawley rats,weighing 180-220 g,were randomly divided into 3 groups (n =24 each) using a random number table:shame operation group (group S),ALI group and PHC group.Sepsis-induced ALl was induced by cecal ligation and puncture in anesthetized rats.In group PHC,PHC 0.45 mg/kg was intramuscularly injected immediately after cecal ligation and puncture.At 6,12,24 and 36 h after ligation,the broncho-alveolar lavage fluid (BALF) was collected for detection of TNF-α and IL-6 concentrations and the lungs were removed for determination of the expression of TLR4 (using RT-PCR and Western blot) and NF-κBp65 (using Western blot) in lung tissues and for microscopic examination.The pathological changes of lungs were scored.The wet to dry lung weight (W/D) ratio was calculated and the activity of myeloperoxidase (MPO) in lung tissues was measured.Results As compared with S group,the IL-6 concentrations in BALF at 6,12 and 24 h after ligation,TNF-α concentration in BALF at 6 and 12 h after ligation,and the expression of TLR4 and NF-κBp65,pathological scores,W/D ratio and MPO activity at each time point were significantly increased in ALI group (P < 0.05).Compared with ALI group,the TNF-α concentration in BALF at 6 and 12 h after ligation,and IL-6 concentrations in BALF,the expression of TLR4 and NF-κBp65,pathological scores,W/D ratio and MPO activity at each time point were significantly decreased in group PHC (P < 0.05).Conclusion PHC inhibits NF-κB activation and decreases inflammatory responses through blocking TLR4 expression in lung tissues,thus attenuating sepsis-induced ALI in rats.
7.Evaluation of functional recovery of unstable ankle fractures after cast immobilization and early weight-bearing: a prospective trial

Objective To evaluate the functional recovery of unstable ankle fractures prospectively after early weight-bearing and cast immobilization. Methods From October 2003 to October 2007, 100 patients who had sought medical management in our department for unstable ankle fractures were enrolled in this prospective study. All patients were treated with open reduction and internal fixation (ORIF), according to the AO/ASIF principles. They were randomized into 2 even groups, one receiving early weight-bearing and the other cast immobilization. The functional recovery was evaluated in terms of Olernd score, Tegner score, pain intensity[merical rating scale (NRS)], full weight-bearing time, return-to-work time, and hospital stay time. Results Forty-six patients in early weight-bearing group and 45 patients in immobilization group finished follow-ups. There was no difference in Olernd score, Tegner score or NRS between the 2 groups, and all pa-tients got a good Olerud score and returned to work. Total weight-beating time in early weight-beating group was 4 weeks less than in immobilization group. Conclusion Early weight-bearing may be recommended for patients who have got a stable osteosynthesis of their fractured ankles as judged by their responsible surgeons, fine compliance, and a high motivation.
8.Determination of ropivacaine in plasma by modified reversed-phase high performance liquid chromatography

AIM: To establish a Reversed-Phase high performance liquid chromatography method for determination of plasma ropivacaine in dog.METHODS:The sample was deproteinated by acetonitrile and determined with RP-HPLC and ultraviolet detection.the analytical column was Intertsil C18 column((4.6) mm?250 mm,5 ?m).The mobile phase consisted of methanol:H_2O(5050),the detection wavelength was 215 nm and the flow rate was(1.4)(ml?min~(-1)),the column temperature was 20 ℃,the sensitivity was 0.02AUFS respectively.The volume of sample for detection is 10 ?l.RESULTS:The linear range was(0.1)-25(?g?ml~(-1))(r=(0.9992)). The recovery of ropivacaine was(91.2)%-(93.6)%,RSD was(2.10)%-(3.40)%(n=6).The detection limit was(0.05)(?g?ml~(-1)).the intra-day and interday precisions of assay for ropivacaine was(1.35)%-(2.88)% and(1.80)%-(3.76)%.The quantative analysis of ropivacaine in plasma was not interfered by impurities.CONCLUSION: The assay method was shown to be suitable for determination the concentration of ropivacaine in plasma.The method is simple,accurate,sensitive and reproducible.
9.General practitioners' demand and evaluation for their own post training

Objective To explore the post training needs,training content,training mode and the advantages and disadvantages of the existing training from the general practitioner perspective,in order to provide reasonable suggestions for the management institutions to improve the quality and effect of the post training for all general practitioners.Methods In October,2013 a questionnaire survey was conducted to 197 family physicians for their post training needs,training content and training mode.According to the interview outline,an in-depth individual interview was conducted to 20 leaders and general practitioners from the community health service center in Beijing Xicheng District on the advantages and disadvantages of general medical training.All survey information databases were established using FoxPro 6.0 software and double entry.Package was statistically analyzed with SPSS 19.0 statistical software,qualitative data using rate to represent; interviews with text descriptions,coding classification analysis.Results More than 70% of general practitioners had the demand for strengthening the training of the clinical medicine,the knowledge of humanities and social sciences as well as clinical research methods,but the demand for the training of general medical theory and basic medicine was small,only accounting for 22.50% general practitioners.More than 90.00% of general practitioners were in favor of targeted flexible training methods.More than 85.00% of general practitioners believed that existing training teachers' teaching abilities were very good,and they agreed to open general practitioner training project in medical colleges and universities.The surveyed physicians also made some criticism about too many training departments,low training quality,too many training programs,and training content repetition etc..Conclusion According to the general medical post training needs,human and scientific research of targeted training should be increased by using case discussion,site visit and many flexible and diverse teaching methods and various departments should also be coordinated and the training incentive and evaluation mechanism should be improved to guarantee the quality of training.
10.Analysis of 3 527 Male Infertile Patients’Semen

Objective To study the influence factors of infertility by analysis of semen sample and reference for clinical treat-ment.Methods 3 527 cases of semen sample were collected from Jan 2012 to Jun 2014.All samples were analysed by SQA-V analyzer and compared with 80 cases of normal semen.Results There were 358 normal samples (10.2%)and 3 169 ab-normal samples (89.8%).Among the abnormal samples low sperm motility had the highest ratio (2.7%)while abnormal pH had the lowest ratio (61.5%).All the indexes had significance difference to normal sample expect pH value (t=0.065, P =0.969).Among them,the comparison of rate had statistical significance(χ2 =3.214~24.712,P <0.05).The compari-son of mean also had statistical significance(t=2.523~15.324,P <0.05).Conclusion Infertility male almost has abnormal index of semen volume,liquefaction time,sperm motility,sperm density,sperm morphology,sperm viability.Accurately sperm analysis can provide objective basis to clinical diagnosis and treatment.
