1.Observation of clinical efficacy of S-1 combined with oxaliplatin in neoadjuvant chemotherapy for local advanced gastric carcinoma

Objective To observe the clinical efficacy of S-1 plus oxaliplatin (SOX) in neoadjuvant chemotherapy for patients with local advanced gastric carcinoma.Methods Clinicopathological data of 68 patients with local advanced gastric cancer were retrospectively analyzed and enrolled as treatment group.All the patients in the treatment group achieved 3-4 cycles chemotherapy of SOX regimen preoperatively.Outcomes of tumor remission after neoadjuvant chemotherapy were evaluated according to RECISI1.1 criteria.Meanwhile,70 patients who underwent operation directly after pathological diagnosis of local advanced gastric cancer during the same period were recruited as control group.Resection rate,R0 resection rate,postoperative complications rate and postoperative progression free survival(PPFS) time were compared between the two groups.Results In the treatment group,after neoadjuvant chemotherapy,4 cases (5.9%) achieved complete renission,36 cases (52.9%) achieved partial remission.Meanwhile,the numbers of cases with stable disease and progressive disease were 15 cases(22.1%) and 13 cases (19.1%) respectively.The objective remission rate and disease control rate in the treatment group were 58.8% and 80.9% respectively.The tumor resection rate,R0 resection rate,postoperative complications rate in the treatment group were 92.6%,85.3% and 14.7% respectively.The R0 resection rate in the treatment group was higher than that in the control group,the difference was statistically significant(x2 =8.042,P < 0.05).However,the differences of tumor resection rate and postoperative complications rate between the two groups were not statistically significant (x2 =1.711,0.673,all P > 0.05).The median time of PPFS in the treatment group and the control group was 36 months and 30 months respectively,there was statistically significant difference in PPFS between the two groups (x2 =4.049,P < 0.05).Conclusion The neoadjuvant chemotherapy of S-1 plus oxaliplatin for local advanced gastric carcminoma has satisfactory short-term effect and can also improve PPFS.However,further investigation is required to observe the effect of this neoadjuvant chemotherapy regimen on postoperative overall survival.

3.Observation of early rehabilitation on patients with severe closed traumatic brain injury

ObjectiveTo observe the effect of early rehabilitation on patients with severe closed traumatic brain injury in intensive care unite (ICU).Methods118 patients with severe closed traumatic brain injury were divided randomly into the treatment group and control group with 59 cases in each group. The patients of control group recieved respiration rehabilitation, bedsore prevention and urinary system nursing; while the patients of treatment group added with motor and cognitive rehabilitation. Scores of Glasgow coma scale (GCS), Fugl-Mayer assessment (FMA), mini-mental status examination (MMSE), disability rating scale (DRS) and activities of daily living (ADL) of all patients were recorded before and after treatment.ResultsThe scores of GCS, FMA (upper limbs), ADL and DRS of the treatment group patients were significantly difference with that of the control group after treatment (P<0.05). The MMSE scores of two groups had no significantly difference after treatment (P>0.05).ConclusionEarly rehabilitation on the patients of severe closed traumatic brain injury in ICU can improve brain function and reduce the disability rating scale. The improvement of cognitive ability is not obviously in a shot period.

5.Comparison of the efficacy and safety of ziprasidone and risperidone in the treatment of schizophrenia

Objective To investigate the clinical efficacy and safety of ziprasidone and risperidone in the treatment of schizophrenia.Methods According to the digital table,64 patients with schizophrenia were randomly divided into the two groups,32 cases in each group.The control group was given risperidone for treatment,while the observation group was given ziprasidone for treatment.The clinical efficacy was assessed by using the positive and negative symptom scale(PANSS),and adverse events were observed.Results The total effective rates of the control group and the observation group were 84.38% and 87.50%,there was no significant difference between the two groups(x2 =2.66,P > 0.05);The PANSS score and each factor score of the two groups after treatment decreased significantly than before treatment (t =8.11,7.86,7.72,7.90,6.77,6.58,7.52,7.40,all P < 0.05),but there was no significant difference between the two groups (t =2.18,P > 0.05) ;The incidence rate of adverse reaction of the observation group was 18.75%,which was significantly lower than 40.63% of the control group (x2 =8.66,P <0.05).Conclusion Ziprasidone has therapeutic equivalence with risperidone in the treatment of schizophrenia,with less adverse reaction,high security,it has good clinical application value.

7.Interferon Effect of Retinoic Acid on Lung Damage of Neonatal Rats Suffered from Hyperoxia

Objective To observe influence of retinoic acid on the lung damage and expression of Connexin26(Cx26) of neonatal rats suffered from hyperoxia.Methods Twenty-four 3-day SD rats were randomly divided into three groups:retinoic acid+ hyperoxia,hyperoxia,air group;rats in the group of hyperoxia were suffered 7 days hyperoxia (the level of oxygen was above 950 mL/L),air group was instead of air.All rats were weighed,value of radical alveolar counts(RAC) and microstructure of the lungs were observed by HE staining under light and electron microscope respectively;and the expression of Cx26 in the lungs was determined by immunohistochemistry.Results Compared with air group,weight,value of RAC were both decreased obviously in hyperoxia,retinoic acid+hyperoxia groups[(13.53?1.27),(13.38?1.29),(17.37?0.89)g,F=30.19 P=0;11.15?1.33,12.49?1.47,13.94?0.98,F=9.59 P=0),while the expression of Cx26 increased [(13.68?1.28)%,(17.82?1.72)%,(19.69?1.77)%,F=29.36 P=0].Conclusion Retinoic acid has a protective effection from damaged lung of neonatal rats suffered from hyperoxia,and it may be related to the alteration of expression of Cx26 protein in the lungs.
8.Bacterial Protein Secretion Pathway with SecA as a Motor

There are one third of synthesized proteins must be secreted to the cell surface or to the surrounding environment to acquire their native functional state. Most of them are exported by Sec translocase (secretion pathway). Sec translocase consists of a membrane embedded protein-conducting channel, termed SecYEG and a peripherally associated motor domain, the ATPase SecA. The SecDFyajC heterotrimeric membrane protein complex can facilitates protein translocation. SecB is a molecular chaperone that functions in the protein translocation pathway. SecM (secretion monitor) encoded by the 5' region of the secM-secA mRNA, which elongation arrest is required for upregulated expression of SecA. The signal sequence in the N terminus of the nascent peptide is first recognized by the signal recognition particle (SRP). SecB, the Sec-system-specific chaperone, channels the preprotein to the Sec translocation pathway and, ad- ditionally, actively targets the bound precursor to the translocase by its ability to bind SecA. The preprotein-bearing SecA then binds to the membrane, at a high-affinity SecA-binding site, SecYEG, which constitutes a channel for polypeptide movement. Continued translocation requires cycles of ATP hydrolysis bySecA, which is thought to occur in a step-wise fashion with a step of 20~30 amino acid residues.
9.Analysis of Clinical Pharmacists Participating in 436 Cases/Times of Clinical Consultation

OBJECTIVE:To evaluate the effects of clinical pharmacist participating in clinical consultation,and to provider ref-erence for clinical pharmacists participating in clinical practice. METHODS:436 cases/times of clinical consultation in which clini-cal pharmacists participated in Zhengzhou Seventh People’s Hospital from 2012 to 2015 were analyzed statistically. RESULTS:2012-2015,the number of clinical consultation in the hospital showed rising tendency;23 departments applied the consultation, among which top 5 were endocrinology department,gynaecology department,neurosurgery department,neurology department and orthopedics department. The reasons for consultation mainly were the formulation of therapy plan (430 cases/times,accounting for 97.95%). The consultation suggestions were adjusting drug types(303 cases/times,69.17%). Consultation suggestions were adopt-ed completely in 405 cases/times(92.89%). There were 307 cases/times of effective outcome,with effective rate of 75.80%(307/405). CONCLUSIONS:Clinical pharmacists participated in clinical consultation is helpful to optimize therapy plan,play a positive play on the safety of drug use. Clinical pharmacists should further learn clinical medical and pharmaceutical knowledge and summa-rize the experience of clinical practice to improve the quality of consultation.
10.Clinical Significance of Changes in NT-proBNP, Hcy and Partial Coagulation-Fibrinolytic Indexes with Acute Cerebral Infarction

Objective To Analyze the examination meaning of b-type brain natriuretic peptide precursor (NT-proBNP),homocysteine and coagulation-fibrinolysis indexes for patients with acute cerebral infarction.Methods Selected 40 patients with acute cerebral infarction (experimental group) to hospital from March 2014 to May 2015 and 40 healthy check-up cases (control group).Then,compared the indicators in blood between the two groups of patients,namely homocysteine (Hcy),NT-proBNP,activated clotting time live enzymes enzyme (APTT),original activator inhibitory factor (PAI-1) and tissuetype fibrinolytic enzyme original activator (tPA).Meanwhile,also compared these indicators for the experimental group before and after treatment.Results Before treatment,the levels of Hcy,t-PA,NT-proBNP,PAI-1 and ATPP for experiment group were 17.7±3.6 μmol/L,29.4±7.9 μmol/L,3 212.8±2 511.4 ng/L,130.1±17.8 μmol/L and 37.8±4.5 s,respectively.The levels of Hey,t-PA,NT-proBNP,PAI-1 and ATPP for control group were 7.2± 2.1 μmol/L,15.1 ± 3.7 μmol/ L,198.7 ± 1 14.8 ng/L,67.8 ± 7.9 μmol/L and 37.8 ± 4.5 s,respectively.After treatment,the levels of Hcy,t-PA,NT-proBNP,PAI-1 and ATPP for experiment group were 12.2±1.5 μmol/L,18.2±2.3 μmol/L,348.7±194.8 ng/L,78.6±9.8 μmol/L and 32.2±4.5 s,respectively.Before treatment,the indicator of APTT for experiment patients was significantly shorter than it after treatment and that of the control group.The other four indicators were significantly higher than them after treatment and those of the control group.The differences were statistically significant (P <0.05).Conclusion Hcy,NT-proBNP,APTT,PAI-1,and t-PA had closely relation with the occurrence of acute cerebral infarction development,and they can be helpful to evaluate disease progression and predict prognosis for patients with acute cerebral infarction.
