1.Effect of Compound Aescin Gel on Massage of the Commonly Encountered Diseases in Orthopedic Traumatology

ObjectiveTo observe the effect of compound Aescin gel on the massage of the commonly encountered diseases in orthopedic traumatology.MethodsAll 219 patients (acute sprain of waist, radiohumeial epicondylitis, frozen shoulder) were divided into the treatment group (n=112) and control group (n=107). The treatment group was treated with compound Aescin gel in massage treatment, and the control group treated only with massage.ResultsThe recovery rate of the treatment group was higher than that of the control group (P<0.05).ConclusionCompound Aescin gel used in massage can improve effect of commonly encountered diseases in orthopedic traumatology.

3.Value of NLR,PLR and their changes in predicting occurrence of anastomotic leakage after surgery in patients with rectal cancer

Objective:To explore the value of preoperative neutrophil-lymphocyte ratio (NLR),preoperative platelet-lymphocyte ratio (PLR),changes between pre and postoperative neutrophil-lymphocyte ratio(postoperative NLR/preoperative NLR) and changes between pre and postoperative platelet-lymphocyte ratio(postoperative PLR/preoperative PLR) in predicting the occurrence of anastomotic leakage after surgery in patients of rectal cancer.Methods: 187 rectal cancer patients in the Gastrointestinal Surgery Department in the First Affiliated Hospital of Southwestern Medical University during March,2015-March,2016 were included in our study.Preoperative NLR,preoperative PLR,changes between pre and postoperative NLR and changes between pre and postoperative PLR were observed.At the same time we included age,gender,pathological staging,stage of lymph node metastasis,tumor differentiation,distance between tumor and anus in our study.χ2 test or Fisher′s exact value was used in the univariate analysis,while Logistic Regression Analysis was used in the multivariate analysis.Results: The univariate analysis showed that preoperative NLR (Fisher′s exact value=7.242),preoperative PLR (χ2=6.787),changes between pre and postoperative NLR (χ2=15.656),PLR (χ2=9.298),age (χ2=4.813) and distance between tumor and anus(χ2=5.951) were related to anastomotic leakage(P<0.05).Other risk factors such as,gender(χ2=0.001),pathological staging (Fisher′s exact value =2.107),stage of lymph node metastasis(Fisher′s exact value =1.298),tumor differentiation (Fisher′s exact value =2.206),and were not associated with anastomotic leakage (P>0.05).Multivariate analysis showed that the preoperative NLR(OR=1.647,P=0.000),change between pre and postoperative NLR (OR=1.880,P=0.000)and distance between tumor and anus(OR=4.364,P=0.048) were associated with anastomotic leakage.Conclusion: preoperative NLR,change between pre and postoperative NLR and distance between tumor and anus are independent risk factors for anastomotic leakage after rectal anterior resection.
4.Apoptosis of hepatocytes and the expression of Bax and Bcl-2 in nonalcoholic steato-hepatitis in rat

Objective To study the apoptotic changes in hepatocytes and the expression of apoptosis-associated proteins Bax and Bcl-2 in the liver tissue of a rat nonalcoholic steato-hepatitis (NASH) model, and explore the relationship between hepatocyte apoptosis and the degree of NASH pathological changes. Methods A rat model of NASH was reproduced with feeding a fat-rich diet for a period of 2, 4, 6, 8 and 12 weeks. Cells apoptosis was evaluated by TdT-mediated dUTP nick end labeling (TUNEL), and the expression of Bax and Bcl-2 proteins was detected by immunohistochemistry in rat liver tissue of control and model groups respectively. Results The number of TUNEL-positive cells was increased in the second week, peaking at the 8th week, and it was significantly correlated with steatosis and hepatic fibrosis (r=0.707 and 0.750, respectively, P0.05). The expressions of Bax and Bcl-2 proteins were mainly located in cytoplasm of hepatocytes, some of which contained lipid droplets. Bax and Bcl-2 positive cells increased gradually with the progression of NASH. In early stage of modeling, the number of Bax positive cells was greater than that of Bcl-2, but the number of the both proteins tended to be about same in the late stage of modeling. Conclusion The cell apoptosis is a main pattern of liver damage in the process of NASH, and it is closely associated with the pathological changes. Proapoptosis protein Bax and antagonisitic protein Bcl-2 might participate in the modulation of hepatocytes apoptosis in the early stage of NASH, and they also have a marked effect in the whole process of NASH.
5.Research of the internal quality controlling method for 17-OHP determination

Objective To find an internal quality controlling method for 17-OHP determination by time-resolved fluoroimmuno-assay .Methods 20 quality control data were collected .The data were analyzed by using L-J method ,instant method and improved instant method .Results were used for the construction of quality control charts .Result The first three quality control data had a great impact on the following judgments of internal quality controlling when instant method was used .The subsequent results might be false acceptance .Improved instant method could effectively reduce the situations of false run-away and false acceptance ,which was suitable for the internal quality control of 17-OHP determination by time-resolved fluoroimmunoassay in newborn screening . Conclusion There are many steps of manual operations in 17-OHP determination of time-resolved fluoroimmunoassay .The details of these operations have great impacts on the experimental results .Thus ,the operations of 17-OHP test should be specified and exe-cuted strictly according to requirement .
6.Ethanol inhibiting the NLRP3 inflammasome activation to protect coronary heart disease

Objective To examine whether ethanol modulates the intracellular processes involved in the secretion of IL-1α,and then exert a protective effect against coronary heart disease. Methods THP-1 cells in human were cultured for 2-3 generations , and put in PMF for 72 h to induce THP-1 into macrophage. ELISA was applied to detect effects for secretion of IL-1α by LPS, cholesterol and ethanol. In the light of ELISA re-sults, western blot was applied to detect the effects of ethanol on caspase-1 and NLRP3. Results Compared with the control group, the secretion of IL-1α in LPS group and LPS + CHOL group increased. Compared withLPS + CHOL group, the concentration of IL -1α in LPS + CHOL + etha group significantly decrease(P < 0.01). The results of western blot showed that ethanol significantly inhibited caspase-1 and NLRP3 activation. Conclu-sion Ethanol can inhibit the NLRP3 inflammasome activation in macrophages , which may represent a biological pathway underlying the protective effect of moderate alcohol consumption on coronary heart disease.
7.Effect of combination lidocaine with bupivacaine or ropivacaine on the spinal neurotoxicity in rats

Objective To observe spinal cord neurotoxicity injury induced by mixture of bupiv-acaine-lidocaine compared with mixture of ropivacaine-lidocaine.Methods Forty-eight SD rats,weig-hing 250-300 g,were randomly allocated into 6 groups (n =8):NS group (group N),1.065% bupi-vacaine group (group B),1.5% ropivacaine group (group R),5% lidocaine group (group L),mix-ture group of 5% lidocaine and 1.065% bupivacaine (group LB)or mixture group of 5% lidocaine and 1.5% ropivacaine (group LR).The rats were given intrathecal cathetering after anesthesia.Tail-flick test (TFL)and the mechanical withdrawal threshold (MWT)of rats in each group were meas-ured 1 d before operation (the baseline values)and 1 d,2 d,3 d and 4 d after intrathecal administra-tion.The movement of rat double hind legs were observed at 10 min,30 min,1 h,2 h,3 h and 4 h and 1 d,2 d,3 d and 4 d after intrathecal administration,and the changes of recovery time of lower limbs were recorded.The rats were sacrificed 4 days postoperatively followed by observation on the indexes mentioned above,and spinal cord and cauda equine were taken for staining.Then the patho-logical changes of spinal cord were observed under light and electron microscope,and caudal nerve was rated for pathological injury.Results TFLs of group LR were obviously longer than those of group N,L,B,R,LB 1-4 d after intrathecal administration (P <0.05);Compared with the group N,there was no statistical difference of TFL in the group L,B,R,LB;there was no statistically sig-nificant difference of rat MWT in comparison among the six groups at each time point.There was no statistically significant difference among six groups in recovery time of movement after intrathecal ad-ministration;and the pathological injury scores were obviously higher in group LR than that of group N,L,B,R,LB (P <0.05),respectively.Compared with the group N,there was no statistical difference of the pathological injury scores in the group L,B,R and LB,respectively.Under light microscopy observa-tion:the structure of spinal cord was normal in group N,B and R.In group L and LB,there was focal ede-ma of neurofibers.In group LR,there was edema of neurofibers or demyelination,accompanied with white mass edema in posterior horn of spinal cord.Under electron microscope,only slight edema of medullated fi-bers and unmyelinated nerve fiber was seen in group B and group R;part of the medullated fibers appeared sparse focal lamellar structure in group L and group LB;swelling and degeneration of neuraxon,separation of neuraxon from myelin sheath and disappearance of unmyelinated nerve fiber were seen in group LR. Conclusion The mixture of 5% lidocaine with 1.5% ropivacaine is more neurotoxic to spinal cord than the single administration of lidocaine or ropivacaine,moreover it is not obviously changed compared with the mixture of lidocaine with equipotent bupivacaine.
8.The electrophysiological characteristics of ventralis intermedius nucleus and its role in localizations of Vim-thalamotomy for Parkinson's disease patients

Objective To investigate the electrophysiological characteristics of the ventralis intermedius nucleus (Vim) in order to find an easy and safe way to confirm the target in Vim-thalamotomy. Method In microelectrode-guided selective Vim-thalamotomy for 23 Parkinson's disease patients, the background activity, amplitude and discharge frequencies of Vim were compared with its surrounding structures. The response of kinesthetic neuron and tremor cell to microstimulation was also compared. Result There were differences in backgroud activity and discharge amplitude for Vim, ventralis lateralis nucleus (VL), ventralis caudalis nucleus (VC), and internal capsule. Based on the response to active or passive movement of contralateral limb tremor cells were divided into two subgroups, which were different in localization. Contralateral tremor showed different response when the two subgroups of tremor cells were mircrostimulated. Conclusion The anterior border of Vim was easily found by microrecording. Only by combining microstimulation with microrecording could the posterior border of Vim and its interior and lateral ordination of target were identified exactly and safely. Kinesthetic neurons and tremor cells which responded to the movement of contralateral limbs should be destroyed.
9.The analysis of clinical effect of individualized cranioplasty with different materials for 75 skull defect patients

Objective To summarize the clinical effect of individualized cranioplasty with different materials. Method The clinical data of 75 patients with skull defect were analyzed retrospectively. Ultrathin cranial CT and three-dimension reconstruction were done to delineate the shape and contour of cranial defect. The templet for repair was fabricated with methyl silicone rubber, bone cement or titanium mest according to the condition of the bone defect with laser rapid prototyping technique. With patient's consent, the individualized templet was implanted to repair the cranial defect. Results Methyl silicone rubber or bone cement were used for 40 of 75 patients to fill the defects, and in the other 35 patients titanium mesh was used to cover the defects with overlapping edges. The patches matched cranial defect perfectly without the necessity of revision. The average operation time was 45 minutes. Perfect or excellent cosmetic results was obtained in 63 of the 75 patients (84%). Postoperative complications were as follows: 5 patients complained of headache, which disappeared in there months. Collection of fluid under the scalp was found in 12 patients, among them the fluid was Absorbed 10 days after operation in 8 cases without any treatment, while in 3 patients it disappeared after suction for 1-4 times. In the remaining one patient, the bone cement templet was removed due to infection, and a titanium mesh was implanted one year later. Loosening and displacement of the templet occurred in 2 patients, and silk sufures were used for fixation in both patients. In these two patients, the templet became stable 3 months later, and in the other titanium was used for repair one year later. 10.Imaging characteristics of hepatocellular adenoma compared with pathologic findings

Objective To retrospectively compare CT and MR features of hepatocellular adenoma with pathologic findings.Methods Twelve patients with histopathologically proved hepatocellular adenoma were classified on the basis of pathologic and genotype phenotype findings into four groups:steatotic type,cytological abnormality type,telangiectatic adenoma with inflammatory infiltrates type and atypical adenoma type.The CT and MR features of each type were reviewed retrospectively compared with the pathological results.Results In this retrospective study,12 patients were examined with CT (8 patients) and MR (8 patients).Among 12 patients,4 patients showed a steatotic type.One patient showed hypo-density on the non-enhanced CT and 3 patients demonstrated hypo-density on all phases of the post-contrast scans.Two lesions showed iso-intense signal on the in-phase T1 WI with signal dropout on the out-of-phase T1WI,and hypo-intense signal on the T2 WI with fat suppression sequences.One lesion demonstrated moderate hypointense signal on all phases of the post-contrast MRI scans.Two patients with the telangiectatic adenoma irflammatory infiltrates type were found.One patient showed hypo-density on the non-enhanced CT scans and hyper-density on all phases of the post-contrast CT scans.One patient demonstrated iso-intense signal and the other hypo-intense signal on the T1 WI,and both displayed moderate hyper-intense signal on the T2WI with fat suppression sequences and hyper-intense signal with gradual enhancement on all phases of post-contrast MR scans.There were 3 patients with a cytological abnormality type.One patient appeared hypodensity and 1 patient showed uniform iso-density on non-enhanced CT scans.All patients who had undergone contrast-enhanced CT scans were found to have hyper-density on the hepatic arterial-dominant phase and became slightly lower on the portal venous phase.On the delay phase the density reduced further.One mass showed iso-intense signal on the T1WI and hyper-intense signal on the T2WI with fat suppression sequences.There were 3 patients with an atypical adenoma type.One patient appeared uniform hypo-density on the nonenhanced CT and hyper-density on the hepatic arterial-dominant phase and became iso-dense on the portal venous phase.On the delay phase,it was slightly hyper-dense.Two out of the three lesions showed isointense and one hypo-intense signal on the in-phase T1 WI,and hypo-intense,hyper-intense,and iso-intense signal on the T2WI with fat suppression sequences,respectively.Two patients examined on all phases of post-contrast MRI scans.The result was similar to the CT findings.Conclusion The imaging features of hepatocellular adenoma are closely associated with pathological characteristics.
