1.Clinical observation of micro-fracture operation combined with sodium hyaluronate in the treatment of talar cartilage injury with ankle arthroscopy

Objective To explore the clinical situation of ankle arthroscopic surgery combined with sodium hyaluronate in the treatment of osteochondral lesions of the talus.Methods 40 cases of osteochondral lesions of the talus were treated in our hospital from June 2014 to October 2016 were treated with ankle arthroscopic surgery combined with sodium hyaluronate,the situation after the treatment were observed, and according to the patient's rehabilitation the rehabilitation exercises were conducted according to the patient's condition, and the therapeutic effect was analyzed.Results The total effective rate was 87.5%;The mean value of ankle function score before treatment was(34.26±2.13)points,the mean value of ankle function score after treatment was(74.35±2.07)points, the difference was statistically significant before and after treatment(P<0.05).Conclusion The clinical observation of ankle arthroscopic surgery combined with sodium hyaluronate in the treatment of osteochondral lesions of the talus, the treatment effect is better, and can help the patient's ankle joint function get improve, and ultimately improve the prognosis, it is worthy of reference.
2.Absorption kinetics of breviscapine in intestine of rats in situ

Objective To investigate the intestinal absorption kinetics of breviscapine in rats.MethodsThe intestinal absorption in small intestine and colon of rats in situ was investigated using circular perfusion and HPLC methods.The in situ absorption of scutellarin in the small intestine was studied and the effects on the intestinal absorption were observed at the various concentrations and different pH values using the same methods.Results The results showed that there were significant differences in both absorptive percentage and absorption rate constant(ka)in the small intestine between the experimental groups of rats with and without bile duct being ligated.The absorption rate constants in small intestine and colon were(0.107 1?0.013 0)and(0.070 7?0.008 9)h-1,respectively.No saturation phenomenon occurred and ka was almost kept unchanged at different concentrations of breviscapine.The absorption of breviscapine was not influenced by pH values ranging from 6.0 to 7.4.Conclusion The results indicates that breviscapine is absorbed more in small intestine than in colon.The absorption of breviscapine complied with the passive diffusion mechanism and first order kinetics.In conclusion,these results suggest that breviscapine could be prepared in oral sustained-release dosage form.

4.Application of post-hospital extended nursing based on WeChat public platform in total knee replacement patients

Objective To explore the effects of post-hospital extended nursing based on WeChat public platform in total knee arthroplosty (TKA) patients' so as to provide a reference for the diversity of extended nursing. Methods From September 2015 to May 2017, we selected 140 TKA patients from Huzhou First People's Hospital of Zhejiang Province by simple random sampling. All of the patients were divided into observation group (n=70) and control group (n=70) according to the discharge order. Patients of control group were treated with routine discharge instructions and follow-up. On this basis, patients of observation group accepted extended nursing based on WeChat public platform. We compared the score of the Hospital for Special Surgery Knee Score (HSS), modified Barthel index (MBI) and satisfaction of patients in two groups. Results There was no significant difference in the score of HSS and MBI of patients in two groups before discharge (P>0.05). There were significant differences in the scores of HSS between observation group [(66.8±5.6), (72.8±5.7), (83.7±5.8) respectively]and control group [(61.2±5.8), (67.5±5.8), (72.8±5.9) respectively]one, three and six months after discharge (P< 0.05). Six months after discharge, the score of MBI of observation group was (92.2±6.3) significantly higher than that of control group (81.4±7.1) (P<0.05). The overall satisfaction of observation group and control group was (96.5±1.2) and (92.8±1.5) respectively with a significant difference (P<0.05). Conclusions Post-hospital extended nursing based on WeChat public platform can promote TKA patients' postoperative rehabilitation and improve patients' activities of daily living.
5.Study on the pharmacological effects and mechanism of Gegen-Zhimu herb pair in preventing and treating Alzheimer's disease by UHPLC-Q/TOF-MS metabolomics strategy

Objective To evaluate the efficacy of Puerariae lobatae radix (PLR) and Anemarrhenae Rhizoma (AR) in preventing and treating Alzheimer’s disease (AD) and explore its potential mechanism of action by LC-MS serum metabolomics strategy. Methods The AD rat model was established by administering aluminum chloride (AlCl3) and D-galactose (D-gal) for 20 weeks. The traditional Chinese medicine intervention group was given the PLR, AR, and PLR-AR extracts for 8 weeks by gavage. The model effect and efficacy were evaluated by Morris water maze test and biochemical indicators including SOD, NO, and MDA; Metabolomics research based on the UHPLC-Q/TOF-MS method was conducted, and relevant metabolic pathways were analyzed through the MetaboAnalyst online website. Results The learning and memory abilities of AD model rats were significantly decreased compared with the control group, and the levels of oxidative stress and lipid peroxides were significantly increased (P<0.05), while the SOD content was decreased considerably (P<0.01). The learning and memory abilities of AD model rats were improved, oxidative stress and lipid peroxidation levels were reversed, and serum SOD content was increased significantly after the intervention of PLR-AR, with better effects than single drugs. Through metabolomics, 70 differential metabolites were identified between the AD model group and the control group, mainly involving 10 pathways, including phenylalanine, tyrosine, and tryptophan biosynthesis, phenylalanine metabolism, and unsaturated fatty acid biosynthesis, et.al. The intervention of PLR-AR could adjust 47 metabolites, with 20 metabolites showing significant differences (P<0.05). The significantly adjusted metabolites involve 6 pathways, including phenylalanine, tyrosine, and tryptophan biosynthesis, et al. Conclusion The combination of PLR and AR could significantly improve the learning and memory abilities of AD rat models. The mechanism may be related to the improvement of oxidative stress and lipid peroxidation levels, the increase of serum SOD content, and the regulation of phenylalanine, tyrosine, and tryptophan biosynthesis pathways.
