1.Chinesization of the HEMO-FISS-QoL questionnaire and its reliability and validity

Objective:To evaluate the reliability and validity of the Chinese version of HEMO-FISS-QoL(HF-QoL) questionnaire (HF-QoL-C) in the Chinese population with hemorrhoids.Methods:From November 2021 to November 2022, a self-constructed general information questionnaire, HF-QoL-C, and the 36-item short form health survey (SF-36), Goligher classification, and Giordano severity of hemorrhoid symptom questionnaire (GSQ) were used to conduct a questionnaire survey on 760 hemorrhoid patients in the anorectal department of six hospitals. The data was analyzed for reliability and validity using SPSS 21.0 and AMOS 26.0 software.Results:The Cronbach's α coefficient of HF-QoL-C and its dimension ranged from 0.831 to 0.960, and the split coefficient was 0.832-0.915. Four common factors were extracted through principal component exploratory factor analysis. Confirmatory factor analysis indicated acceptable structural validity( χ2/ df=8.152, RSMEA=0.097, CFI=0.881, IFI=0.881, NFI=0.867). HF-QoL-C was correlated with SF36 and GSQ( r=-0.694, 0.501, both P<0.01). There were differences in the total score and dimensional scores of HF-QoL-C between surgical and drug treated patients, different grades of Goligher classification for hemorrhoidal disease, and different ranges of hemorrhoid prolapse (all P<0.001). No ceiling effect was found in the total score and the scores of each dimension(0.3%-2.0%). There was a floor effect in both psychological function and sexual activity dimensions (16.7%, 35.1%). Conclusion:HF-QoL-C has good reliability and validity, which can be used to measure the quality of life of Chinese hemorrhoid patients.
2.Network meta-analysis of first-line treatments for metastatic urothelial carcinoma

OBJECTIVE To systematically evaluate the effectiveness of different treatment regimens as first-line treatment for metastatic urothelial carcinoma (UC) using a network meta-analysis (NMA) approach. METHODS Electronic databases including PubMed, the Cochrane Library, Embase, Wanfang Data, CNKI, and VIP were searched for randomized controlled clinical trials (RCTs) on first-line treatment for metastatic UC from January 1, 2010 to January 31, 2024. After literature screening and data extraction, a risk of bias assessment of included studies was conducted. R software (version 4.3.2) was used to perform the NMA. RESULTS A total of 11 RCTs involving 14 treatment interventions were included. No significant differences were noted in objective response rate among groups, with the combination of pembrolizumab, gemcitabine and cisplatin having the highest probability of ranking first. Regarding progression-free survival (PFS) and overall survival (OS), no significant differences were observed among groups, while enfortumab vedotin combined with pembrolizumab showed a trend towards better PFS extension compared to gemcitabine combined with cisplatin [HR=0.45, 95%CI(0.20,1.06), P=0.049], and it had the highest probability of ranking first in both PFS and OS. CONCLUSIONS The combination of enfortumab vedotin and pembrolizumab may have an advantage in prolonging survival in the first-line treatments for metastatic UC.
4.Postoperative serum triglyceride levels in predicting risk of new-onset diabetes mellitus in patients following liver transplantation.

To investigate the postoperative serum triglyceride (TG) levels in predicting the risk of new-onset diabetes mellitus (NODM) in patients following allogeneic liver transplantation. One hundred and forty three patients undergoing allogeneic liver transplantation in Shanghai General Hospital from July 2007 to July 2014 were enrolled in this study. The NODM developed in 33 patients after liver transplantation. The curve of dynamic TG levels in the early period after liver transplantation was generated. Independent risk factors of NODM were determined by univariate and multivariant logistic regression analyses. The clinical value of TG in predicting NODM was analyzed by area under the ROC curve (AUC). Serum TG levels were gradually rising in the first week and then reached the plateau phase (stable TG, sTG) in patients after surgery. The sTG in NODM group were significantly higher than that in non-NODM group (=-2.31, <0.05). Glucocorticoid therapy (=4.054, <0.01), FK506 drug concentration in the first week after operation (=3.482, <0.05) and sTG (=3.156, <0.05) were independent risk factors of NODM. ROC curve analysis showed that the AUC of sTG in predicting NODM was 0.72. TG shows a gradual recovery process in the early period after liver transplantation, and the higher TG level in stable phase may significantly increase the risk of NODM in patients.

6.Current status and prospects of cognitive training for older adults with cognitive impairment

Cognitive training is an important non-pharmaceutical intervention for older adults with cognitive impairment.It can improve cognitive function and life skills through training in different cognitive domains.Here we review the previous research on different types of cognitive training, examine the effect of cognitive training on cognitive improvement in older adults with cognitive impairment, and analyze the underlying neural mechanisms.In addition, we also identify deficiencies in the designs of previous research and propose how research protocols and training efficacy can be improved in the future.
7.Determination of Fluconazole in Human Plasma by UPLC-MS/MS with Isotopes Dilution

OBJECTIVE: To establish a method for determination of fluconazole concentration in human plasma. METHODS: UPLC-MS/MS method was adopted to determine plasma after precipitated with acetonitrile. Using isotope fluconazole-d4 as internal standard, the determination was performed on ACQUITY UPLC BEH C18 column with mobile phase consisted of 0.1% formic acid-acetonitrile (gradient elution) at the flow rate of 0.3 mL/min. The column temperature was 40 ℃, and the sample size was 3 μL. ESI was used for positive ion scanning by multiple reaction monitoring mode. The ion pairs for quantitative analysis were m/z 307.1→220.0 (fluconazole) and m/z 311.1→223.0 (internal standard). RESULTS: The linear range of fluconazole was 10-5 000 ng/mL (r=0.998 1). The limits of quantitation was 10 ng/mL. RSDs of intra-day and inter-day were less than 8%; accuracy ranged 95.8%-106.7%. The extraction recovery ranged 97.3%-107.3% (RSD<5.0%, n=6), and matrix effect, dilution effect and residual effect didn’t influence quantitative analysis of the substance to be measured. CONCLUSIONS: The method is simple, rapid, specific and accurate, which can be used for therapeutic drug monitoring and pharmacokinetic study of fluconazole.
8.Meta-analysis of the Effects of Sh enqi Fuzheng Injection Combined with Conventional Chemotherapy on the Immune Function of Patients with Advanced NSCLC

OBJECTIVE: To systematically evaluate the effects of Shenqi fuzheng injection combined with conventional chemotherapy on the immune function of patients with advanced non-small cell lung cancer (NSCLC), and provide evidence-based reference for clinical medication. METHODS: Retrieved from Cochrane Library, PubMed, Medline, Embase, CNKI, Wanfang database, VIP and CBM, Shenqi fuzheng injection combined with conventional chemotherapy (trial group) versus conventional therapy (control group) for advanced NSCLC were collected. After literature screening, data extraction and quality evaluation with Cochrane system evaluator manual 5.1.0 risk evaluation tool, Meta-analysis was performed by using Rev Man 5.3 statistical software. RESULTS: A total of 16 literatures were included, involving 1 324 cases. Results of Meta-analysis showed that there were no statistical significance in the difference of objective remission rate (ORR) [RR=1.14, 95%CI(0.91,1.43), P=0.25] and the level of CD8+ [MD=-1.26,95%CI(-4.10, 1.59),P=0.39] between 2 groups. The levels of CD3+ [MD=17.48, 95%CI(13.40, 21.56), P<0.000 01 ], CD4+[MD=12.26, 95%CI(8.39 16.13), P<0.000 01], CD4+/CD8+ [MD=0.33,95%CI(0.27, 0.39),P<0.000 01] and the percentage of NK cells [MD=9.33, 95%CI(5.72, 12.94), P<0.000 01] in trial group were significantly higher than control group. Results of subgroup analysis for medication duration of Shenqi fuzheng injection showed that after 1-14 d treatment of Shenqi fuzheng injection, the levels of CD3+ [MD=17.11, 95%CI(12.37 ,23.17),P<0.000 01], CD4+[MD=13.28,95%CI(8.33, 18.23),P<0.000 01], CD4+/CD8+[MD=0.36,95%CI(0.28,0.43),P<0.000 01] and the percentage of NK cells [MD=12.06,95%CI(7.52,16.61),P<0.000 1] in trial group were significantly higher than control group. After 21 d treatment of Shenqi fuzheng injection, the levels of CD3+[MD=14.88, 95%CI(7.51,22.26),P<0.000 01], CD4+[MD=10.56,95%CI(5.57,15.56),P<0.000 01], CD8+[MD=3.02, 95%CI(1.80, 4.23),P<0.000 01], CD4+/CD8+[MD=0.29,95%CI(0.23, 0.35),P<0.000 01] and the percentage of NK cells [MD=5.58,95%CI(2.49, 8.67),P=0.000 4] in trial group were significantly higher than control group. There was no statistical significance in the level of CD8+ between 2 groups after 7-14 d treatment of Shenqi fuzheng injection [MD=-4.26,95%CI(-12.60, 4.09),P=0.32]. The incidence of leukopenia, nausea and vomiting, and renal dysfunction in trial group were significantly lower than control group. There was no statistical significance in the incidence of hemoglobin decreased and thrombocytopenia between 2 groups. The results of publication bias showed that there was a greater possibility of publication bias in this study. CONCLUSIONS: Shenqi fuzheng injection combined with conventional chemotherapy may improve the immune function of patients with advanced NSCLC and improve the safety after chemotherapy. But more large-scale, milltiple-center and high-quality RCT are needed to validate this conclusion.
9.Anti-inflammatory effect of interleukin-35 in mice with colitis and its mechanism.

OBJECTIVE:
To investigate the anti-inflammatory effect and mechanisms of interleukin-35 (IL-35) in inflammatory bowel disease.
METHODS:
BALB/c mice were divided into three groups with 10 mice in each group:control group, model group (oral administration of 4% glucan sodium sulfate for 7 d) and IL-35-treated group (oral administration of 4% glucan sodium sulfate for 7 d, intraperitoneal injection of 2 μg IL-35 at d2-5). Disease activity index (DAI) was scored every day. After 7 d, the mice were sacrificed, and the serum and intestinal tissue samples were collected. The gross morphology of the colon was observed; HE staining was used to observe the pathological changes of colon tissue; flow cytometry was employed to detect the change of macrophage polarization ratio in colon tissue; the mRNA expression levels of cytokines IL-6, TNF-α, IFN-γ, IL-10 and SHIP1 in colon tissue were determined by real-time quantitative RT-PCR; the expression and distribution of SHIP1 in colon tissue was measured by immunohistochemistry; Western blotting was adopted to detect the expression level of SHIP1 protein in colonic intestinal tissues of each group.
RESULTS:
The DAI scores of the mice in the model group were higher than those in the control group, while the DAI scores in the IL-35-treated group were lower than those in the model group (all <0.01). Compared with the control group, the colon length was significantly shortened in the model group (<0.05), while the colon length of the IL-35-treated group had an increasing trend compared with the model group, but the difference was not statistically significant (>0.05). Compared with the model group, microscopic inflammatory infiltration score was decreased and microscopic crypt destruction and score was significantly lower in IL-35-treated group (all <0.05). The relative expression of proinflammatory cytokines IL-6, TNF-α and IFN-γ in the colon tissue of IL-35-treated group was decreased compared with the model group, while the relative expression of IL-10 mRNA was higher than that of the model group (all <0.05). Compared with the control group, the proportion of M1 macrophages in the model group increased (<0.05), while the proportion of M1 macrophages in the IL-35-treated group was lower than that in the model group (<0.05). The relative expression of SHIP1 mRNA and protein in the colon tissue of IL-35-treated group was higher than that in the model group (all <0.05).
CONCLUSIONS
IL-35 can inhibit the polarization of M1 macrophages and regulate inflammatory cytokines to promote anti-inflammatory effect on mice with colitis.

10.Simultaneous Determination of 7 Components in Kuhuang Injection by UPLC-MS/MS

OBJECTIVE:To establish a method for the content determination of matrine,sophocarpine,emodin,rhein,chloro-genic acid,sarkosaponin a,aloe-emodin in Kuhuang injection.METHODS:UPLC-MS/MS was adopted. The determination was per-formed on Phenomenex Kenetix C18 column with mobile phase consisted of methanol-0.1% formic acid(gradient elution)at the flow rate of 0.5 mL/min. The column temperature was set at 20 ℃,and injector temperature was set at 10 ℃. The equilibrium time was 4 min and sample size was 5 μL. Ionization mode was electrospray ionization with spray voltage of 5500 V and -4500 V,at-omizing air pressure of 4.14×105 Pa,heater pressure of 4.48×105 Pa,curtain air of 1.72×105 Pa and ion source temperature of 600 ℃. The work mode was multiple reaction monitoring mode. RESULTS:The linear range were 1.25-80.0 ng/mL for matrine (r=0.9993),1.10-70.0 ng/mL for sophocarpine(r=0.9995),0.16-10.5 ng/mL for emodin(r=0.9993),2.61-168 ng/mL for rhe-in (r=0.9993),1.50-96.0 ng/mL for chlorogenic acid (r=0.9993),1.48-94.5 ng/mL for sarkosaponin a (r=0.9996) and 6.11-391 ng/mL for aloe-emodin(r=0.9991). The limits of quantification were 0.061,0.109,0.041,1.313,0.500,0.492,3.055 ng/mL,limits of detection were 0.025,0.054,0.016,0.656,0.150,0.148,1.528 ng/mL. RSDs of precision,stability and reproducibility tests were all no more than 3%. The recoveries of them were 95.45%-99.45%(RSD=1.43%,n=6),97.50%-101.00%(RSD=1.50%,n=6),95.67%-101.73%(RSD=2.85%,n=6),97.17%-100.57%(RSD=1.16%,n=6),95.19%-98.90%(RSD=1.71%, n=6),95.38%-103.85%(RSD=3.39%,n=6),95.50%-101.17%(RSD=1.20%,n=6),respectively. CONCLUSIONS:The method is simple,effective,accurate,reliable and suitable for simultaneous determination for 7 components in Kuhuang injection.
