Objective To explore the effects of glucocorticoid(GC)and lipid metabolism on the pathogenesis of avascular necrosis of femoral head(ANFH).Methods Thirty-two New Zealand rabbits were randomly assigned into two groups as experiment group(n=24)and control group(n=8).Each rabbit in experiment group was given injection with 8.0mg/kg of hydrocortisone acetate for two times per week, while rabbits in control group were given normal saline in same quantity.Serum glucocorticoid(GC)concentration, total cholesterol(TC)and triglyceride(TG)were determined respectively before and 2,4,6 and 8 weeks after treatment.Rabbits were sacrificed at the 2nd,4th,6th and 8th week. The rabbits, sacrificed at the same time point, were stratified into group A(low concentration group)and group B(high concentration group)according to the GC concentration,in an attempt to observe the rate of empty bone lacuna and glucocorticoid receptor(GR)expression in the soft tissue around hips. Results The concentration of TC of experiment group began to increase since the 6th week, and that of TG since 2nd week(P0.05).In addition, the rate of bone lacuna and OD scores of GR were positively correlated at the 4th and 8th week(r=0.699 0,0.605 0,P

Objective To investigate the effect of self-designed closed negative pressure drainage combined with sponge dressings on refractory wounds. Methods Sixty patients with phase III-IV pressure ulcers were randomly divided into experiment group and control group in equal number. The self-designed closed negative pressure drainage combined with sponge dressing was applied in the experiment group and in the control group the conventional dressings were used. The two groups were compared in terms of hyperplasia of fresh granulation tissue, time for filling the defect and the healing time and the medical expense. Results Compared to the control group, the time for hyperplasia of fresh granulation tissue, the time for filling the defect and the healing time in the experiment group were all significantly shorter, and the medical expense of the experiment group was significantly less(all P<0.01). Conclusion The self-designed closed negative pressure drainage combined with sponge dressings in the treatment of phase III-IV refractory pressure ulcers may effectively shorten the healing time, improve the curative effects and reduce the economic burden of patients.

Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2015;29(14):1248-1252
OBJECTIVE:
To evaluate the effect of the vestibular diagnosis and treatment system (SRM-IV ) in diagnosis and treatment of patients with benign paroxysmal positional vertigo (BPPV).
METHOD:
Patients who were diagnosed as BPPV by SRM-TV in the clinic of our hospital from November 2013 to October 2014 were retrospectively analyzed in this study.
RESULT:
Among 425 suspected cases, 230 BPPV-positive patients were diagnosed including 131 cases of posterior SC (57.0%), 95 cases of horizontal SC (41.3%) and 4 cases of more than two SC (1.7%). The cure rate by SRM-V was 94.6% and the effective rate was 100.0%. The relapsed occurred in 10 patients (4.8%), which contained 4 men and 6 women.
CONCLUSION
SRM-V can realize 360° reasonable repositioning procedure while Canalish reposition procedure cannot. SRM-V can improve both the corrective rate of diagnosis and the cure rate, especially for the patients who suffered from complex BPPV.

Platelet-rich plasma (PRP) currently has been widely used in various medical fields, such as tissue regeneration, wound healing, scar repair, skin and hair regeneration etc..PRP is rich in platelets, growth factors and other blood components, which can effectively promote tissue repair and healing. However, there is no optimal preparation method and unified standard of composition ratio for PRP, so its clinical application value has not been satisfactorily interpreted yet. In this paper, the preparation and quality standard of PRP were reviewed to provide basis for standardization of RPP in clinical application.

【Objective】 To discuss the repair effect of lyophilized platelet lysate (PL) products on articular cartilage injury model of rats. 【Methods】 A total of 25 SD rats were injected with typeⅡcollagenase at the right knuckle articular cavity respectively on day 1, 3 and 5 of experiment, and the modeling conditions were observed 14 days after the last injection of collagenase. The SD rats with successful modeling were randomly divided into three groups, and were injected with lyophilized PL [Group A, 1 mL/(mouse·time)], PL [Group B, 1 mL/(mouse·time)], and normal saline[Group C, 1 mL/(mouse·time)]. The above three substances were injected with corresponding drugs on day 0, 7, 14 and 21 of experiment based on the grouping conditions, and the changes of knee joint diameters of the rats from the three groups were observed and compared. On day 14 and 28, one rat in each group was randomly killed and two knuckle articular cavities of each were taken for tissue sampling, using hematoxylin-eosin staining (HE) and immunohistochemistry. 【Results】 After 14 days of modeling by injection of type Ⅱ collagenase, the proportion of successful modeling in rats was 84% (21/25), with the knee joint diameter (mm) before and after modeling at 12.84±1.14 vs 14.11±1.17(P<0.01). On day 14, 21 and 28, groups A and B were superior to group C in the knee joint diameter and activity improvement (P<0.05), with 13.33±1.16 vs 13.37±1.08 vs 14.21±1.08, 13.10±1.09 vs 13.01±1.04 vs 14.09±1.09 and 12.38±1.08 vs 12.51±1.03 vs 14.01±1.07, respectively. Histological observation showed that group A and B were superior to group C in the production and arrangement of chondrocytes and the positive expression of type Ⅱ collagen, and there was no significant difference between group A and group B. 【Conclusion】 The lyophilized PL has similar therapeutic effect to PL in the treatment and repair of articular cartilage injury, and is worthy of clinical application.
