1.Long-term outcome of orthotopic ileal neobladder (Camey Ⅱ) in the treatment of bladder cancer

Objective To report the long-term follow-up results on treating bladder transitional cell carcinoma with orthotropic ileal neobladder after radical cystectomy.Methods Thirty-three male patients with a mean age of 64 years were subjected to Carney Ⅱ orthotropic ileal neobladder oper-ation after cystectomy.The entire length of ileum was opened along the antimesenteric border,placed into a transverse U shape,and sutured with a running suture.The bottom of the U shape was anasto-mosed with urethra to reconstruct the urinary continuity.The perioperation mortality,complications,urinary continence,renal function,image findings,and urodynamic examination were all retrospec-tively investigated.The quality of life was evaluated using the Functional Assessment of Cancer Ther-apy-Bladder(FATC) and International Prostate Symptom Score (IPSS).Results Thirty patients were followed up for 12-60 months.Urodynamics was carried out after oporation.The average ca-pacity of the urinary reservoir,the peak flow rate and postvoid residual were (380±65) ml,(12.6±2.3) ml/s and (25±11) ml,respectively.But the intra-reservoir pressure was significant lower than the intra-urethra pressure(20.4±8.7 cm H2O vs.57.2±10.5 cm H2O).Twenty-eight patients had a satisfactory daytime urinary continence and 26 patients had a satisfactory nocturnal urinary continence.The total FACT was 110.5±16.0 and IPSS was 14.5±4.2.Conclusion CamyⅡ ileal neobladder is a satisfactory method for treatment of invasive bladder cancer after radical cystectomy; it has few com-plication and is clinically valuable.

Objective To study the mechanism of senile escaping of benign prostatic hyperplasia(BPH) condition. Methods With polymerase chain reaction based telomeric repeat amplification protocol assay(TRAP),telomerase activity was examined in 13 normal prostate tissues,35 benign prostatic hyperplasia tissues,33 hyperplastic tubercle and diaphragm. 3.Clinical investigation on the correlation between lower urinary tract infection and cystitis glandularis.

In order to study the association between lower urinary tract infection and cystitis glandularis (CG), 120 cases of CG were diagnosed by cystoscopic biopsy in the suspicious foci of the bladder. Among them, 72 cases were subjected to bacterial counting culture of urine and microscopic examination of urinary sediment, and 60 cases to fluorescence quantitative polymerase chain reaction (FQ-PCR) assay to detect HPV, CMV and HSV DNA in urine samples. In the 72 cases of CG, the positive rate of bacterial counting culture of urine was 15.3% (11/72), and gray zone rate was 18.1% (13/72). 31.9% (23/72) patients were positive in bacterioscopy of urinary sediment. There was statistically significant difference as compared with the control group (P<0.01). Only 4 of 60 urine samples were positive by FQ-PCR in detection of the three viruses mentioned above with the positive rate being 6.67%. Compared with the control group, there was no significant difference (P>0.05). It was concluded that the genesis of CG was closely correlated with the chronic lower urinary tract infection, especially caused by Esch coli.

4.Modified madigan prostatectomy: a procedure preserved prostatic urethra intact.

A total of 92 patients with benign prostatic hyperplasia (BPH) were subjected to modified Madigan prostatectomy (MPC) for a much satisfactory effect in open prostatectomy surgery. Exposing anterior prostatic urethra near the bladder neck and conjunct cystotomy modified the MPC procedure. This modified procedure preserved prostatic urethra intact and could also deal with intracystic lesions at the same time. The intact of prostatic urethra was kept completely or largely in 86 cases. The amount of blood loss during modified procedure was less. The mean operative time was 105 min. Seventy patients had been followed up for 3-24 months. The postoperative average Qmax was 19. 2 ml/s. The cystourethrography revealed that the urethra and bladder neck were intact in 10 patients postoperatively. Furthermore, the prostatic urethra was obviously wider after modified MPC. The modified MPC can reduce the occurrence of urethra injury and enlarge the MPC indications. The modified technique is easy to perform with less complications and much satisfactory clinical result.

5.The Expression and Significance of Interleukin-6 in Prostate Cancer

Objective To study the role of interleukin-6(IL-6)in the pathogenesis of prostate cancer and its clinical significance. Methods Immunohistochemistry and RT-PCR were used to detect the expression of IL-6 protein and mRNA in frozen prostatic adenocarcinoma,adjacent benign prostatic tissue,and prostate cancer cell lines PC-3 and LNCaP. The serum levels of IL-6 in patients with prostate cancer and healthy controls,and the supernatants of prostate cancer cell cultures were measured by using ELISA. Results The IL-6 protein levels in prostate cancer tissue and PC-3 cells were significantly higher than those in adjacent benign prostatic tissue and LNCaP cells. The serum IL-6 levels in the patients with prostate cancer were markedly higher than those in the healthy controls. The IL-6 levels in supernatants in PC-3 cells were notably higher than those in the LNCaP cells. Conclusion The IL-6 gene may act as an important regulator in prostate cancer progression and may be one of the causes of prostate cancer conversion from an initially androgen-dependent state into an androgen-independent state.
6.The micro-morphological changes of rats' ventral prostates following castration

Objective To reveal the morphologic changes of rats' prostates after castration, and to explore the possible mechanism that causes these morphologic changes. Methods 90 male adult rats were randomized into 9 groups (10 per group), one group as the control, while the others were sacrificed 1, 2, 3, 5, 7, 10, 14, 21 days post castration. Prostates were harvested, then fixed, embedded, sectioned, and observed by light microscope and electric telemicroscope. Furthermore, sections of the prostates were detected for apoptosis by TUNEL and PCNA by immunohistochemistry. Sections stained by HE were applied to computer-based imaging analysis system in order to count the area of glands, stroma and the width of the epithelium separately. Results Prostatic glands were atrophied, and the secretion of glands was diminished post castration. Apoptosis and necrosis occurred in the epithelia as well as in the stromal cells and the proliferation index was decreased. Conclusions Weight and volume loss of the prostates after castration was due to the loss and atrophy of the prostate cells as well as the decrease of secretion. Apoptosis and proliferation might play a role in the atrophy of the prostate.
7.Ethical issues and countermeasure on organ transplantation.

The ethical problems had directly affected the applying and development of organ transplantation.The serious shortage of donors restricted this technique deeply.The analysis on the ethical and moral problem about organ transplantation had been performed from ethical angel.It was proposed that implementation of brain-death legislation,setting up the newly ethical conception and prohibiting the organ business.Voluntary contribution was the first ethical principle in the source of the donor.Medical principles and social standards should be taken into account on the problem of acceptor selection.

Glutamate is one of the important neurotransmitters and participates in micturition reflex in the cerebral cortex,pons and spinal cord micturition center.Glutamate receptors include metabotropic and ionotropic and the latter again include NMDA and non-NMDA receptors.They all have different effects on micturition reflex.NMDA and non-NMDA receptors may interact on each other,and so may glutamate and other neurotransmitters in the central nervous system.

10.Etiological study on cystitis glandularis caused by bacterial infection.

To study the relationship between bacterial infection and the etiology of cystitis glandularis, 36 female Wistar rats were divided into 3 groups. No intervention was given to the rats in the blank group. NS was infused into the bladder of the rats of the control group, and solution containing E. coli was injected into the bladder of experimental group. Three months later, tissue samples of bladder were collected and observed visually and under light microscope. The results showed that tissues of the blank group were normal; one sample in the control group showed Brunn's nests and cystitis cystica, and 10 in the experimental group had the change of cystitis glandularis. Compared to the blank and control group, samples in the experimental group showed significant change (P<0.05). There were no significant difference between blank group and control group (P>0.05). It is concluded that bladder instillation of E. coli can induce cystitis glandularis, which confirms that infection is the cause of cystitis glandularis.
