1.Analysis on Hot Spots of Innovative Technology of Chinese Medicine Industry in Shanghai Based on The IPC Classification of Patents

Objective To analysis the hot spots of innovative technology of Chinese medicine industry in Shanghai during the recent years.Methods Patent Database of State Intelletual Property Office was retrieved by IPC class numbers combined with keywords and the IPC classification of the Chinese medicine patents in Shanghai area during 2002-2013 were under statistical analysis.Results The hot spot distribution of therapeutic activity of TCM preparations:Antineoplastic is the most important activity of Chinese medicine development, followed by the activities of treatment of inadequate blood flow or cardiovascular system diseases, such as hyperglycemia, high cholesterol, and so on. The activity of treatment of the digestive tract or digestive system diseases, followed by the activities of anti infection, antiviral, antibacterial, anti-inflammatory treatment, etc. The hot spot distribution of dosage:Researches on new dosage of Chinese medicine were mainly reflected in the improvement of traditional dosage form and development of new formulations. The capsules, tablets, oral liquid, and injections were the new dosage forms under the most researches. The distribution of medical resources:Medicinal plants or animals mainly came from “angiosperm”. Medicinal animals were less than medicinal plants, which mainly belonged to insecta.ConclusionThe hot spots of innovative technology of Chinese medicine industry in Shanghai are mainly distributed in the medicine of anti-tumor, cardiocerebral vascular diseases, digestive system diseases and so on in recent years. The researches of new formulation focus on the traditional dosage form improvement and new formulations development. “Angiosperm” is the key to the development of medicinal plants.
2.Static balance ability of functional ankle instability patients

BACKGROUND:At present, the study to balance ability of functional ankle instability patients is less. The
balance ability deficiency may also be one of the important reasons for ankle repeated injury, so the research can effectively reduce the damage probability.
OBJECTIVE:To explore the static balance ability characteristics of functional ankle instability patients.
METHODS:Ten male subjects with unilateral functional ankle instability were enrol ed as the experimental group in the study, and other 10 healthy match-paired subjects were served as the control group. Their static balance ability was measured under double feet standing and single foot standing using win-pod system, the double feet testing lasted for 30 seconds and the single foot testing lasted for 10 seconds. Evaluation parameters of balance ability were obtained.
RESULTS AND CONCLUSION:During double feet testing, there were no significant differences in balance
ability parameters between the experimental group and the control group whether eyes be opened or closed (P>0.05), but the index values of double feet testing were obviously less than those of single foot testing. So we
concluded that in the double feet standing, there was no significant difference in the static balance ability between functional ankle instability patients and normal persons. Moreover, the static balance ability of double feet was
better than that of single feet. In the single foot standing of the affected foot, the static balance ability of functional ankle instability patients was slightly worse than that of the normal persons, especial y with eyes closed.
Comparison of the average swing between the X-axis and Y-axis found that in the process of maintaining balance posture, the swing direction of the body center of gravity was mainly in the anteroposterior direction.
3.Preparation of titanium implant surface bioactive membrane

BACKGROUND: The coating induced by conventional simulated body fluid (SBF) and 1.5-folds SBF as biomimetic method has the shortcomings of slowly growth with weeks or months growth cycle. OBJECTIVE: To evaluate Ti-6Al-4V alloy surface deposition of hydroxyapatite coating induced by 3-folds SBF. DESIGN, TIME AND SETTING: In vitro biological analysis. The experiment was performed at the Institute of Metal Research, Chinese Academy of Sciences from November 2008 to March 2009. MATERIALS: Ti-6A1-4V alloy was supported by Boda Metel Materials Co., Ltd. METHODS: The pretreated Ti-6Al-4V alloy was immersed in 37 'C 3-folds SBF. The SBF was replaced every 24 hours for 3 days. MAIN OUTCOME MEASURES: The morphology, phase and elemental composition of calcium phosphate coating were analyzed by scanning electron microscopy, X-ray diffraction and infrared spectroscopy analysis. RESULTS: The scanning electron microscopy exhibited that the surface of Ti-6A1 -4V alloy was covered with deposited film with granular crystal on local areas at day 1 after immersion. The deposited film covered surface of Ti-6A1-4V alloy at day 3 after immersion, and granular or scale-shaped crystal could be seen. The crystal deposition displayed sustained growth with certain area as center. The product comprised Ca, P, O, C elements without Al, V. X-ray diffraction and infrared spectroscopy analysis revealed that the main component of the coating was hydroxyapatite. CONCLUSION: The 3-folds SBF can accelerate the deposition as well as shorten formation duration of hydroxyapatite coating on Ti-6A1-4V alloy.
4.Effect of fibrocystin on the proliferation of kidney cyst epithelial cells in autosomal recessive polycystic kidney disease

Objective To explore whether the inhibited expression of fibrocystin by RNA interference can increase epidermal growth factor (EGF)-induced cell proliferation and its possible mechanism . Methods A stable PKHD1-silenced HEK 293 cell line was established . Cell proliferation rate, intracellular Ca2+ concentration and extracellular signal-reguhted kinase 1/2(ERK1/2) activity were assessed after treatment with EGF, verapamil and Bay K8644 . Results The proliferation rate of PKHD1-silenced HEK-293 cells was found to be significantly higher after EGF stimulation compared to the control HEK 293 cell (231 .5% vs 152 .8%, P<0 .01) . PKHD1-silencing lowered the intracellular Ca2+ concentration and caused EGF-induced ERK1/2 overactivation in the cells(P<0 .01 ) . When cells were treated with verapamil for 4 hours to lower the intracellular Ca2+ concentration, the cell proliferation rate was significantly increased after 20 ng EGF for 24 hours . The verapamil treatment increased the level of activated ERK1/2 in EGF-treated cells . An increase of intracellular Ca2 + in PKHD1-silenced ceils repressed the EGF-dependent ERK1/2 activation and the hyperproliferative response to EGF stimulation . Conclusions Inhibition of fibrocystin can cause EGF-induced excessive proliferation through decreasing intracellular Ca2+ resulting in EGF-induced ERK1/2 activation . The loss of fibrocystin may lead to abnormal proliferation in kidney epithelial cells and cyst formation in ARPKD through modulation of intracellular Ca2+ concentration .
5.Precocious puberty girls relationship between depression and E2, FSH, LH levels

56 girls with central precocious puberty were divided into the depression group and non-depression group according to the short mood and feelings questionnaire;then the levels of basic estradiol ( E2 ) , follicle stimu-lating hormone ( FSH ) and luteinizing hormone ( LH ) at various time points were compared between the two groups. The E2 level of the non-depression group was higher compared with the depression group ( P <0.01 ) . However, the significant difference of FSH and LH between the two groups was not discovered (P>0.05). The re-sults above demonstrated a negative correlation between depression and the E2 level of precocious puberty girls.
6.Risk factor analysis of antineutrophil cytoplasmic antibody-associated systemic vasculitis with pulmonary infection

Objective To analyze the risk factors of ANCA-associated systemic vasculitis with pulmonary infection to improve the alertness of clinicians to imprave the survival of patients.Methods Seventyfive AASV patients in Shanghai Renji Hospital from January 2010 to June 2013 were selected,and logistic regression models was applied to analyze the correlation.Results Of 75 AASV patients,39 (52%) cases were males,36(48%) cases were females,the average ages were (56±19),56(75%) cases were MPA,17(23%) cases were WG and 2(3%) cases were CSS.42(56%) cases were diagnosed to have pulmonary infection.Among the patients with pulmonary infection,23 (55%) were males,19 (45%) were females,24 (57%) were older than 60 years old,17(12%) had WG,37(88%) had MPA,and 8(19%) died.Meanwhile,11(26%) had bacterial infection,2(5%) had tuberculosis,2(5%) had fungi infection,and 27(64%) had mixed infections respectively.8(19%) cases died.Multivariate logistic regression analysis showed that those who were older than 60 years (OR=15.13,95%CI 2.16-106.04,P<0.01),who were diagnosed as MPA (OR=9.00,95%CI 1.37-59.14,P<0.05),who had hypoproteinemia (OR=17.61,95%CI 3.14-98.74,P<0.01) and interstitial lung disease (OR=11.83,95%CI 2.19-63.85,P<0.01) were more likely to complicate with pulmonary infection.Conclusion Those who are elders,diagnosed as MPA,with hypoproteinemia and interstitial lung disease have higher risk to have pulmonary infection among AASV patients,which underscores that clinicians should pay more attention to this complication of AAV patients.
7.Effects of Qichu Fujin Recipe on regeneration and repair of injured sciatic nerve in rats.

Objective: To explore the effects of Qichu Fujin Recipe (QCFJR), a compound traditional Chinese medicine, in repairing sciatic nerve injury in rats. Methods: A total of 60 male Sprague-Dawley (SD) rats were randomly divided into four groups: normal control group, untreated group, mecobalamin group and QCFJR group. Except the normal control group, sciatic nerve injury was induced by crushing of left sciatic nerve. Rats in the mecobalamin group were intragastrically administered with mecobalamin solutions 150 mug/(kg.d), and rats in the QCFJR group were intragastrically administered with QCFJR 35.2 g/(kg.d), while rats in the untreated group were intragastrically administered with normal saline (0.5 mL/d), once a day for 4 weeks respectively. Sciatic function index (SFI) was determined by walking tract analysis 1 week and 2 and 4 weeks after crushing. Five rats in each group were sacrificed for histological observation at the different time points. The remnant rate of gastrocnemius wet weight and the diameter of gastrocnemius cells were calculated. Expression of S-100 protein in the distal stump of injured nerve was observed by using immunohistochemical method. Distal injured sciatic nerves were determined with toluidine blue staining and observed under a light microscope 1 week and 2 and 4 weeks after operation. Diameter of axon and depth of myelin sheath were calculated by an image analysis system. Ultrastructure of nerve fibers was determined with uranyl acetate and lead citrate staining and observed under an electron microscope. Results: Compared with the normal control group, SFI, remnant rate of gastrocnemius wet weight, diameter of gastrocnemius cells, expression of S-100, diameter of axon and depth of myelin sheath in the mecobalamin group and the QCFJR group were significantly decreased at different time points (P<0.05), superior to those of the untreated group (P<0.05, P<0.01). Except the remnant rate of gastrocnemius wet weight and the diameter of gastrocnemius cells, there were no significant differences in other indexes between the mecobalamin group and the QCFJR group 1 week and 2 and 4 weeks after crushing (P>0.05). Conclusion: QCFJR can stimulate the regeneration and repair of nerve fiber and delay the skeletal muscle atrophy after sciatic nerve injury in rats.
8.Meta analysis of BISAP score system in predicting the severity of acute pancreatitis

Objective To investigate the value of BISAP scoring system in predicting the severity of acute pancreatitis.Methods Medline,EMBASE,Science Direct,Springer link,CBM,Cnki,Wan fang and VIP database were retrieved by computer between January 2000 and March 2013,and articles of BISAP score system in predicting the acute pancreatitis were collected according to strict inclusion criteria.Quality assessment was made by QUADAS scale.Meta-Disc1.4 software was used to analyze the heterogeneity of included articles and perform quantitative synthesis,as well as calculate the pooled sensitivity and specificity,positive likelihood ratio,negative likelihood ratio,and draw the ROC curve,and the results were presented with 95% CI.Results A total of 11 articles were included,and there were 7 Chinese articles and 4 English articles.According to QUADAS scale,there were 4 articles of A grade,5 articles of B grade,2 articles of C grade.There were 6 studies using 2 as BISAP cut-off value,and 9 articles using 3 as cut-off value,while 4 studies using 2 cut-off values.Pooled analysis showed diagnostic odds ratio of 8.03 (95 % CI 5.66-11.38) when cut-off value was 2,and diagnostic odds ratio of 7.49 (95% CI 5.35-10.49) when cut-off value was 3.There were moderate heterogeneity in both groups (I2 =63.3 %,P =0.018; I2 =56.1%,P =0.019).When BISAP cut-off value was 2,the pooled analysis of sensitivity,specificity,positive likelihood ratio,negative likelihood ratio and area under curve (AUC) were 59% (95% CI56%-63%),82% (95% CI 80%-83%),3.50 (95% CI2.96-4.14),0.45 (95% CI0.36-0.56) and0.82; and when BISAP cut-off value was 3,the corresponding values were 44% (95% CI41%-47%),90% (95% CI 89%-91%),4.59 (95% CI 3.31-6.37),0.64 (95% CI 0.61-0.68) and 0.64.The former had a high sensitivity,low specificity and large AUC,while the latter had a low sensitivity,high specificity and small AUC.Conclusions The best cut-off value of BISAP in predicting SAP is 2.When cut-off value is 2,the misdiagnosis rate is low,and the false positive rate is in the acceptable range,which is suitable for clinical application.
9.Design of integrative confocal-microscopic hard laryngoscope system

Objective The existing laryngoscope can not be used to observe the ultrastructure of cells.As a result,doctors always have to conduct biopsies on some patients who can not be diagnosed.The timeconsuming procedure of biopsy may be detrimental to the timely diagnosis and treatment.To solve this problem,we aimed to design the integrative confocal-microscopic hard laryngoscope system (ICMHL).Methods ICMHL system integrated the technique of existing laryngoscope and confocal microscopy,which allows doctors to observe the macrostructure and microstructure of lesions at the same time. Results With the effect of fluorescein sodium,ICMHL system could record the dynamic changes of cells at a rate of 30 fps,and the confocal-microscopic host machine could photograph images at a rate of 230 fps and diagnose invisible minute lesions.Conclusion ICMHL system provides a new,real-time approach for clinic biopsy.
10.Research progress of liver hanging maneuver

Liver hanging maneuver was firstly introduced by Belghiti et al in 2001,since then,the new technique has aroused widespread interest.The fatal weakness of liver hanging maneuver is bleeding resulting from blind dissection of the anterior plane of the inferior vena cava.In this review,the anatomic basis of retrohepatic tunnel and the surgical techniques were summarized to provide surgeons with suggestions in hepatectomy,liver transplantation or operations for hepatic trauma,so as to reduce the incidence of complications and raise the safety and success rate.
