1.The prognosis of acinetobacter baumannii infection associated with drug resistance in the intensive care unit

Objective To research the association between the prognosis of Acinetobacter baumannii infection and drug resistance among patients in the intensive care unit( ICU). Methods Eighty-four patients with Acine tobacter baumannii infection from February 2003 to April 2009 in Peking University Shenzhen Hospital ICU were retrospectively analyzed and divided into alive group(59 patients) and dead group(25 patients) by using 30-day mortality. The impact of drug resistant Acinetobacter baumannii on mortality was assessed by binary logistic regression. Results The mortality of patients with Acinetobacter baumannii infection was related to age, haematological malignancy, the Chanson Comorbidity Index, APACHEII score, multidrug-resistant Acinetobacter baumannii ( MDRAB) , carbapenem-resistant Acinetobacter baumannii (CRAB ) , septic shock and antimicrobial inappropriate therapy by univariate analysis( P < 0.05). Binary logistic regression revealed that CRAB( OR = 3. 360,95%CI: 1. 563 ~7.223,P< 0.05)and the Charison Comorbidity Index( OR = 1. 633,95% CI: 1. 201 ~2. 221 ,P <0.05) were independent risk factors. Conclusions The prognosis of Acinetobacter baumannii infection was closely correlated with drug resistance. The patients with CRAB infection had a higher mortality.
2.Transition from endocapillary proliferative glomerulonephritis to membrane proliferative glomerulonephritis:a case report

ObjectiveTo investigate the mechanism, diagnosis and treatment of membrane proliferative glome-rulo-nephritis (MPGN) transitioned from endocapillary proliferative glomerulonephritis (EnPGN).Methods The clinical data and the results of pathological examination of one case of MPGN transitioned from EnPGN were retrospectively analyzed.Results The child was presented with proteinuria, microscopic hematuria, and persistent low level of complement C3. The type of renal pathology was transitioned from EnPGN to MPGN. Complete remission was achieved in this child with the treatment of oral prednisolone and tacrolimus, but the level of plasma complement C3 remained low.Conclusions The type of renal pathology in children with persistent low level of complement C3 could make a transition, and the early diagnosis, timely and effective treat-ment are important.

Anti-retroviral (ARV) drugs have been successfully used for HIV/AIDS therapy,and their uses in pre-exposure prophylaxis (PrEP) for HIV infection is becoming increasingly important,and is drawing more attentions.Reports on seven large scale,multicentric clinical trials provide significant clinical data for further study and implementation of PrEP.This article reviews the current situation,necessity,questions,problems and prospects of PrEP for HIV infection.
4.Anterior cruciate ligament reconstruction with harmstring tendon transplantation

The reconstruction with harmstring tendons is the commonly therapic method for the anterior cruciate ligament (ACL) injury. The graft-implant fixation is important for the initial biomechanics stability and the graft-tunnel healing. The methods of fixation are variable, which can be classified by direct fixation and indirect fixation or by in-tunnel fixation and non-in-tunnel fixation. The interface screw, endobutton, cross pin and tendon knot are most commonly used in clinic. No fixation method is superior to all others, for every method has advantages and disadvantages respectively.
5.Effects of Allopurinol on Inhalation Pulmonary Injury in Rats

Six hours after inhalation pulmonary injury,the pulmonary microvascular permeability (PMVP),pulmonary malondialdehyde (MDA) level,superoxide dismu-tase activity (SOD),the contents of protein and leucocytes in broncho-alveolar lavage fluid(BALF),and serum MDA level were determined in rats pretreated with allopurinol.In addition,the pathological changes of the lung tissues were also studied.It was found that early administration of allopurinol resulted in marked reduction of PMVP,and the protein content and leucocyte count in BALF.His-topathological examinations revealed that allopurinol could reduce the leucocyte accumulation in pulmonary vessels,interstitial leucocyte infiltration,interstitial and alveolar edema and hemorrhage,and vascular congestion.The serum and pulmonary levels of MDA also markedly decreased.The changes of pulmonary level of MDA were parallel to those of PMVP,and the protein content and leucocyte count in BALF in time course and in degree.The results listed here imply that the mechanism of the therapeutic effects of allopurinol on inhalation pulmonary injury is to reduce the production of superoxide anions through the inhibition of xanthine oxidase,in turn to inhibit the peroxidation of pulmonary lipids to alleviate pulmonary damage.

8.Esthetic evaluation in restoring anterior teeth with porcelain fused to metal crown

Objective To compare and analyse the esthetic effect of four kinds of the porcelain fused to metal crowns (PFMs) in restoring anterior teeth. Methods Based on single standard, a total of 371 anterior teeth from 110 patients were restored by PFM, in which 206 teeth of 59 patients were restored with Ni-Cr alloy PFM, 105 teeth of 29 patients with full circular procelain shoulder PFM, 35 teeth of 16 patients with titanium metal PFM, and 25 teeth of 6 patients with noble metal PFM. The esthetic effect was examined and analysed by single-blind method in one week and after one year. Results Between three days and one week, there was no difference among four PFMs in shapes and colors, which corresponded with adjacent teeth. The edge suitability was good and no significance was found in them. After one year, there was no difference of four PFMs in shapes and colors, which corresponded with adjacent teeth. The edge suitability of noble metal PFM was better than the other three, and the difference was statistically significant. Conclusion The esthetic effect is the best when noble metal PFM is used to restore anterior teeth and better with full circular procelain shoulder PFM.

10.The expression of focal adhesion kinase and Ki67 in invasive breast ductal carcinoma and their clinical significance

Objective:To investigate the expression of focal adhesion kinase(FAK) and Ki67 in the invasive breast ductal carcinoma tissues, seeking their relationship with tumor invasion and metastasis and to study the mechanisms for cell signaling transduction mediated by FAK in the carcinogenesis and progress of breast carcinoma.Methods:Tissues were obtained from 88 patients with invasive breast ductal carcinoma and 25 with breast ductal epithelial benign proliferative disease.
