1.Influence of operative effect from postoperative early complication in elderly patients with hip fractures

[Objective]To analyze the influence of operation effect from complication of cardiac,pulmanary and hematology system in elder patients with hip fracture.[Method]Fifty-nine cases with hip fracture from Jan.2001 to Jul.2005 were admitted in the department.Thirty-nine patients were acomplicated with medical diseases,most of them were hypertension,chronic bronchitis and diabetes mellitus.Among these patients,56 cases were operated.[Result]Fifty-two cases were operated successfully.One died of pulmonary interstitial fibrosis.The incidence of postoperative complications,including infection of pulmonary,urinary and wound,bedsore,irritable allergic!ulcer and lower extremities venous thrombosis were 19%,10%,8%,5%,2%and 4% respectively.[Conclusion]In elderly patients with hip fracture,the incidence of pulmonary infection is the highest.The lower extremities venous thrombosis is the most serious complication.So it is helpful to do quadriceps femories function exercise.The operative method is also a very important cause to define the postoperative effect.
2.How far away is the stem cell therapy from the glaucoma management.A ten-year review.

This mini review presents a summary of the stem ceil studies associating with the glaucoma treatment.The sources of stem cell.the means of cell delivery,and the restoration of structure and function of the optic nerve were discussed.Based on the current research,the potential benefit and risk of stem cell therapy for the glaucoma patients was evaluated.Some suggestions were given for the future research on these issues.
3.Application of gene-sequencing technique in the monitoring of bacterial infectious diseases

Gene-sequencing technique is the most important techniques in the research of modern molecular biology. After decades of development, it has been widely used in disease detection and monitoring due to its high throughput, fast speed and high accuracy. Therefore, the principle and development process of gene-sequencing technique were summarized in this paper, and the principle of gene-sequencing technique of the first generation, the second generation and the third generation and all kind of gene-sequencing equipment based on various principle of technique were systematic introduced in this paper. Besides, the paper also explored and discussed the selection and application of various kind of gene-sequencing equipment in the emergency and early warning monitoring for bacterial infectious disease. On the other hand, the paper also summarized and prospected the application and development of gene-sequencing technique in biology and biomedical research.
5.Application of magnetic resonance imaging diffusion weighted imaging in lung cancer

Magnetic resonance imaging diffusion weighted imaging (DWI) as a non-radiation and noninvasive examination method,has great help in diagnosis and staging of lung cancer,and the apparent diffusion coefficient (ADC) determination has demonstrated clinical application value in the therapeutic assessment.As an essential complement of CT,MRI-CT imaging infusion improves the accuracy of the target volume sketching in the radiotherapy of lung cancer.
6.Prognostic evaluation of microRNA-192 levels in diabetic nephropathy patients

Objective To explore the relevance of the microRNA-192 (miR-192) levels and prognosis in patients with diabetic nephropathy (DN).Methods A total of 59 cases with duration of type 2 diabetes mellitus (T2DM) ≥10 years from January 2010 to July 2011 were retrospectively admitted.The miR-192 levels were determined after admission,and other relevant clinical data were collected.And the patients were followed-up for 24 months.According to the patient's prognosis,the patients were divided into DN group(20 cases) and non-DN group(39 cases),and the relevant information was analyzed in two groups.Results The miR-192 levels in DN group were lower than those in non-DN group[(4.47 ± 2.27) μ g/L vs.(7.28 ± 2.39) μ g/L],and there was significant difference (P < 0.05).Multivariate Logistic regression analysis showed that hypertension history,smoking history,high glycosylated hemoglobin,low fasting C-peptide,low 24 h urine protein,low miR-192 level,high body mass index was the independent risk factors of prognosis of T2DM (P < 0.05).The area under the receiver operating characteristic (ROC) curve was 0.697 in miR-192 levels individual predicting progression to DN stage,0.823 in six others without miR-192.After joining seven indicators forecast area under the curve increased significantly(0.856,95% CI:0.832-0.904).The miR-192 levels 5.14 μ g/L was the best predictive value to the critical value of community,and the patients were divided into miR-192 < 5.14 μ g/L group (28 cases) and miR-192 ≥ 5.14 μ g/L group (31 cases).The number of progression to DN within 24 months in miR-192 < 5.14 μg/L group was more than that in miR-192 ≥5.14 μ g/L group (16/28 vs.4/31,P < 0.05).Conclusions A reduced level of miR-192 is an independent risk factor that can lead to poor prognosis in patients with DN,and can predict adverse outcomes in patients with T2DM effectively.
7.Cytotoxicity evaluation of nano-Ag-SiO2urethral catheter for external urethral mucosa cells

BACKGROUND:Nano-Ag is increasingly used as antibacterial materials in medical products due to its excelent antibacterial activity. Antibacterial activated nano-Ag-SiO2 OBJECTIVE: To evaluate the biosecurity of nano-Ag-SiO urethral catheter can reduce the incidence of catheter-associated urinarytract infections obviously. As more and more nano-Ag medical products have been developed, their biological security is becoming a focused issue. 2 urethral catheter through comparing the active influence of nano-Ag-SiO2 METHODS: Mechanical separation and enzymatic digestion were used to isolate and culture urothelial mucosa of benign prostatic hyperplasia patients to make cellsuspension after proliferationin vitro. The medium was replaced by nano-Ag-SiO urethral catheter and ordinary urethral catheter extracts onin vitro cels of human prostatic urethra epithelial cels in different periods. 2 and general catheter tube extracts, respectively. MTT assay was performed to quantify thein vitro cytotoxicity of two extracts on human urethra epithelial cels which were cultured for 2, 5 and 7 days. Absorbance values of nano-Ag-SiO2 RESULTS AND CONCLUSION:The cytotoxicity of extract of nano-Ag-SiO and general urinary catheter groups were measured by enzyme-linked immunosorbent assay to calculate the relative growth rate, and toxicity evaluation was compared and evaluated. Under an inverted microscope, cellmorphology, growth and proliferation were observed. 2 catheter was 1 grade, and that of general medical catheter extract was 0 to 1 grade. There was no statistical difference in group effect of relative proliferation rate (F=0.544,P=0.475); there was statistical difference in time effect of relative proliferation rate (F=3.031,P=0.086); no statistical difference was found in the interaction between group and time (F=0.130, P=0.879). These findings indicate that nano-Ag-SiO2 catheter has no or low cytotoxicity effect on cellgrowth and proliferation, which is consistent with the biological evaluation standards of medical devices.
8.Rehabilitation and functional prognosis of acute brain infarct complicated with depression

Objective To observe rehabilitation and functional prognosis of patients suffered from brain infarction of acute phase complicated by depression. Method We selected 38 cases of acute brain infarct complicated with depression disorder as depression group, 40 cases of non- depression patient admitted at the same period were selected as non depression group.Antidepressants drugs administration and rehabilitation therapy were performed.We evaluated patients according to functional independence measure(FIM)of Chinese edition. HAMD grading comparison was carried out in depression group. Result Before treatment,two groups score nearly the same,compared with pretreatment. After treatment,patients in depression group and non depression group showed significant improvement esp non- depression group (P<0.01).11 cases with severe depression in the depression showed no changes in FIM score before and after treatment. Conclusion Compared with non- depression patients,functional recovery of patients with depression following brain infarction of acute stage is much slower,especially for patients with severe depression.
9.Comparison of the effect of different methods of anesthesia on cerebral autoregulation in patients undergoing neurosurgery

Objective To compare the effect of different methods of anesthesia on cerebral autoregulation in patients undergoing neurosurgery.Methods Sixty-nine ASA Ⅱ orⅢ patients with brain tumor, aged 23-62 yr, scheduled for neurosurgery under general anesthesia, were randomly divided into 3 groups ( n = 23 each) : propofol-remifentanil group (group PR), sevoflurane-remifentanil group (group SR) and propofol-sevoflurane-remifentanil group (group PSR) . Anesthesia was induced with target-controlled infusion (TCI) of propofol (target plasma concentration3 μg/ml, PR and PSR groups) or inhalation of 8% sevoflurane (group SR) and iv injection of remifentanil 1 mg/kg and atracurium 0.5 mg/kg. The patients were mechanically ventilated after tracheal intubation. PETCO2 was maintained at 32-35 mm Hg. Anesthesia was maintained with TCI of propofol (target plasma concentration 2.0-3.5 μg/ml) in group PR, with inhalation of 1.5%-2.5% sevoflurane in group SR, with TCI of propofol (target plasma concentration 1.5-3.0 μg/ml) and inhalation of 1% sevoflurane in group PSR, and with TCI of remifentanil (target plasma concentration 2.0-4.5 ng/ml) and iv infusion of atracurium at 6 μg · kg-1 · min-1 in all groups. Auditory evoked potential index was maintained between 40-45. The middle cerebral artery time-average peak flow velocity was recorded before induction (baseline) , immediately after intubation, immediately before craniotomy and at the beginning of skin suture. The unilateral carotid artery was compressed for 7 s at the corresponding time points mentioned above. The transient hyperemic response ratio (THRR) was calculated to reflect cerebral autoregulation. Results Compared with the baseline value at T0, THRR was significantly increased at T2in group PR and decreased at T2,3 in group SR (P <0.05) ,while no significant change was found in THRR at T1-3in group PSR (P >0.05). The THRR was significantly lower in SR and PSR groups than in group PR, and higher in group PSR than in group SR ( P < 0.05). Conclusion Propofol-remifentanil anesthesia can improve cerebral autoregulation, sevoflurane-remifentanil anesthesia can reduce cerebral autoregulation, and propofol-sevofluraneremifentanil anesthesia exerts no effect on cerebral autoregulation in patients undergoing neurosurgery.
10.Optimal fields and dose of postoperative radiation in esophageal carcinoma

Among esophageal cancer patients who had undergone radical surgery, the overall 5-year survival rate ranges from 20% to 30%. Postoperative radiotherapy is important adjunctive and remedial treatment. However, currently, there is no clear consensus on the indication of postoperative radiotherapy, the optimal radiation fields and dose. We review and summarize the relevant studies for clinical references.
