1.Efficacy Observation of Zhuanggu Huoxue Recipe for Necrosis of Femoral Head in Patients with Osteoporotic Femoral Neck Fracture

OBJECTIVE:To investigate the clinical efficacy of Zhuanggu huoxue recipe for necrosis of femoral head in patients with osteoporotic femoral neck fracture. METHODS:305 patients with osteoporotic femoral neck fracture randomized into therapy group and control group. Both groups were given internal fixation therapy and continuous traction treatment for skin. Therapy group were given Zhuanggu huoxue decoction (p.o. b.i.d) for 12 weeks. Control group were given ossotide and puerarin. At 1st month,2nd month,4th month,6th month,12th month,18th month after operation,roentgenograms of bone in femoral lateral position were taken to observe the fracture healing and the occurrence of femoral head necrosis. At 4th month,6th month,8th month after radiotherapy,MRI of double lateral hip was taken to observe the occurrence of femoral head necrosis. Objective To study the application value of cytological examination and biopsy for the transbronchial needle aspiration (TBNA) in the diagnosis of cancer. Methods 101 patients with lung disease by CT that need TBNA examination to make final diagnosis were selected. They were executed with routine ex-amination of TBNA. And then pathologic biopsy and cytological examination were applied on the collection sam-pies. Results 86 patients among the 96 subjects were finally diagnosed by TBNA. So the ratio of final diagno-sis was 94. 5%. And the positive ratios about transbronchial needle aspiration and ring clamp biopsy, also brush biopsy, were 71.4% 、75.8%、 61.5%. Only 5 patients were histodiagnosed by just transbrenchial needle aspiration. Conclusion TBNA plays an important role on the diagnosis of lung tumor with different types. And the combination of three methods can improve the diagnosis pro-gressively.
3.Efficacy of transurenthral resection of muscle-invasive bladder cancer in the elderly

Objective To investigate the clinical effects of transurenthral resection of muscleinvasive bladder tumor (TURBT) in senile. Methods This study was clinical prospective random research.46 senile cases with muscle-invasive bladder cancer treated by TURBT was divided into 2 groups:control group and observed group.The observed group was treated with TURBT combined with intravesical instillation of pirarubi 30 mg immediately after TURBT,the bladder wall around tumor was resected thoroughly.The control group underwent radical cystectomy and pelvic lymph node dissection. Results The clinical improvement rate was higher in observed group than control group (85.7% s.63.6%,X2 =5.3008,P=0.021).There was not difference in 3 years recurrence rate between observed group(25.0%) and control group(13.6%) (x2 =S.3008,P>0.05).The 1 year survival rate were 95.8%,90.9% and 3 years survival rate were 79.2%,72.7% in observed group and control group,respectively,and there were no differences between the two groups(X2 =0.262,P=0.609). Conclusions TURBT combined with intravesical instillation of pirarubi can enhance clinical effect and reduce recurrence rate in the treatment of the elderly with muscle-invasive bladder cancer.
5.Dimethyl sulfoxide acts as a protective agent to perfuse rabbit amputated limbs:the relative recovery of local drug concentrations

BACKGROUND:The effect of dimethyl sulfoxide cryoprotectants has been got a lot of verification in the low-temperature medical applications. But there is no literature addressing microdialysis detection of dimethyl sulfoxide cryoprotectants. OBJECTIVE:To investigate the microdialysis relative recovery of different concentrations of dimethyl sulfoxide cryoprotectants used for limb reattachment. METHODS:In vitro linear probe relative recovery of different concentrations of dimethyl sulfoxide (2%, 5%, 8%) was detected by retrodialysis gain and loss method. The correction in vivo experiment was done to estimate dimethyl sulfoxide relative recovery in rabbit amputated limbs. RESULTS AND CONCLUSION:The relative recoveries of different concentrations of dimethyl sulfoxide (2%, 5%, 8%) were (49.49±3.56)%, (46.30±1.48)%, (52.66±2.54)%using retrodialysis gain method and (50.99±6.89)%, (43.86±1.35)%, (50.67±0.75)%using retrodialysis loss method. The average recoveries were (49.48±3.18)%and (48.51±4.03)%, respectively. There was no difference in the relative recovery of dimethyl sulfoxide detected using two methods. The change of dimethyl sulfoxide concentration could not influence the retrodialysis gain and loss method calibration results. The recovery was (15.45±4.8)%in vivo. These findings indicate that the microdialysis technology is suitable for dimethyl sulfoxide sampling in vivo that has no obvious influence on the relative recovery.
6.Peritonsillar peripheral space infections caused by acute tonsillitis: pathogen characteristic, influencing risk factors and prevention

Objective To research the pathogen characteristics and influencing factors for peritonsillar peripheral space infections in acute tonsillitis patients,so as to take measure to reduce the peritonsillar peripheral space infections.Methods 898 acute tonsillitis patients who received treatment were chosen as study subjects.Bacteria analyzer was adopted to identify pathogens.Chi-square test was used to analyze the univariate factor,and logistic regression analysis was conducted to different factors.Results 228 patients were involved the complications of peritonsillar peripheral space infections(cellulitis in 52 cases or abscesses in 176 cases),and the infection rate was 25.4%.Bacterial cultivation performed in 177 patients and positive results were detected in 107.The top three infections bacteria were Staphylococcus aureus (21.5%),Streptococcus viridians (19.6%) and Klebsiella pneumoniae (15.9%).Univariate factor analysis found that the history diabetes and peritonsillar peripheral space infections,age,acute phase of smoking and drinking,fatigue,nutritional status,class of antibiotic use,tonsillitis type,educational level and the long first diagnosis time were risk factors for peritonsillar peripheral space infections,and the differences were significant (x2 =39.851,8.818,89.041,87.266,30.417,21.499,13.472,64.548,12.223,62.871,70.749,all P < 0.05).Logistic regression analysis found that acute phase of smoking and drinking,age,fatigue,tonsillitis type and the long first diagnosis time were independent risk factors for peritonsillar peripheral space infections patients with acute tonsillitis (OR =2.873,5.311,5.876,0.290,2.244,2.369,all P < 0.05).Nutritional status and educational level had beneficial effect to peritonsillar infections event (OR =0.290,0.461,all P < 0.05).Conclusion Acute tonsillitis is easy to cause peritonsillar peripheral space infection,it should be paid much attention and strengthened prevention.The pathogens causing peritonsillar peripheral space infections in acute tonsillitis is still mainly Staphylococcus aureus.After the analysis of related factors,the certain measures should be taken to reduce the complications rate of peritonsillar peripheral space infections in acute tonsillitis.

Objective To investigate the diagnostic value of the ratio of % Micro to % Hypo in the diagnosis of three kinds of common thalassemia and iron‐deficiency anemia (IDA ) .Methods Forty‐nine cases of IDA ,24 cases of mildα‐thalassemia ,24 cases of mildβ‐thalassemia ,24 cases of silent α‐thalassemia and 120 individuals undergoing healthy physical examination were selected as the research subjects and divided into 6 groups:normal group ,IDA group ,mildα‐thalassemia group ,mildβ‐thalassemia group ,silentα‐thalassemia and mild thalassemia group(in duding mild α‐thalassemia group and mild β‐thalassemia group) .The % Micro and %Hypo were deteced in each group and their ratio was calculated .The results were performed the analysis and comparison .Results The % Micro/% Hypo ratio had statistical difference between the mild thalassemia group and IDA group ,between the mild thalasse‐mia and IDA group with the normal group(P<0 .01) .The% Micro/% Hypo ratio had no statistical difference between the silent α‐thalassemia group and normal group ,and between the mild α‐thalassemia group and mildβ‐thalassemia group (P>0 .05) .With the% Micro/% Hypo ratio of 0 .9 as the discriminant value to diagnose mild thalassemia and IDA ,its sensitivity ,specificity and accuracy for diagnosing mild thalassemia were 91 .67% ,91 .89% and 91 .72% respectively ,the sensitivity ,specificity and accuracy in diagno‐sing IDA were 91 .94% ,91 .25% and 91 .18% respectively .Conclusion The ratio of % Micro/% Hypo has good differential diagno‐sis and assisted diagnosis screening value ,but has little value for diagnosing silent α‐thalassemia .
8.Expression of p53 in breast cancer and regulatory effects of 5-aza-2′-deoxycytidine on p53

Objective Tumor suppressor gene p53 can inhibit tumor cell growth, arrest cell cycle, and promote apoptosis.Howev-er, the effects of p53 on the pathogenesis of breast cancer have not been fully elucidated.The aim of this study was to explore the expression of p53 protein and the correlation with clinical pathologic features in breast cancer.Furthermore, the regulatory effects of 5-aza-2′-deoxycyti-dine on p53 in breast cancer cell line were also studied. Methods The expression of p53 protein in 80 cases of breast cancer and normal and adjacent tissue were determined by the immunohistochemical staining .The expressions of p53 mRNA and p53 protein in breast cancer cell line were determined by RT-PCR and Western blotting. Results The positive rate of p53 in breast cancer (41.25%) was higher than that in the normal and adjacent tissue (22.5%) (P<0.01).The expression of p53 was not significantly correlated with age, grade, stage and lymph node metastasis (P>0.05).The low expression of p53 both in mRNA and in protein levels were found in breast cancer cell line of MCF-7.The expres-sion of p53 increased after 5-aza-2′-deoxycytidine administration . Conclusion p53 is highly expressed in breast cancer , which may play an im-portant role in the development and progression of breast cancer. 5-aza-2′-deoxycytidine, up-regulating the p53 expression in breast cancer cell line, which provides the evidents for the development of therapeutic drugs for the patients with low expression of p53 breast cancer.
9.Association of insulin resistance with common carotid arterial stiffness in hemodialysis patients

Objective To investigate the association between arterial stiffness of the common carotid artery(CCA)and insulin resistance in hemodialysis patients. Methods Arterial stiffness index β of CCA was evaluated by an ultrasonic phase-lock Echo-tracking system in 80stable non-diabetic hemodialysis patients.Insulin resistance was detected by the homeostasis model assessment method(HOMA-IR).Plasma hemoglobin,serum albumin,total cholesterol,high density lipoprotein,low density lipoprotein,triglyceride,lipoprotein(a),ApoA1,ApoB,CRP,calcium,phosphorus and creatinine were determined by standard methods. Results The stiffness index β was 11.41±4.13 in patients with previous cardiovascular disease(CVD)and 9.75±3.63 in those without CVD(P<0.05).The stiffness index β was positively correlated with HOMA-IR(r=0.321,P<0.01),as well as with age(r=0.376,P<0.01),pulse pressure(r=0.267,P<0.05),and duration of hemodialysis(r=0.219,P<0.05).In stepwise multiple regression analysis,HOMA-IR(β=0.228,P<0.05)and age(β=0.308,P<0.01)were identified as significant independent variables for stiffness index β of CCA. Conclusions Insulin resistance is associated with aaefial stiffness in nondiabetic hemodialysis patients.The increased arterial stiffness may be the link between insulin resistance and cardiovascular morbidity as well as mortality in hemodialysis patients.
10.Clinical analysis of ameloblastoma in nasal cavity and paranasal sinuses

OBJECTIVE To investigate the clinical features and therapeutic methods of ameloblastoma in nasal cavity and paranasal sinuses. METHODS The clinical data of 6 cases of ameloblastoma in nasal cavity and paranasal sinuses diagnosed by pathology were analyzed retrospectively, and the relevant literature were reviewed. RESULTS All of the 6 cases received surgical treatment. The follow-up period was 15 months to 10 years after operation.3 patients recurred until the last follow up. Among them, 2 cases recurred once, 1 case recurred 2 times, no patient died of this tumor. CONCLUSION The incidence of ameloblastoma in nasal cavity and paranasal sinuses is low and the symptoms are nonspecific, the diagnosis mainly depends on pathology and immunohistochemistry. CT and MRI examinations of ameloblastoma are helpful in diagnosis and differential diagnosis of the lesion. The main treatment is surgical resection of the lesion, the recurrence rate is higher. It has malignant tendency, and long-term follow-up is needed.
