1.USE OF AORTIC VALVED HOMOGRAFT IN COMPLEX CONGENITAL HERAT DISEASE
Medical Journal of Chinese People's Liberation Army 1983;0(02):-
From April 1990 to February 1991, 7 cases of complex congenital cardiac malformations were treated with transplantation of aortic valved homograft sterilized and preserved in our department. Conduit-constructing from right ventricle to pulmonary arteries and from left ventricles to pulmonary artries was performed in 2 and 5 cases repectiredly. When the bore between aortic homograft and artificial blood vessel was 2-4mm, good result could be achieved in clinical hemodynamics and pressure difference was seldom found. No death occurred in these cases. The follow-up with color Doppler echocardiography and cineangiography proved that anastomosis was unobstracted, extracardiac conduits had no distortion and compression, and no stenosis and incomptence were found in homograft valves.

Objective:To explore the effect of group painting art therapy on negative symptoms, the cognitive status and social function of patients with schizophrenia during rehabilitation. Methods:A total of 114 patients with schizophrenia recovered from May 2018 to May 2019 in Zhangjiakou Shalingzi Hospital. They were divided into two groups according to the random number table method with 57 case in each group, the control group was given routine nursing, and the observation group received group painting art therapy on the basis of routine nursing. The Positive And Negative Symptoms Scale (PANSS), MATRICS Consensus Cognitive Battery(MCCB), and Social Disability Screening Schedule (SDSS) were used to evaluate the symptoms, cognitive status, and social function improvement of the two groups before and after intervention. Results:There were no significant differences in the negative symptom scores and positive symptom scores of the PANSS between the two groups of patients before the intervention ( P>0.05). After the intervention, the scores of negative symptoms and general psychopathology were (11.83 ± 3.19), (17.30 ± 4.93) points in the observation group and (13.57 ± 2.85), (20.34 ± 5.12) points in the control group, the differences were significant ( t values were 2.789, 2.932, P<0.05). After the intervention, the 7-dimensional MCCB scores of the two groups of patients decreased, and the scores of the observation group were significantly lower than those of the control group ( t values were 2.046-3.238, P<0.05). After the intervention, the score of SDSS was (11.63±2.71) points in the observation group and (13.84 ±3.14) points in the control group, the differences were significant( t value was 3.653, P<0.01). Conclusions:The use of group painting art therapy in patients with schizophrenia during rehabilitation can promote the improvement of negative symptoms and improve the cognitive and social functions of patients.

Objective:To clarify the effect of miR-200c on the proliferation of retroperitoneal fibroblast, and analyze its molecular mechanism, which provide theoretical basis for the inhibition of retroperitoneal fibrosis. Methods: 36 cases of retroperitoneal tissues were collected, and the primary fibroblasts were cultured. The cells stimulated by 5 ng/ml transforming growth factor-β1 (TGF-β1) were used as TGF-β1 group;the cells were transfected with pcDNA3.1-miR-200c first, and then stimulated by 5 ng/ml TGF-β1 were used as miR-200c group;the cells transfected only by pcDNA 3.1 were used as pcDNA 3.1 group;the normal cultured cells were used as control group. The MTT assay, cell scratch and Transwell chamber were used to detect the proliferation and migration of fibroblast, respectively. The ELISA experiment was used to detect the content of Akt protein in the lysis liquid of each group. Results: The OD value of fibroblast proliferation in miR-200c group was obviously lower than that of TGF-β1 group (P<0.01);compared with TGF-β1 group, the cell migration rate was decreased significantly in miR-200c group (P<0.01);the content of Akt protein in miR-200c was obviously lower than that of TGF-β1 group (P<0.01). Conclusion: miR-200c inhibits the effect of TGF-β1 on the proliferation and migration of fibroblast by down-regulating Akt signaling pathway, which has important significance in inhibiting peritoneal fibrosis.

Objective To study the radiofrequency ablation (RA) and vagal denervafion (VD) in surgical treatment of long-standing atrial fibrillation (AF) associated with rheumatic heart disease (RHD).Methods Retrospective analysis the cardiac rhythm by 24-hour Holter monitoring during 5-year follow-up after total Maze procedure accompanied rheumatic mitral valve replacement.Between June 2006 and December 2007,a total of 173 consecutive patients with long-standing AF-associated RHD underwent mitral valve replacement and ablation maze procedure,92 cases had RA alone and 81 had RA + VD.Results Although Kaplan-Meier curve shows that the freedom from AF at 5 years follow-up time were similar(P =0.718),the percentage of antiarrhythmic drug therapy was significant higher in the RA group during early postoperative period(4th month,54.1% vs.34.7%,P=0.017;5th month,39.2% vs.21.3%,P=0.018;6th month,23.0% vs.10.7%,P =0.044),and the percentage of those free by AF was significant lower(6th month,82.2% vs.93.8%,P =0.023;1st year,76.1% vs.89.9%,P=0.019).Conclusion Total maze procedure with bipolar radiofrequency ablation is effective to treat longstanding AF associated with rheumatic valve disease.Vagal denervation helped to maintain stable sinus rhythm and lower antiarrhythmic drug therapy at the early stage, but there was no additional benefit after the 1 st year of follow-up,it may be caused from the reactivation of vagal plexus electrical activity.

BACKGROUND:For patients with degenerative mitral annulus dilatation complicated by atrial fibrilation, atrial fibrilation is needed to be treated concomitant with artificial valve ring implantation. Although Cox maze III procedure is the gold standard for treatment of atrial fibrilation, its safety has been questioned. OBJECTIVE:To evaluate the safety of artificial valve rings implantation and Cox Maze III procedure for treatment of degenerative mitral annulus dilatation complicated by atrial fibrilation. METHODS: A total of 43 patients with degenerative mitral annulus dilatation and atrial fibrilation were enroled and were randomly divided into test group (n=21) and control group (n=22). Patients in the test group were treated with mitral valvuloplasty and Cox Maze III procedure. Patients in the control group were only treated with mitral valvuloplasty. RESULTS AND CONCLUSION: (1) Intraoperative safety indexes: the durations of cardiopulmonary bypass and aortic clamping. and the incidence of hemoglobinuria in the test group were higher than those in the control group (P < 0.001). There was no significant difference in the urine output between these two groups. (2) Postoperative safety indexes: the incidence of atrial dysrhythmia (i.e., any atrial dysrhythmia other than atrial fibrilation), proportion of patients requiring a temporary pacemaker and hospital stays in the test group were higher than those in the control group (P < 0.05). There were no significant differences in the secondary-thoracotomy bleeding rate and incidence of stroke between these two groups. (3) Postoperative outcomes: the rates of cardioversion to achieve sinus rhythm at the day of surgery and at discharge in the test group were significantly higher than those in the control group (P < 0.001). At discharge, patients in both groups were al at NYHA functional class Ior II. Echocardiography showed trace to mild mitral regurgitation and complete closure of the tricuspid valve. There were no significant differences in the left atrial diameter, left ventricular diameter, left ventricular volume, and ejection fraction between these two groups. However, the proportions of mitral and tricuspid valve peak A-waves were significantly higher than those in the control group (P < 0.001). These results show that artificial valve ring implantation and Cox Maze III procedure is a safe and effective method for treating degenerative mitral annulus dilatation complicated by atrial fibrilation during operation.
