2.Effect of Niaoduqing granules on chronic renal failure in patients with blood lipid and blood homocysteine

Objective To observe the effect of Niaoduqing granules on chronic renal failure in patients with blood lipid and blood homocysteine.Methods Patients with chronic renal failure were divided into observation group and control group by number table method,two groups of patients were both given low salt,low fat and high quality low protein diet,controlling blood pressure and blood sugar,correcting anemia,correcting calcium and phosphorus meta-bolic disorders,correcting of acidosis,while patients with combined infection were given positive anti infection treat-ment.Observation group was treated with Niaoduqing granules on that basis.The changes of triglyceride(TG),total cholesterol( HDL-C) ,total cholesterol( CHO) ,low-density lipoprotein( LDL-C) ,and plasma homocysteine( Hcy) were observed before and after treatment in the two groups.Results After treatment,CHO,TG,HDL-C,LDL-C, Hcy in the control group were respectively (4.83 ±0.22)mmol/L,(2.21 ±0.30)mmol/L,(0.69 ±0.11)mmol/L, (4.30 ±0.32)mmol/L,(24.35 ±7.05)μmol/L,that threre was no significant changes before and after treatment.In the observation group,TG,LDL-C,Hcy were (1.38 ±0.18)mmol/L,(3.29 ±0.13)mmol/L,(14.54 ±3.56)umol/L respectively after treatment,and which were significantly lower than that before treatment ( t=1.45,2.13,4.28,all P<0.05).HDL-C (1.42 ±0.31)mmol/L,were significantly increased after treatment as compared with that before treatment (t=3.88,P<0.05).The difference of Hcy,TG,LDL-C,HDL-C and CHO after treatment was statisti-cally significantin between the two groups (t=1.34,2.08,1.10,3.88,all P<0.05).Conclusion Niaoduqing gran-ules can improve lipid metabolism disorders in patients with chronic renal failure,and can reduce the serum homocys-teine.

The efficacy of traditional Chinese medicine(TCM) is the summary for the effects of traditional Chinese medicine.The effect of traditional Chinese medical pharmacology is studying the interaction mechanism between TCM and body,using the modern technical methods and guiding by TCM theories.Most efficacies of TCM are identical with pharmacological effects.The clinical use of TCM pharmacy will give a full play of treatment only by guided with the theory of the concept of the whole and treatment based on syndrome differentiation and integrated with the efficacy of TCM and the principal compatibility of medicine.
4.The role of microglial activation in spinal cord in a rat model of persistent postoperative pain evoked by skin/muscle incision and retraction

Objective To investigate the role of microglial activation in spinal cord in a rat model of persistent postoperative pain evoked by skin/muscle incision and retraction (SMIR) .Methods Seventy-two male SD rats weighing 200-250 g in which intrathecal (IT) catheter was successfully inserted were randomly divided into 3 groups ( n = 24 each) : group sham operation; group SMIR and group SMIR + FT minocycline (a specific microglia inhibitor) . The rat model of persistent postoperative pain evoked by SMIR was established according to the method described by Flatters. Pain behavior was assessed by paw mechanical withdrawal threshold ( MWT) to von Frey filament stimulation at 1 day before (T0,baseline) and 3, 7, 12, 22 and 32 days after operation (T1-5,) . Four animals were sacrificed at each time point in each group for detection of the expression of Iba-1 (a specific marker of microglia) in the spinal dorsal horn by immunofluorescence and the microglia was counted. Results MWT was significantly decreasedat T1-4, while the expression of Iba-1 and microglia counts in the spinal dorsal horn were significantly increased at T1, 2 by SMIR in group Ⅱ. IT minocycline significantly attenuated the hyperalgesia induced by SMIR at T1-4 and decreased Iba-1 expression and microglia counts at T1,2 in group Ⅲ. Conclusion Microglial activation in the spinal cord plays an important role in the development and maintenance of SMIR-evoked persistent postoperative pain in rats.
5.Characterization of DNA Antigen from Immune Complexes Deposited in the Skin Lesions of Patients with Lupus Erythematosus

Objective To explore the characteristics of DNA antigen deposited in the skin lesions of patients with lupus erythematosus (LE). Methods Thirty-two LE skin specimens were collected. The deposited immune complexes were obtained by cryoprecipitation methods. Then the samples were digested by protease. Finally DNA was extracted with phenol and chloroform. The size of DNA fragments was detected on agarose gel electrophoresis. Ten kinds of probes were used to analyze the origin of these DNA molecules by dot hybridization. Results DNA fragments were successfully isolated from 27 skin specimens with four kinds of different sizes including band-I (20 000 bp), band-Ⅱ (1 300 bp), band-Ⅲ (800-900 bp) and band-Ⅳ (100-120 bp). In 15 specimens most of DNA was identified as band-I. In 2 specimens band-Ⅰ, -Ⅱ and -Ⅲ were all noticed, while all four bands were detected in 10 specimens. Objective To investigate whether endogenous retinal stem cells can be activated in the retina of Royal College of Surgeons (RCS) rats during the onset and development of retinitis pigmentosa. Methods The RCS-p+ rats with inherited retinal dystrophy were divided into 3 groups: the initial stage group (15-day RCS rats), the mid-stage group (30-day RCS rats) and the advanced stage group (90-day RCS rats) according to the severity of degeneration (n=4 in each group). RCS-rdy+p+ rats without retinal degeneration served as controls, and divide into three groups (15-day control, 30-day control, 90-day control) matched with RCS-p+ rats. A transcription factor (Chx10) expressed by embryonic retinal progenitors was detected using immunofluorescence and Western blotting. Results All of the retinal layers in the three control groups and in the 15-day RCS rats did not express Chx10, while the positive expression was observed in the photoreceptor layers of the 30-day and 90-day RCS rats. Objective:To investigate the postoperative analgesia effect of continuous femoral nerve block (CFNB) combined with parecoxib on the patients with Total knee arthroplasty (TKA).Methods:A total of 100 patients,who underwent unilateral TKA in Affiliated Hospital of Guizhou Medical University from January 2015 to December 2016,were selected and randomly divided into study group (n=50) and control group (n=50).After TKA,the patients of two groups were received CFNB,CFNB combined with parecoxib analgesia respectively,analgesic time was 2 days.The visual analogue pain scores (VAS) of the patients of two groups at rest and during exercise and the incidence of postoperative adverse reactions were observed at 6h,12 h,24 h,48 h after operation.The Hospital Special Surgery knee score (HSS) was used to evaluate the joint function of patients in the two groups 1 d before operation and 1 d,2 d,3 d after operation.Results:Resting state VAS and motion state VAS in the study group were significantly lower than those in the control group 6 h,12 h,24 h,and 48 h after operation (P<0.05).There were no significant differences in the incidence of nausea / vomiting,respiratory depression,catheter related problems,and urinary retention between the two groups (P>0.05).There was no significant difference in HSS score between the two groups 1 d before operation(P>0.05).The HSS scores of the patients in the two groups 1 d,2 d,3 d after operation were significantly higher than those 1d before operation,and the HSS scores of the patients in the study group were significantly higher than those in the control group (P<0.05).Conclusion:CFNB combined with parecoxib has the advantages of good analgesic effect and safety for postoperative analgesia of the patients undergoing TKA,which is good for the patients' early knee joint function exercise and is worthy of clinical promotion.

Luminal surface of vascular endothelium is decorated with a variety of polysaccharide-protein complexes,which constitute the glycocalyx.It has been demonstrated that vascular endothelial glycocalyx plays an important role in modulation of selective permeability of vessels,mediation of the blood cell-endothelial cell interactions and the release of nitric oxide induced by fluid shear stress under physiological condition.In inflammation condition,sheding of glycocalyx due to inflammation mediator leads to its functional weakening in vessel protection.At the same time,heparan sulfate as a major constituent of vascular endothelial glycocalyx could be involved in regulating the evolution of inflammation.Heparan sulfate interacts with L-selectin to mediating leukocyte rolling,presents chemokines on luminal surfaces of endothelial cells to mediate leukocyte crawling and firm adhesion,participates in transcytosis of chemokines from tissue to luminal side of endothelial cells during inflammation.Various risk factors of atherosclerosis,as an inflammatory disease,are closely associated with vascular endothelial glycocalyx.This paper is aimed to review the role of vascular endothelial glycocalyx in inflammation and atherosclerosis.
9.Correlation of visual acuity and metamorphopsia with foveal morphology before and after vitrectomy with internal limiting membrane peeling in idiopathic macular epiretinal membrane eyes

Objective To analyze the correlation of visual acuity and metamorphopsia with foveal morphology before and after vitrectomy with internal limiting membrane peeling (ILMP) in idiopathic macular epiretinal membrane (IMEM) eyes.Methods This is a retrospective case series of 47 IMEM patients (49 eyes).All the patients underwent 25G pars plana vitrectomy with IMEM removal and ILMP.The best corrected visual acuity (BCVA) was measured using the international standard visual acuity chart,and the results were converted to the logarithm of the minimum angle of resolution (logMAR) visual acuity.The severity of metamorphopsia was measured using M-charts.The central macular thickness (CMT),inner nuclear layer thickness (INT),inner retinal layer thickness (IRT),outer retinal layer thickness (ORT),the status of ellipsoid zone (EZ) were assessed with spectral-domain optical coherence tomography at baseline and each month postoperatively.The differences in BCVA,CMT,INT,IRT,ORT and status of EZ before and after surgery were analyzed,so did the correlations between these indexes at the same time.Results Compared with baseline,the postoperative BCVA was significantly increased (F=6.133,P<0.001),but the M value,CMT,INT,IRT,ORT were significantly decreased (F=12.481,10.565,15.739,6.046,10.569;P<0.001);the integrity of EZ was improved significantly (x2=12.309,P<0.001).Preoperative BCVA was positively related to the CMT (r=0.720) and ORT (r=0.720,0.887;P<0.05),while negatively related to preoperative integrity of EZ (r=-0.295,P<0.05).The postoperative BCVA was positively related to the preoperative BCVA and postoperative CMT,ORT (r=0.774,0.754,0.842;P<0.05),while negatively related to postoperative integrity of EZ (r=-0.676,P<0.05).The preoperative M value was positively related to the preoperative CMT,INT,IRT,and ORT (r=0.931,0.668,0.840,0.637;P< 0.05).The postoperative M value was positively related to the preoperative M value and postoperative CMT,INT,IRT,and ORT (r=0.723,0.722,0.767,0.825,0.387;P<0.05).Conclusions Vitrectomy with ILMP for IMEM can improve the visual acuity,metamorphopsia and foveal morphology.Both visual acuity and metamorphopsia correlate with foveal morphology.
10.Comparation of three-dimensional transesophageal echocardiography and other imaging techniques in conducting percutaneous left atrial appendage occlusion

Objective To investigate the value of 3D-TEE,2D-TEE,CTA and X-ray contrast in assessing left atrial appendage (LAA) shape,size and guidance for the choice of occlusion.Methods Forty-three cases of left atrial fibrillation who were referred for LAA closure were performed TEE to measure the opening maximum diameter,and maximum depth of 2D-TEE at 0°,45°,90° and 135°.When LAA showed most clearly,the opening maximum and minimum diameter of LAA were measured by using 3D-ROOM mode,and the results were compared with those measured by CTA and X-ray contrast.Results There was no significant difference for opening maximum diameter of LAA measured by 3D-TEE,2D-TEE and X-ray contrast (all P>0.05),but that of CTA statistically larger than 3D-TEE,2D-TEE,X-ray contrast (all P<0.01).There were significant difference between 2D-TEE at 90°,135° and CTA,2D-TEE at 135° and X-ray contrast in assessing maximum depth of LAA (all P<0.05).Opening maximum diameter of LAA measured by 3D-TEE was significantly correlated with each angle of 2D-TEE,CTA,and X-ray contrast (r=0.70-0.77,0.57,0.58,all P<0.01),and the opening maximum diameter measured by 2D-TEE also correlated with CTA and X-ray contrast (r =0.57-0.71,0.45-0.51,all P<0.01).Also the opening maximum diameter measured by 3D-TEE,2D-TEE,CTA and x-ray contrast were correlated with occluder size (r=0.93,0.70-0.77,0.57,0.47,all P<0.01).Conclusion Compared with 2D-TEE,CTA and X-ray contrast,3D-TEE is more instructive for occluder selection.
