1.Reflections on surgical operation to treat cancer after a long term follow-up survey

Objective To review and reflect the development and achievement in surgical therapy for cancer in 20th century, and to inquire and foresee the objectives and tasks of the study on tumor surgery in 21st century. Methods Based on the data of a long term follow-up survey of more than 3000 cancer patients who had undertaken thoracic or abdominal operations, and also on a review, analysis, evaluation and reflection of clinical cases accumulated for 40 years, clinical experiences of both positive and negative aspects were summed up, a few new personal ideas and knowledge were raised. Results It was found that: (1) Postoperative recurrence and metastases played as the key factors which affected the curative effect. Therefore therapies and procedures to control postoperative recurrence and metastases are vital in improving the curative effect. (2) Improvement on curative effect of cancer surgery can be hardly expected if no breakthrough in basic researches is made. Conclusion Experimental and clinical researches should be aimed at the radical cure of postoperative recurrence and metastases, so as to improve the surgical treatment of malignancies in the future.
2.Efficacy observation on benign prostatic hyperplasia treated with acupuncture and moxibustion.

OBJECTIVETo compare the difference in the clinical efficacy on benign prostatic hyperplasia (BPH) between the acupuncture-moxibustion therapy and the medication of Qianliekang tablets.
METHODSOne hundred and twenty-eight patients were randomized into an acupuncture-moxibustion group and a Qianliekang group, 64 cases in each one. In the acupuncture-moxibustion group, acupuncture was applied to Shenshu (BL 23), Pangguangshu (BL 28), Zhongji (CV 3), Guanyuan (CV 4) and Shuidao (ST 28), and the warm moxibustion therapy with moxa stick was used at Shenshu (BL 23), Guanyuan (CV 4) and Shenque (CV 8), once every day. In the Qianliekang group, Qianliekang tablets were prescribed for oral administration, 4 tablets each time, three times a day, for 3 months. The International Prostate Symptom Score (I-PSS) and the changes in residual urine (Ru) and maximal urine flow rate (Qmax) determined with the ultrasonic B test were compared before and after treatment in the two groups.
RESULTSThe results of I-PSS, Qmax and Ru were improved obviously after treatment as compared with those before treatment in the two groups (all P < 0.05). The improvements in the acupuncture-moxibustion group were much more obvious than those in the Qianliekang group [8.62 +/- 2.18 vs 15.26 +/- 2.81, (16.04 +/- 4.33) mL/s vs (12.47 +/- 2.13) mL/s, (10.43 +/- 2.14) mL vs (32.13 +/- 3.24) mL, all P < 0.01]. The total effective rate was 89.1% (57/64) in the acupuncture-moxibustion group, which was better than 68.7% (44/64) in the Qianliekang group.
CONCLUSIONAcupuncture-moxibustion therapy achieves the significant efficacy on BPH, which is better than the oral administration of Qianliekang tablets.

6.Significance and progress in detection of HBV cccDNA in peripheral blood mononuclear cells

Hepatitis B virus (HBV)covalently closed circular DNA (cccDNA)is an intermediate replicative episome which is stable and not easy to eliminate.cccDNA serves as the template for HBV replication and plays an important role in persistence of HBV infection in hep-atocytes,and it is also a biomarker of HBV activity.Studies suggested that HBV has extrahepatic infectivity and cccDNA could be detected in peripheral blood mononuclear cells (PBMC).The predictive values of HBV cccDNA in PBMC for HBV recurrence after liver transplanta-tion and antiviral therapy and mother-to -child transmission of HBV in patients with hepatitis B,as well as the possible mechanism of cccDNA involved in extrahepatic HBV infection,are summarized.Therefore,the detection of HBV cccDNA is of great significance for the study and treatment of HBV.
7.Recent progress in development of antibiotics against Gram-negative bacteria.

Multidrug-resistant (MDR) bacterial infections, especially those caused by Gram-negative pathogens, have emerged to be one of the world's greatest health threats. However, not only have recent decades shown a steady decline in the number of approved antimicrobial agents but a disappointing discovery also void. The development of novel antibiotics to treat MDR Gram-negative bacteria has been stagnated over the last half century. Though few compounds have shown activities in vitro, in animal models or even in clinical studies, the global antibiotic pipeline is not encouraging. There are a plethora of unexpected challenges that may arise and cannot always be solved to cause promising drugs to fail. This review intends to summarize recent research and development activities to meet the inevitable challenge in restricting the proliferation of MDR Gram-negative bacteria, with focus on compounds that have entered into clinical development stage. In addition to new analogues of existing antibiotic molecules, attention is also directed to alternative strategies to develop antibacterial agents with novel mechanisms of action.

8.An additional augmentative locking compression plate combined with bone graft for treatment of aseptic subtrochanteric fracture nonunion after intramedullary nailing

Objective:To investigate the efficacy of an additional augmentative locking compression plate combined with bone graft for aseptic subtrochanteric fracture nonunion after intramedullary nailing.Methods:From October 2016 to October 2019, 32 patients (25 males and 7 females) were treated at Department of Orthopedics, The Sixth People's Hospital Affiliated to Shanghai Jiaotong University for aseptic subtrochanteric fracture nonunion after intramedullary nailing. Their ages ranged from 27 to 68 years (average, 50.5 years) and their nonunion time from 9 to 24 months (average, 12.2 months). According to the Weber-Cech classification for nonunions, 9 cases belonged to the highly vascular type and 23 to the ischemic type. With the original intramedullary nails retained, all patients were fixated with an additional augmentative locking compression plate before simultaneous autologous iliac bone grafting. All patients were followed up regularly for fracture union, function of the affected hip and complications.Results:The 32 patients were followed up for 6 to 24 months (average, 12.9 months). All nonunions obtained clinical and imaging union. The clinical union time ranged from 3 to 8 months (average, 5.2 months), and the imaging union time from 6 to 10 months (average, 7.4 months). At the last follow-up, the therapeutic efficacy was evaluated as excellent in 28 cases and as good in 4, giving a good to excellent rate of 100%(32/32), according to the Sanders scoring for post-traumatic hip joint; the efficacy was evaluated as excellent in 21 cases, as good in 10 cases and as fair in one, giving a good to excellent rate of 96.9%(31/32), according to the Harris hip scoring. No such complications occurred as infection or loosening or breakage of internal fixation.Conclusion:An additional augmentative locking compression plate combined with bone graft is a reliable and effective treatment for aseptic subtrochanteric fracture nonunion after intramedullary nailing.
