Background and purpose:The prognosis of ovarian cancer is poor and the diagnosis is relatively late. It is needed to search for early diagnosis and treatment for ovarian cancer. This study investigated the expression of serum miRNAs in patients with malignancy or benign ovarian tumor preoperativey and analyzed its correlation with clinicopathological progress and prognostic features of epithelial ovarian cancer.Methods:Forty-eight miRNAs which have been reported to be related to ovarian cancer were ordered. The differential expression of 48 miRNAs in the serum of patients with malignant or benign epithelial ovarian tumors was detected by TaqMan low density array. The differentially expressed miRNAs were further confirmed by real-time fluorescent quantitative polymerase chain reaction (RTFQ-PCR). The relationship between the expression level of selected miRNA and clinical clinicopathological factors, progress and prognosis were analyzed.Results:TaqMan low density array and further RTFQ-PCR showed that only miR-125b was sig-nificantly increased in 135 ovarian cancer patients as compared with 38 individuals with benign tumor. The expression of miR-125b was higher in early stage patients than that in advanced stage patients (P=0.039). The patients without residual tumor expressed more miR-125b than patients with residual tumor (P=0.013). The high level of miR-125b was signifi-cantly correlated with longer progress free survival (PFS) (P=0.003), but not correlated with overall survival (OS) (P=0.069). Conclusion:MiR-125b may play an important role in the pathogenesis of epithelial ovarian cancer and prognosis. It may be a potential gene to predict the recurrence of epithelial ovarian cancer, but the change of gene expression at different stages and its underlying mechanism need further research.

Vitamin D deficiency is highly prevalent in the general population including rheumatoid arthritis (RA) patients.Autocrine regulation of vitamin D modulates important immune response.Vitamin D deficiency may be involved in the pathogenesis of many autoimmune diseases including RA.Vitamin D regulates both the innate and adaptive immune responses.Vitamin D's effects on the innate immune system are mainly through the toll-like receptor and on the adaptive immune system through T cell differentiation,particularly the Helper T cell (Th) 17 response.As Thl7 cells are critical in the pathogenesis in RA,what effect will vitamin D deficiency lead to on the RA? Many data have indicated the relationship between vitamin D deficiency and RA disease activities,but not all the results are consistent.Here,we review the immune-modulatory roles of vitamin D and its effects on disease activity,osteoporosis and cardiovascular disease risk in RA.

Objective:To explore the effect of comprehensive intervention program based on the concept of fast track surgery in early enteral nutrition tolerance and rehabilitation in patients with esopha geal cancer.Methods:93 selected patients were divided into the intervention group and the conventional control group according to the admission time.Patients in intervention group were treated with comprehensive intervention,and control group were given routine nursing care.Enteral nutrition intolerance incidence,the first time of bowel sounds,the first time exhaust and defecation and complications were compared between the two groups.Results:36 patients from 47 patients (76.60％) in the intervention group could tolerate the early feeding,while 24 patients from 46 patients (52.17％) in the control group could tolerate.There was significant difference between two groups (P ＜ 0.05).The first time of exhaust,bowel sounds and defecation,and the hospitalization time of patients in the intervention group were better than that in the conventional control group (P ＜ 0.01).Incidence rate of incision infection,pulmonary infection and intestinal obstruction in patients of the comprehensive intervention group was lower than that in the control group (P ＜ 0.05).Conclusion:Compared with the traditional treatment,the comprehensive intervention can effectively reduce the enteral nutrition intolerance incidence,promote the recovery,reduce hospitalization time and the incidence of postoperative complications,and improve the life quality of patients after surgery.

Objective To investigate the expression of interleukin (IL)-34/colony stimulating factor(CSF)-1R in the process of transforming growth factor ( TGF)-β1 inducing epithelial-mesenchymal transition ( EMT) of human alveolar epithelial cells A549 cells.Methods A549 cells were cultured in vitro.CCK 8 was used to test the influence of the proliferative rate of A549 cells which were stimulated by TGF-β1 at different concentrations and time points .A549 cells were stimulated by 5μg/L TGF-β1 at 0 hour, the 12th hour, the 24th hour, and the 48th hour.Western blotting was adopted to detect changes of the following proteins: α-smooth muscle actin (α-SMA ) , E-cadherin ( E-Cad ) , matrix metalloproteinase-2 (MMP-2), matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1(TIMP-1). Real-time PCR was adopted to detect changes of the following genes: IL-34 mRNA, CSF-1R mRNA, MMP-2 mRNA and MMP-9 mRNA.Results TGF-β1 had no significant influence in the proliferation of A 549 cells compared with the control group(P>0.05).TGF-β1(5μg/L)stimulated A549 cells at different time point (0 hour, 12, 24, 48 hours), compared with the control group .The epithelial phenotype E-Cad protein was gradually down-regulated ( P <0.01 ) , while the mesenchymal phenotype α-SMA protein was gradually up-regulated ( P <0.01 ) and the protein of MMP-2 increased gradually (P<0.01).The protein of MMP-9 increased firstly and then was reduced (P<0.01),the peak was at the 24th hour.The protein of TIMP-1 was firstly transiently increased and then reduced (P<0.01), the minimum was at the 48th hour.Compared with the control group , the gene of IL-34 mRNA increased gradually (P<0.01), and the genes of CSF-1R mRNA, MMP-2 mRNA and MMP-9 mRNA increased firstly and then decreased ( P <0.01), which were peaked respectively at the 24th hour, the 24th hour, the 12th hour, respectively.Conclusion In the process of TGF-β1 inducing A549 cells transition,there is accompanied with the expression of IL-34/CSF-1R.

Objective To analyse the causes of medical complaints ,seek methods to prevent medical disputes .Methods All medical complaints were collected and analyzed retrospectively in 2011 .Results In 278 medical complaints ,there were 161 (57 .91% ) complaints by telephone ,101 (36 .33% ) complaints by hospital electronic touch screen ,16 (5 .76% ) complaints by E-mail;the top three complaint departments were vasculocardiology department 33(11 .87% ) ,department of gynaecology and obstet-rics 29(10 .43% ) ,urology department 26(9 .35% ) ,respectively ;the top three complaint reason were poor doctor-patient communi-cation 101(36 .33% ) ,poor service level 69(24 .82% ) and ward disadvantaged 45(16 .19% ) ,respectively .Conclusion Prevention of medical complaints of the most important measures is to improve the skill of doctor-patient communication and technical level of medical staff .Furthermore ,hospital should strengthen the construction of infrastructure .

Objective To explore pre-hospital delay factor of coronary reperfusion therapy for ST-elevation acute myocardial infarction (STEAMI) patients presenting with non-chest pains. Methods A retrospective observation was conducted. The clinical data of STEAMI patients underwent emergency percutaneous coronary intervention (PCI) admitted to Luoyang Central Hospital Affiliated to Zhengzhou University from August 2013 to August 2015 were analyzed. The patients were divided into chest pain group and non-chest pain group according to the presence of chest pain or not. Clinical characteristics were compared between the two groups, and incidence of major adverse cardiac events (MACE), door-to-balloon time, door-to-electrocardiograms (ECG) time and ECG-to-balloon time were evaluated. Influencing factors of pre-hospital delay was analyzed by logistic multiple stepwise regression. Results A total of 259 patients with STEAMI were enrolled, including 154 patients with chest pain and 105 presented with non-chest pains. Compared with chest pain group, the patients in the non-chest pain group were older (years: 68.12±8.93 vs. 62.34±7.12, P < 0.05), less female (26.67% vs. 42.20%, P< 0.05), and had a higher past history of angina, stroke and heart failure (27.61% vs. 13.63%, 31.42% vs. 18.83%, 26.67% vs. 11.68%, respectively, all P < 0.05), and higher percentage of Killip ≥ Ⅲ patients (15.24% vs. 6.49%, P < 0.05), the lower ambulance use (26.67% vs. 44.81%, P < 0.01), longer hospitalization time (days: 12.50±2.89 vs. 9.50±2.67, P < 0.05), higher incidence of MACE (19.05% vs. 9.09%, P < 0.05), longer door-to-balloon time and door-to-ECG time (minutes: 159.01±51.21 vs. 115.31±36.74, 53.06±18.17 vs. 30.35±9.93, both P < 0.01). It was shown by logistic multivariate regression analysis that no-chest pain [odds ratio (OR) = 5.14, 95% confidence interval (95%CI) = 2.34-10.81, P < 0.001], age ≥ 65 years old (OR = 1.43, 95%CI = 0.93-2.99, P = 0.022), diabetes (OR = 1.57, 95%CI = 0.66-2.15, P = 0.015) and no-ambulance transport (OR = 1.55, 95%CI = 0.73-2.75, P < 0.001) were risks factors of coronary reperfusion delay ≥ 2 hours. Conclusions STEAMI patients presenting without chest pain showed higher incidences of MACE, longer time of ECG obtained and initial PCI time delay. Clinicians should try to reduce the delay time of the patients in order to improve patient survival rates.

Objective To investigate clinical significance of diagnosis of the anti saccharomyces cerevisiae antibody (ASCA), pancreatic acini antibody (PAB)resistance,resistance to small goblet cell antibody (GAB),anti neutrophil cytoplasm anti-bodies (ANCA)for inflammatory bowel disease (IBD)and the differential diagnosis of ulcerative colitis (UC)and crohn's disease (CD).Methods Collected 200 cases of test group sets of inflammatory bowel disease,serum by indirect immunofluo-rescence (IF).Results In the serum of 200 patients,106 cases with positive or weakly positive.Among them,the positive ASCA/weak positive 24 cases,14 cases of PAB,GAB 63 cases,28 cases ANCA,and included in ASCA group respectively and PAB group,GAB group,ANCA group.Positive rate of ANCA and GAB in the diagnosis of UC were 34% and 58%. Positive rate of ASCA and PAB in the diagnosis of CD were 28.6% and 38.1%.ANCA associated with GAB detection in the diagnosis of UC specific degree was 60%,ASCA associated with PAB detection in the diagnosis of CD specific degree was 75%.Conclusion Serum inflammatory bowel disease antibody spectrum in ASCA,ANCA,GBA and PAB four antibod-ies of joint detection has important guiding value to the diagnosis of IBD,also can be used as one of UC and CD in the differ-ential diagnosis methods.

Objective To investigate the diagnostic values of serum free fatty acids (FFA) and urine podocalyxin (PCX) in the patients with type 2 diabetic nephropathy(DN).Methods A total of 120 patients with type 2 diabetes mellitus (DM) were enrolled,and they were divided into three groups,normal albumin group(NA),microalbuminuria group(MA) and heavy albuminuria group(HA),based on the urinary albumin-creatinine ratio(UACR).In addition,40 healthy persons participated in medical examination were selected as the control group.Serum FFA levels were detected by a biochemical analyzer and urine PCX levels by ELISA.The correlation between them was analyzed by Pearson correlation,and their diagnostic values in type 2 DN were evaluated by the receiver operating characteristic curve (ROC).Results Serum FFA levels in the NA,MA,HA and control groups were (0.61 ± 0.14),(0.81 ± 0.13),(0.95 ± 0.18) and (0.49 ± 0.11) mmol/L,respectively,and urine PCX levels were (1.86 ± 0.45),(4.47 ± 1.48),(6.72 ± 1.40)and(1.38 ±0.24) ng/mL,respectively.The levels of serum FFA and urine PCX in type 2 DM patients were significantly higher than those in the control group(P ＜ 0.05),and increased with the progression of type 2 DM.Pearson correlation analysis showed that serum FFA levels were positively correlated with urine PCX levels(r =0.73,P ＜ 0.05).The sensitivity of serum FFA,urine PCX and combined detection in the diagnosis of type 2 DN were 74.1％,80.6％ and 89.5％,respectively,and the latter significantly higher than the former two (P ＜ 0.05).Conclusion Serum FFA and urine PCX levels increase significantly in diabetic patients with renal injury,and both of them have important clinical values in the diagnosis of type 2 DN.
