1.Clinical observation of the effect of umbilical cord blood stem cell transplantation in treatment of decompensated cirrhosis

Objective To explore the clinical eficacy of umbilical cord blood stem cell transplantation in treatment of decompensated cirrhosis.Methods Thirty patients with decompensated cirrhosis were given umbilical cord blood stem cell transplantation (treatment group) and 30 patients with decompensated cirrhosis were given traditional treatment (control group).Liver function and blood coagulation function was tested after 4,8 weeks treatment respectively,and adverse effects were recorded at the same time.Results After 8 weeks treatment,total bilirubin,albumin and prothrombin time in treatment group was improved compared with that before treatment[(71.3 ± 37.8) μ mol/L vs.(107.3 ± 53.2) μ mol/L,(30.1 ± 4.9) g/L vs.(27.5 ± 5.1) g/L,(15.0 ± 2.9) s vs.(16.7 ± 3.9) s],and there was significant difference (P < 0.05).There was no significant difference in the index before and after treatment in control group (P> 0.05).No obvious adverse reactions were observed in the process of umbilical cord blood stem cell transplantation.Conclusion Umbilical cord blood stem cell transplantation is safe and effective in treatment of deeompensated cirrhosis.
2.HLA-DR and DP antigens expression on human aortic smooth muscle cells in vitro

To observe whether human aortic smooth muscle cells (SMC) may expressHLA-DR and DP antigens, we used ?-IFN and lymphocytes conditioned media (LCM) to induce SMC in vitro. The results showed that ?-IFN increased HLA-DR and HLA-DP expression on SMC by 80% and 73%, and LCM did so by 74% and 70% respec-tively. HLA-DR and DP (11% and 7%) in both groups were significantly higher thanthat of the control group. These results suggested that activated T lymphocytes may in-crease HLA-DR and HLA-DP antigens expression on human SMC by secreating ?-IFN (orwith other cytokines), which may play certain important roles in atherogenesis.
3.Cost analysis of hemodialysis and peritoneal dialysis

Objective To compare the difference in cost formation between hemodialysis and peritoneal dialysis so as to provide basis for the formulation and perfection of medical security policies. Methods An analysis was made of the cost formation and profits of hemodialysis and peritoneal dialysis in 4 hospitals of Shanghai in 1999. Results Of the average cost of each performance of hemodialysis, the cost of materials accounted for 35% to 47% and the cost of equipment accounted for 38% to 44%, while the scope of increase in profits was affected both by the number of patients and the average profits per performance. Of the average cost of each performance of peritoneal dialysis, the cost of materials exceeded 97% while profits were related only to the number of patients. Conclusion There is a marked difference in profits between the two forms of dialysis, leading to an improper expansion in the use of hemodialysis and an inhibition in the rational use of the technique of peritoneal dialysis.
4.Lung transplantation for end-stage pulmonary diseases: report of 18 cases

Objective To discuss the feasibility of the lung transplantation as an effective treatment for end-stage pulmonary diseases domestically and to compare the major issues involving the practice of lung transplantation domestically and overseas.Methods After the foundation of group of lung transplantation in May 2002, lung transplantation models of porcine were set up. From September 2002 to April 2005, in the 18 cases undergoing lung transplantation, there were subjected to single-lung transplantion (SLT, 83.3%) and 3 bilateral sequential single lung transplants without CPB (DSSLT, 16.7%). Indications for SLT (n=15) included emphysema (n=9), pulmonary fibrosis (n=3), pneumosilicosis (n=1), lymphangioleiomyomatosis (n=1) and ventricular septal defect (VSD) induced Eisenmenger's syndrome (n=1); for DSSLT (n=3) bronchiectasis (n=1) and pulmonary emphysema (n=2). Among the 15 SLTs, there were 9 cases of left SLT and 6 right SLT. Among them, 2 cases shared one same donor's lung block, one case received contralateral lung transplantation―a left donor lung implanted in the recipient’s right thorax, and one case simultaneous right SLT and VSD repair. Results In hospital mortality (HM) was 3/18 ( 16.7%). Among SLTs, early death was due to severe rejection on the 30th postoperative day in one patient and acute rejection on the 15th postoperative day in other patients, and another patient died due to pulmonary vein embolism on the 36th day. There were 3 and 2 patients with the survival time longer than 1 and 2 years respectively. The median overall survival was 10 (2 to 32) months. Conclusions Our LT program shows similar results to those reported by the International Society for Heart and Lung Transplantation for developed countries. The key of successful operation depends on the establishment of group of lung transplantation and cooperation of multi-department. Ischemic-reperfusion injury, acute-rejection and infection are the major reasons of deaths shortly after the operations.
5.An analysis on current situation and development tendency of hospital infection management in China

Objective To understand the current situation of hospital infection management in some hospitals in China and explore its development tendency. Methods Surveys were made by means of questionnaires on hospital infection management in 16 hospitals of various tiers in Beijing, Shanghai and Guangzhou and the results were compared with those of the two nation-wide surveys conducted respectively in 1994 and 1999. At the same time a dynamic assessment was made of hand hygiene implementation. Results The infection management systems in large and medium-sized hospitals in China were becoming increasingly perfect and a fairly high level was reached in function construction, team structure and expertise of hospital infection management departments. However, there were still many problems. 60% of medical staff washed their hands after patient-contact while only 35% did so before patient-contact and after item contact. Conclusion Hospital infection management in China has reached an important stage of accelerated development with substantial accumulation and more solid foundation. However, as far as development tendency is concerned, we should introduce technical indexes for quality control, put more emphasis on process-evaluation rather than result-evaluation in quality control management, stress the guiding role of evidence-based medicine in hospital management and promote a benign interaction between the hospital infection management and medical treatment process.
6.Investigation of hand hygiene compliance after patient contact

Objective To evaluate hand hygiene compliance (HHC) after patient contact and the influence of different factors on HHC in China. Methods HHC by different levels of medical workers from different departments were observed on 8 tertiary hospitals in Beijing, Shanghai and Guangzhou. Results The average HHC was 56. 5%( 61. 0% by the doctors and 53. 8% by the nurses). HHC was 52. 8% in the presence of gloves and 82. 0% in the absence of gloves; the difference was statistically significant. 7.Application of ROC curve to determine the ELISA cut-off value for the diagnosis of alveolar echinococcosis

Objective To determine the ELISA cut‐off value for the diagnosis of alveolar echinococcosis by applying ROC curve . Methods 34 cases of alveolar echinococcosis specimens confirmed by the gold standard were selected and detected by ELISA .The diagnostic cut‐off value was determined by ROC curve ,and the sensitivity and specificity were calculated according to the cut‐off value .Results When the cut‐off value was 0 .44 ,the ROC curve had the largest area under the curve (AUC) accounting of 0 .995 , and the sensitivity and specificity w ere 97 .06% and 97 .06% respectively .Conclusion T he ROC curve is the best w ay to determine the cut‐off value of ELISA .
8.Expression of matrix metalloproteinase-2 and-9 in the PBMC of patients with polymyositis and dermatomyositis

Objective To investigate the relation of the expressions of matrix metalloproteinase-2(MMP-2)and-9(MMP-9)with polymyositis/dermatomyositis(PM/DM).Methods Blood samples were obtained from 8 patients with PM,12 patients with DM and 10 normal human controls.Real-time quantitative RT-PCR and Western-blot were applied to examine the mRNA and protein expressions of MMP-2 and MMP-9 respectively in the peripheral blood mononuclear cells(PBMC)of these patients.Results The mRNA expression levels of MMP-2 and MMP-9 in the PBMC of patients with PM or DM were 2.41 and 1.66 folds as high as those of the normal controls,respectively;these difierences were significant between the two groups.Increased expressions of MMP-2 and MMP-9 were also noted in the patients with PM or DM.Conclusion MMP-2 and MMP-9 may play a part in the development of polymyositis and dermatomyositis.
9.Overexpression of HRAD17 sensitizes A549 cells to radiation-induced apoptosis

Objective To study the effects of overexpression of HRAD17 on radiation-induced apoptosis of A549 cells.Methods Transfection of FLAC-tagged HRAD17 expression plasmid into human A549 cells was carried out and flow cytometry was employed tO observe the apoptotic effect.In addition,phosphorylation of p53 Ser46 was detected by Westem blot and the transcription of p53 AIP1 was measured by RT-PCR.Results Overexpression of HRAD17 gene significantly increased radiation-induced apoptosis of A549 cells.Further.Ser46 phosphorylation of P53 protein was increased and transcription of p53 AIP1 gene was elevated in cells overexpressing HRAD17.Conclusions HRAD17 protein can sensitize A549 cells to radiation-induced apoptosis in part by increasing p53 AIP1 expression.
10.Synthesis of diethylenetriamine polydentate ligands and their DNA-cleaving activity.

A series of multinuclear diethylenetriamine ligands were synthesized and used as artificial nuclease enzyme model. Target compounds were characterized by 1H NMR, 13C NMR, IR and ESI-MS. Preliminary studies on the cleavage of pUC19 DNA in the presence of metal complexes have also been performed and the results revealed that these complexes could act as powerful catalysts for the cleavage of pUC19 DNA after 48 h under physiological conditions. The hydrolytic cleavage mechanism of DNA plasmid by title compound was confirmed by T4 DNA ligase experiment.
