1.Laparoscopic resection for colorectal cancer: the main progress of surgery for colorectal cancer

Whether resection for colorectal cancer could achieve the same effect as open surgery had been doubted. Recently, the international large prospective random control tests and large retrospective studies have revealed that laparoscopic resection for colorectal cancer not only has short-term advantages such as minor injuries, less postoperative pain, quicker recovery of bowel function and less postoperative hospital stay, but also had at least the equivalent long-term effects in local recurrence, distant metastasis, incision implantation and long term survival. It is promising that the roll of laparoscopic surgery for colorectal cancer will be defined by more evidence-based medicine study in the near future.
2.Clinical significance of the number of lymph nodes detected for stage Ⅱ colorectal cancer

Colorectal cancer is a threat to human health, the disease rate of stage Ⅱ colorectal cancer is increasing. Some prognostic factors have been researched, and the number of harvested lymph nodes is a very important factor. Harvesting more lymph nodes will reduce the relapse rate and increase the survival rate. It remains controversial for the minimum number of lymph nodes, and UICC and AJCC recommended that the minimum number was 12. Some factors will influence the number of harvested lymph nodes,for ex-ample, the patient's situation, tumor itself, surgeon and pathologist's skills , and so on.

p53,the widely studied tumor suppressor gene ,plays a key role in regulating cell cycle ,repai-ring DNA damages , eliminating free radicals and regulating immune responses and so on .The recent studies showed that p53 mutation was in close biological connections with the occurrence , development , infiltration and metastasis in over 50 percent digestive systemic malignant tumors .In view of the current researches ,we review the biological features of p53,the relationship between p53 and digestive systemic malignant tumors ,and gene therapy of p53 to provide references for the pathogenesis ,diagnosis,treatment and prognosis of digestive systemic malig-nant tumors in this paper .

6.SPLENIC ARTERY ANEURYSMS IN PATIENTS WITH ADVANCED SCHISTOSOMIASIS AND PORTAL HYPERTENSION

Objective To probe into diagnosis and treatment of splenic artery aneurysms (SAA) in patients with advanced schistosomiasis and portal hypertension. Methods Clinical data on 12 cases of SAA treated in the First Hospital of Jiaxing from January 1974 to December 2000 were analyzed retrospectively. Results One case was found by color duplex flow imaging (CDFI) and one was by BUS before operation. Other 10 cases were all found during operation period. The diameters of SAA ranged from 1.8 cm to 3.5 cm. Three aneurysms were excised together with splenectomy. Ligation of proximal and distal were done in 7 cases, one case excised the aneurysm, one case had distal ligation only and was death post-operation due to rupture of the aneurysm, after splenectomy. Conclusion The incidence of SAA in patients with advanced schistosomal portal hypertension is not very rare. If found during operation, proper management of SAA and portal hypertension is important in order to get good results.
7.Contrast analysis of chronic atrophic gastritis under gastroscopy and their pathologic results in 55 cases

Objective To study the significances of various gastroscopic findings in chronic atrophic gastritis.Methods To contrastively analyse various findings of CAG under gastroscopic direct vision and their corresponding pathologic diagnosis in 55 cases, and to calculate respectively the sensitivity and specificity of CAG, intestinal metaplasia (IM), atypical hyperplasia (ATP) through various gastroscopic expressions.Results Of the 55 patients CAG 30 cases, CAG and CSG 25 cases were diagnosed by endoscopy; the pathological diagnoses were CAG 36, CSG 8 and CAG plus CSG 11 cases. The coincidence rate of diagnosis between endoscopy and pathology was 85 45%. The sensitivities of gastroscopic findings in CAG to their pathology were about 12 77%~51 06%. Some manifestations in gastroscopy such as piebald change and flat protrusion in CAG have high specificity to pathology about 87 50% and 75 00% respectively. The dominated white phase, prominent vascular pattern with flat protrusion and piebald with erosive protrusion attained the specificity of 100% to the pathology. The asperous and granular changes had higher diagnostic value about IM, its sensitivity and specificity was 66 67% and 71 74% respectively. The dominated white phase and prominent vascular pattern had higher sensitivity, 71 74% in diagnosing ATP. Erosive protrusion had higher specificity, 68 89% in diagnosing ATP.Conclusion Piebald change, asperous and granular changes, dominated white phase and prominent vascular pattern under gastroscopy are highly helpful to the pathological diagnosis of CAG, IM and ATP. But the combination of gastroscopic and pathologic examination is the inevitable procedure in correct diagnosis of CAG, IM, and ATP.
9.Clinical Observation of Cilostazol Combined with Alprostadil in the Treatment of Peripheral Arterial Disease

OBJECTIVE:To investigate the clinical efficacy and safety of cilostazol combined with alprostadil in the treatment of peripheral arterial disease(PAD). METHODS:A total of 68 PAD patients in our hospital from Jan. 2015 to Jan. 2016 were di-vided into observation group(34 cases)and control group(34 cases)according to random number table. Control group was given basical treatment,and Alprostadil injection 2 mL+0.9% Sodium chloride injection 100 mL,ivgtt,qd. Observation group was addi-tionally given Cilostazol tablets 100 mg,po,bid,on the basis of control group. Both groups were treated for 30 days. Clinical effi-cacies of 2 groups were observed. The hemorheology indexes(hematocrit,whole blood high-shearing viscosity,erythrocyte aggre-gation index,erythrocyte deformation index,plasma viscosity),inflammatory factor indexes(TNF-α,IL-6,IL-8, hs-CRP),oxida-tion stress indexes(GSH-Px,SOD,T-Aoc,MDA),arteriosclerosis indexes(ABI,TBI,dorsalis pedis artery blood flow)were de-termined before and after treatment. The occurrence of ADR was recorded. RESULTS:Total response rate of observation group (91.18%)was significantly higher than that of control group(79.41%),with statistical significance(P<0.05). Before treatment, there was no statistical significance in hemorheology indexes,inflammatory factors indexes,oxidant stress indexes or arteriosclero-sis indexes between 2 groups(P>0.05). After treatment,hematocrit,whole blood high-shearing viscosity,erythrocyte aggregation index,erythrocyte deformation index,plasma viscosity,the levels of TNF-α,IL-6,IL-8,hs-CRP and MDA in 2 groups were de-creased significantly;while the levels of GSH-Px,SOD,T-Aoc,ABI,TBI and dorsalis pedis artery blood flow were increased sig-nificantly;the improvement of above indexes in observation group was significantly better than control group,with statistical signif-icance(P<0.05). There was no statistical significance in the incidence of ADR between observation group(14.71%)and control group(11.76%)(P>0.05). CONCLUSIONS:For PAD,cilostazol combined with alprostadil can effectively improve hemorheolo-gy indexes,inflammatory factor indexes,oxidation stress indexes and arteriosclerosis indexes with good safety.
10.Transforming Activity of a Novel Mutant of HPV16 E6E7 Fusion Gene

An optimized recombinant HPV16 E6E7 fusion gene(HPV16 ofE6E7)was constructed according to codon usage for mammalian cell expression,and a mutant of HPV16 ofE6E7 fusion gene(HPV16 omfE6E7)was generated by site-directed mutagenesis at L57G,C113R for the E6 protein and C24G,E26G for the E7 protein for HPV16 ofE6E7 [patent pending(CN 101100672)].The HPV16 omfE6E7 gene constructed in this work not only lost the transformation capability to NIH 3T3 cells and tumorigenicity in SCID mice,but also maintained very good stability and antigenicity.These results suggests that the HPV16 omfE6E7 gene should undergo further study for application as a safe antigen-specific therapeutic vaccine for HPV16-associated tumors.
