Multidrug resistance (MDR) has been a main culprit behind the failure of chemotherapy in patients with malignant tumors and a major obstacle to improving the life quality and prolonging the survival of patients. Hepatocellular carcinoma cells, the innate drug-resistant cells, are generally insensitive to radiotherapy and chemotherapy. Moreover, as the early symptoms of hepatocellular carcinoma are atypical, most patients are diagnosed at the advanced stage, with short survival period and high recurrence rate. Thus, the sensitivity to chemotherapy drugs is decreased. This explains how MDR becomes one of the important reasons for the failure of primary hepatocellular carcinoma (PHC) treatment. Therefore, it is an urgent task to search for safe and effective chemosensitizers with little adverse effect in the research on the drug resistance of hepatocellular carcinoma. As Chinese medicine has been widely applied in the treatment of tumors, the mechanisms of compound Chinese medicine prescriptions, Chinese medicine injections, and single Chinese medicinal in reversing chemotherapy resistance in liver cancer have attracted the interest of scholars. According to previous reports, the mechanisms can be summarized as increasing intracellular drug concentration, influencing changes in enzyme activity, inducing apoptosis, reversing abnormalities in cellular signaling pathways, and regulating the tumor microenvironment. Traditional Chinese medicine reduces the chemotherapy resistance of hepatocellular carcinoma cells through multiple targets and multiple pathways, thereby improving the chemotherapy sensitivity of the cancer cells and enhancing the toxicity of chemotherapeutic drugs to hepatocellular carcinoma cells. Therefore, exploring the mechanism of MDR of hepatocellular carcinoma from the perspective of traditional Chinese medicine is important for reversing the MDR and is of great reference value for clinical treatment of hepatocellular carcinoma. However, there are few experimental types and adverse effects available. Thus, the multi-mechanism and multi-target experiments and clinical research should be carried out in the future.

ObjectiveTo investigate the effect of Qi-invigorating and blood-activating therapy on the miR216b/Beclin1 pathway in mice with atrophic precancerous lesions of gastric cancer (PLGC) and analyze its mechanism in autophagy of PLGC. MethodSeventy-five healthy male SPF KM mice were randomly divided into a blank group and a model group. Mice in the model group were given 1-methyl-3-nitroso-1-nitrosoguanidine (MNNG) solution (150 mg·L-1) for free drinking and gavage and ranitidine solution (0.03 g·kg-1) daily for 12 weeks. According to the random control table, mice were divided into a model group, a Qi-invigorating group (3.5 g·kg-1 of Astragali Radix), a blood-activating group (0.7 g·kg-1 of Notoginseng Radix et Rhizoma powder), a Qi-invigorating and blood-activating group (3.5 g·kg-1 of Astragali Radix + 0.7 g·kg-1 of Notoginseng Radix et Rhizoma powder), and a folic acid group (2 mg·kg-1). The corresponding drugs were given to mice in each group for 8 weeks and then the tissues were collected. Hematoxylin-eosin (HE) staining was carried out to observe the changes in gastric mucosa. Western blot was used to detect the protein expression of microtuble-associated protein 1 light chain 3 (LC3)Ⅰ, LC3Ⅱ, and Beclin1. Real-time polymerase chain reaction (Real-time PCR) was used to detect the mRNA expression of Beclin1 and miR-216b. ResultPathological observation showed that as compared with the blank group, the intrinsic glands of gastric mucosa decreased with atrophy and intestinal metaplasia in the model group, which were improved in all treatment groups, and the improvement of the Qi-invigorating and blood-activating group was the most obvious. As compared with the blank group, the content of LC3Ⅰ, LC3Ⅱ, LC3Ⅱ/LC3Ⅰ, and Beclin1 protein in gastric tissues of the model group was significantly decreased (P<0.05). As compared with the model group, the content of LC3Ⅰ, LC3Ⅱ, LC3Ⅱ/LC3Ⅰ, and Beclin1 protein in gastric tissues of each treatment group was increased (P<0.05, P<0.01). The increase was most obvious in the Qi-invigorating and blood-activating group. As compared with the blank group, the mRNA expression of Beclin1 in the model group was decreased (P<0.05), and that of miR216b was increased (P<0.05). As compared with the model group, the mRNA expression of Beclin1 was increased and that of miR216b was decreased in each treatment group (P<0.05), and the changes were the most obvious in the Qi-invigorating and blood-activating group. ConclusionThe mechanism of the Qi-invigorating and blood-activating therapy, represented by Astragali Radix and Notoginseng Radix et Rhizoma, in treating PLGC may be through inhibiting the expression of miR216b and activating Beclin1, thus promoting autophagy and repairing gastric mucosa.

ObjectiveTo investigate the changes of intestinal microbiota in type 2 diabetic diarrhea (T2DD) patients with dampness-heat syndrome and spleen-kidney deficiency syndrome. MethodT2DD patients who were admitted to the Department of Endocrinology Ⅰ of the Affiliated Hospital of Shaanxi University of Chinese Medicine from March 2020 to May 2021 were selected, including 14 patients with type 2 diabetes mellitus (T2DM), 12 T2DD patients with dampness-heat syndrome, and 13 T2DD patients with spleen-kidney deficiency syndrome. Twelve healthy subjects receiving medical examination were selected as control group. Their body mass index (BMI), fasting plasma glucose (FPG), 2 h postprandial blood glucose (2 h PBG) and glycosylated hemoglobin (HbA1c) were compared. Fecal samples were collected for DNA extraction to build a database. High-throughput 16S rDNA sequencing was used to compare the composition of intestinal microbiota and the differential bacteria among the four groups. ResultCompared with the conditions in control group, the levels of FPG, 2 h PBG and HbA1c in the other groups were increased (P<0.05). Alpha diversity showed no significant difference in species richness, evenness and diversity of intestinal microbiota among the groups. Beta diversity indicated that intestinal microbiota tended to be consistent in each group, and there was no marked difference between groups. The top 5 phylum by relative abundance were Bacteroidetes,Proteobacteria, Firmicutes, Actinobacteria, and Fusobacteria, among which,Bacteroidetes, Proteobacteria and Firmicutes were dominant. Compared with the control group, the three diabetic groups had elevated relative abundance of Bacteroidetes while decreased relative abundance of Firmicutes. The relative abundance of Actinomycetes in spleen-kidney deficiency T2DD group was significantly higher than that in the other groups, and the relative abundance of Firmicutes and Fusobacteria in the dampness-heat T2DD group was significantly lower than that in the other groups. At the genus level, the top 10 bacteria by relative abundance were Phocaeicola, Bacteroides, Pseudescherichia, Prevotella, Bifidobacterium, Faecalibacterium, Fusobacterium, Roseburia, Citrobacter, and Cetobacterium. LEfSe analysis revealed that the relative abundance of Prevotella, Mediterraneibacter, Parabacteroides, and Fusicatenibacter in diabetic patients was remarkably higher than that in healthy patients. Bacteroides and Sutterella might be the characteristic microbiota of T2DD patients with dampness-heat syndrome, while Faecalibacterium, Limosilactobacillus, Eubacterium, Gemmiger, Enterocloster, Alistipes, Parasutterella and Oscillibacter might be the characteristic microbiota of T2DD patients with spleen-kidney deficiency syndrome. ConclusionBacteroides and Parasutterella might be the characteristic microbiota of T2DD patients with dampness-heat syndrome and spleen-kidney deficiency syndrome, respectively. This paper provided reference for studying the mechanism, diagnosis and treatment of modern traditional Chinese medicine for T2DD of dampness-heat type and spleen-kidney deficiency type.

This study aimed to explore the possible effect of Xixin Decoction(XXD) on the learning and memory ability of Alzheimer's disease(AD) model senescence-accelerated mouse-prone 8(SAMP8) and the related mechanism in enhancing neuroprotective effect and reducing neuroinflammation. Forty SAMP8 were randomly divided into a model group(10 mL·kg~(-1)·d~(-1)), a probiotics group(0.39 g·kg~(-1)·d~(-1)), a high-dose group of XXD granules(H-XXD, 5.07 g·kg~(-1)·d~(-1)), a medium-dose group of XXD granules(M-XXD, 2.535 g·kg~(-1)·d~(-1)), and a low-dose group of XXD granules(L-XXD, 1.267 5 g·kg~(-1)·d~(-1)). Eight senescence-accelerated mouse-resistant 1(SAMR1) of the same age and strain were assigned to the control group(10 mL·kg~(-1)·d~(-1)). After ten weeks of intragastric administration, the Morris water maze was used to test the changes in spatial learning and memory ability of mice after treatment. Meanwhile, immunofluorescence staining was used to detect the positive expression of receptor for advanced glycation end products(AGER), Toll-like receptor 1(TLR1), and Toll-like receptor 2(TLR2) in the hippocampal CA1 region of mice. Western blot was employed to test the protein expression levels of silencing information regulator 2 related enzyme 1(SIRT1), AGER, TLR1, and TLR2 in the hippocampus of mice. Enzyme linked immunosorbent assay(ELISA) was applied to assess the levels of Aβ_(1-42) in the hippocampus of mice and the levels of nuclear factor κB p65(NF-κB p65), NOD-like receptor protein 3(NLRP3), tumor necrosis factor-α(TNF-α), and interleukin-1β(IL-1β) in the serum and hippocampus of mice. Compared with the model group, XXD significantly improved the spatial learning and memory ability of SAMP8, increased the expression of neuroprotective factors in the hippocampus, decreased the levels of neuroinflammatory factors, and inhibited the expression of Aβ_(1-42). In particular, H-XXD significantly increased the expression of SIRT1 in the hippocampus of mice, reduced the expression levels of NF-κB p65, NLRP3, TNF-α, and IL-1β in the serum and hippocampus of mice, and decreased the expression of AGER, TLR1, and TLR2 in the hippocampus of mice(P<0.05 or P<0.01). XXD may improve the spatial learning and memory ability of AD model SAMP8 by enhancing the neuroprotective effect and inhibiting neuroinflammation.

Chinese Journal of Experimental Traditional Medical Formulae 2022;28(3):275-282
Chronic kidney disease (CKD) has become a public health problem worldwide with renal interstitial fibrosis (RIF) serving as the important pathological feature and pathological outcome of various CKD. Therefore, anti-fibrosis therapy has important practical significance for delaying the progression of CKD and improving the prognosis of CKD patients. The Wnt/β-catenin signaling pathway is a conserved signaling pathway through evolution, which plays a vital role in organ formation, tissue homeostasis, and disease progression during embryonic development. A growing body of research has confirmed that the Wnt/β-catenin signaling pathway is one of the key signaling pathways in a variety of kidney diseases and its activation is closely related to RIF. RIF is aggravated by the specific regulation of the expression of downstream target genes, such as fibroblasts, zinc finger transcription factor 1 (Snail1), M2 macrophages, matrix metalloproteinase-7 (MMP-7), plasminogen activator inhibitor-1 (PAI-1), and renin-angiotensin system(RAS), and relieved by targeting the signaling pathways, such as Klotho, Dickkopf-related protein 1 (DKK1), secreted frizzled-related protein 1 (Sfrp1), and indocyanine green-001. In addition, the pathological view of RIF in traditional Chinese medicine (TCM) coincides with that in western medicine. Based on the etiology and pathogenesis of TCM in the combination of deficiency and excess, TCM regulates fibrosis-promoting mediators by tonifying deficiency, eliminating turbidity, removing the toxin, resolving stasis, and treating both symptoms and root causes in a multi-target, multi-layer, and multi-pathway manner to inhibit the Wnt/β-catenin signaling pathway and play an important role in renal protection. Therefore, this study reviewed the regulatory mechanism of the Wnt/β-catenin signaling pathway in RIF and the protective effect of targeting this signaling pathway on renal function and discussed the potential role of TCM in delaying the progression of RIF, which is expected to provide new targets and strategies for the prevention and treatment of RIF.

Chinese Journal of Experimental Traditional Medical Formulae 2021;27(22):232-240
Acute pancreatitis (AP) is one of the most common digestive system diseases in clinic. Its pathogenesis is complex and has not yet been fully clarified. It easily progresses to severe AP if the treatment is not provided in time, and the resulting condition is dangerous with high mortality. Intestinal mucosal barrier (IMB) injury is the key link leading to the aggravation of AP. The IMB injury in the late stage of AP promotes the translocation of harmful intestinal bacteria, the entry of bacteria and the produced endotoxins into blood circulation triggers endotoxemia and enterogenous infection, causing multiple organ failure and even death. Western medicine has limitations in the treatment of IMB injury induced by AP. By contrast, Chinese medicine has been proved effective and reliable in repairing the IMB injury induced by AP through oral administration and external application, and has been widely recognized by physicians and patients. AP falls into the categories of "precordial pain due to spleen disorder", "thoracic accumulation", and "pancreas-heat syndrome" in traditional Chinese medicine. The main causes of AP are excessive intake of sweet and greasy food, improper diet, and cholelithiasis, which lead to damp-heat accumulation in the middle energizer, stagnation of spleen and stomach, and obstruction of fu-organ intestine. Therefore, dredging the interior, purging, clearing heat, removing toxin, moving Qi, and activating blood should be emphasized in treatment. According to the related literature both in China and abroad in the past five years, this paper summarized the action mechanisms of Chinese medicine in the treatment of AP-induced IMB injury as follows: It protects the mechanical barrier by improving intestinal microcirculation disorders, relieving intestinal ischemia-reperfusion injury and oxidative stress response, reducing the release of inflammatory mediators and cytokines, and inhibiting the apoptosis of intestinal epithelial cells. It restores the chemical barrier by promoting gastrointestinal functional recovery and shortening enteral nutrition time. It improved the biological barrier by regulating intestinal microecological imbalance. It reinforces the immune barrier by adjusting the level of immune cells. This paper reviewed the characteristics of IMB injury in AP as well as its therapeutic principles and mechanisms with Chinese medicine, aiming to provide a theoretical and scientific basis for the in-depth study and rational application of Chinese medicine for the treatment of IMB injury in AP.

Chinese Medical Ethics 2024;35(3):332-337
The life view of "human life is the most important" in Traditional Chinese Medicine (TCM) originates from the people-oriented theory of Chinese traditional culture, the benevolence thought of Confucianism, the rebirth thought of Taoism and the compassion thought of Buddhism. Taking the life view of "human life is the most important" as the starting point, it has formed medical ethics thoughts such as the social responsibility of saving the world, the medical practice purpose of treating all patients equally, the meticulous working attitude, the diligent and tireless learning attitude, and the exploration spirit of widely exploring the medical roots, which still has important educational significance. Therefore, the life view of "human life is the most important" should be integrated into the medical ethics education of medical students, and the medical ethics literacy of medical students should be improved through the following ways: strengthen the vocational ideal education of saving the dying and healing the wounded, and enhance the sense of social responsibility of medical students; consolidate the theme of life first classroom education and improve the professional identity of medical students; enrich and innovate the practice forms of protecting life, pay attention to the practicality of medical ethics education.
