1.The application of contact lenses in the treatment of corneal epithelial shedding

Objective:To observe the efficacy of contact lenses on a wide range of corneal epithelial shedding. Methods:The information has collected a wide range of corneal epithelium of thirty-six patients admitted to our hospital in the January 2010-December 2010, tracking observed in patients with epithelial wear contact lenses after the healing. Results:The patients wear contact lens immediate relief of symptoms with eye pain, foreign body sensation. Patients with continuous wear contact lenses after five to seven days, including twenty-eight patients with corneal epithelial fully restored, the remaining eight cases without contact lens continue to use eye drops of one to two days corneal epithelial fully restored in the ointment;thirty-six patients were followed up in three to six months, not fall off again. Conclusion:Therefore, use contact lenses to treat corneal epithelial shedding, the patients suffered less pain, treatment is easy to operate, low cost, the treatment effect is significant.

3.Status quo of burden on family caregivers of elderly patients with chronic obstructive pulmonary disease and its influencing factors

Objective? To explore burden on family caregivers of elderly patients with chronic obstructive pulmonary disease (COPD) and to discuss the influencing factors of caregiver burden. Methods? Totally 190 family caregivers of elderly COPD patients treated in a Class III Grade A hospital in a city between January and December 2017 were selected by convenient sampling. The caregiver burden of these elderly COPD patients were investigated with the general information questionnaire, Zarit Burden Interview (ZBI) and Social Support Rating Scale (SSRS). Multiple linear regression analysis was used to analyze the influencing factors to caregiver burden. Results? The total score of caregiver burden of elderly COPD patients was (52.32±7.42), and the total score of social support was (25.23±5.93). Multiple linear regression analysis showed that age, occupation, time of daily care,medical service availability and social support were the main influencing factors to caregiver burden (P< 0.05). Conclusions? The caregiver burden of elderly COPD patients is at a medium level and is affected by multiple factors. More disease and nursing care related knowledge and information should be provided to the caregivers based on their actual conditions so as to improve the social support for caregivers as well as the caregivers' physiological and mental health and their quality of life.
4.Genotype-phenotype analysis of five families with congenital ectopia lentis accompanied with cardiovascular abnormalities

Objective To analyze the genotype-phenotype correlation in 5 families with congenital ectopia lentis (CEL) accompanied with cardiovascular abnormal manifestation.Methods Detailed clinical data of 15 family members in 5 families were collected from August 2017 to March 2018 in Zhongshan Ophthalmic Center,including examination of the condition of lens before and after mydriasis by slit-lamp,evaluation of the cardiovascular system using transthoracic echocardiography,and evaluation of the degree of involvement of the subjects' skeletal system using X-ray images.Genomic DNAs were extracted from whole blood sample of the 5 probands and 10 relatives,and screened for FBN1 mutation by targeted exome sequencing.The possible genotype-phenotype correlation was analyzed by reviewing previous literatures into these mutation sites.The study followed the principles of the Helsinki Declaration and written informed consent was obtained from each subject prior to any examination.Results All of the five probands were diagnosed as CEL accompanied with cardiovascular abnormal manifestation.FBN1 gene mutations were identified in all of the five probands,including four missense mutations (c.2741G>T,c.2585G>T,c.1633C>T,c.4260C>G) and one splicing mutation (c.2114-1G>C).It was predicted that all of the 5 mutations would alter the protein structure.Conclusions FBN1 gene has a high degree of clinical heterogeneity,and the early detection of ocular phenotypes combined with genetic screening is of great significance in the diagnosis of cardiovascular abnormalities.
5.Totally endoscopic vs open thyroidectomy for differentiated thyroid cancer

Objective:To compare toally endoscopic thyroidectomy(TET) with open thyroidectomy(OT) in the treatment of differentiated thyroid cancer.Methods:Data of 190 patients at Liuzhou People's Hospital from Jul 2017 to Dec 2019 were analyzed. Patients were divided into endoscopic surgery group ( n=95) and open surgery group ( n=95). Results:The operation time and the hospital stay in endoscopic group were longer than that in open surgery group[(153±25) min vs. (116±17) min, (5.56±1.08) d vs.(5.08±1.04) d, t=11.827,3.083, both P<0.05)]. There was no significant difference in intraoperative blood loss [(33±14) ml vs. (37±16) ml, t=-1.851 P>0.05], recurrent laryngeal nerve paralysis, hypoparathyroidism and wound complications (4% vs. 9%, 9% vs. 15%,1% vs. 3%, all P>0.05). There was no significant difference in the number of central lymph node dissection between the two groups (6.12±3.54 vs. 6.35±4.75, t=-0.404, P<0.05). The length of scar in endoscopic group was shorter [(3.03±0.27) cm vs. (6.47±0.53) cm, t=-56.138, P<0.05), and the postoperative cosmetic score evaluated by the patients was higher (8.76±0.75 vs. 7.39±0.76, P<0.05), than those in open group. There were no tumor recurrence nor metastasis in neither group by short term follow-up. Conclusions:TET is similar to OT on clinical curative effectiveness for differentiated thyroid carcinoma while carries a better cosmetic result .
6.Analyzing the results of finger systolic blood pressure tests in 28 healthy young adults

Objective To investigate the level of finger systolic blood pressure (FSBP) in healthy young adults. Methods A total of 28 healthy young adults were selected as the study subjects by convenient sampling method. The FSBP of the study subjects was detected at 30 and 10 ℃, and the FSBP index (Fi) was calculated. Results The FSBP of the study subjects at 30 and 10 ℃ were (102.0±16.5) and (104.4±15.2) mmHg, respectively. The FSBP in male group at 30 and 10 ℃ was (99.6±18.6) and (107.2±17.0) mmHg, respectively. The FSBP in female group at 30 and 10 ℃ was (104.4±13.9) and (101.5±2.8) mmHg, respectively. The results of factorial analysis showed that the interaction between gender and temperature on FSBP was statistically significant (P<0.05). FSBP in male group was higher at 10 than 30 ℃ (P<0.05) and higher than female group at 10 ℃ (P<0.05). There was no statistical significance for the main effect of gender, temperature, finger, or the interaction effect of gender and finger, temperature and finger for FSBP (all P>0.05). The average Fi of the study subjects was (98.0±16.6)%, with males and females having the average Fi of (100.7±20.7) % and (95.2±10.6) % respectively. The results of factorial analysis of variance showed that there was no significant difference on Fi in the main effect gender and fingers or the interaction effect between them(all P>0.05). Conclusion The FSBP test could be used as a detection method for assessing peripheral microcirculation function in Chinese population. However, further research is needed to establish reference ranges and influencing factors.
7.Application and effect of information management in nursing of incontinence associated dermatitis among elderly patients

Objective:To explore the application and effect of information management in nursing of incontinence associated dermatitis (IAD) among elderly patients.Methods:From April 2019, the Cadre Department of Zhejiang Provincial People's Hospital carried out information management of IAD in elderly patients. Relying on the Ding Talk platform to provide nurses with information training and assessment of IAD knowledge, and to carry out information management of skin management in the elderly incontinent patients. Using before-after study, we randomly selected data of 80 elderly patients from October 2018 to March 2019 as control group, and selected data of 80 elderly incontinent patients from April to September 2019 as experimental group. We observed the incidence and time of IAD between two groups.Results:After information management, the incidence of IAD of experimental group patients was 13.75% (11/80) lower than that 33.75% (27/80) of control group with a statistical difference ( P<0.05) . The time of IAD of experimental group was (10.46±2.01) d, which was significantly later than that [ (5.47±2.29) d] of control group ( P<0.05) . Conclusions:Information management can effectively reduce the incidence of IAD in elderly patients, delay the occurrence time and improve the nursing effect.

Background and Objectives:
p21, an important member of the Cip/Kip family, is involved in inhibitory effects of RUNX1b overexpression during the early stage of human hematopoiesis.
Methods:
and Results: We established a human embryonic stem cell (hESC) line with inducible expression of p21 (p21/hESCs). Overexpression of p21 did not influence either mesoderm induction or emergence of CD34+ cells, but it significantly decreased the production of CD43+ cells and changed the expression profile of hematopoiesis-related factors, leading to the negative effects of p21 on hematopoiesis.
Conclusions
In RUNX1b/hESC co-cultures when RUNX1b was induced from D0, perturbation of the cell cycle caused by upregulation of p21 probably prevented the appearance of CD43+ cells, but not CD34+ cells. The mechanisms via which CD34+ cells are blocked by RUNX1b overexpression remain to be elucidated.
9.Survey on the testing accuracy of HBV serological markers among grassroots medical institutions in the demonstration areas of infectious diseases

Objective:To evaluate the detection accuracy of hepatitis B virus (HBV) serological markers among grassroots medical institutions in the demonstration areas of infectious diseases.Methods:A fixed sampling method was used among the followed-up hepatitis B cohort in general population of six infectious disease demonstration areas nationwide. Blood samples of chosen objects were collected, in which HBsAg and anti-HBs were tested by grassroots medical institutions and were also parallely tested by the third-party platform. The test results were compared between the two groups. Statistical analyses were conducted by SAS 9.4 software.Results:A total of 5 756 and 5 263 samples of HBsAg and anti-HBs were successfully tested, respectively. Comparing the results of HBsAg and anti-HBs from grassroots medical institutions with the results from the third platform, the agreement of HBsAg and anti-HBs was 97.13% and 77.33%, respectively. The Kappa value was 0.56 (95% CI 0.50-0.62) for HBsAg and 0.54 (95% CI 0.52-0.56) for anti-HBs, respectively; and the McNemar tests indicated the difference between the results (all P<0.01). There were also significant differences in agreement of testing results with the third platformin among different regions ( P<0.05 or <0.01). The Kappa values indicated that Jiangsu province and Guangdong province had high accordance rates of HBsAg (0.87 and 0.81, respectively), and Gansu province and Guangdong province had high accordance rates of anti-HBs (Both were 0.74). Regarding the results from the third platform as the standard, the sensitivity of HBsAg testing in grassroots medical institutions was moderate (40.51%) and the specificity was well (99.96%). The sensitivity of anti-HBs testing was substantial (73.18%) and the specificity was well (84.31%). Guangdong province (Youden index: 0.69) and Jiangsu province (Youden index: 0.80) had high identification ability for HBsAg, and for indicator of anti-HBs, Gansu province (Youden index: 0.78) and Guangdong province (Youden index: 0.76) had high identification ability. Conclusion:There are certain differences in results of HBV serological markers tests between the grassroots medical institutions in the demonstration areas of infectious diseases and the third platform. Current testing strategies in grassroots medical institutions are suitable for identifying people without hepatitis B infection, while it is necessary to pay attention to the situation of potential false negative error.
10.Optimization of Extraction Process of Gastropathy Oral Thick Paste 1 by Orthogonal Design Based on Analytic Hierarchy Process Method–Entropy Weight Method-Independent Weight Method

Abstract
OBJECTIVE To determine the index weight, optimize the extraction process of gastropathy oral thick paste 1. METHODS Taking the amount of water added, extraction time and extraction times as factors, depend on the single factor experiment, L9(34) orthogonal table was selected to design the experiment. The content of chlorogenic acid was determined by HPLC and the content of total flavonoids content was determined by ultraviolet spectrophotometry. With the above two contents plus the yield of dry extractum as the evaluation index, the weights were assigned based on the analytic hierarchy process, entropy weight method and independence weight method, and the extraction process was optimized by orthogonal experimental design. RESULTS Multi-index scoring was more scientific and reasonable, and the final optimal extraction conditions for gastropathy oral thick paste 1 were as follows: add 8 times the amount of water, extract twice and 0.5 h for each time. CONCLUSION The optimized extraction process using analytic hierarchy process, entropy weight method and independence weight method is reasonable, feasible, stable, and reproducible, which is suitable for the preparation of gastropathy oral thick paste 1, providing reference for subsequent research and development.
