1.Comparison of pharmacokinetics characteristics of vancomycin in cerebrospinal fluid after administration by continuous and interim intravenous infusion

Objective To compare the difference in pharmacokinetics characteristics of vancomycin in cerebrospinal fluid between administration by continuous infusion and interim infusion.Methods Twenty postoperative patients in the Department of Neurosurgery of Beijing Tiantan Hospital, Capital Medical University admitted into intensive care unit (ICU) to receive vancomycin for prophylaxis of intracranial infection were enrolled, and they were randomly distributed to a continuous intravenous infusion group and a interim intravenous infusion group, each group 10 cases. In continuous intravenous infusion group, the patients received a loading dose of vancomycin (15 mg/kg) by continuous intravenous pump infusion for 1 - 2 hours followed by 30 mg/kg vancomycin in a constant pump infusion rate for 24 hours; while in interim intravenous infusion group, the patients received 15 mg/kg vancomycin administered by intravenous pump infusion for 1 - 2 hours, once every 12 hours. The concentration of vancomycin in the cerebrospinal fluid at different time points was measured by two-dimensional liquid chromatography (2D-LC) method, the parameters of pharmacokinetics were calculated in the two groups, and the adverse reaction was observed.Results The comparison between the ratio of areas under the concentration-time curves (AUC) and minimum inhibitory concentration (MIC) of the continuous and interim groups showed no significant difference (19.7±14.0 vs. 16.1±6.4,P > 0.05). However, in the continuous intravenous infusion group, the drug concentration reached the peak value (0.96± 0.77)μg/mL at 12 hours, and later revealed a plateau concentration 0.91-0.93μg/mL for 12 hours; while in the intravenous infusion interim group, the drug concentration reached the peak value (0.92±0.47)μg/mL at 16 hours, in the later 2 hours declined to (0.84±0.45)μg/mL, and afterwards still had a tendency of persistent declination. In all the patients, no any adverse reaction related to the drug occurred.Conclusion Continuous intravenous infusion and interim intravenous infusion of vancomycin for the postoperative neurosurgical patients without intracranial infection have the similar efficacy of medication, but the former can achieve the peak concentration faster and later the fluctuation of drug concentration in cerebrospinal fluid is smaller than those in the latter.
2.Study on the HPLC Fingerprint of Anemone raddeana

OBJECTIVE:To establish the HPLC fingerprint for Anemone raddeana. METHODS:HPLC was performed on the column of Phemomenex Gemini C18 with mobile phase of 0.1%phosphoric acid-acetonitrile(gradient elution)at a flow rate of 1 ml/min,the detection wavelength was 206 nm,the column temperature was 30℃,and the injection volume was 20μl. With the refer-ence of raddeanin A,13 batches of A. raddeana were analyzed,chromatographic fingerprint similarity evaluation system software was conducted for similarity analysis,and SPSS 13.0 was conducted for cluster analysis. RESULTS:There were 11 common peaks in the 13 batches of A. raddeana with similarity of higher than 0.90. According to the verification,the fingerprint and control fin-gerprint shows good consistency. The drugs in Huadian,Jiaohe, Tiangang,Shulan,Tonghua and Fusong of Jilin and Shangzhi of Heilongjiang were regarded as category 1,and in Harbin,Yabuli town and Yimianpo of Heilongjiang,Qingyuan of Liaoning,Ji-nan of Shandong were category 2. CONCLUSIONS:The established fingerprint can provide reference for the identification and quality evaluation of A. raddeana.
3.The effect of Tanshinone IIA on the expression of NF-?B、I?B-? and the mRNA of ICAM-1 and VCAM-1 of ECV304 induced by TNF-?

Aim To illustrate the role of Tanshinone IIA(TanIIA)extracted from dan-shen root in atherosclerosis and its different targets by investigating the effect on the expression of NF-?B、I?B-? and the mRNA of ICAM-1 and VCAM-1 in human umbilical vascular endothelial cell 304(ECV304).Mehods The TNF-?-induced cultured ECV-304 cell injury model was established to analyze the expression of NF-?B、I?B-? by cell ELISA method and the mRNA of ICAM-1 and VCAM-1 by RT-PCR method of the cells cultured with different denses of TanIIA.Results The results of ELISA method showed that the expression of the NF-?B was reduced significantly and the expression of the I?B-? was enhanced significantly in ECV304 which were cultured with higher denses of TanIIA.The results of RT-PCR method showed that the mRNA of ICAM-1 and VCAM-1 induced by TNF-? was significantly refrained.Conclusions TanIIA has a significant effect on inhibiting the mRNA of ICAM-1 and VCAM-1 induced by the NF-?B activation,so it can play an impotant role in resisting atherosclerosis.
4.ULTRASTRUCTURE OF THE INTRAMURAL LYMPH CAPILLARIES OF THE STOMACH IN THE RAT

The distribution of the vesicles was as following: 29.1% opened onto or touched the abluminal membrane, 44,9% without visible connection to either surfaces, 26% opened onto or touched the luminal membrane. It was concluded that the mechanism of lymph formation in the stomach seems to be different from that of those organs such as heart, liver, kidney and thyroid. The present study suggested that the intercellular pathway may act as the primary route for lymph formation and macromolecule transportation, and the intracytoplasmic vesicular system may also play certain role in the transport of macromolecules.

6.Progress in the study of some important natural bioactive cyclopeptides.

Natural cyclopeptides are hot spots in chemical and pharmaceutical fields because of the wide spreading bio-resources, complex molecular structures and various bioactivities. Bio-producers of cyclopeptides distribute over almost every kingdom from bacteria to plants and animals. Many cyclopeptides contain non-coded amino acids and non-pepditic bonds. Most exciting characteristic of cyclopeptides is a range of interesting bioactivities such as antibiotics gramicidin-S (2), vancomycin (3) and daptomycin (4), immunosuppressive cyclosporin-A (1) and astin-C (8), and anti-tumor aplidine (5), RA-V (6) and RA-VII (7). Compounds 1-4 are being used in clinics; compounds 5-8 are in the stages of clinical trial or as a candidate for drug research. In this review, the progress in chemical and bioactive studies on these important natural bioactive cyclopeptides 1-8 are introduced, mainly including discovery, bioactivity, mechanism, QSAR and synthesis.
7.Association of HLA-Cw alleles with systemic lupus erythematosus

Objective To explore the association between HLA-Cw alleles with systemic lupuserythematosus. Methods Polymerase chain reaction-sequence specific primer method was used to analyze thedistribution of HLA-Cw01-08 alleles among 108 patients with SLE and 102 healthy controls. The allelefrequencies was compared between various patient groups and the control population. Results The frequencyof HLA-Cw07 alleles in patients with SLE was significantly increased in patients with SLE. Conclusion Theresults indicate that HLA-Cw07 may be the susceptible alleles or may be closely linked to the susceptiblegenes for SLE.
8.Determination of Tigecycline in Human Plasma by LC-MS/MS and Its Clinical Application

OBJECTIVE:To establish the method for the determination of tigecycline (TGC) in human plasma. METHODS:After precipitated by acetonitrile,the plasma sample was determined by LC-MS/MS. Using d9-TGC as internal standard,Kromasil C18 column was used with mobile phase consisted of water (containing 0.05% TFA)-acetonitrile (gradient elution) at flow rate of 0.6 ml/min,column temperature of 40 ℃. The ion transitions were performed under ESI positive MRM model at m/z 586.3→513.2 and m/z 595.3→514.3 for TGC and internal standard,respectively. RESULTS:The linear range of TGC was 25-2 000 ng/ml (r=0.999 8),and lowest quantification limit was 25 ng/ml;intra-day and inter-day RSD was 3.15%-7.23%,and relative error was-4.53%-10.48%. Plasma sample kept stable after 3 times of freezing and thawing cycle,at room temperature for 24 h,in automat-ic sample injector for 24 h and freezing for 42 d (RSD<15%). Plasma concentration of TGC was 0-438.0 ng/ml in one patient with pan-drug resistant bacteria infection(0-12 h after administration). CONCLUSIONS:The developed method is accurate,sensi-tive and specific,and can be used for plasma concentration determination of TGC and pharmacokinetic study.
9.Clinical Features and Characteristics of Coronary Artery Lesion between Hui and Han Nationality Young Patient with Acute Myocardial Infarction

Objective To investigate the clinical features and characteristics of coronary artery lesion between Hui and Han nationality young pa?tient with acute myocardial infarction(AMI)who were referred to the affiliated hospital of Ningxia Medical University. Methods A total of 189 con?secutive AMI young patients(age≤44 years)who underwent coronary angiography were retrospectively retrieved from the database.Those patients with AMI were divided into Hui group(46 cases)and Han group(143 cases). The clinical features and results of coronary angiogram were com?pared between the two group. Results Compared with Han group,Hui group are more younger than Han group,high prevalence rate of diabetes, lower smoking history and lower drinking history(P<0.05). Coronary angiography showed the incidence of three?vessel lesions was significant low?er in Han group than in Hui group(P<0.05). Both group showed single vessel was the most common lesion. Conclusion Hui nationality patients with acute myocardial infraction are more younger and are are more prone to suffering from diabetes history、lower smoking history and lower drinking history than Han nationality patients. The coronary artery lesions of Hui nationality patients with acute acute myocardial infraction are more three?branch lesions than Han nationality patients.
10.Pre- and postoperative changes of regional cortical cerebral blood flow in patients with cerebral arteriovenous malformation.

OBJECTIVETo investigate pre- and postoperative changes of regional cerebral cortical blood flow in patients with cerebral arteriovenous malformation.
METHODTwenty-two adult patients with arteriovenous malformation (AVM) were recruited into this study at Beijing Tiantan Hospital from September 2001 to May 2002. Eight patients had giant cerebral AVM and the other 14 had a small one. Cortical cerebral blood flow (CBF) was measured by laser Doppler flowmetry (LDF) before and after AVM resections. After surgery, the probe of LDF was implanted adjacent to the area of AVM and monitored for 24 hours.
RESULTSCBF increased significantly after the resection in all patients regardless of AVM size. In patients with small AVM, CBF returned to the baseline level within 4 hours, but in patients with giant AVM, CBF remained high even after 24 hours.
CONCLUSIONSMonitoring CBF is helpful to understand pre- and postoperative changes of regional cortical CBF in patients with cerebral AVM.
