1.The influence factors and meanings of tracheotomy after cervical spinal cord injury.

Cervical spinal cord injury is a common and serious disease in clinic, and tracheotomy combined with mechanical ventilation is an effective way to prevent respiratory complications. Although tracheotomy is used widely, there are not unified indications of tracheotomy after cervical spinal cord injury in the practical application. At the same time, the advantages and disadvantages of the application of tracheotomy in patients with cervical spinal cord injury are still on dispute. Based on the recent literature, we summarize the influence factors and meanings of tracheotomy after cervical spinal cord injury.

2.Clinical analysis for patients with severe chronic emphysema treated by lung volume reduction surgery

Objective To study effectiveness and safety of lung volume reduction surgery(LVRS) in treatment for severe chronic emphysema(chronic obstructive pulmonary disease).Methods Clinical data of 24 patients of severe chronic emphysema undergone with LVRS during January 2004 to June 2007 were analyzed retrospectively.LVRS was performed for the patients after respiratory tract preparation based on their pulmonary function,results of blood gas analysis,cardiac function,as well as physical activity capacity.and surgical incision Was selected based on"target"location of emphysema with chest computerized tomography and isotope lung perfusion scanning,22 cases with standard unilateral LVRS via outer post-lateral incision and two with bilateral LVRS via mid-sternal incison.A linear device for cutting and stitching was used in surgical operation to excise the lung tissues of severe emphysema with strips of bovine pericardium to prevent air leakage.Results All the patients were followed-up for 19 months in average and complications occurred in seven of them after operation,including four with leakage of the alveoli.two with cardiac arrhythmia and one with pneumonia,and no death was observed.Pulmonary function and symptoms of dyspnea improved obviously in all the patients after operation,and they all could care for themselves,in general,with scales of dyspnea increased to grade Ⅰ in one case,to grade Ⅱ in 10 cases and to grade Ⅲ in three cases.Conclusions LVRS can improve pulmonary function of selected patients with severe emphysema,to certain extent,and its safety will depend on strict selection of the patients and correct peri-operative care.
3.PROPHYLAXIS OF INFECTIONS IN PATIENTS WITH CHRONIC RENAL FAILURE

Infection is a common, serious and costly complication in patients with chronic renal failure (CRF) and has been the major cause of high mortality in these patients. Increased evidences suggest that prophylaxis can significantly decrease the prevalence of infections in CRF patients, such as tuberculosis, S. aureus infection and hepatitis virus induced liver diseases. It remains an important issue for clinical nephrologists to investigate and to provide strategies for prevention and treatment of various infections in CRF patients.
4.Surgical Treatment and Prognostic Analysis in Patients with Malignant Thymoma

Objective: To observe the clinical outcome of surgical treatment for malignant thymoma,and evaluate the prognostic factors thereof. Mothods:All patients (63 eases)were operated and followed by radiotherapy. The Logistie regression analysis was used for the relationship between the prognosis, Masaoka staging,L/B pathology ,and the differences of the operation given.Rosults:Patients with early Masaoka stage and who received radical resection of thymoma had higher 3 and 5 year survival rates (P<0.05).The radiotherapy after surgery was related to 3 and 5 year survival rates. The differences of L/B pathology was not related to 3 and 5 year survival rates.Conclusion :The survival rate is enhanced for patients with malignant thymoma when both surgical and radiotherapy intervention axe given as early as possible.The Masaoka staging, not L/B pathology, is closely related to the prognosis.
5.Clinical observation on treatment of sudden sensorineural hearing loss with tinnitus with Ginkgo mihuan oral liquid combined with Tinnitus-curing instrument

Objective:To observe the clinical treatment effect of Ginkgo mihuan oral liquid combined with tinnitus-curing instrument on the patients with sudden deafness associated with tinnitus.Methods: One hundred and twenty patients with sudden deafness associated with tinnitus were randomly divided into treatment group and control group one and control group two. Treatment group were treated with Ginkgo mihuan oral liquid combined with Tinnitus-curing instrument. The patients in control group were given Ginkgo mihuan oral liquid, the other control group used tinnitus-curing instrument. Observe the tinnitus and hearing improved progress.Results: In treatment group, the effective rate of tinnitus improvement was higher than that in control group, the differences were statistically significant(x2=7.040,x2=5.952;P<0.05), but the effective rate of hearing improvement was not higher than that in control group, the differences was not statistically significant(x2=0.621,x2=2.813;P<0.05).Conclusion: Ginkgo mihuan oral liquid combined with Tinnitus-curing instrument has certain curative effect on tinnitus recovery of patients with sudden hearing loss.
7.Targeting angiogenesis and vascular remodeling as a novel therapeutic approach to liver fibrosis.

Development of liver fibrosis is closely associated with angiogenesis and abnormal vascular remodeling. Recent studies have highlighted the importance of angiogenesis and vascular remodeling in fibrogenesis, the results that inhibition of angiogenesis is effective in suppression of liver fibrosis demonstrate that therapies with several molecular targets against angiogenesis, inflammation and fibrosis might be beneficial for the treatment of cirrhosis. However, there is some evidence that inhibition of angiogenesis can even worsen fibrosis. This article outlines recent advances regarding the interplay between inflammation, angiogenesis and fibrogenesis in terms of cellular and molecular mechanisms, and suggests a requirement of greater understanding to intervene in these key processes, such as liver sinusoidal endothelial cell fenestration and impact distinct chemokine actions driving monocyte migration and differentiation, for therapeutic benefit in the future.

8.APOPTOSIS INDUCED BY CYCLOSPORINE A IN RENAL TUBULAR AND INTERSTITIAL CELLS AND THE PROTECTIVE EFFECTS OF ENALAPRIL AND LOSARTAN ON IT

The rats were fed on low salt diet for seven days,and then were randomly divided into five groups: control (n=7), CsA treated (n=7), CsA+Verapamil (n=7), CsA+Enalapril (n=7) and CsA+Losartan (n=7).CsA was given by subcutaneous injection (15mg?kg -1?d -1) for four weeks. The apoptosis of tubular and interstitial cells was detected by TUNEL assay. The proliferating cell nuclear antigen and Fas antigen expression were examined by immunohistochemical staining. Apoptosis of tubular and interstitial cells increased in CsA treated rats. 9.CLINICAL CHARACTERISTICS OF PATIENTS WITH POST-TRAUMATIC ACUTE RENAL FAILURE

To investigate the clinical characteristics of patients with post traumatic acute renal failure (ARF)effect of early renal replacement therapy on their propnosis, a case control study was carried out in 71 cases of post traumatic ARF and 40 cases of post operative ARF. The effect of early renal replacement therapy on prognosis was evaluated. The incidence of post traumatic ARF in traumatic patients was 1 06%, and 16 21% in ARF. Rhabomyolysis and hypovolemia were the most common causes of ARF. Respiratory failure and coma were the predisposing factors of ARF. The incidence of multiple organ failure was 85 9% and the mortality was 70 4%. Early renal replacement therapy might improve the prognosis. The mortality of patients with Post traumatic ARF remains high, renal replacement therapy should be given as early as possible.
10.Clinical characteristics and prognosis in patients with acute renal failure due to sepsis: a multivariate analysis

Objective To evaluate the clinical characteristics and prognostic factors in patients with acute renal failure (ARF) due to sepsis. Methods A case-control retrospective study was carried out in patients with acute renal failure due to sepsis and non-sepsis. Univariate and multivariate logistic regression analysis was performed to find the correlation among prognosis, clinico-biochemical parameters and scoring indexes of APACHE II and ATN-ISI. Results The incidence of septic ARF was 15. 6% in all ARF patients. Multiple organ failure was most common (87.9% ) in these patients with a high mortality (69. 7% ). Single-variable analysis showed that surgery, respiratory failure, hepatic failure, mechanical ventilation, oliguria, coma, multiple organ failure, ARF occurred during ICU stay and chronic health points were factors influencing prognosis. Multivariate logistic regression analysis showed that oliguria, ARF occurred during ICU stay, chronic health points, and multiple organ failure were the independent predictors of hospital mortality. Conclusion Outcome of septic ARF is correlated with factors including oliguria, ARF occurred during ICU stay, chronic health points and multiple organ failure.
