1.Changes of axillary lymph node status in breast cancer after neoadjuvant chemotherapy

Objective To evaluate effects of neoadjuvant chemotherapy on axillary lymph node status in patients with breast cancer.Methods A total of 45 patients with stage Ⅱ or Ⅲ breast cancer treated with neoadjuvant chemotherapy followed by surgery(Combination Group) were compared with 79 patients with breast cancer treated with initial surgery only(Surgery Group) in respect of axillary lymph node status,including the total number of the lymph nodes and the number of positive and negative lymph nodes,according to findings of physical examination,B-ultrasonography,and molybdenum target radiography.Results The total number of the lymph nodes and the number of positive lymph nodes were significantly less in the Combination Group(16.9?5.9 and 2.5?2.2) than in the Surgery Group(20.8?8.0 and 3.9?3.0)(t=-2.856,P=0.005;t=2.790,P=0.006),whereas the number of negative lymph nodes were 14.4?5.4 and 16.7?7.0 in the Combination Group and the Surgery Group,respectively,without statistical significance(t=-1.904,P=0.055).Four patients were found loco-regional relapse in each group during follow-up checkups for 6~19 months(mean,10 months) in 40 patients in the Combination Group and 7~21 months(mean,12 months) in 67 patients in the Surgery Group.Conclusions Total and positive axillary lymph nodes retrieved after axillary lymph node dissection decrease in number after neoadjuvant chemotherapy.
2.Iris pigment epithelial cells of rabbits stained by carboxyfluorescein diacetate succinimidyl ester in vitro

Objective To investigate the feasibility of labeling iris pigment epithelial(IPE)cells of rabbits with 5-(and 6-)carboxyfluorescein diacetate succinimidyl ester(CFSE). Methods Enzyme-assisted microdissection was used to isolate the cultured rabbit′s IPE cells.The third or forth subcultured IPE cells were incubated with 2.5,5,10,20,and 40 ?mol/L of CFSE for 1,5,and10min respectively.The fluorescence intensity was detected by flow cytometry,and the leakage of CFSE and its dyeing were observed by fluorescence antibody labeling. Results Incubation with 20 ?mol/L CFSE under 37℃for1minute was the most optimal condition for IPE cells labeling.The coloration of IPE cells stained by CFSE lasted 4 weeks.There was no leakage of dye from labeled rabbit IPE cells to non-labeled human IPE cells in mixed culture process. Conclusion With the advantages of high rate of dyeing,long time of tracing,safety and convenience,CFSE can be used as a new method to label the rabbit′s IPE cells.
3.Relationship between choroidal neovascularization and macular choroidal watershed zones in patients with age-related macular degeneration

ObjectiveTo investigate the possible relationship between choroidal neovascularization (CNV) and macular choroidal watershed zones (CWZ) in patients with age-related macular degeneration (AMD). MethodsFifty-seven selected indocyanine green video angiograms (ICGA) of 57 patients (57 eyes) with AMD were evaluated, and 35 ones of the healthy fellow eyes of 35 patients with unilateral non-AMD fundus diseases were selected as age-matched control. The video angiograms were evaluated for investigating the relationship between CNV and macular CWZ. 4.Multi-factor analysis of the effects on visual acuity prognosis of photodynamic therapy for choroidal neovascularization

Objective To investigate the related factors of effects on distant visual acuity after photodynamic therapy (PDT) for choroidal neovascularization (CNV). Methods One hundred and thirty-five cases (135 eyes) of CNV treated with PDT were observed. The gender, preoperative distant and near visual acuity, disease course, pathogeny, area of CNV, types of CNV ascertained by fundus fluorescein angiography (FFA), and changes of CNV in FFA were recorded. Multi-factor regression analysis of visual acuity within 1 month and 3 months after PDT was performed with SPSS statistics software. Results The distant visual acuity within 1 month postoperatively was related to the preoperative distant visual acuity, the area of CNV and the changes in the FFA(P=0.000,0.030,0.062), and 3 months after PDT, it was related to the distant and near visual acuity preoperatively and the changes in the FFA(P=0.000,0.054,0.034). The condition of distant visual acuity within 1 month postoperatively was related to the FFA type of CNV and the disease course(P =0.018,0.08). Conclusion The smaller the area of CNV is, the better postoperative distant visual acuity would be. The proportion of improvement of visual acuity is relatively higher in patients with classic CNV. Early treatment for the patients with the indicatio may improve the visual acuity effectively.
5.Nursing experience of patients undergoing liver biopsy after liver transplantation

ObjectiveTo summarize the nursing experience of postoperative liver biopsy in patients after liver transplantation. Methods89 patients undergoing postoperative liver biopsy after liver transplantation from June 2010 to April 2011 were retrospectively analyzed.Their nursing experience was summarized. Results81 patients were confirmed of having rejection reaction.89 patients had no severe complications,only 28 cases(31.5%)had mild local pain. ConclusionsSufficient preoperative preparation,good nursing cooperation during the operation,close postoperative observation and nursing can prevent and timely detect and cope complications,thereby reduce the risk of liver puncture.

8.Correlative factors of cognitive impairment in patients with lacunar infarcts

Objective To study the relationships between cognitive impairment in patients with lacunar infarcts and quantitative CT measures and to determine the independent correlative factors of cognitive impairment. Methods Neuropsychological examination was conducted for 128 patients with acute lacunar infarct. Number, location, and volume of infarcts, cerebral atrophy index and severity of white matter lesions (WMLs) were measured and recorded. Results The number of lacunar infarcts in cognitive impairment (CI) group was significantly larger than that in cognitive normal (CN) group. Mean width of sulcus and sylvian fissure, index of frontal horn and ventricular-brain ratio (VBR) were significantly different in both groups. There were more patients with 3 grades or 4 grades WMLs in CI group (62%) than those in CN group (22%). The total volume of lacunar infarcts showed no statistically significant difference. Logistic regression analysis indicated that the number of lacunar infarcts in frontal subcortex and thalamus, the volume of infarcts in anterior periventricular white matter, width of cerebral sulcus and sylvian fissure were correlated with cognitive impairment respectively. Additionally, age and education were correlative factors of cognitive impairment in patients with lacunar infarct. Conclusion Correlative factors of cognitive impairment in patients with lacunar infarct are not merely one feature, but a combination of infarct features (number, location, and volume), cortical atrophy and host factors (age and education).
9.Treatment of symptomatic cerebral artery stenosis and occlusion with Xenon CT guided cerebral revascularization

Objective To investigate the roles of Xenon-CT cerebral blood flow perfusion imaging in cerebral revascularization before surgery and efficacy evaluation.Methods The clinical data of 15 patients with symptomatic cerebral artery stenosis/occlusion of anterior circulation were analyzed retrospectively.Eight patients were treated with endovascular stenting,1 was treated with internal carotid endarterectomy,and 6 were treated with superficial temporal artery-middle cerebral artery bypass grafting.The regional cerebral blood flow (rCBF) detected by Xenon CT within 2 weeks before and after procedure and the modified Ranking scale (mRS) scores at 6 months after procedure were compared.Results (1) The mean rCBF value of 12 patients with abnormal blood perfusion of target vessels before procedure was 30±10 ml/(100 g·min) and that was 32±14 ml/(100 g·min) after procedure.Compared with before procedure,the difference was statistically significant (P<0.05).The mean rCBF value of 3 patients with normal blood perfusion of target vessels before procedure was 48±6 ml/(100 g·min) and that was 50±7 ml/(100 g·min) after procedure.Compared with before procedure,the difference was not statistically significant (P>0.05).(2) The postoperative mRS score was decreased in 8 cases and stable in 7 cases.Compared with before procedure,there were significant differences in mRS scores after procedure in 15 cases (P<0.05).During the follow-up period,none of the patients had new neurological impairment.Conclusion Revascularization can improve the presence of hemodynamic disorders in patients with symptomatic anterior circulation cerebral artery stenosis or occlusion of the target blood vessels in the distal local cerebral perfusion and neurological deficit symptoms.The patients with abnormal perfusion of preoperative Xenon-CT cerebral blood flow perfusion imaging may be more beneficial than those with normal perfusion.
10.The use of short dental implants in rigid fixed bridge for multiple missing teeth:a 2-year follow-up visit

BACKGROUND: Short dental implants have been widely used in the posterior area when one tooth loses. How effective is it when we use the short implants in the rigid fixed bridge for the multiple teeth lost area?OBJECTIVE: To observe the clinical effect of short dental implants used in the rigid fixed bridge.METHODS: Forty-three patients with multiple missing teeth were enrolled and underwent the rigid fixed bridge supported by short dental implants, including 26 males and 17 females, with an age range of 44-67 years. After 2 years of dental implantation, the success rate of dental implants, amount of alveolar bone loss and dental implant-related complication were followed up.RESULTS AND CONCLUSION: A total of 92 short dental implants, 6-8 mm in length, were implanted,including 44 in the maxilla and 48 in the mandible. There were two cases of peri-implantitis and the success rate of implants was 97.8%. There was no significant difference in the amount of alveolar bone loss between the 8 mm, 7 mm and 6 mm length implants as well as between the C/I ≤ 1, 1 < C/I ≤2, C/I > 2 groups at 1-2 years after implantation. There were four cases of central screw loose and one case of porcelain fracture after restoration. To conclude, the clinical use of short dental implants in the rigid fixed bridge has achieved good outcomes during the short period, but long-term observation is needed.
