1.THE USE OF MICROCOMPUTER IN THE DIAGNOSIS OF INTRACRANIAL TUMOR

This is a preliminary report using APPLE I type microcomputer to diagnose the nature of intra-cranial tumor (ICT). A computer program was made. All clinical data of 520 cases of intracranial tumors proved by operation and pathology were put into the computer for analysis. The cases were di vided into two groups: the 1st 120 cases were used to make a stepwise regression analysis to get a multiple linear regression equation; and the 2nd 400 cases were tested to prove the reliability of the e quation and diagnose the nature of ICT. The reliability was 85.6%.From the preliminary study, the formula that was derived for clinical purpose to evaluate the nature of ICT depends upon 7 dependent variables. It may be a supplement to evaluate the nature of ICT in early stage.
2.BASIC AND CLINICAL RESEARCHES ON SEVERE HEAD INJURY

Severe head injury is the main cause of disability and death. And it is one of the most concerned topics in military medical research. A great deal of achievements involving basic and clinical researches on the severe head injury have been made in recent years. Many animal models of head injury have been established.The mechanisms of brain edema, secondary brain and diffuse axonal injury have been preliminary elucidated,broadening the understanding in the mechanism of repair and clinical treatment. We analyed and summarized these achievements retrospectively in this article, the aim of which is to improve the clinical treatment result for severe head injury.

6.Influence of post-discharge continuous nursing intervention on psychological quality and compliance with immunosuppressants treatment in patients with renal transplantation

Objective To explore the effects of postdischarge follow-up continuous nursing intervention on the psychological quality and compliance of immunosuppressants treatment in patients with kidney transplantation.Methods 157 patients with kidney transplantation post-operation were recruited,81 cases in the study group and 76 cases in the control group randomly.The control group received routine hospital discharge care and immunosuppressants treatment.The study group received postdischarge followup nursing intervention and immunosuppressants treatment.After 6 months of postdischarge intervention,the psychological quality of patients in two groups was assessed by the emotion application measure scale separately.The Morisky Medication Adherence Scale 8-item version (MMAS 8-item version) was used for the assessment of the immunosuppressants treatment compliance of the two groups of patients.Results After 6-month intervention,the patients in the study group showed better psychological quality than the patients in the control groups.There were significant differences in the various items of assessments of the psychological quality between the two groups.The high compliance percentage of immunosuppressants treatment of the study group was 45.7%,whereas the high compliance percentage of the control group was 22.4%.The differences between the two groups were significant.Conclusions Post-discharge follow-up nursing intervention to patients with kidney tranplantantion is effective for improving the psychological quality and the immunosuppressants treatment compliance and may be helpful to improve the life quality of the patients.
7.Bone marrow mesenchymal stem cells for osteoarthritis:its possibility and future

BACKGROUND:Nowadays treatment of osteoarthritis with drugs is not ideal. In recent years, more and more scientists try to use stem cells to treat osteoarthritis.
OBJECTIVE:To explore the stem celltreatment for osteoarthritis in order to promote its clinical application and find out its difficulties.
METHODS:A computer-based online search of PubMed database and CNKI database between March 1998 and October 2013 was performed to search related articles with the key words of“stem cell, osteoarthritis, bone metabolism, bone marrow mesenchymal stem cells”in English or in Chinese, respectively. The word“AND”was used for the connection between the word retrieval. Literatures related to stem cells treatment for osteoarthritis were selected;in the same field, the articles published lately in authoritative journals were preferred.
RESULTS AND CONCLUSION:A total of 79 literatures were primarily selected, and 50 documents were involved for summary according to inclusion criteria. The stem celltreatment for osteoarthritis is realized mainly by promoting the repair of cartilage tissue. Commonly used methods are stem celltransplantation and induced differentiation of stem cells. Stem celltreatment has broad application prospects for the treatment of osteoarthritis.
8.Effects of p38 Mitogen-Activated Protein Kinase on Apoptosis of Small Intestinal Epithelial Cells after Transplantation in Rats

Objective To explore the effects of p38 mitogen-activated protein kinase (MAPK) on apoptosis of small intestinal epithelial cells after transplantation in rats. Methods Small intestinal transplantation was performed in SD and Wistar rats. The recipients were divided into three groups: isograft group (Wistar→Wistar group), allograft group (SD→Wistar group) and allograft+cyclosporine A group (SD→Wistar+CsA group). The grafts were harvested on day 1, 3, 5 and 7 after operation. All graft samples were subjected to histological examination. The apoptosis of graft epithelial cells was detected by TUNEL method. p38 MAPK was measured by Western-blotting method and serum TNF-? was determined by ELISA. Results Mild, moderate and severe rejection reaction occurred in the SD→Wistar group, it was showed that the number of apoptotic cells increased with the severity of the rejection reaction by TUNEL. In SD→Wistar group, the numbers of apoptotic cells were significantly higher than those of the other two groups (P0.05). 9.The reasons of internal fixation failure for subtrochanteric fractures of the femur

Objective To analyze the reasons of internal fixation failure for subtrochanteric fractures of the femur and indicate the methods for avoiding the complications. Methods Thirty-nine patients with internal fixation failure for subtrochanteric fractures of the femur were treated from January 1993 to January 2004. There are 34 males and 5 females with an average age of 37 years (range from 21 to 65 years). The causes of injury included falls from bicycle in 3 cases, pedestrain injury in 1 case, traffic accident in 28 cases and falls from heights in 7 cases. The pattern of the fractures included Seinsheimer ⅡA type in 5 cases,ⅡB type in 2 cases,ⅡC type in 1 case,ⅢA type in 18 cases,ⅢB type in 1 case,Ⅳ type in 3 cases,Ⅴ type in 9 cases. The initial operation included open reduction and fixed with cerclage wires in 1 case, open reduction and fixed with Kuntscher nail and cerclage wire in 13 cases, closed reduction and fixed with Russell-Taylor reconstruction nail in 1 case, open reduction and fixed with 135? DHS in 6 cases, open reduction and fixed with plate in 18 cases. Two cases failed in 8 weeks postoperation, 26 cases failed in 6 months postoperation and 11 cases failed over 6 months. The pattern of failure of internal fixation include varus deformity of fracture in 35 cases, cutout of the lag screw from the femoral head in 2 cases and over medial migration of the distal femoral shaft in 2 cases. The reasons of internal fixation failure were nonanatomically reduction of fractures, unrigid interal fixation, over early weight bearing and poor placement of lag screw. Results The internal fixtion of fractures were revised, including open reduction and fixed with kuntscher nail in 2 cases, fixed with interlocking nail in 23 cases, fixed with 135? DHS in 6 cases, fix with plate in 4 cases, total hip replacement in 4 cases. The mean duration of follow up was 23 months (range from 19 to 37 months). The average time of fracture healing was 11.5 months (range from 11 to 13 months). Conclusion There are higher bending loads at the subtrochanteric area of the femur due to the eccentric loading. If extramedullary implant is selected, the stability at the medial cortex of the femur must be reconstructed intraoperatively and bone grafting is required.
10.Surgical treatment for esophageal cancer:current status and future directions

Carcinoma of the esophagus is the second most common cause of cancer-related-death in women,and fourth in men in China.The prevalence rate of adenocarcinoma of esophagus has increased in the western countries recently too.Unfortunately,the treatment result of esophageal cancer is very poor,and only 15%~20% of all esophageal cancer patients will be alive 5 years after diagnosis.With the development of radiologic techniques and molecular biology,the esophageal cancer patients are given on individualized treatment modality.This paper reviewed several interesting topics,including operation for esophageal cancer,new technologies for pre-operative staging,multidisciplinary treatment as well some data from the Shanghai Cancer Hospital.
