OBJECTIVE To explore the situation of bacteriological examination in every clinical department,know distribution and drug resistance of pathogenic bacteria,and offer the help about diagnosis and reasonable using antibiotics.METHODS Using the Microscan A/S-4 to identify the microbes and microbial sensitivity tests according to pathogens from more than twenties clinical departments between 2004 and 2005.RESULTS The clinical delivering rate was low,and samples were dispersive.The samples mostly were sputum,pus,urine and blood,and their rate was 52.15%.The first three pathogenic bacteria were totally 13 kinds of species in all kinds of departments.The main pathogenic bacteria in the internal medicine such as in the department of respiration,department of gerontology,department of endocrinology and so on were mostly Klebsiella pneumonia and Escherichia coli.The main pathogenic bacteria in department of neurosurgery,ICU,department of burn and department of trauma were mostly Pseudomonas aeruginosa,Acinetobacter baumannii/haemolyticus,Enterobacter cloacae and Staphylococcus aureus,These bacteria might lead to nosocomial infections in our hospital too.Their rate was 52% and drug-resistance was serious.A.baumannii/haemolyticus and E.cloacae were only sensitive to imipenem.P.aeruginosa was most sensitive to piperacillin/tazobactem and next to ceftazidime,amikacin and ciprofloxacin.CONCLUSIONS There are the serious phenomena that P.aeruginosa,A.baumannii/haemolyticus and E.cloacae are highly resistant to many antibiotics.We will much more attend and study it.The clinics must attend it and improve the delivering rates of cultured sample.

Objective To observe the characteristic variation of the patients' inner and outer retina who had chronic central serous chorioretinopathy (CSC) after being treated of photodynamic therapy (PDT).Methods Nineteen patients with chronic CSC were recruited,including 15 eye of men and 4 eye of women,logMAR BCVA was 0.1-1.0,0.39 ± 0.30.Meanwhile,24 healthy people were located in the control group.All the patients received PDT for the first time.All subjects including 24 healthy people underwent fourier domain optical coherence tomography (FD-OCT).Retinal thickness were investigated before PDT and 1,4,12,20 weeks after PDT respectively.Data were recorded including inner layer and outer layer.Retinal thickness were compared in fovea (1 mm),parafovea (3 mm)and perifovea(5 mm).Paired-samples t test was used to compare retinal thickness before and after PDT.The statistical differences of patients and control group were evaluated by independent-samples t test.The correlations between the best logMAR corrected visual acuity (BCVA) was analyzed by Pearson statistical analyses.Results The inner(F=13.814,10.095,4.689) and outer(F＝ 9.354,5.878,3.978) layer fovea thickness of CSC subjects in 1,4,12 week was thinner,the difference was statistically significant (P＜0.05).The outer layer fovea thickness at P12 (t =-3.725),parafovea of inner and outer retinal (t =-3.198,-2.722) was reduced when compared with control group,and differences have statistical sense,respectively (P＜0.05).There was correlation between logMAR BCVA and outer retinal thickness in fovea and parafovea (r =0.465,-0.728,-0.687; P＜0.05).Conclusion In our study,the inner and outer layer retinal thickness decreased generally after the first time PDT in CSC patients.

Objective In order to elucidate neurotoxic actions of toluene in the air of submarine, the effect of toluene on biochemical alteration in the brain was studied in rat. Methods Adult healthy male Sprague-Dawley (SD) rats were randomly grouped into five groups (blank control group, excipient group, low dose group, medium dose group and high dose group). Toluene-exposed rats were administered with toluene in different doses (once a time every other day). Rats of excipient group were given olive oil with the volume as same as toluene. Nothing was given to rats of blank control group. 30 days later, all rats were sacrificed and the hippocampus and cortex were harvested for determination of nitric oxide synthase (NOS), nitric oxide (NO), malondialdehyde (MDA), Na~+-K~+-ATPase, reactive oxygen substances(ROS) and glutamic acid levels. Objective To analyze utilization of antibiotic drugs and resistance rate of gram negative bacteria in the 3rd People's Hospital of Bengbu during 2011-2013,to provide references for clinical rational use of antimicrobial drugs and reduce bacterial resistance.Methods Retrospective investigation method was used to statistically analyze the pathogenic bacteria culture results,antimicrobial cumulative frequency of each categories and results of drug resistance test.Results The total DDDs of antibacterial drugs reduced year by year,down from 255 456 DDD in 2011 to 155 024 DDD in 2013,cephalosporin and enzyme inhibitors and macrolide consumption ranked increased year by year,the three or four generation cephalosporins and quinolones such amount was reduced year by year,antifungal fungi and carbapenem ranking had no obvious change,cefoperazone shubatan,cefuroxime and etimicin ranking and the dosage ratio did not change significantly,the cefathiamidine,cefepime and aztreonam ranking and the dosage ratio was reduced.Bauman acinetobacter and Pseudomonas aeruginosa to piperacillin-tazobactam resistant rate increased year by year,Bauman acinetobacter to beta lactam antibiotics drugs showed a gradual trend of drug resistance.Conclusion The frequency of antimicrobial drug use showed a declining trend in our hospital,and drug-resistant bacteria rate increased,in order to reduce the resistance present form,we need to more rational use antimicrobial agents.

Objective To explore the mechanism of gene Nelin on the differentiation of vascular smooth muscle cells(VSMC).Methods Human VSMCs were stably infected with over-express lentiviral vector [Nelin-VSMC] and interference lentiviral vector [LV-Nelin-SiRNA-VSMC] by Western blotting and RT-PCR.The effect of Nelin expression on phenotypic transition of VSMCs was observed and the mechanism involved was explored.Results The Nelin-VSMC cells take on the contractile phenotype of long slender cells possessing hill-valley characteristics.Overexpression of Nelin can enhance SMoα-actin expression with the increase of total RhoA and nuclear serum response factor(SRF) expression.Treated with a selective Rho kinase inhibitor Y-27632,the expression of SMα-actin and nuclear SRF protein induced by Nelin was effectively reduced.Meanwhile,the opposite results were observed in the Nelin-depression VSMC.Conclusions Nelin can enhance the expression of SMα-actin and carry a pivotal role in the process of VSMCs differentiation and phenotype modulation by activating Rho and SRF.

OBJECTIVE:To probe into the characteristics and regularity of adverse drug reactions(ADR)cases occurred in our hospital.METHODS:535 ADR cases reported in our hospital in 2008 were collected and analyzed statistically.RESULTS:In the incidence of 535 ADR cases,the ratio of male to female were 1.10∶1.Patients over the age of 60 years showed the highest incidence,accounted for 36.82%(n=197).430 ADR cases were induced by intravenous route(80.37%);254 cases were induced by antimicrobials(47.48%),followed by TCM preparations(n=75,14.02%).The ADR were chiefly manifested as lesion of skin and its appendants in 212 cases(39.63%),followed by 164 cases of lesion of digestive system(30.65%).Most ADR cases had a favorable prognosis.CONCLUSION:It should be strengthened to offer education and guidance of ADR monitoring for medical staff so as to make sure the safety,effectiveness and convenience of drug use in the clinic.

Objective:To evaluate the clinical efficacy of paclitaxel combined with epirubicin neoadjuvant chemotherapy in the treatment of triple negative breast cancer and the effect on the Ki-67,p53,P-glycoprotein (P-gp) and glutathione transferase (GST-π).Methods:84 patients with triple-negative breast cancer admitted in our hospital from June 2010 to June 2012 were selected and divided into the observation group and the control group according to the order of admission.The control group was treated with epirubicin,and cyclophosphamide.The observation group was given paclitaxel neoadjuvant chemotherapy combined with epirubicin.The clinical efficacy,expressions of Ki-67,p53,P-gp and GST-π were compared between the two groups.Results:After treatment,the total remission rate of observation group was significantly higher than that of the control group [76.19％(32/42) vs 45.24％(19/42)] (P ＜0.05).Before chemotherapy,the positive expression rates of Ki-67,p53,P-gp and GST-π in the two groups showed no statistical difference(P＞0.05).After chemotherapy,the positive expression rates of Ki-67,p53,P-gp and GST-π in the observation group were significantly lower than those of the control group (P＜0.05),but the positive expression rates of Ki-67,p53,P-gp and GST-π in the control group had no significant difference compared with those before chemotherapy (P＞0.05).The positive expression rates of Ki-67,p53,P-gp and GST-π in the observation group were significantly lower than those of the control group (P ＜0.05).There was no significant difference in the incidence of adverse reaction rate between the observation group and the control group (P＞ 0.05).Conclusion:Paclitaxel combined with Epirubicin neoadjuvant chemotherapy could effectively reduce the expression of Ki-67,p53,P-gp and GST-π in triple-negative breast cancer with exact clinical efficacy.

ObjectiveTo explore the relationship between the expression of smooth muscle 22 alpha ( SM22α),α-smooth muscle actin (α-SMA) and the development of varicose veins of the lower extremities. MethodsWe collected tissues of varicose saphenous vein in the experimental group,and what left over of normal saphenous vein tissues harvested for a coronary bypass operation ( control group).RT-PCR were applied to investigate the expression level of SM22αmRNA and immunohistochemical techniques were applied to investigate the expression level of SM22α and α-SMA protein in normal and varicose veins.ResultsThe expression level of SM22α mRNA in normal and abnormal veins was 0.2614 ±0.1168 and 0.5114 ± 0.1554 respectively ( t =5.020,P ＜ 0.01 ).The immunohistochemical staining showed that signals of SM22α and α-SMA protein existed in the cytoplasm of smooth muscle cells.The integrate optical density (IOD) of SM22α in normal and abnormal veins was 10 055 ± 3584 and 16 226 ± 3378 respectively ( t =4.991,P ＜ 0.01 ).The IOD of α-SMA in normal and abnormal veins was 9746 ± 3903 and 15 371 ± 4318 respectively (t =3.861,P ＜ 0.05 ). ConclusionsSM22α and α-SMA was down-regulated in varicose veins,indicating that the VSMC had transformed the typical contractile phenotype into the typical synthetic phenotype.Phenotype changes of VSMC further promote venous wall remodeling leading to the occurrence of varicose veins.
