2.Study on the effect of chuan kezhi injection atomizing inhalation in the prevention of respiratory infection after general anesthesia postoperative

Objective To investigate the effect of chuan kezhi injection atomizing inhalation in the prevention of respiratory infection after general anesthesia postoperative.MethodsA total of 74 cases with fractures and brain surgery postoperative with intubation anesthesia from Jiaxing First Hospital were collected and randomly divided into two groups with 37 cases in each group.Patients in the control group were treated by routine atomizing inhalation, patients in the experiment group were treated by chuan kezhi injection atomizing inhalation, before and after treatment empty stomach sampled venous blood 5 mL, determined T cell subgroup, observed cough and expectoration eased and throat discomfort alleviated situations of the patients after operation 1 to 4 days, and compared the clinical efficacy.ResultsAfter treatment the effective rate of the experiment group 94.59% was higher than the control group 81.08%, with no statistical significance;the cough and expectoration symptoms eased of the experiment group were significant better than the control group after treatment 1, 2, 3d (P<0.05), the throat discomfort symptoms eased of the experiment group was significant better than the control group at treatment 3 d (P<0.05), after treatment the CD3+, CD4+/CD8+ and NK cell levels of the experiment group were higher than the control group(P<0.05).ConclusionChuan kezhi injection atomizing inhalation can effectively ease throat symptoms after general anesthesia, improve immune status, the effect is remarkable.
3.The relationship between sepsis-induced high mobility group-1 gene expression and endotoxemia in rats

ObjectiveTo investigate the relationship between sepsis induced high mobility group 1 (HMG 1) gene expression and endotoxemia in rats. MethodsSepsis was induced by cecal ligation puncture (CLP). One hundred Wistar model rats were randomly divided into four groups: normal control group ( n =10), sham operation (SO) group ( n =10), CLP group (further subdivided into 2?h, 6?h, 12?h, 24?h, 48?h and 72?h post CLP, n =10 in each subgroup), and bactericidal/permeability increasing protein(rBPI 21 ) treatment group (further subdivided into 12?h, 24?h post CLP, n =10 in each subgroup). AIM: To investigate the feasibility to inhibit the expression of MHCⅡ by special siRNA targeting class Ⅱ major histocompatibility complex (MHC Ⅱ) transactivator (CⅡTA), which might regulate MHC Ⅱexpression for suppressing immune rejection. METHODS: Five different siRNA were designed, synthesized and transfected into freshly isolated rat corneal keratocytes. 5.Surgical treatment of congenital blepharophimosis syndrome and long-term follow-up

ObjectiveTo summarize long-term effects of different op eration methods to cure congenital blepharophimosis syndrome in the past twenty years. MethodsIn the past twenty years, according to the clini cal features of 74 cases of congenital blepharophimosis syndrome,we adopted one -stage or two-stage technique to do the medial and lateral canthoplasty and/or to correct blepharoptosis. 24 patients accepted post-operative follow-up as l ong as 9 years. ResultsGenerally, Mustrarde technique was adop ted to correct inverse epicanthus for 54 cases, but for 20 patients with mild ep icanthus we adopted other methods. When enlarging the medial canthus, we selecti vely shortened the medial canthal ligament for 36 patients. When transverse tens ion on the upper eyelids was not increased, 33 cases selected one-stage techniq ue to correct this syndrome and obtained the same effects as two-stage techniqu e did for the other 41 cases. Other combined deformations of the patients should be treated in adolescence according to the patients' requirements. ConclusionBy adopting one-stage or two-stage technique to perform the medial and lateral canthoplasty and/or to correct blepharophimosis for con genital blepharophimosis syndrome can reach the satisfactory results if a correc t indication is selected.
6.Significance of fluctuated level of endogenous bactericidal/permeability-increasing protein in patients after major abdominal surgery

Objective To investigate the fluctuated pattern of endogenous bactericidal/permeability-increasing protein(BPI)levels,and to elucidate its significance in endotoxin response after major abdominal surgery.Methods Eleven patients undergoing major abdominal surgery were involved in present study.The levels of plasma BPI,lipopolysaccharide-binding protein(LBP)and interleukin-6(IL-6)were measured preoperatively and on postoperative days 1,3,5,7,and 14 by ELISA.Plasma endotoxin concentration was determined with the chromogenic limulus amebocyte lysate(LAL),which was modified by perchloric acid(PCA)pretreatment of samples.The polymorphonuclear neutrophil count was also done at each time point.Results In comparison with normal controls,plasma BPI/LBP ratios [(1.058?0.074)?10-3,P=0.004] were significantly increased on postoperative day 1,maintained at same level on day 3,and declined on day 5.No plasma IL-6 was detected from the blood of all the 11 patients prior to operation,but plasma IL-6 level rose rapidly on day 1 after operation(27.440?12.144pg/ml,P=0.04),maintaining a high level(11.530?5.312pg/ml)on postoperative day 3,and disappeared on day 5.Conclusions Plasma BPI/LBP ratios were markedly elevated during 3 days after a major abdominal surgery,and endogenous elevated BPI may play an important role in prevention from the development of endotoxemia in surgical patients.

Objective To analyse the prevalence rate of the cardiovascular and cerebrovascular disease and their susceptible factors in employees of AISCO.Methods A total 95 912 persons,to analyse the history of cardiovascular and cerebrovascular disease,smoking,blood pressure,diabetes mellitus,TC,HDL-C,BMI,and the value of TC/HDL-C so as to find the risk stratification of cardiovascular and cerebrovascular disease.Results There were 1 910 persons with CAD(2.00%),607 with cerebrovascular(0.63%),19453 smoking(20.28%),788 with diabete(0.82%),31698 with hypertention(33.05%),8347 with hypercholesteremia(8.70%),55462 over weight(57.83%),and 10 487 with TC/HDL-C≥3.5.the probability suffer from cardiovascular were mostly in very-low-risk(68.14%)and low-risk(5.75%),and carotid atherosclerosis were mostly in high-risk(44.23%),intermediate-risd(23.21%)and very-high-risk(10.51%)without low-risk and very-low-risk.Conclusion The health condition of the investigation population was not optimism and we will make more efforts for health education.
8.Spiral CT localization of the point where superior epigastric artery perforates rectus abdominis muscle

Objective To locate the points where the superior epigastric artery perforates the rectus abdominis muscle by using the spiral CT,in order to evaluate its control on abdominal complications after breast reconstruction.Methods Fifty cases had received abdominal spiral CT angiography,respectively.Then the coordinate system was established,with the umbilicus as the origin,the umbilicus horizontal line as X axis,vertical umbilical level line as Y axis.The point where superior epigastric artery perforates the rectus abdominis muscle in the coordinate system was located as described above.Results There were 392 perforating points in the rectus abdominis muscle in all 50 cases,with a mean of 7.84 perforators per patient.In the first quadrant,the points where superior epigastric artery perforate the rectus abdominis muscle were located in the range from 1.45 cm to 5.47 cm on the X axis,and in the range from 2.52 cm to 16.38 cm on the Y axis,respectively.In the second quadrant,the points where superior epigastric artery perforates the rectus abdominis muscle were located in the range from 2.02 cm to 6.80 cm on the X axis,and in the range from 2.30 cm to 14.46 cm on the Y axis,respectively.Conclusions The spiral CT angiography has high sensitivity and specificity,it can locate the point where the superior epigastric artery perforates the rectus abdominis muscle,which can avoid to cut extra rectus abdominis muscle.It is significant to reach best postoperative effect with the lowest donor site complications.
9.Complications of breast reconstruction with pedicled TRAM and LDF: a Meta analysis

Objective To analyze the complications of breast reconstruction with pedicled transverse rectus abdominis myocutaneous flap (TRAM flap) and latissimus dorsi flap (LDF).Methods From databases such as CNKI,SinoMed,PubMed,VIP and Cochrane Library and manual search of articles on breast reconstruction with pedicled TRAM and LDF from 1992 to 2002 for the language of both Chinese and English,1493 cases were collected.47 cases were selected after they all were inspected by the excluded and included standards.RevMan 5.0 software was used to perform the Mantele-Haenszel fixed effect model.Results Ten studies reporting outcomes of pedicled TRAM and LDF were used to estimate relative risk (RR) and 95 % confidence interval (CI) for complications.There was a 1.7 times increase in the risk of partial flap necrosis (relative risk,1.72; 95 % CI,1.02to 2.88) in pedicled TRAM flap patients compared with LDF patients.There was no difference in the risk for fat necrosis (relative risk,1.01; 95 % CI,0.60 to 1.72) and total flap loss (relative risk,2.13; 95 % CI,0.82 to 5.54) between pedicled TRAM flap and LDF patients (P>0.05).Conclusions Compared with the pedicled TRAM flap,partial flap loss risk of breast reconstruction with LDF is lower,and the risk of fat necrosis and total flap loss have no statistical difference (P>0.05).
10.The effect of S1PR2/3 during myocardial ischemia-reperfusion injury

Objective To observe the effect of S1PR2/3 on heart during myocardial ischemia-reperfusion (I/R) in rats. Methods Healthy adult male Sprague-Dawley rats were randomly divided into 7 groups: control group, sham operation group, IR group, IR group treated with DMSO, IR group treated with Cym5541( agonist of S1P3), IR group treated with Cay10444 (antagonist of S1P3), IR group treated with Cay10444/Jte-013 (antagonist both S1P3 and S1P2). In vivo model of myocardial ischemia-reperfusion was established. The hemodynamics, infarction area and mortality was recorded. Results Compared with IR, the S1PR3 antagonist group and S1PR2/3 antagonist group showed signiifcantly reduction of heart rate(HR) and increament left ventricular end-diastolic pressure(LVEDP)(P<0.05). In addition, the infarction area was increased in the S1PR3 antagonist group and S1PR2/3 antagonist treated group (55.7%:28.8%, 51.6%:28.8%), respectively. Treatment with S1PR3 agonist reduced the infarct size compared with IR group(18.6%:28.8%). Blocking S1P2/3 receptors increased IR-induced mortality signiifcantly (53%:22%, P<0.05). Conclusion S1PR2/3 have a beneifcial effect on heart. S1PR2 and S1PR3 were involved in the IR-induced SCD.
