1.Clinical study on alprostadil combined with torasemide in the treatment of intractable heart failure

Objective To observe the short-term effect of alprostadil combined with torasemide in the treatment of intractable heart failure.Methods Thirty cases of patients with intractable heart failure and ineffective in routine therapy were given alprostadil and torasemide treatment,with alprostadil 20 mg in 5% glucose or 0.9% sodium chloride solution 100 ml intravenous infusion once a day and torasemide 20-40 mg intravenous twice a day 3 days later torasemide dosage was adjusted according to the disease,and the remaining conventional anti-heart failure therapy unchanged.Course was one week.The blood pressure,body weight,urine output,creatinine,electrolytes,N-terminal pro-brain natriuretic peptide (NT-proBNP)and left ventricular ejection fraction (LVEF) before and after treatment was observed.Results After treatment,clinical symptoms of dyspnea,edema,pulmonary rales,such as wet and dry significantly improved in all patients.During the course of the treatment,3 patients appeared mild hypokalemia,and potassium was promptly corrected.Body weight after treatment reduced compared with that before treatment [(63.8 ± 7.6) kg vs.(82.6 ± 10.7) kg],urine output increased after treatment compared with that before treatment [(2 328.3 ±367.8) ml/d vs.(568.7 ± 104.6) ml/d],and the differences were statistically significant (P < 0.01).Systolic blood pressure,diastohc blood pressure difference was not statistically significant after treatment,compared with that before treatment (P > 0.05).LVEF after treatment increased compared with that before treatment [(44.5 ± 8.3)% vs.(31.9 ± 10.2)%],serum creatinine levels reduced [(97.8 ± 18.6) μmol/L vs.(143.8 ±21.7) μmol/L],and the difference was statistically significant (P< 0.05) ; NT-proBNP after treatment reduced compared with that before treatment [(567.4 ± 212.3) ng/L vs.(2 726.5 ± 525.3) ng/L],and the difference was statistically significant(P < 0.01).Conclusion Alprostadil combined with torasemide treatment can quickly and effectively improve symptoms and help improve heart and kidney function in intractable heart failure,and has no significant adverse reactions.
2.Effect of atorvastatin on osteopontin of patients with chronic heart failure

Objective To investigate atorvastatin administration on osteopontin and cardiac function of patients with chronic heart failure(CHF).Methods Sixty CHF patients were randomly divided into control (30 cases) and atorvastatin groups (30 cases).Patients in control group were given regular treatment including conventional oxygen inhalation,digitalis,angiotensin converting enzyme inhibitors,β blockers,diuretics,vasodilator therapy.Patients in treatment group were given atorvastatin at dose of 20 mg/d for 2 months plus regular treatment.The NHYA cardiac function,echocardiography determination of left ventricular ejection fraction(LVEF) were recorded.The levels of plasma osteopontin,plasma N-terminal brain natriuretic peptide (NT-proBNP) and C-reactive protein (CRP) were measured.Results The levels of plasma osteopontin,NT-proBNP and CRP in atorvastatin group were (1 062.68 ± 130.63) ng/L,(609.00 ±62.39) ng/L and ((5.84 ± 0.70) mg/L respectively,lower than those of control group ((1609.94 ± 201.87) ng/L,(922.33 ± 108.68) ng/L,(8.90 ± 0.86) mg/L),and the differences were statistically significant (Z =-1.981,t =2.766,Z =-2.092,P <0.05).LVEF in atorvastatin group was (58.7 ± 1.2)%,higher than that in control group ((52.8 ± 1.6) % ; P < 0.05).Conclusion Atorvastatin treatment can decrease plasma osteopontin density,alleviate the inflammatory response and inhibit cardiac remodeling in patients with heart failure,which was good for recovery of heart failure.
3.Change of Formaldehyde Level in Indoor Air in Newly Decorated Houses in Beijing

Objective To investigate the formaldehyde pollution in the indoor air in some newly decorated houses in Beijing and analyze the dynamic changes in order to find the change law of formaldehyde. Methods 54 residences were randomly chosen in Beijing to monitor the formaldehyde levels. The temperature and humidity were recorded at the same time. Covariance analysis was used to analyze the influence of temperature and humidity on formaldehyde pollution levels. Results In six consecutive months of monitoring after the decoration, the monthly average concentrations of formaldehyde in the indoor air were 0.04 mg/m3, 0.04 mg/m3, 0.13 mg/m3, 0.15 mg/m3, 0.02 mg/m3 and 0.03 mg/m3 respectively. The influence of temperature and humidity on formaldehyde levels was significant. Conclusion The formaldehyde level in the indoor air after decoration is mainly affected by the season and time. Residents should open the windows more often and it is suggested to live in the newly decorated houses in six months as the decoration finished.
4.Dynamic Changes of Benzene Series Level in Indoor Air in Newly Decorated Houses

Objective To know the current status of the benzene series pollution in the indoor air in newly decorated houses in Beijing and analyze its dynamic changes in order to find the changes rule of benzene series level. Methods 54 houses were randomly chosen in Beijing to monitor their benzene series levels during Apr.-Oct.,2005. Results The monthly average concentrations of benzene series in six months after decoration changed obviously while the temperature and time changed. Conclusion The benzene series level in the indoor air after decoration is affected by temperature mainly. Residents should open the windows for 3 months at least in order to avoid benzene series pollution.
5.Biological effect of laser-induced osteosarcoma cell apoptosis resistant to multidrugs

OBJECTIVE; To review the molecule mechanism of multidrug resistant osteosarcoma cell apoptosis induced by laser. DATA SOURCES; An online search of Pubmed database and CNKI database was undertaken to identify the articles published from 1996 to 2006 by using the keywords of “laser, osteosarcoma and apoptosis”, and language was limited to both Chinese and English. STUDY SELECTION; The articles were primarily screened and then the full-texts were searched. Included criteria: relevant to the mechanism of tumor cell apoptosis induced by laser. Articles were excluded because of repetition and obsolescence. DATA EXTRACTION; Totally 74 articles were found and 31 of them were selected according to the inclusive criteria as described above. Of the 31 articles, 5 articles were related to reactive oxygen species, 5 were associated with intracellular calcium concentration, 6 were involved in protein kinase C and 5 in Bcl-2 respectively. The last 8 were about P53 in the tumor apoptosis induced by laser. DATA SYNTHESIS; The mechanisms that laser induces osteosarcoma cell apoptosis include mainly reactive oxygen species, intracellular calcium concentration, protein kinase C, Bcl-2, p 53 and so on. The recent reports focus on the proliferation improvement of low-intensity laser on normal cells, but few reviews the biologic effect of tumor cells, especially multidrug resistant osteosarcoma and animal models. It is presumed to be correlated with the mechanism involved above. CONCLUSION; The mechanism that laser induces osteosarcoma cell apoptosis is still unclear, and is mainly related to reactive oxygen species, intracellular calcium concentration, protein kinase C, bcl-2, p 53 and so on.
6.Effect of fast track surgery on insulin resistance indexes of esophageal cancer patients

ObjectiveTo investigate the influence of fast track surgery (FTS) on insulin resistance indexes in esophageal cancer patients.MethodsA total of 68 esophageal cancer patients receiving radical operation in our hospital from November 2009 to March 2011 were randomly divided into 2 groups by envelope method:FTS group ( n =34) and control group ( n =34 ).The postoperative first passage of gas by annus and defecation,postoperative hospital stay,hospitalization expenditure,and postoperative complications were recorded.Insulin resistance indexes were measured before operation as well as on the 1st,3rd,and 7th postoperative days,including fasting blood-glucose (FBG),fasting insulin (FINS),serum level of interleukin-6 (IL-6),and C-reactive protein (CRP).The insulin resistance index (HOMA-IR) was calculated at the same time points.ResultsThe postoperative time of first passage of gas [ ( 1.91 ± 1.13 ) d vs.(2.95 ± 1.25 ) d,P =0.000 ] and defecation [ ( 3.75 ± 1.54 ) d vs.(4.84 ± 1.76) d,P =0.007] in FTS group were significantly shorter compared with control group,and the total hospitalization expenditure was significantly lower in FTS group [ (3.19±0.42) ten-thousand yuan vs.(3.98 ±0.36) ten-thousand yuan,P =0.000 ].The preoperative insulin resistance indexes showed no signifcant differences between the two groups ( all P > 0.05 ).One day after operation,the levels of log-HOMA-IR (0.97 ± 0.52vs.1.54±0.57,P=0.000),FINS [ (11.87±4.55) μU/ml vs.(17.16±6.90) μU/ml,P=0.000],IL-6[ (124.15±21.39) ng/Lvs.(138.78±23.60) ng/L,P.=0.009],and CRP [ (62.92±14.78) mg/L vs.(71.07 ± 14.51 ) mg/L,P =0.025 ] in FTS group were significantly lower than those in control group.Three days after operation,the levels of log-HOMA-IR (0.86 ± 0.47 vs.1.27 ± 0.58,P =0.002),FINS [ ( 9.56 ± 4.37 )μU/mlvs.(14.91±6.54) μU/ml,P=0.000],IL-6 [ (140.94±25.12) ng/Lvs.(153.89±24.06) ng/L,P=0.035],and CRP [ (138.59 ±21.61) mg/L vs.(153.17 ±25.20) mg/L,P=0.019] in FTS group were significantly lower than those in control group.Seven days after operation,the level of CRP in FTS group was significantly lower than that in control group [ (53.07 ± 15.84 ) mg/L vs.( 61.59 ± 16.28 ) mg/L,P =0.032 ].ConclusionFTS may promote the early recovery of gastrointestinal function,reduce stress reaction and postoperative insulin,thus being conducive to fast rehabilitation of esophageal cancer patients.

Objective To observe the effect of monosialoteterahexosyl ganglioside (GM1) combined with Shuxuening injection on nerve function in patients with acute cerebral infarction (ACI) and its mechanism.Methods A total of 94 patients with ACI admitted to the Department of Neurology in Xiangyang First Peoples' Hospital Affiliated to Hubei Medical College from January 2013 to June 2016 were enrolled,and they were divided into two groups by random number table,each group 47 cases.The patients in two groups were all given conventional western medicine treatment;The patients in one group (single group) were treated by intravenous (Ⅳ) drip of GM1,100 mg once a day;and the patients in another group (combined group),by above GM1 Ⅳ drip combined with Shuxuening intramuscular injection,once 2 mL,twice a day;the therapeutic course in two groups was 14 days.Before and after treatment,the changes of China stroke clinical neurological impairment score (CSS score),glasgow coma score (GCS),nerve factor,oxidative stress index and hemodynamics index of two groups were observed.Results Compared with those before treatment,after treatment the CSS score,the levels of neuron specific enolase (NSE),and malondialdehyde (MDA) were significantly lower,while the GCS score,the levels of nerve growth factor (NGF),neurotrophic factor (NTF),maximum blood flow velocity (Vmax),minimum blood flow velocity (Vmin),mean blood flow velocity (Vmean),mean blood flow quantity (Qmean),glutathione peroxidase (GSH-Px),catalase (CAT) and superoxide dismutase (SOD) were all significantly higher in the combined group (all P < 0.05).After treatment,the CSS score,levels of NSE and MDA in the combined group were significantly lower than those of the single group [CSS:11.20 ± 1.78 vs.16.24 ± 1.95,NSE (μg/L):13.17± 1.00 vs.17.68 ± 1.84,MDA (μmol/L):4.14±0.49 vs.5.61 ±0.50,all P < 0.05],GCS score,NGF,NTF,GSH-Px,CAT,SOD,Vmax,Vmin,Vmean and Qmean of the combined group were all significantly higher than those of the single group [GCS:13.68± 1.85 vs.12.01±1.76,NGF (ng/L):88.10±8.83 vs.68.13±7.16,NTF (pg/L):5.13±0.38 vs.3.71±0.30,GSH-Px (U/L):128.13±8.07 vs.103.90±6.58,CAT (U/L):25.74±2.15 vs.19.43± 1.84,SOD (μU/L):94.36±8.00 vs.77.29±7.34,Vmax (cm/min):48.23±3.36 vs.43.17±2.88,Vmin (cm/'min):8.11±0.76 vs.6.85 ± 0.64,Vmean (cm/min):18.69 ± 1.37 vs.15.60 ± 1.24,Qmean (mL/min):9.10 ± 0.74 vs.7.79 ± 0.66,all P < 0.05].Conclusions GM1 combined with Shuxuening injection can improve nerve function in patients with ACI by improving brain blood flow,secreting neurotrophic related factors and inhibiting oxidative stress reaction,thus it has important clinical significance for repairing the damaged nerve function.
9.The role of autophagy in TRAIL-induced apoptosis of medullary thyroid cancer TT cells

Objective To observe the level of autophagy induced by TRAIL in TT cell line and identify the role of autophagy in TRAIL-inducing apoptosis of TT cell line.Methods The growth inhibition of TT cells was measured by MTT assay.MDC staining was used to identify the happening of autophagy.Annexin V/PI double staining was used to analyze the apoptosis rate of TT cells by flowcytometry.The protein expression of caspase-8 and Beclin1 was detected by Western blot.Results ① The growth inhibition ratio of TT cells induced by TRAIL at the concentration of 250,500,1000 and 2000 ng/ml was (3.02 ± 1.82)%,(4.87 ± 1.45)%,(7.51 ± 1.57) %,(12.76 ± 3.23) % respectively,which suggested significant resistance of TT cells to TRAIL.② MDC-labeled green light vesicles was significantly increased after the treatment of TRAIL for 48 h.③ The apoptosis rate of TT cells induced by TRAIL at the concentration of 500 ng/ml and 1000 ng/ml after the pretreat ment of 3-MA for 4 h was(17.83 ± 1.54) % and(27.81 ± 1.79) % respectively,which was significantly higher than the apoptosis rate induced by TRAIL(3.70 ± 0.34) %,(6.55 ± 0.59) % alone and that induced by 3-MA(7.71 ± 0.64) % (t =3.282,P < 0.05 ; t =7.830,P < 0.01).④ The combination treatment of TRAIL and 3-MA increased the cleavage of caspase-8 and down-regulated the expression of Beclin 1.Conclusion Autophagy induced by TRAIL may contributes to the resistance of TT cells to TRAIL,which can be reversed by the inhibition of autophagy.
