6.Analysis of Expression of Multidrug Resistance Gene and Multidrug Resistance-Associated Protein Gene in Patients with Non-small-cell Lung Cancer
Tianjin Medical Journal 2000;28(12):738-739
Objective:To investigate the expression of multidrug resistance (MDR1) gene and multidrug resistance-associated pro-tein (MRP) gene in patients with non-small-cell lung cancer (NSCLC) and its relation to histopathological type. Methods:The MDRi and MRP genes were determined by reverse transcription-polymerase chain reaction (RT-PCR) in 48 NSCLCspecimens. Results:The positive rates of the MDR1 and MRP gene expression were 62.5% and 66.7% respectively in the48 patients. Twenty-one of the 48 NSCLCs (43.8%) expressed both MDR1 and MRP genes. The positive rates of the MRPgene expression in squamous cell carcinoma was significantly higher than those in adenocarcinoma (P<0.01), and therewas no significant difference in MDR1 gene expression between the two groups (P>0.05). Conclusion: The MDR1 andMRP gene expressions were both or respectively in the NSCLCs. The incidence of the MRP gene expression in squamous cellcarcinoma was higher than that in adenocarcinoma.

BACKGROUNDTo study the diagnostic value of detection of p16 methylation of sputum exfoliated cells for peripheral lung cancer.
METHODSBefore operation, p16 methylation of the sputum exfoliated cells was detected in 20 normal persons and 50 patients with peripheral pulmonary nodi by p16 methylation-specific PCR (MSP) for judging the nature of the nodi. The results were compared with pathological reports after operation.
RESULTSThe p16 MSP positive rate of sputum exfoliated cells of peripheral lung cancer patients (27/44, 61.4%) was much higher than that of benign pulmonary nodi (1/6, 16.7%) and normal persons (3/20, 15.0%)(Chi-Square=4.281 and 11.869 respectively, both P < 0.05). No significant difference of p16 MSP positive rate of sputum exfoliated cells was found between benign pulmonary nodi and normal persons (Chi-Square= 0.136, P > 0.05). No significant difference of p16 MSP positive rate of sputum exfoliated cells was found between squamous cell carcinoma (13/18, 72.2%) and adenocarcinoma (8/19, 42.1%) (Chi-Square=3.416, P > 0.05 ). If the positive p16 MSP of sputum was chosen as criterion for peripheral lung cancer, its positive predictive value, negative predictive value, sensitivity and specificity were 96.4%, 22.7%, 61.4% and 83.0% respectively.
CONCLUSIONSThe detection of p16 methylation of sputum exfoliated cells is contributable to the diagnosis of lung cancer.

Objective To investigate the clinical efficacy and safety of recombinant tissue type plasminogen activator(rr-PA) and urokinase(UK) in the treatment of acute myocardial infarction (AMI).Methods According to the digital table,128 patients with AMI were randomly divided into two groups,64 cases in each group. The control group adopted UK intravenous thrombolytic therapy, while the observation group adopted rr-PA intravenous thrombolytic therapy. The recanalization condition at different time after thrombolytic therapy, the incidence rate of cardiovascular events and death rate were compared between the two groups. Results The recanalization rate at 30min,60min and 120min after thrombolysis of the observation group were 29.69 %,59.38 % and 93.75 %,respectively, which were significantly higher than 14.06%,35.94% and 68.75% of the control group(x2 =6.34,7.01,7.45,all P < 0.05) ;The incidence rate of cardiovascular events and mortality rate of the observation group after thrombolytic therapy were 17.19% and 4.64%,respectively,which were significantly lower than 40.63% and 12.50% of the control group (x2 =7.83,7.62,all P < 0.05).Conclusion Recombinant tissue type plasminogen activator intravenous thrombolytic therapy has significant effect in the treatment of acute myocardial infarction after thrombolysis, which can reduce the incidence of cardiovascular events and mortality, with good clinical application value.

OBJECTIVE To investigate antibiotic resistance and distribution of bacteria to provide reference for using antibiotics reasonably.METHODS Antibiotic susceptibility test was performed by K-B method,meticillin-resistant Staphylococcus(MRS)and ESBLs were identified according to NCCLS(2004)of the USA.RESULTS From in 118 strains of bacteria there were 75 Staphylococcus strains, and the isolated rate of meticillin-resistant S.aureus(MRSA)was 35.0%,and that of meticillin-resistant coagulase-negative S.aureus(MRCNS)was 40.0%.Among 41 Gram-negative bacilli,ESBLs isolating rates in Escherichia coli and Klebsiella pneumoniae were 18.2% and 30.0%,respectively.The resistant rate to clindamycin, erythromycin and ciprofloxacin in MRS was higher than in meticillin-sensitive Staphylococcus(MSS)significantly.Imipenem showed proper antibiotic activity to Gram-negative bacilli.CONCLUSIONS We should pay more attention to resistant strains to control the nosocomial infection outbreaking.
