Lymph node metastasis is one of the main methods of metastasis and it affects the the prognosis of patients with gastric cancer. It is reported that the rate of tumor recurrence in patients with tumor-negative lymph nodes detected by routine pathology is approximately 25%, which is a factor that affects the survival. It is reported that the phenomenon associates with lymph nodes micrometastasis closely. The article will tou-ch the latest development on micrometsstasis.
2.Preliminary report of laparoscopic approach for anterior lumbar inter body fusion

Objective To evaluate the effectiveness of laparoscopy in anterio r lumbar interbody fusion. Methods Twenty-three laparoscopic anterior lumbar int erbody fusions were performed in the period between December 2000 and December 2 002. There were 9 males and 14 females with a mean age of 52 years (range, 46 to 66 years). In this group, there were 18 of spondylolisthesis, 3 of lumbar spina l instability and 2 of degenerative disc treated with anterior lumbar interbody fusion using Syncage and autograft with balloon-assisted endoscopic extraperiton eal gasless exposure. The main complaints were low back pain radiating to leg. 1 3 cases were fused at L4,5 and 10 at L5S1. The Oswestry Disability Index (ODI) w as recorded to evaluate the function preoperatively, and in the 2nd week, 3rd, 6 th and 12th month postoperatively. X-ray was taken to observe the fusion and the subsided cage. Results The operative time ranged from 50 to 180 min with a mean of 120 min, and the bleeding volume ranged from 50 to 300 ml with a mean of 80 ml. There were complications in 2 cases, one which was caused by laparoscopic mi sguidance, and the other was bleeding due to a small vessel injury. All the case s were followed-up from 12 to 24 months, with a mean of 16.6 months. 3.THE STUDY OF NON-CYTOTOXIC CHEMOTHERAPY IN GASTRIC CANCER

objective To study the inhibition effect and its mechanism of a non - cytotoxic chemotherapy drug in the growth of gastric cancer. Methods The inhibition effect was observed in the growth of xenotransplanted gastric cancer cells MKN45 with a gastrin receptor(GR) antagonist. On the basis of the determination of expressed GR in 34 patients with gastric cancer, we evaluated the preliminary clinical effect of PGL to gastric cancer with the positive GR. Results It is less tumor area, weight and gastric cancer cells DNA Index, average DNA contents and S phase fraction in PGL group than in control group. It is the least in PGL + SST group. GR is easy to be expressed in advanced gastric cancer of the body, cardia and fundus. With the 16-21 months follow- up of 14 patients with positive GR gastric cancer, we find that 2 patients died in control group of 8 patients, and all patients are alive in PGL group. Conclusion PGL can regulate the growth of gastric cancer cells MKN45. It would be able to improve the prognosis of gastric cancer.
4.Open door expansive laminoplasty(Kurokawa’s) for the treatment of cervical spinal stenotic myelopathy

Objective To investigate the efficacy of Kurokawa s open door expansive laminoplasty for the treatment of cervical spinal stenotic myelopathy. Methods Sixty seven patients admitted to Zhongshan hospital with cervical spinal stenotic myelopathy undergone Kurokawa s procedure in the last 6 years were included in this study. All of the patients were followed up after operation. The follow up period ranged from 10 months to more than 6 years. The average age at operation was 55.2 years. Japanese Orthopedic Association (JOA)score were used to measure the severity of cervical spinal stenotic myelopathy and recovery rates. Methods The study was conducted by integrating hospital internal data resources, structuring information platform and standardizing consultation process. Results The information sharing platform was achieved based on long-distance medical information network, thus achieving the greatest degree of information sharing and raising the level of long-distance medical consultation.
6.Frequencies distribution of 5-HT2A T102C genotypes in the schizophrenia patients

Objective To investigate the geographic distribution of 5-HT2A T102C genotypes frequency in the PLA schizophrenia patients. Methods The 5-HT2A receptor gene T102C polymorphism was analyzed for genotyping by the method of polymerase chain reaction(PCR)in 177 male PLA schizophrenia patients. Results The distribution of 5-HT2A genotypic frequencies in 177 male PLA schizophrenia patients were 0.07~0.33(? 2=4.44,P=0.617 1) for A1A1, 0.50~0.72 for A1A2 (? 2=1.14, P=0.942 2), and 0.17~0.29 for A2A2 (? 2=0.93, P=0.985 7), respectively. There was no significant correlation between 5-HT2A frequencies and different populations. Conclusion The frequencies of 5-HT2A T102C genotypes in the PLA schizophrenia patients were uniform in different populations.
7.Pseudoenhancement of renal cyst study of multiple phases of contrast enhancement with multi-slice CT

Objective To evaluate the rules and features of the pseudoenhancement phenomenon of renal cysts during the multi-phases of contrast enhanced MSCT scan.Methods Ninety one patients with 112 simple renal cysts with B-ultrasound,CT examination,improved clinically enrolled in this retrospective study.The attenuation of the renal cysts were measured blindly in the images of CT plain scan and scans of arterial,venal and secrete phases,and the attenuation change of the cysts between pre-and post-enhanced scans were calculated; the accuracies of pseudoenhancement judging were calculated with 10 HU,15 HU and 18 HU as a threshold;the size were recorded; degree of intra renal parenchyma of the cyst were also confirmed,and so renal cysts were divided into three groups:type Ⅰ,Ⅱ and Ⅲ; The differences of attenuation among 3 enhancement phases,different size,different type were analyzed statistically with the Kruskal-wallis rank sum test,the correlation between the diameter and the pseudoenhancement in each enhancement phases were analyzed statistically with Spearman test.Results The attenuation median of the 112 cysts in plain scan,arterial,venal and secrete phase was 6.0 HU,11.0 HU,12.0 HU and 12.0 HU respectively,there was significant difference(x2=53.32,P<0.01).The attenuation of the cysts in enhanced phases was higher than unenhanced.The range of attenuation change of the cysts between pre-and postenhanced scans was-10 to 31 HU,the number of cysts in groups of pseudoenhancement of 1 to 5 HU and 6 to 10 HU in each enhanced phase was dominant.The pseudoenhancement median of arterial,venal and secrete phase was 4.0 HU,5.0 HU and 6.0 HU respectively,There was significant difference among three groups(x2=10.062,P<0.01).Taking 10 HU,15 HU and 18 HU as threshold for judging pseudoenhancement,the accuracy was 83.0%(239/288),95.1%(274/288) and 96.9%(279/288) respectively; Pseudoenhancement of small cysts(≤10 mm) was higher than other groups in each enhancement phase,and there was significant difference(P<0.05).Type Ⅰ,Ⅱ and Ⅲ renal cysts were 62,23 and 27 respectively,the pseudoenhancement median of type Ⅰ renal cyst was 5.0 HU,6.0 HU and 8.0 HU respectively in arterial,venous,and excretion phase,which were higher than Type Ⅱ and Ⅲ (P<0.05).It showed low negative correlation between pseudoenhancemen and diameter in arterial,venous and secrete phase(r =-0.326,-0.332 and-0.447,P< 0.01).Conclusion The pseudoenhancement correlated with the renal cyst size,the type and the enhance phases,which should be considered when making diagnosis.
8.Efficacy of patient-controlled epidural analgesia combined with ganciclovir for treatment of postherpetic neuralgia

Objective To evalvate the efficacy of patient-controlled epidural analgesia (PCEA) combined with ganciclovir for treatment of postherpetic neuralgia in patients.Methods A total of 60 patients with PHN,aged 51-86 yr,of ASA physical status Ⅰ-Ⅲ,were randomly assigned into 2 groups (n =30 each) using a random number table:ganciclovir group (F group) and PCEA combined with ganciclovir group (FB group).In FB and F groups,ganciclovir 0.25 g was infused intravenously twice a day for 7 consecutive days.In addition,continuous PCEA was performed simultaneously in group FB and the PCEA solution contained ropivacaine 300 mg,prednisolone 20 mg and dezocine 10 mg in 250 ml of normal saline.At 1 day before treatment (T0) and 7 days,1 month,and 6 months after treatment (T1-3),pain was assessed with visual analogue scale (VAS),and the quality of life (QOL) score and pair relief rate were recorded.Results VAS and QOL scores were significantly lower at T1-3 than at T0 in the two groups.Compared with F group,VAS scores were significantly decreased,and QOL score and pain relief rate were increased at T1-3 in FB group.Conclusion PCEA combined with ganciclovir can effectively alleviate postherpetic neuralgia in the patients.
9.The correlation between the merger of hyponatremia and malnutrition in the elderly acute cerebral infarction patients in intensive care unit

Objective To analyze relationship between elderly patients with acute cerebral infarction hyponatremia and their nutrition status.Methods One hundred and twenty patients were selected who hospitalized from January 2011 to December 2012.All the patiens' fasting venous blood samples were taken in the next day morning after hospitalied.and then performed the routine blood test,blood biochemical testing items,observed the level of serum sodium (Na),hemoglobin (Hb),total protein (TP),serum albumin (ALB),prealbumin (PA),C-reactive protein (CRP) and lymphatic cell counts (LY) were measured.According to the value of serum sodium,all patients were classified to lower sodium group (serum sodium < 135 mmol/L,36cases),normal group (serum sodium range from 136 to 145 mmol/L,76 cases),and higher sodium group (serum sodium > 145 mmol/L,8 cases).The results were statistically analyzed.Results The incidence of hyponatremia was 30% (36/120).The levels of TP,ALB,PA,Hb and LY in lower sodium group were (50.35 ± 8.61) g/L,(28.35 ± 6.98) g/L,(89.96 ± 12.13) mg/L,(94.13 ± 25.36) g/L and (0.87 ±0.51) × 109 respectively,lower than that of normal group ((65.30 ± 5.48) g/L,(37.50 ± 3.63) g/L,(178.14 ± 18.61) mg/L,(124.87 ± 29.08) g/L,(1.67 ± 0.98) × 109 respectively,t =5.0897,7.1058,4.3216,3.8174,5.4237 respectively,P < 0.01),but the level of CRP ((76.55 ± 49.95) mg/L) in lower sodium group was obviously higher than the normal serum sodium group ((21.33 ±35.04) mg/L,t =0.1287,P < 0.01).The level of the serum sodium in ICU was related to the TP,ALB,PA,Hb and CRP(r =0.3176,0.4369,0.3695,0.2408,0.3612,0.0753 respectively,P <0.05).Conclusion The level of the merger of hyponatremia was correlated with malnutrition in the elderly acute cerebral infarction patients in ICU.We must strengthen the nutrition support treatment while correcting hyponatremia,in order to improve the clinical curative effect.
10.Analysis on effect of vacuum sealing drainage combined with routine debridement in treatment of frontal sinusitis after craniotomy

Objective To investigate the clinical effect of using uninterrupted vacuum sealing drainage combined with routine debridement and pedicled periosteal flap sealing frontal sinusitis in treating frontal sinusitis after craniotomy.Methods The clinical data in 31 cases of frontal sinusitis after craniotomy in our hospital from January 2006 to December 2014 were retrospectively analyzed.Seventeen cases adopted simple debridement and drainage treatment and 14 cases were treated with continuous vacuum sealing drainage combined with routine debridement treatment.Postoperative follow up lasted over 1 year.Results In simple debridement and drainage,12 cases were cured,the other 5 cases recurred after operation,and the cure rate was 70.6 %.But in the continuous vacuum sealing drainage combined with routine debridement treatment,14 cases were cured,no case recurred after operation,and the cure rate was 100%.Therefore,the cure rate of continuous vacuum sealing drainage treatment combined with routine debridement was higher than that of simple debridement and drainage treatment(P<0.05).Conclusion Adopting vacuum sealing drainage combined with routine debridement and pedicled periosteal flap sealing frontal sinusitis can promote the infection focus clearance and wound healing,and increases the cure rate.
