The Journal of Practical Medicine 2016;32(16):2652-2655
Objective To assess the efficacy and safety of multimedia visual training system for ambly-opic children through a randomized, controlled clinical trial. Methods A total of 203 amblyopic children (253 eyes) ages 3 to 12 years were randomly divided into a study group (100 patients, 170 eyes) and a control group (103 patients, 183 eyes). The study group received multimedia training system; while the control group received conventional integrated therapies. Best corrected visual acuity was examined 3 and 6 months after treatment. The efficacy and safety of multimedia visual training system for amblyopic children was assessed by comparing the ef-fect of treatment in both groups. Results 6 months after treatment, the total effectiveness rate was 74.1% in the study group and 52.5% in the control group, and there was a statistical difference (P < 0.0001). The effective-ness rate for refractive amblyopia was higher in the study group than in the control group (80.5% vs. 50.8%), and there was a significant difference (P < 0.001). As compared with the effectiveness rate for different ages, the subgroups of 3 to 6 years and 7 to 12 years in the study group was 80.8% and 58.0%, much higher than those in the control group (54.9% and 43.9%), and there were statistical differences(P < 0.001 and P < 0.05). Conclusions The efficacy of multimedia visual training system is superior to that of the traditional treatment , especially for refractive amblyopia. It is effective in all age groups.

OBJECTIVE:To improve the formulation of Kangfu cream,in order to increase the stability of it. METHODS:Ac-cording to the problem that demulsion of zinc oxide and poor stability of Kangfu cream, orthogonal test was used to optimize the formulation of cream matrix with oil phase ratio (stearic acid-white vaseline-glycerol monostearate),with the amount of polysor-bate 80,triethanolamine and SG-10 as factors, using comprehensive score of glossiness, delicacy, consistence, coating and stratifi-cation(10 points as full mark)as index. The stability of the cream was validated and investigated, such as compatibility,heat-re-sisting,cold-resistant,lighting. RESULTS:The formulation were as follows as 100 g cream containing stearic acid 8 g,white vase-line 8 g,glycerol monostearate 6 g,polysorbate 80 4 g,triethanolamine 3 g,SG-10 2 g and appropriate amount of water. The comprehensive scores of the cream were >9.5 points (n=3). Compared with the cream prepared by previous formulation, the cream prepared by modified formulation had no significant change in appearance and color, and demulsion and layering hadn’t been found. CONCLUSIONS:Kangfu cream with better stability than original formulation is successfully prepared.

Objective To evaluate cardiac biomarkers as biological risk factors for cardiovascular and all-cause motality in HD patients. In addition,a multimarker approach including inflammatory index was performed to improve the cardiovascular and all-canse risk assessment of these patients. Methods The author measured Troponin-T (TnT),N-terminal pro brain natriuretic peptide (NT-proBNP) and high-sensitivity C-reactive protein (HsCRP),collected the clinical data at baseline (January 2012) in 229 HD patients in three hemodialysis centers in Haidian District of Beijing, recorded time and cause of death in the next 1000 days.Kaplan-Meier was used to calculate survival rate and impact factors of prognosis.Cox proportional hazard model was used to estimate significance of =TnT,NT-proBNP and HsCRP and adjusted hazard ratios (HRs) of death. Results During the followup,37 patients died, mainly from cardiac cause (54.05%,20/37).Univariate analysis found old age, diabetes, cardiovascular disease, low serum albumin,CRP≥3 mg/L,TnT≥0.1 mg/L,NT-proBNP≥ 4381 ng/L were associated with prognosis.Elevated cTnT,NT-proBNP or HsCRP were all associated with increased cardiovascular and all-cause motality. Moreover, the combination of all parameters (NTproBNP≥4381 ng/L and TnT≥0.1 mg/L and HsCRP≥3 mg/L) were dramatically associated with increased cardiovascular cause mortality (HR=25.25,P < 0.01) and all-cause mortality (HR=27.33,P < 0.01).The association were significant even after full adjustment for cardiovascular (HR=14.33,P < 0.01) and all-cause mortality (HR=1 1.54,P < 0.01) respectively. Conclusions A combined index of cardiovascular risk factors could provide supplementary risk stratification in HD patients for cardiovascular mortality and all-cause mortality, strongly supporting the annual routine determination of these biomarkers.

Objective To retrospectively review and compare the clinical results of allogeneic peripheral blood stem cell transplantation (allo-PBSCT) from HLA- matched sibling donors mobilized with different regimens. Methods Seventy-one patients with hematological malignant diseases received allo-PBSCT from HLA-matched sibling donors in our department. Among them, 24 received allografts mobilized with G-CSF (group G), and the remaining (47 cases) were mobilized with G-CSF and GM-CSF (group G+ M). CD34+ subsets and T cell subsets in the allografts were analyzed, and the time of hematopoietic reconstitution and the incidence of graft versus host diseases (GVHD) were compared. The adverse effects on the donors after mobilization were also observed. Results The enough targeted CD34+ cells in all donors were harvested by 1-3 aphereses. Ninety-six h after mobilization, WBC counts of the donors were significantly higher in group G than in group G + M [(49. 6± 19. 5) 109/L vs (25.4 ± 10. 4) 109/L, P<0. 05]. Analysis of the CD34+ subsets showed that the percentage of cells with the CD34+/CD38- phenotype was significantly higher in group G + M than in group G [(37. 7 ± 5. 7) % vs (31.4 ± 4. 5) %, P<0. 05]. There was no significant difference in T cells and subsets of grafts. There was no significant difference in the number of total CD34+ cells and CD34+ CD38- cells, and infusion of T cells between two groups. The days required for the recovery of neutrophils and platelets was inversely correlated with the infused CD34+ and CD34+ /CD38- cell number. There was no significant difference in incidence of acute and chronic GVHD between two recipient groups. Seventeen cases and 10 eases among 71 eases died of relapses of primarydiseases, and complications of transplantation such as severe GVHD and infections respectively.Fourteen cases in group G (58.3 %) and 31 cases in group G+ M (66.0 %) survived. The most common adverse events in the donors were bone pain and fever, which mostly occurred 36 h after mobilization and could be relieved by non-steroidal anti-inflammatory drugs. Conclusion Two mobilization regimens showed equivalent clinical results. But the combined regimen of G-CSF and GM-CSF demonstrated a significantly greater mobilization of cells with the CD34+/CD38- phenotype.Meanwhile in allogeneic PBSCT, a greater number of total CD34+ cells and CD34+ CD38- cells infused may be associated with faster hematopoietic reconstitution of recipients.
