1.Comparison of wound healing in two mouse models of type 2 diabetes

Objective To compare the characteristics of ulcer wound healing in current commonly used C57BL/6J-db/db mouse models of spontaneous gene mutation-induced type 2 diabetes and in C57BL/6J mice with diabetes induced by streptozotocin (STZ), and to provide a basis for related experimental studies on diabetic ulcer in animal models.Methods To establish the mouse models of diabetic ulcer wound, observe the healing time and calculate the wound healing rate at 0, 3, 5, 7, 10, 14 days.Tissue samples were collected at days 7 and 14.HE and Masson staining, and immunohistochemistry (CD31 and PCNA) were used to observe the pathological changes of the wound tissues.Gene expressions of collagen-IIIα, fibronectin and α-SMA were detected by fluorescent quantitative analysis.Results The wound healing time of db/db mice was significantly delayed compared with the STZ mice, which was extended from 16.6±0.8 d to 20.2±1.3 d (P< 0.001).Compared with the STZ group, the growth of granulation tissue in the db/db group was slow, the length of newly formed epithelium was insufficient, the collagen deposition was disordered, and the wound healing was poor.At 7 days, the expression of CD31 and PCNA was significantly lower in the db/db group (P< 0.01), and at 7 and 14 days, the increase of collagen-IIIα and α-SMA genes up-regulation was significantly lower in the db/db group than in the STZ group.Conclusions Both the two types of diabetic mice show delayed wound healing.However, compared with the STZ-induced diabetic mice, the gene mutation db/db mice are more suitable for studies of diabetic ulcer wound healing as regarding the extent of the delay and the degree of difficulty of wound healing.
2.Evaluation of the role of professional nurses in atrial fibrillation anticoagulation management system

Objective To evaluate the effectiveness of professional nurses in atrial fibrillation (AF) anticoagulation management system.Methods 217 consecutive patients with nonvalvular AF were enrolled.All patients received warfarin therapy for 3-6 months after catheter ablation for AF,who were divided into experimental group (n=102) and control group (n=115) by random digits table.The patients of control group adjusted their warfarin doses by following doctor's advice when discharged and by visiting the outpatient clinic regularly after discharged.The patients of treatment group adjusted their warfarin doses under the guidance of one well-trained nurse.Days of the international normalized ratio (INR) value achieved therapeutic anticoagulation range (2.0-3.0) for the first time,the effective anticoagulation rate that defined as more than 70% of INR values between 2.0-3.0 after titration period,the ratio of times of INR value 2.0-3.0 to total times after titrating and INR monitoring frequency were compared between the 2 groups.Results In treatment group,days of the INR value achieved therapeutic 2.0-3.0 for the first time were less than that in control group (8 d vs.15 d,P<0.01),the effective anticoagulation rate [45.1%(46/102)] and the ratio of times of INR value 2.0-3.0 to total times after titrating (67.6%±18.5%) was significantly higher than that in control group [31.3%(36/115) and 62.0%±23.1% respectively,P<0.05].INR monitoring during the whole period and after titrating were more frequently in treatment group [(9.4±2.2)times vs.(8.4±2.7) times,P<0.05;(7.9±2.4) times vs.(6.3±2.8) times,P<0.01].The number of patients in treatment group who monitor INR less than 3 times after titrating was larger than that in control group (18 vs.1,P<0.01).Conclusions The participation of professional nurses in atrial fibrillation anticoagulation management system was helpful not only in achieving INR 2.0-3.0 more quickly but also in improving the effective anticoagulation rate.
3.Correlation between uncertainty in illness and family support of patients with coronary heart disease

Objective To explore the correlation between uncertainty in illness and family support of patients with coronary heart disease (CHD) in order to provide the basis for taking effective individual nursing intervention and promoting the rehabilitation of patients .Methods One hundred and thirty-two CHD patients were surveyed by Mishel uncertainty in illness scale and perceived social support from family scale (PSS-Fa). Results The total score of uncertainty in illness was (69.11 ±9.398) in the patients with CHD, and the family support was negatively correlated with uncertainty in illness (r=-0.359,P<0.05).The differences were found in the score of uncertainty in illness in CHD patients with different education background , occupation, degree of discomfort, and history of coronary artery stent (F/t=5.047,3.131,4.229,-3.972, respectively;P <0.05 ).Conclusions The family support of CHD patients is negatively correlated with uncertainty in illness , and the clinical nurses should pay attention to the family support situation , so as to reduce the uncertainty in illness and improve their quality of life .

5.Effect of preoperative transcatheter arterial chemoembolization on apoptosis of hepatocellular carcinoma cells.

OBJECTIVETo evaluate the effect of preoperative transcatheter arterial chemoembolization(TACE) on apoptosis of hepatocellular carcinoma (HCC) cells.
METHODSA total of 136 patients with HCC underwent liver resection. One to five courses of TACE prior to liver resection were performed in 79 patients (TACE group), in which one to four courses of chemotherapy alone were performed in 11 patients (group A), one to five courses of chemotherapy combined with iodized oil were performed in 33 patients (group B), one to three courses of chemotherapy combined with iodized oil and gelatin sponge were performed in 23 patients group C) and one to three courses of chemotherapy combined with iodized oil, ethanol and gelatin sponge were performed in 12 patients (group D). The other 57 patients only received liver resection (non-TACE group). The extent of apoptosis was analyzed by transferase-mediated dUTP nick end labeling (TUNEL) staining. The expressions of Bcl-2 and Bax protein were detected by immunohistochemical method.
RESULTSThe apoptotic index(AI) and level of Bax protein in HCC cells were significantly higher in groups A, B, C and D than those in the non-TACE group (P < 0.05). The level of Bcl-2 protein and ratio of Bcl-2 to Bax protein of HCC cells were significantly lower in Groups A, B, C and D than those in the non-TACE group (P < 0.05).
CONCLUSIONPreoperative TACE regimens may enhance apoptosis of HCC cells by up-regulating the expression of Bax protein and down-regulating the expression of Bcl-2 protein and ratio of Bcl-2 to Bax protein expression.

6.Comparison of diagnostic value of TIRADS, BSRTC, BRAFV600E mutation detection and their combined use in differentiating thyroid nodules

Objective To compare the diagnostic efficiency of the thyroid imaging reporting and data system (TIRADS), the Bethesda system for reporting thyroid cytopathology(BSRTC) and BRAFV600E detection, and their combined use in the differentiation between benign and malignant thyroid nodules. Methods One hundred and twenty eight patients with 128 thyroid nodules who were scheduled for ultrasound-guided fine-needle aspiration biopsy (FNAB) were recruited for the study. All of them underwent ultrasound, fine-needle aspiration cytology(FNAC) examination, and BRAFV600E detection. TIRADS and BSRTC systems were adopted to judge the ultrasound and FNAC results. The receiver operating characteristic curve was established to assess the diagnostic value of each method. Results The sensitivity, specificity, and AUC of TIRADS were 74. 3%, 84. 5%, and 0. 794, respectively. BSRTC had higher specificity(98. 3%) and equal sensitivity compared to TIRADS. The sensitivity, specificity, and AUC of BRAFV600E detection were the highest ones among the three methods. Combinations of different methods could increase the diagnostic sensitivity and accuracy. The combination of FNAC and BRAFV600E detection significantly increased the diagnostic efficiency(AUC=0. 984), with sensitivity 98. 6% and specificity 98. 3%. Conclusions The diagnostic value of BRAFV600E detection in the differentiation of benign and malignant thyroid nodules is better than both TIRADS and BSRTC, and the combination of FNAC and BRAFV600E detection reaches the best diagnostic efficiency.
7.Correlation between career maturity and professional identity among secondary and higher vocational run-through education nursing interns

Objective? To understand the career maturity and professional identity among secondary and higher vocational run-through education nursing interns and to analyze the correlation between them. Methods In November 2018, we selected 84 nursing students from two "4+2" secondary and higher vocational run-through education classes in a higher vocational nursing college in Beijing as subjects by purposive sampling. All of the students were investigated with the Career Maturity Inventory(CMI)and Professional Identity Questionnaire for Nurse Students(PIQNS). A total of 84 questionnaire were sent out and 82 valid of them were collected. Results? Among 82 nursing interns, the total scores of CMI and PIQNS were (118.24±16.29) and (74.99±6.74). Career maturity had a positive correlation with professional identity (r=0.416,P< 0.01). The influencing factors of career maturity included independence of career choice and professional self-image among nursing interns (P<0.01). Conclusions Nursing students have high levels of career maturity and professional identity. Educators should pay attention to the development of professional identity of them so as to improve their career maturity.

? This paper reviews the application of WeChat in continuous care for patients with coronary heart disease (CHD), namely the WeChat group-based intervention, the WeChat public platform-based intervention, the combination of WeChat group and WeChat public platform, and the combination of WeChat group and other information technologies. In addition, this paper analyzes the effect of WeChat intervention, that is, its impact on disease knowledge, self-management behavior, quality of life and the occurrence of adverse cardiac events in patients with CHD, in order to provide a reference for enriching continuous care for patients with CHD, so that continuous care can be effectively combined with WeChat information technology.
