1.Study of relationship between fibrinogen, aldosterone and left ventricular mass index in patients with essential hypertension

Objective To investigate the relationship between plasma fibrinogen levels,aldosterion levels and left ventricular mass in patients with essential hypertension.Methods Two hundred and seventy-three patients with essential hypertension were selected as subjects.Eldow vonous blood sample were taken to measure the rennin,aldosterion,fibrinogen,D-dimer,prothrombin fragment 1 + 2 (F1 + 2),and plasminogen activator inhibitor-1 (PAI-1) concentration by radioimmunoassay.And assessed cardiac structure and function in patients ultrasound heartbeat by application of ultrasound heartbeat.According to fibrinogen levels,all patients were divided to low(A group,n =95),medium (B group,n =87) and highest (C group,n =91) groups,and comparative analyzed the general clinical data,biochemical indexes,ultrasound heartbeat graph test results of the 3 groups.Meanwhile to compare the differences between 3 groups of left ventricular mass index on the two different concentrations of plasma aldosterone average ≥ 125 pmol/L or < 125 pmol/L.Results Left ventricular mass index (LVMI) increased in patients with aldosterone levels ≥ 125 pmol/L as the plasma aldosterone increased significantly.The left ventricular mass index in A,B,C group were (35.0 ± 3.1),(45.0 ± 3.6),(51.0 ± 2.9) g/m2 respectively,the difference between the 3 groups have statistical significance (F =6.65,P < 0.01).Within C group,LVMI in patients with aldosterone concentration(≥ 125 pmol/L) was greater than < 125 pmol/L(t =11.65,P < 0.01).Meanwhile the fibrinogen concentration gradually increased in A,B and C group (2630 (2420-2790),3290 (3170-3430),4210 (3791-4580),respectively).Fibrinogen concentration in C group was higher than that in the other two groups (Z =-2.46,P < 0.01).Ultrasonic heartbeat graph examination Results showed that left ventricular diameter in C group was the biggest than that in A and B groups (A,B,C group were (20.0 ± 2.3),(22.0 ± 3.9),(23.0 ± 3.1) mm/m2 respectively,F =8.16,P < 0.05);The same trends were also seen regarding of interventricular septal thickness (A,B,C group were(9.2 ± 1.9),(10.1 ± 2.0),(10.5 ± 2.4) mm respectively,F =10.52,P < 0.01),left ventricular posterior wall thickness (A,B,C group were (8.8 ± 1.5),(9.2 ± 1.6),(9.7 ± 1.7) mm respectively,F =10.6,P<0.01) and left ventricular mass(A,B,C group were(37 ±9),(43 ± 11),(45 ± 13) g/m2 respectively,F =11.6,P < 0.01).Conclusion Plasma fibrinogen and aldosterone concentrations are associated with left ventricular mass in patients with essential hypertension.Both them of interaction and participate to left ventricular hypertrophy commonly.
2.Research progress in Cre recombinase mouse models in ophthalmology

Cre/LoxP site specific recombinase system is an effective tool for gene recombination in the body to produce specific gene deletion or inversion.The study of gene function is the experimental basis for the development of experimental animal models.Study the Cre/LoxP system,compare the different function characteristics of mouse models for Cre recombinase system in the research of ophthalmology,epecially in the tissue of retina,such as retinal pigment epithelium (RPE) cells,Müller cells and photoreceptor cells,get the Cre transgenic mouse model to establish proper experimental animal models of ophthalmology will lay a solid foundation for the research of the lack of single gene for a particular organization study.Here,the research progress of Cre/LoxP system,the Cre transgenic mouse models and eye specific Cre recombinant enzyme model mice will be summarized.

The conformable rate to the results of operation was 100%, while it is 96% for 50 Patients with congenital heart disease.The study was shown that there was a high degree of diagnostic accuracy of DCFI (r=0.9401) in the quantitative value of 20 patients with VSD by correlation analysis

5.Risk stratification and significance of adult diabetic patients with cardiovascular disease

Objective To explore the related risk factors between adults with diabetes and cardiovascular disease risk.Methods Five hundred and fifty-seven patients whose age were over 20 years with diabetes mellitus were hospitalized in the First People's Hospital of Shangqiu from Jan.2008 to Dec.2013.According to the major risk factors of cardiovascular risk assessment,Framingham cardiac risk score was use to assess and stratify patients;and then graded as low risk group(< 10%),intermediate risk group(10% to 20%) and the high risk group(> 20%).Predicted the next 10 years the probability of the risk of cardiovascular disease.Results Among 557 patients,275 cases were male and 282 cases were female,79 cases (14.18%) were type Ⅰ diabetes mellitus and 478 cases (85.82%) were type Ⅱ diabetes mellitus.Meanwhile,10 years the probability of the risk of cardiovascular disease showed that there were 103 cases of low risk group(18.5%),91 cases of intermediate risk group (16.3%),195 cases of high risk group (30.2%) and 168 cases of cardiovascular disease group who had cardiovascular disease (30.2%).(1) Characteristics of risk factors for cardiovascular disease of risk stratification:①There was high incidence of metabolic syndrome in patients with diabetes mellitus,which was up to 37.9% even in low risk group and 60.5% in high risk group.②The average age of high risk group was (63.5 ± 11.7) years old which was higher than the low risk group ((50.3 ± 15.4) years old).③Systolic pressure of high risk group was (143.4 ± 18.1) mmHg,higher than that of the low risk group ((125.7 ± 13.2) mmHg).④It was hard to control the blood pressure in high risk group,so as a few cases(32.3%) were up to the standard.⑤The incidence of chronic kidney disease in high risk group was 21.5%,which was significant higher than that in intermediate risk group(7.7%) and low risk group(7.8%).(2) In high risk groupn,type Ⅱ diabetes mellitus's incidence was 31.6% which was higher than type Ⅰ diabetes mellitus's(16.4%).(3)In high risk group,the proportion of age range from 70 to 79 years old was up to 39.2%.Conclusion The overall risk of cardiovascular disease increases in patients with type Ⅱ diabetes.Metabolic factors may be the main risk.The risk assessment of diabetic patients will help to increase the intensity of treatment.
6.The applications value of CT biliary imaging in diagnosis of the biliary duct obstruction diseases

Objective To analyze the multi-slice spiral CT (MSCT) combined negative helical CT cholangiography (N-CTC) technical inspection applications in diagnosis of the biliary duct obstruction diseases.Methods Retrospective analysis of clinical data of 72 patients admitted to hospital by MSCT combined with N-CTC technical inspection and confirmed by pathology or surgery in patients with biliary duct obstructive diseases,and compared with pathological findings.Results Seventy-two cases had 81 paragraph obstruction lesions,with the biliary obstruction of pancreatic head to the level of duodenal ampullary were more common,a total of 44 (54.3%,44/81);among the 216 biliary paragraph,the missed diagnosis rate of MSCT combined with N-CTC was 9.9% (8/81),and misdiagnosis rate of obstruction was 4.4% (6/135),MSCT combined with N-CTC technology in diagnosis of biliary duct obstruction in comparison with surgical or pathological had no significant difference (P > 0.05).MSCT combined with N-CTC easily missed small low-density stones.Conclusions MSCT combined with N-CTC can clearly positioning biliary duct obstruction and qualitatively,consistency with surgery or pathological diagnosis is good,it is practical,non-invasive,can be used as the first choice for suspected biliary duct obstruction means of inspection,has a wide range of applications.
7.Clinical effect of cemented long stem prosthesis and memory alloy embracing fixator in the treatment of type B2 periprosthetic fractures

Objective To study the clinical effect of cemented long stem prosthesis and memory alloy embracing fixator in the treatment of type B2 periprosthetic fractures.Methods Fom April 2014 to April 2016,102 cases of peripheral B2 periprosthetic fractures patients were selected and randomly divided into bone cement long stem prosthesis revision surgery(group A) and revision of cemented long stem prosthesis combined with memory alloy embracing fixator(group B),each group with 51 cases.Clinical efficacy was evaluated according to Beals and Tower criteria,used Harris score to evaluate the hip the joint function,used the Visual Analogue Scale(VAS) score to evaluate the pain,after follow-up of 40 months,the clinical curative effect of 2 groups of patients,fracture healing time,complications and Harris score and VAS score were observed.Results The excellent rate of B group was significantly higher than that of A group,with statistical significance(90.2% vs.72.5%,χ2=5.239,P<0.05).Fracture healing time of group B was shorter than that of group A,with statistically significant ((10.2±2.5) weeks vs.(21.3±8.9) weeks,t=8.575,P<0.05).Harris the score of group B was higher than the group A,with statistically significant((89.9±13.5) points vs.(71.2±18.9) points,t=5.750,P<0.05).VAS score of group B was significantly lower than the group A,with statistically significant((1.23±0.25) points vs.(5.98±1.08) points,t=30.600,P<0.05).There were no complications such as infection,nonunion,loss of reduction and loosening of internal fixation in the 2 groups.Conclusion Revision of cemented long stem prosthesis combined with memory alloy embracing fixator is an ideal methods for the treatment of type B2 periprosthetic femoral fractures,which provides a good initial stability for fracture healing.
8.Aspero saponins phospholipid preparation and absorption in rats' intestine in situ

AIM: To prepare the asperosaponin—phospholipid compound to improve the bioavailability of asperosaponin in rat intestine in situ. METHODS: Ratio of asperosaponin to phospholipid was chosen as index to screen the optimal proportion based on the orthogonal design.Rat intestine perfusion was use to compare aspersaponin absorption under with and without phospholipid. RESULTS: Asperosaponin—phospholipid compound showed the markedly improvement in absorption capacity and absorption rate. CONCLUSION: The experiment recommends that asperosaponin in the combination with phospholipid is better than only used asperosaponin concerning bioavailability.
9.Extraction Optimization of Qishan Sugar-free Granule by Orthogonal Design

Objective:To study the best extraction technology of Qishan sugar-free granule. Methods:The effects of boiling dura-tion, boiling times and water addition ratio on the extraction process were investigated by using L9 (34 ) orthogonal design, and the con-tents of total flavonoids, baicalin and berberine were taken as the indices. Results:The optimum conditions were as follows:10-fold a-mount of water for 2 cycles, and every extracting time was 1 hour. Conclusion: The best extraction technology of Qishan sugar-free granule is obtained, and the process is stable and feasible, and the contents of active components are high.
10.Chemical components analysis by GC-MS of supercritical-CO_2 extracts from Tibetan medicine Pazhu Capsule

AIM:To study the main chemical components of traditional Tibetan medicine Pazhu Capsule(Calcitum,Fructus Chebulae,Semen Granati,Fructus Piperis,Fructus Piperis Longi,etc.) in the treatment of gastropathy. METHODS: The soluble components of Pazhu Capsule were extracted by supercritical-CO_2 fluid.The components were measured by GC-MS and the relative amount of them were determined by peak area normalization.(RESULTS:)22 components were identified.The main components were piperine(44.2%),9,12-Octadecadienoic acid(z,z)(21.1%),Azuleno[45-b] furan-2(3H)-one,3a,4,6a,7,8,9,9a,9b-octahydro-6-methyl-3,9-bis(methylene)-,[3as-(3a.alpha.,9a.alpha.,9b.beta.)](10.55%),n-Hexadecanoic acid(4.8%) and 2(3H)-Benzofuranone,6-ethenylhexahydro-6-methyl(33.5%).CONCLUSION: The main components of the Tibetan medicine Pazhu Capsule are piperine and 9,12-Ocatadecadienoic acid,both contents are up to 65.3% of the total extracts.
