1.Sdudy on Subchronic Toxicity of Fipronil in Rats

Objective To study the subchronic toxicity of fipronil in rats and to protect and improve the environment and human health. Methods 80 SD rats were randomly divided into four groups,20 in each groups (10 males and 10 females). The rats were treated with fipronil by gavage,in doses of 25,10,4,0 mg/kg,once a day,10 ml/(kg?d),for 90 consecutive days. The test method was according to GB15670—1995. Results The acute toxicity symptom of rats in high dose showed that cachexia,dull,hair floppy. No acute toxicity symptoms were found in the moderate and low dose groups. No significant difference was found in the serum and urine routine test indexes. Liver index increased significantly in the high dose(25 mg/kg)and moderate dose(10 mg/kg) exposed rats. The activity of aspartateaminotransferase (AST) in the serum increased only in male rats. No significant changes in indicators were found in the low exposed rats. The histopathologic examination showed that in the high and moderate dose groups,in the liver,spot and focus necrosis,the hepatocytes around central veins became bigger and cloudy swelling,some hepatocytes presented ballooning degeneration. No obvious pathological change was seen in the low dose group. Conclusion Fipronil has a toxicity to the liver,based on 90 day oral toxic test in rats,no-observed adverse effect level (NOAEL) is 4 mg/(kg?d).
2.EFFECTS OF AUTOPHAGY ON CLEARANCE OF THE APOPTOTIC LYMPHOCYTES BY MACROPHAGES

Objective To investigate the morphological characteristics of the autophagic structures in macrophages after phagocytosis of apoptotic lymphocytes and to explore the effects of autophagy on clearance of the apoptotic cells by macrophages. Methods Apoptosis of lymphocytes was induced with cyclophosphamide.The morphological changes of macrophages phagocytizing the apoptotic cells were viewed with a scanning electron microscope.The structural features of the autophagosome precursors,autophagosomes and autophagolysosomes in macrophages were examined with a transmission electron microscope,and the cross-section areas of the autophagic structures were measured with an image analyzer.The autophagosomes of macrophages were labeled with monodansylcaolaverine(MDC) staining and quantitated using laser scanning confocal microscopy.Results Macrophages actively phagocytized the apoptotic lymphocytes,apoptotic nuclei,apoptotic bodies and other cell debris to form heterophagosomes.When compared with the control group,numbers of autophagic cells and autophagosomes in these cells increase in the group of macrophages that engulfed the apoptotic cells.In addition,the ratios of the cross-sectional areas of the autophagic structures to that of the cytoplasm of the macrophages were greater.There were also apoptotic bodies or other cell debris in many the autophagosomes,and these autophagosomes were large and near the cell membrane.Autophagosomes containing the whole apoptotic cell or apoptotic nucleus were not observed.Conclusion The autophagic abilities of macrophages were significantly enhanced when the cells removed the apoptotic lymphocytes.Autophagy also plays an important direct or indirect roles in clearance of the apoptotic lymphocytes by macrophages.
3.Clinical study of fast track surgery in colorectal cancer patients

ObjectiveControl study on the clinical efficiency and costs of fast track surgery(FTS) and traditional method was carried out in colorectal cancer patients. Methodsone hundred colorectal cancer patients were randomLy selected, 50 cases treated with conventional therapy as control group and 50 cases treated with FTS programme. The postoperative initial venting time, the incidence of complications, the hospital stay and cost index were compared between the two groups. ResultsThe postoperative initial venting time was in advance and postoperative stay was obviously shortened in FTS group. Hospitalization expenditure in FTS group was lower than that in control group. Patients recovered quickly, the result was satisfactory. The complication was not significantly different between the two groups (P > 0.05 ) . ConclusionsFTS treatment can accelerate postoperative rehabilitation and elevate clinical efficiency in colorectal cancer patients during operation period. FTS treatment is a safe and effective method.
4.Effects of dust particles on autophagy of macrophages

AIM:To investigate the changes of autophagy of macrophages after phagocytizing dust particles and explore the mechanisms of dust particle-induced autophagy of the cells.METHODS:The bronchopulmonary lymph nodes were collected from patients with lung operation.The paraffin sections were prepared and then stained with Wilder's method.The tissue structures were viewed.Peritoneal macrophages were harvested from rats and then treated with carbon particles.Influences of carbon particles in autophagic activities of macrophages were examined.The ultrathin sections of the lymph nodes and the cells phagocytized carbon particles were prepared.The structures and distribution of phagosomes,autophagosomes and lysosomes were viewed.The apoptotic cells in the dust cells of the lymph nodes and the cells having phagocytized carbon particles were examined using transmission electron microscope and TUNEL staining.RESULTS:In adult lymph nodes,dust particles were deposited significantly in macrophages,collagen fibres and density of microvessels increased.There were autophagosome precursors,autophagosomes,autophagolysosomes as well as phagosomes in the dust cells and the cells phagocytized carbon particles.In autophagosomes,mitochondrion,dust particle or carbon particle were usually observed.There were positive cells by TUNEL staining in the dust cells and the cells phagocytized carbon particles.Nuclear condensation or apoptotic body in the apoptotic cells were observed under transmission electron microscope.CONCLUSION:Deposition of dust particles induces enhancement of autophagic activities and apoptosis of macrophages.Autophagy plays an important role in cleaning dust particles and the injured mitochondria.
5.Design of sample collection system for clinical inspection

6.Clinical study on Jianpihufei decoction combined with chemotherapy in the treatment of advanced non small cell lung cancer

Objective To observe the short term curative effect,life quality,adverse reactions of Jianpibufei decoction combined with chemotherapy in the treatment of patients with advanced non small cell lung cenacer.Methods 60 patients with advanced non small cell lung cenacer were randomly divided into the treatment group and the control gnoup,each group 30 cases.The treatment group was treated with TP scheme and Jianpibufei decoction.The contwl group was only treated with TP scheme.The effect,life quality and the injury of liver and kidney function was observed and compared between the two groups.Results The difference of the remission rate between the two groups had no statistical significance(x2 =0.418,P > 0.05).The improving rate of life quality in the treatment group was 70%,and higher than that in the control group(40%)(x2 =6.545,P <0.05).The reduction of the white blood cell,the hemoglobin,platelet and the injury of liver function in the treatment group were lower than those in the control group obviously(Z=-2.516,-2.184,-2.059,-2.097,all P<0.05).Conclusion Jianpibufei decoction combined with chemotherapy in the treatment of patients with advanced non small cell long cenacer could not increase the effect,but could reduce the adverse reaction of the chemotherapy and improve the life quality of patients.
7.Clinic analysis of cerebrospinal rhinorrhea in 24 cases

Objective:To discuss the clinical experience of diagnosing and managing of cerebrospinal fluid(CSF)rhinorrhea.Method:Twenty-four cases of cerebrospinal rhinorrhea were analysed retrospectively from Janu 2003 to Sept 2008, among which 18 cases from department of Otolaryngology Head and Neck Surgery and 6 cases from Neurosurgery.Result:Postoperative follow-up lasted from 4 months to 72 months. All the cases were successfully cured, among which 6 cases with conservative treatment and 18 cases under surgery,and no relapse case was found. The 18 cases under surgery included endoscopic approach(12 cases), extra-nasal approach(4 cases), transnasal approach under microscope(2 cases).Conclusion:It is not only minimally invasive, safety and efficiency of transnasal endoscopic technique for CSF leaks, but also without facial scarring after operation. Transnasal endoscopic approach can be preferred for the closure of uncomplicated CSF leak, located at the cribriform plate or the sphenoid sinus. The extra-nasal or intracranial approach may be an attractive option for more complicated and large CSF leak, or the leak site is not easily found with endoscopic.

10.Clinical analysis on 55 cases of paranasal sinus fungus ball.

OBJECTIVE:
To explore the etiology of paranasal sinus fungus ball and evaluate the curative effect of endoscopic sinus surgery on paranasal sinus fungus ball.
METHOD:
The clinical and follow-up data of 55 patients with paranasal sinus fungus ball were retrospectively analyzed.
RESULT:
All patients were cured by endoscopic sinus surgery, no complications were observed. Following up for 6-48 months, two cases were recurred.
CONCLUSION
Anatomic abnormalities of the paranasal sinuses and wound in the mucosa are the main pathogenesis of paranasal sinus fungus ball. Endoscopic sinus surgery is an effective treatment of paranasal sinus fungus ball. Relapse rate is related with regular irrigation after operation.
