Objective To evaluate the effectiveness of health management on quality of life of hypertensive patients living in underdeveloped rural regions. Methods Minqin County of Gansu Province was taken as research field, and health education covered all the population. Individual follow-up was adopted by quasi-experiment,and SF-8 scale was used to evaluate the change of scores of quality of life at baseline and the end of the study. Results The score of various dimension of quality life of interventive group showed a significant decrease at the end of follow-up ( P < 0. 05) , and the net score of general health status was 10. 92,the net score of impact to social role exerted by physical function was 9. 59,and the net score of social function was 4.61. Moreover, there was statistical difference between the intervention group and the control group for their quality of life(P <0. 05) , which showed in detail that each dimension of quality of life of the intervention group had higher score than that of the control group, after adjusting baseline difference by analysis of covariance. Conclusions All these results suggest that the active screening, following up and health education, conducted by the primary health care staff of township hospitals, under the idea of health management, can improve the quality of life of hypertension patients effectively in the rural area of underdeveloped region.

BACKGROUND: At present, common therapies of obstructive sleep apnea syndrome (OSAS) are uvulo-palato-pharyngoplasty and nasal continuous positive airway pressure (N-CPAP). The effective rate of surgical treatment was 50% and the prostecdtive efficacy is decreased gradually. As to treatment with breathing machine, some of the patients can not adapt to it.OBJECTIVE: To investigate the effects of auricular acupoint pressing therapy on clinical indexes of patients with OSAS and its clinical effect, and analyze the possible mechanism.DESIGN: Randomized control observation.SETTING: Department of Acupuncture and Department of Respiration,Second Hospital of Hebei Medical University.PARTICIPANTS: Forty-five outpatients with OSAS between April 2001 and April 2004, who were diagnosed as OSAS in the room of sleep-monitoring of Department of Respiration, were selected from the Clinic of Respiration, Second Hospital of Hebei Medical University, including 44 males and one female. All subjects were randomly divided into treatment group (n=30) and control group (n=15).METHODS: ①Patients in the treatment group were treated by auricular acupoint pressing therapy, acupoint-locating: shenmen, sympathesis, subcortex, heart, lung, spleen, kidney, anterior ear lobe. Each acupoint was pressed about 10-20 times with 3-5 times every day and 10 days as onecourse. Patients in the control group were treated with placebo and orally took 100 mg of Vitamin C three times a day. Patients in both groups were detected by 7hour nocturnal polysomnography before treatment and 10 days after the treatment, and the curative effects were evaluated.②Criteria of the curative effects on clinical symptoms: Effective: clinical symptoms such as dizzy, somnolence, oppressed chest and obstructing etc. were significantly ameliorated with nocturnal times decreased from 4-5 times to once or none and awake times decreased from 4-5 to once or none. Valid: clinical symptoms were improved. Invalid: no amelioration was found in clinicalsymptoms.MAIN OUTCOME MEASURES: Changes of clinical symptoms before and after the treatment and hypopnea index (HI), apnea index (AI), apnea hypopnea index (AHI), minimum blood oxygen saturation as well as the longest apnea time detected by polysomnography were observed.RESULTS: A total of 45 patients with OSAS were involved in the analysis of results, and no one withdrew from the study.①The clinical symptoms in auricular acupoint treatment group were significantly ameliorated than control group. In the treatment group, 17 patients were effective (56.7%), 12 patients were valid and one patient was invalid. The clinical symptoms in the control group were not improved before and after the treatment, and there were significant differences between the treatment group and control group (P < 0.05). ②Comparison in each inspecting indexes by polysomnogram: those in the auricular pressing therapy group were remarkably different from those in the control group after treatment (P < 0.001). There was no significant difference in all indexes of the control group between and after the treatment (P > 0.05).③The differences in AI, HI, AHI, minimum blood oxygen saturation as well as the longest apnea time of the auricular pressing treatment group after the treatment were markedly different from those before treatment [(65.25±7.44), (53.69±10.80) times/hour; (72.40±7.92), (59.22±10.55) times/hour; (9.46±2.6), (6.5±2.9) times/hour; (73.3±4.8)%,(77.67±3.78)%; (90.16±33), (45.33±24.69) s,P < 0.001]. Among them, the maximum decrease of AI was 19.3 times/hour, the maximum decrease of AHI was 17.8 times/hour, the maximum decrease of HI was 12.9 times/hour and the maximum increase of blood oxygen saturation was 7% CONCLUSION: Auricular acupoint pressing therapy, in the treatment of OSAS, can significantly ameliorate the AI, HI; AHI, minimum blood oxygen saturation as well as the longest apnea time with better curative effects.

The present study intended to provide some informations about the relationship between the supraependymal structures (SES) and the periventrieular neural tissue (PVNT), and with gross dissection and scanning electron microscopy the anterior medullary velum (AMV) was observed on 20 adult rabbits. The AMV may be divided into three portions: 1. the posterior membraneous wall of the recess of the inferior colliculus; 2. the anterior roof of the fourth ventricle; and 3. the transiional portion between the two portions mentioned above. On the ependymal surface of the entire AMV. There were numerous microvilli and cilia, except for the second portion of AMV, there were also some spherical-like structures, which were 2-6?m in diameter and had lace-like processes on its surface, and supraependymal cells(SEC) which were stellate, triangular, and spindle in shape. On the cellular surface with very few SEC, the secretory granules may be seen, which were 0.1-0.3?m in diameter. The SEC often extended out 2-5 processes, and the distal parts of which expanded into a flattened shovel-like structure, which lay on or inserted into the ependymal surface. The SEC here are similar to type Ⅱ SEC seen in the third and fourth ventricles, but they may differ significantly in structures on cellular surface, e.g. the secretory granules, and in shapes of their processes. Thus, it may suggest that the SEC here may play an intermediary role between PVNT and CSF, and be another route of neurohumoral modulation.

Objective To evaluate biochemical characteristics and the trend of diabetic complications in patients with newly-diagnosed type 2 diabetes from 1994 to 2008. Methods We utilized the database of the diabetes complications assessment and analyzed the metabolic disorder and the diabetic complications in the patients with newly diagnosed diabetes. Results 2 085 cases were collected, including 1189 males and 896 females. The average age of onset of diabetes was 51.6±13.1 and 54.6±7.9 yrs respectively in 2008 and 1994. During 1994,no case was found in subjects aged 20-29 yrs and 5% of the patients were aged 30-39; but 2% of patients aged 20-29 and 16% aged 30-39 yrs were found in 2008. BMI was increased from 24.48±4.15 in 1994 to 26.03±3.63 in 2008. Percentage of patients with abnormal BMI ( ≥25 kg/m2 ), WHR [≥0.90 (male) or ≥0.85 (female)]increased significantly from 63.6%, 75.0%, and 71.4% in 1994 to 79.6%, 95.2%, and 93.8% in 2008,respectively. Both SBP and DBP were not significantly changed. The fasting blood and postprandial blood glucose,HbA1c decreased from 10.3 mmol/L, 15.2 mmol/L, 11.1% in 1994 to 9.0 mmol/L, 14.3 mmol/L, and 8.6% in 2008, respectively. The average TG level increased from 1.7 mmol/L in 1994 to 2. 1 mmol/L in 2008,however, TC and HDL level were not significantly changed. The prevalence of diabetic retinopathy decreased from 28.2% in 1994 to 3.9% in 2008. The prevalence of diabetic nephropathy increased from 17.7% in 1994 to 24.6% in 2008. The prevalence of diabetic cardiovascular disease increased from 14.3% in 1994 to 24. 1% in 2008. Compared with the patients without microvascular complications, the patients with microvascular complications had higher SBP, DBP, and HbA1c( 136/78 vs 130/77 mm Hg, 9.41% vs 9.11% ). The patients with macrovascular complications had older age, higher SBP, TC, and TG than those without macrovascular complications (53.4 vs 50.0 yrs; 132 vs 129 mm Hg ; 5.3 vs 5.1 mmol/L and 2.6 vs 2.1 mmol/L). Conclusions In the studied newly-diagnosed diabetic patients from 1994 to 2008, there were increasing incidences of obesity and hypertriglyceridemia. However, the prevalence of diabetic retinopathy decreased significantly, while that of nephropathy showed no significant change.Cardiovascular complications were markedly increased.
