1.Coexistence of sarcoidosis and primary Sj(o)gren syndrome: a clinical analysis and literature review

Four patients with coexistence of sarcoidosis and primary Sj(o)gren syndrome (pSS) were retrospectively analyzed.All patients were female,who were referred to our department mainly because of respiratory symptoms.Positive antinuclear antibody (ANA) was detected in 2 patients and anti-Sj(o)grens syndrome A (SSA) antibody positive in 1 patient.All patients presented specific histologic patterns of both sarcoidosis and pSS.Publications related to coexistence of these two diseases were reviewed.Forty-one patients were finally included in the analysis,among whom 37 confirmed patients were from literature search.There were 37 women and 4 men.The main clinical features presentation were xerophthalmia in 40,xerostomia in 38,hilaradenopathies in 28,interstitial lung disease in 15,respiratory symptoms in 13.The main immunologic data were positive ANA in 23,SSA antibody in 19,anti-Sj(o)grens syndrome B antibody in 10 and rheumatoid factor in 12.All patients presented specific histologic patterns of both diseases.Patients with both sarcoidosis and pSS of ten represent multisystemic involvement and positive immunologic parameters,as well as the dual expression of specific histologic characteristics.
2.Experimental Study on Therapeutic Effects of Xileisan Pessary on Rats with Cervicitis

Objective To study the therapeutic effects of Xileisan Pessary on rats with cervicitis based on the idea of treating different diseases with the same therapy. Methods Vagina perfusion of hydroxybenzene mucilage was used to establish rat cervicitis models. Rats were randomly divided into normal group, model group, positive medicine group, low-, medium-, and high-dose Xileisan Pessary groups. Rats with cervicitis were treated with Xileisan Pessary from the next day after modeling for 10 days. The amount of neutrophilic granulocytes and leukocytes were detected by blood routine analysis meter; cervial histopathologic examination was conducted by HE staining. Results Compared with normal group, the numbers of granulocyte and leucocyte increased significantly in the model group (P<0.01). Compared with the model group, the numbers of granulocyte and leucocyte in the low-dose Xileisan Pessary group showed a decreasing trend (P<0.05, P<0.01); cervical inflammation mucosal epithelium and tissue necrosis were signally alleviated; granulocyte in the medium-dose Xileisan Pessary group showed a decreasing trend (P>0.05);the content of leucocyte hardly changed;granulocyte in the high-dose Xileisan Pessary group increased observably (P<0.01); the number of leucocyte showed an increasing trend (P>0.05), and the numbers of granulocyte and leucocyte in positive medicine group decreased observably (P<0.01), and cervical inflammation mucosal epithelium and tissue necrosis were also signally alleviated. Conclusion Xileisan Perssary can relieve the inflammation of cervix and repair injured mucosa. It has the function of treating different diseases with the same therapy.

4.Investigation of the occurrence of musculoskeletal disorders at shoulder, neck and elbow of digestive endoscopy operators

Objective:To investigate the effect of endoscopic operation volume on musculoskeletal disorders (MSDs) of shoulder, neck and elbow.Methods:From December 2019 to March 2020, random sampling was used to select 200 gastroenterologists from hospitals at or above the municipal level in Sichuan Province as the research object. Gastroenterologists were divided into 108 eadoscop in the low operation group (The operation volume is less than 5 000 cases) and 92 eadoscop in the high operation group (The operation volume is more than 5 000 cases) according to the operation volume. MSDS was investigated by UCLA shoulder scoring system, Tanaka Jingjiu cervical spondylosis scale and improved An and Morrey elbow scoring system, and the chi-square test was used to compare the basic conditions of different body parts in the two groups. The basic condition of each part of the two groups was tested and compared.Results:Compared with the low operation group, the age and working years of the high operation group were longer, and the difference was statistically significant ( P<0.001). The most painful part between the two groups was the shoulder, accounting for 25.9% (28/108) and 37.0% (34/92), respectively. Occasional shoulder pain was the most (41.9%, 26/62) and normal function was the most (32.3%, 20/62) ; The forward flexion angle exceeds 150° at most (40.3%, 25/62). Slight pain often occurred in the neck, accounting for 57.7% (15/26), the occasional impact of pain on the upper limbs accounted for 61.5% (16/26), and the impact on the fingers accounted for 53.8% (14/26) ; 65.0% (15/26) had mild disturbance to the sense of signs; Normal tendon reflex accounted for 88.5% (23/26) ; The influence of opponents was weakness, discomfort and no dysfunction, accounting for 53.8% (14/26) ; Endoscopists with normal muscle strength (2 points) and mild decline accounted for 50.0% (13/26) respectively, and there was no significant difference among the groups ( P=0.234、1.000、1.000、0.050、0.680、0.539、0.431、0.239). The elbow score scale showed that mild loss accounted for 55.0% (11/20), mild instability accounted for 65% (13/20), and mild pain accounted for 55.0% (11/20) . Conclusion:MSDs is a health problem faced by endoscopists, and targeted preventive measures should be taken for its possible causes.

6.Effect of Feixian Fang extract on angiogenesis of pulmonary interstitial fi-brosis rats

Objective Feixian Fang is a Chinese medicine compound,formulated by the principle of in-vigorating lung and kidney,reducing phlegm and resolving masses,and promoting blood circulation to re-move meridian obstruction.This paper aimed to investigate the effect of Feixian Fang extract (FXF)on degree of pulmonary fibrosis,change of microvessels and platelet endothelial cell adhesion molecules (CD 31,CD34)in rats with pulmonary interstitial fibrosis.Methods All rats were divided into sham opera-tion group,model group,prednisone group,losartan group,FXF high-,mid-and low-dose group.Ex-cept the sham operation group,the rats of other six groups were instilled bleomycin of 3 mg/kg through tracheal intubation.The next day,the rats were administered intragastrically corresponding medicines for consecutive 14 d or 28 d.The degree of pulmonary fibrosis,change of microvessels were observed under electron microscope and the expression of CD31 and CD34 were detected using immuhistochemistrical methods.Results High dose and middle dose FXF inhibited the pulmonary fibrosis and pulmonary alve-olar inflammation and decreased microvascular density (P <0.05).Conclusion The effect of FXF a-gainst pulmonary interstitial fibrosis can be via the way of decreasing microvascular density to induce less abnormal angiogenesis in pulmonary fibrosis.
7.Research and development of teaching software for medical image processing under the background of"new medical science"

In response to the current situation and teaching status of the medical image processing course on the background of"new medical science",a teaching software which is highly compatible with the teaching process of medical image processing is developed.The teaching software allows for linear grayscale transformation,windowing display,scaling,rotation,mirroring,median filtering,differential sharpening,edge detection,histogram acquisition,and histogram equalization of medical images.Additionally,it enables parameter adjustments within a certain range for linear grayscale transformation,windowing display,scaling,rotation,median filtering,differential sharpening,and edge detection.Meanwhile,it employs different algorithms to achieve the scaling of medical images.The teaching software is used in the theoretical and experimental teaching of medical image processing courses at Baotou Medical College.It can improve students'initiative and enthusiasm in learning,strengthen their understanding of the examination points for radiology technicians,lay a solid foundation for subsequent courses,and ultimately achieve the goal of in-depth integration of"Medical Engineering"and"Medical Science"in the medical image technology major at Baotou Medical College under the background of"new medical science".
8.Risk factors of infection with polymyxin resistant and carbapenemase-resistant Klebsiella pneumoniae

Objective:To investigate the risk factors of infection with polymyxin resistant and carbapenemase-resistant Klebsiella pneumoniae(PR-CRKP). Methods:A total of 170 patients with CRKP infection admitted in the Third Affiliated Hospital of Soochow University from July 2020 to October 2023 were enrolled,including 123 cases of CRKP infection and 47 cases of PR-CRKP infection. The general conditions,exposure of antibacterial drugs 6 months before admission,laboratory test indicators at admission,antibacterial drug use when target bacteria were detected,length of hospital stay and time of invasive procedures in two groups were retrospectively analyzed. The risk factors of PR-CRKP infection were analyzed with univariate and multivariate logistic regression. SPSS 26.0 software was used to analyze the data.Results:Univariate analysis showed that compared with the CRKP group,the average age of patients in PR-CRKP group was older( Z = -2.186, P = 0.029),the proportion of patients with exposure history to semisynthetic penicillins,carbapenems,polymyxins,and quinolones 6 months before admission was higher( χ 2= 3.930,5.414,11.939,8.478,all P < 0.05),the proportion of infections diagnosed at admission and blood urea nitrogen levels( χ 2= 7.268, Z = -2.406, P = 0.007 and 0.016)was higher,the hemoglobin level( t = 2.641, P = 0.009)was lower,the length of hospital stay was longer,the rates of tracheal intubation,urinary catheter,and deep vein catheterization were higher( Z = -4.243,-4.660,-5.341,-4.583,all P < 0.001),the duration of carbapenem and polymyxin B use was longer( Z = -4.757,-7.554,both P < 0.001),the proportion of combined quinolone-resistant Escherichia coli(QREC)and carbapenem-resistant organism(CRO)infections and bloodstream infections,and the rate of admission to intensive care units was higher( χ 2 = 33.737,42.041,5.426,12.991, P < 0.05 or < 0.01). Multivariate analysis showed that time to polymyxin B use( OR = 1.179, 95%CI 1.059-1.312, P = 0.003),combined QREC infection( OR = 5.357, 95%CI 2.100-13.669, P < 0.001)and combined CRO infection( OR = 3.302, 95%CI 1.146-9.514, P= 0.027)were independent risk factors for PR-CRKP. Conclusion:Prolonged use of polymyxin B is an independent risk factor for PR-CRKP,and mixed QREC and CRO infection can increase the risk of PR-CRKP.
9. Study on Relationship of Anxiety and/or Depression With Dyspepsia in Outpatients Undergoing Gastroscopy

Background: Patients with dyspepsia are the major group of outpatient department of gastroenterology. Dyspeptic symptoms are closely related to anxiety and/or depression. However, the relationship of anxiety and/or depression to dyspeptic symptoms in outpatients undergoing gastroscopy is still unclear. Aims: To investigate the incidence of anxiety and/or depression in outpatients undergoing gastroscopy and its correlation with the severity of dyspepsia. Methods: Outpatients underwent gastroscopy from November 2021 to December 2021 at West China Hospital, Sichuan University were recruited. Generalized anxiety disorder-7 (GAD-7), patient health questionnaire-9 (PHQ-9) were used to evaluate anxiety and depression, respectively. Rome diagnostic questionnaire for functional gastrointestinal disorders, severity of dyspepsia assessment questionnaire, visceral sensitivity index were used to evaluate dyspepsia. The correlation of anxiety and/or depression with dyspepsia was analyzed. Results: A total of 458 questionnaires were collected effectively, including 146 (31.9%) patients with anxiety and/or depression. Compared with the non-anxiety and depression group, the number of gastroscopy in the anxiety and/or depression group was significantly higher (P<0.05). Compared with the non-anxiety and depression group, the incidences of extremely mild/mild dyspepsia, moderate/severe epigastric fullness, postprandial fullness, epigastric pain and belching were significantly increased in the anxiety and/or depression group (P<0.05). Anxiety and/or depression was significantly correlated with the severity of dyspepsia and visceral sensitivity (r=0.421, r=0.143; P< 0.05). Conclusions: Anxiety and/or depression is common in outpatients undergoing gastroscopy; anxiety and/or depression may lead to repeated endoscopic examinations; anxiety and/or depression is significantly correlated with dyspepsia and visceral sensitivity.
