1.Study on the application of alprostadil and benazepril in patients with diabetes and the effect on blood glucose fluctuation

Objective To analyze the effects of alprostadil combined with benazepril in the treatment of diabetes and the effect on blood glucose fluctuation.MethodsEighty cases with diabetes were randomly divided into the control group and the observation group with 40 cases in each group.The control group was treated with benazepril while the observation group was additionally treated with alprostadil.The treatment effects, the changes of blood glucose, blood lipids and renal function were compared between the two groups, and the incidence of adverse reactions was recorded.ResultsThe total effective rate in the observation group was higher than that in the control group (P<0.05).After treatment, the fasting plasma glucose (FPG), postprandial 2h plasma glucose (2hPG) and glycosylated hemoglobin (HbA1c) were reduced to the normal level but the volatility differences of FPG and 2hPG in the observation group were lower than those in the control group (P<0.05).After 4 weeks of treatment, the total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) decreased while high-density lipoprotein cholesterol (HDL-C) increased (P<0.05) but the differences between the two groups showed no statistical significance.After treatment, the improvement of renal function indexes in the observation group was better than that in the control group (P<0.05), and the incidence of adverse reactions showed no significant difference between the two groups.ConclusionThe application of alprostadil combined with benazepril in the treatment of diabetes can reduce blood glucose fluctuation, improve renal function, and reduce the risk of diabetic nephropathy.
2.Primary study on resting myocardial perfusion in type 2 diabetes mellitus patients with real-time myocardial contrast echocardiography

Objective To explore the characteristics of resting myocardial perfusion of type 2 diabetes mellitus(T2DM) by real-time myocardial contrast echocardiography(RT-MCE). Methods Thirteen patients with T2DM and 12 in control were enrolled in the study. RT-MCE was performed using a continuous infusion of SonoVue with vena mediana in elbow. Images of left ventricle filled with contrast were acquired from apical 4-, 2-chamber and long axis views with real-time myocardial contrast mode of GE Vivid 7 Dimension system. All above images were captured in continuous 3 cardiac cycles before "flash" ,and then 15 cardiac cycles after "flash". All clips were stored for off-line analysis. Results In quiescent condition, there were significant decrease on indices of A,k and A × k in patients compared with control group (A:6.46 ±1.60 vs 6.81 ± 1.53, P <0.05;k:1.04 ± 0.39 vs 1.28 ± 0.31*, P <0.01;A× k:6.55 ± 2.72 vs 8.78 ±3.16, P < 0.01, respectively). Conclusions RT-MCE could evaluate the early change of myocardial perfusion in T2DM patients and offers important reference for clinical diagnosis and treatment.
3.Factors influencing the recovery of ability in the activities of daily living after cerebral hemorrhage

Objective To explore the factors influencing the recovery of ability in the activities of daily living (ADL) after intracerebral hemorrhage. Methods A total of 108 patients with intracerebral hemorrhage admitted for rehabilitation to the rehabilitation medicine department of Huashan Hospital between January 2007 and June 2011 were studied.Twelve items of clinical data were collected with regard to the patients' medical history,physical status,modified Barthel index (MBI) score and Brunnstrom stage at admission.Functional status was classified according to the MBI scores and Brunnstrom stages assessed at admission and before discharge.Linear regression analysis was used to relate the variables. Results After rehabilitation,the MBI scores and Brunnstrom stages had improved relative to the scores at admission.Factors influencing the MBI improvements included the intervention timing of rehabilitation and the course of therapy employed. Conclusions It is very important to comprehend the factors influencing the recovery of ADL ability after cerebral hemorrhage in order to design effective rehabilitation strategies,better predict functional outcomes and improve patients' ADL ability effectively.
4.Protective Effect of Diclipterachinensis Polysaccharide P2 B on Carbon Tetrachloride-induced Injury of Liv-er Cell Line L-02

Objective:To evaluate the protective effect of Diclipterachinensis polysaccharide P2B on liver cell line L-02 injury in-duced by carbon tetrachloride ( CCl4 ) . Methods:The human liver L-02 cells were cultured, and the injury model was built by CCl4 . The L-02 cells were divided into the normal control group, the CCl4-damaged group, and the P2B sample groups (0. 125, 0. 250 and 0. 500 mg· ml-1 ). The contents of alanine aminotransferase ( ALT), aspartate aminotransferase ( AST), superoxide dismutase (SOD) and malondialdehyde (MDA) were determined by MTT assay. Results:Compared with the CCl4-damaged group, P2B could improve the activity of L-02 cells, and the activity of AST and ALT in the supernatant was significantly reduced, and the content of SOD in the cells was increased and that of MDA was decreased. Conclusion:P2B can significantly prevent L-02 cells from the damage induced by CCl4 in a dose-dependent manner, and the mechanism may be related with the anti-oxidative activity of P2B.
5.Evaluation of the difference of rotation between subendocardium and subepicardium in diastolic heart failure patients by two-dimensional speckle tracking imaging

Objective To observe the rotation of subendocardium and subepidium by two-dimensional speckle tracking imaging(2D-STI),and to evaluate its performance in diastolic heart failure patients(DHF)with a normal left ventricular ejection fraction. MethodsNinety-seven consecutive clinically stable patients were enrolled in this study [41 healthy controls,36 with diastolic heart failure,20 with systolic heart failure (SHF)]. High frame rate dynamic two-dimensional images were recorded at the left ventricular short-axis view,including basal, papillary muscle and apical planes. Subendocardial and subepicardial global rotation were measured using Q-lab 7.0 software offline. Results ① In all the subjects, the rotation of the subendocardium was obviously greater than that of subepicardium. ②As seen from the apex,left ventricular subendocardium and subepicardium performed a wringing motion with a clockwise rotation at the base and countclockwise rotation at the apex. ③In the apical plane, subendocardial rotation was significantly lower in both heart failure groups than in controls,and was depressed to a larger extent in SHF patients than in those with DHF. Subepicardial rotation was no significant difference between the DHF group and the control group, though it was significantly lower in patients with SHF. ④At the base, the rotation of subendocardium and subepicardium were not different between DHF and control groups, but it was significantly reduced in patients with SHF. Conclusions The subendocardial rotation is reduced, but subepicardial rotation is normal in DHF patients. On the other hand, in patients with SHF, subendocardial and subepicardial rotation are both reduced. The left ventricular systolic properties are impaired in DHF patients.

Objective To explore the detection rate of colorectal polyps and colorectal cancer under colonoscopy,and their clinicopathological characteristics and changing trend.Methods From January 1,1991 to December 31,2017,at Guangzhou First People's Hospital,the colorectal polyps and colorectal cancer diagnosed by colonoscopy were collected.The gender,age,location of colorectal polyps and colorectal cancer and pathological type were retrospectively analyzed.Chi-square test was performed for statistical analysis.Results Among 77 978 patients underwent routine colonoscope examination,male and female accounted for 50.18% (39 132/77 978) and 49.82% (38 846/77 978),respectively;and the age was (52.8 ± 15.7)years.A total of 23 240 patients with colorectal polyps were detected,and the total detection rate was 29.80%.The detection rate of colorectal polyps of male patients was higher than that of female patients (35.11%,13 741/39 132 vs.24.45%,9 499/38 846),and the difference was statistically significant (x2 =1 059.16,P <0.01),the age was (59.2 ± 14.6) years.The most common location of polyps was left colon,accounted for 48.56% (11 286/23 240).The pathological type was adenoma (60.99%,13 726/22 504).Totally 5 204 patients with colorectal cancer were detected,the detection rate of 6.67%;the detection rate of male was higher than that of female (7.71%,3 018/39 132 vs.5.63%,2 186/38 846),and the difference was statistically significant (x2 =136.07,P <0.01),the age was (63.5 ± 12.5) years.The colorectal cancer was predominantly located in the left colon (73.71%,3 836/5 204).And the common pathological type was moderately differentiated adenocarcinoma (83.61%,4 351/5 204).Based on a nine-year as a time period,the detection rate of eolorectal polyps was 12.91% (599/4 639) from 1991 to 1999,22.74% (3 784/16 642) from 2000 to 2008,and 33.26% (18 857/56 697) from 2009 to 2017,showing a significant upward trend (x2 =1 353.45,P <0.01).The detection rate of colorectal cancer was 9.03% (419/4 639) from 1991 to 1999,8.59% (1 430/ 16 642) from 2000 to 2008,and 5.92% (3 355/56 697) from 2009 to 2017,showing a significant downward trend (x2 =191.89,P < 0.01).There was no significant change in pathological types of colorectal polyps in 27 years (x2 =201.08,P > 0.05);while moderately differentiated adenocarcinoma increased,however,the highly and poorly differentiated type decreased (x2 =249.27,P < 0.01).No significant changes in lesion location was found (x2 =117.11 and 134.82,both P > 0.05).Conclusions The detection rate of colorectal polyps increased in the past 27 years,while colorectal cancer decreased.The elderly men are susceptible populations.The most common location of colorectal polyps and cancer are left colon,which should be carefully examined during colonoscope examination.

This paper presents a three-dimensional electronic laparoscopy system, including three-dimensional laparoscope pipe and medical video system. The three-dimensional laparoscope pipe adopts a dual-optical structure, which can collect three-dimensional information of the surgical region. By selecting a reasonable initial structure, the MTF curve of the objective lens is close to the diffraction limit, and the distortion is less than 25%. The medical video system also achieved high-definition image with 1 080 P, 30 Hz by GPU. At the mean time, the three-dimensional electronic laparoscope has achieved quantitative production and has been tested in a number of animals, which has broad application prospects and significant clinical application value.
