1.The association between obstructive sleep apneahypopnea syndrome and glaucoma:a Meta analysis
Chinese Journal of Experimental Ophthalmology 2016;34(7):630-634
Background Studies have showed an increasing prevalence of glaucoma in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS).However, this view remains controversial. Objective This Meta analysis was to assess whether there is an association between glaucoma and OSAHS.Methods A systematic search in PubMed database was carried out with the subject headingssleeping apnea andglaucoma. The literature type was limited to cases-controlled studies and prospective cohort studies with the publishing duration from January 1,1982 to January 30,2015 in English. The risk of glaucoma among OSAHS patients was analyzed, and Review Manager 5.2 was applicated for Meta analysis. The pooled odds ratio (OR) and 95% confidence interval (CI) was calculated to assess the strength of the association. Results Twelve independent retrospective casescontrolled studies were included in this review, including 11 592 subjects in the OSAHS group and 25 931 subjects in the control group. The study quality was scored 6-8 by Newcastle-Ottawa Scale which was acceptable. The random effects model was accepted because of the heterogeneity among the studies (x2=34.20,P<0.05,I2 =68%).The prevalence of glaucoma was higher in the OSAHS group than that in the control group (OR =1.87,95% CI:1.21-2.90,Z=2.82,P<0.05).The subgroup analysis of OSAHS showed that the OR (95% CI) of mild, moderate and severe OSAHS groups versus control group was 3.61 (0.56-23.43),4.17 (0.47-36.91) and 6.95 (1.14-42.26),respectively.Sensitivity analysis confirmed that the OR value fluctuated in 1.74-2.16 and closed to 1.87.Funnel graphy exhibited a asymmetry appearance among the literatures, which suggested a possible publication bias. Conclusions OSAHS is one of the risk factors of glaucoma. The serious OSAHS is associated with an increased risk of glaucoma.

OBJECTIVE To investigate the bacterial distribution and antibiotic resistance of main pathogens from urinary tract infection (UTI) in our hospital for the guidance of rational use of antibiotics. METHODS The antibiotic resistance of clinical pathogen isolates from urinary tract infection by routine bacterium culture from Jan 2006 to Dec 2007 were studied retrospectively. The extended spectrum ?-lactamases (ESBLs) were detected out to the Gram-negative bacilli. RESULTS There were 722 strains of pathogens in the whole 2773 urinary samples with the isolating rate 26.0%. Most of the Urinary tract infections of patients were caused by Gram-negative bacilli (69.4%),then by Gram-positive cocci (16.3%) and fungi (14.3%). The most common pathogens in urinary tract infection were Escherichia coli (47.9%),Enterococcus faecais (7.9%),E. faecium (4.0%),Proteus mirabilis (3.9%),and Pseudomonas aeruginosa (3.5%). The Gram-negative bacilli were found to be sensitive to imipenem and meropenem, but highly resistant to the most other antibiotics, while P. mirabilis was susceptive to amikacin and cephalosporins. The Gram-positive cocci were more sensitive to nitrofuvantoin and vancomycin, but highly resistant to penicillin, oxacillin and SMT. CONCLUSIONS E. coli is still the primary urinary pathogen among patients, and highly resistant to a lot of antibiotics,We should carry out cultivation, isolation, and antimicrobial susceptibility testing as soon as possible to guide reasonable clinical drug therapy.

Objective To observe the effect of early non-invasive positive pressure ventilation(NIPPV)on the treatment of serious hypoxemia induced by acute left heart failure. Methods Forty patients with acute left heart failure( Grade Ⅳ heart function)were randomly divided into two groups. Patients in both groups accepted supportive treatment included cardiotonics, diuretics, vasodilators, in additional to these high concentrations of oxygen were given in conventional group, and non-invasive positive pressure ventilation were given in NIPPV group by biphasic positive airway pressure(BiPAP). Systolic blood pressure, heart rate, respiratory rate, blood-gas analysis( pH, PaO2, PaCO2, SaO2 )and clinical signs were observed at 2 hours after treatments. Results Compared to control,RR( [ 19.55 ± 1.88] vs [21.85 ±3.51 ] ) BPM] ,HR ( [96.40 ±2.80] vs[ 103.20 ±6.78 ] BPM), SBP ( [ 116.30 ± 8.95 ] mm Hg vs [ 122.50 ± 6.13 ] mm Hg), pH (7.404 ± 0.027 vs 7.358 ±0.05) ,SaO2 ( [93.57 ± 1.18]% vs [91.97 ± 1.85]% ) ,PaO2 ( [75.58 ±4.61 ]mm Hg vs [68.38 ±7.95]mm Hg), PaCO2 ( [ 38.69 ± 3.06 ] mm Hg vs [ 43.61 ± 2.65 ] mmHg) were significantly different in NIPPV group( t = 2.582,4.146,2.558,3.534,3.256,3.505,5.428, Ps < 0.05 ). We found no significant differences in the comparisons before treatments. Hypoxia improved in NIPPV group, and the total effective rate was 95% in NIPPV group and 70% in control group, which showed significant difference( x2 =4.329 ,P <0.05 ) Conclusion BiPAP non-invasive positive pressure ventilation combined with routine treatment in treating heart failure, could promptly correct hypoxia, improve heart function and shortening disease course.
