1.Separation of Ephedrine Isomers with Non-aqueous Capillary Electrophoresis

AIM Here the development of a simple, rapid and simultaneous method for the separation of five ephedrine alkaloids obtained with non-aqueous capillary electrophoresis (NACE) were reported. The effects of the electrolyte, non-aqueous solvent on the separation were also discussed. METHODS A running buffer of 50 mmol/L ammonium acetate in methanol was found to be the most suitable for this separation. RESULT The best separation was achieved within 8 minutes. CONCLUSION The separation is not only dependent on differences in the pK value of the alkaloids but also on the intramolecular interaction and solvation. So much improved selectivity could be achieved in nonaqueous medium.
2.Alkaloids isolated from Buyang Huanwu Decoction and their structure identification

Objective To study the antithrombotic compounds and bioactive fraction from Buyang(Huanwu) Decoction(BHD).Methods Four alkaloids have been isolated from BHD by repeated chromatographies.Their structures were elucidated by means of spectral and single-crystal Xray diffraction analysis.Results They are N-(3′-maleimidyl)-5-hydroxymethyl-2-pyrrole formaldehyde(Ⅰ),4-carbamoyl-2-pyrrole carboxylic acid(Ⅱ),N-(1′-D-deoxyxylitolyl)-6,7-dimethyl-1,4-dihydro-2,3-quinoxalinedione(Ⅲ),and 2,3,4,9-tetrahydro-1H-pyrido indole-3-carboxylic acid(Ⅳ).Conclusion All these compounds are isolated from BHD for the first time and compounds Ⅰ-Ⅲ are new alkaloids.
3.Isomers of amygdalin in BUYANG HUANWU TANG and its production

Object To study the isomers of amygdalin in BUYANG HUANWU TANG and its production Methods Amygdalin was isolated from both peach seeds and BUYANG HUANWU TANG by using various column chromatography Their structures were identified by the various spectral data Results Amygdalin had been isolated from n BuOH fraction of aqueous extract of BUYANG HUANWU TANG and found to be a pair of D , L epimers and their ration was 1∶1 It was also found that the structures and the ration of D , L epimers of amygdalin in decoction of single peach seed were similar to that in BUYANG HUANWU TANG. The peach seed only gave D amygdalin when it was extracted in 95% EtOH at reflux temperature, and D amygdalin cannot be isomerized when it was treated in water at 100 ℃ Conclusion Isomerization of D amygdalin results from interaction between it and other compounds of peach seed in water at high temperature, and has no evident relation to other constituents in BUYANG HUANWU TANG L amygdalin is a new compound generated due to decoction of peach seed
4.Determination of ephedrine alkaloids in Ephedra sinica by nonaqueous capillary electrophoresis

Object To develop a rapid method for the determination of ephedrine alkaloids in Ephedra sinica Stapf by nonaqueous capillary electrophoresis (NACE) and evaluate the extracting method by determining the amount of alkaloids Methods The buffer contained 50 mmol/L ammonium acetate in methanol without any additives was used And the detection wavelength was 210 nm Results The best separation result was achieved within 8 min Linearity was obtained in range of 9 8-147 0 ?g/mL pseudoephedrine, 6 8-102 0 ?g/mL for norephedrin, 9 4-141 0 ?g/mL for ephedrine, 4 8-72 0 ?g/mL for norpseudoephdrine, 6 8-102 0 ?g/mL for methylephedrine respectively The recovery range of these five alkaloids was 95 0%-100 4% Conclusion This method is rapid and accurate for the quantitative analysis of ephedrin alkaloids in E sinica
5.Clinical analysis of 12 cases of nasal surgery in patients with coronary heart disease after accepted PCI.

OBJECTIVE:
To investigate the perioperative treatments of endoscopic sinus surgery for nasal diseases in patients with coronary heart disease after PCI.
METHOD:
Clinical data of 12 cases of endoscopie-assised surgery such as nasal tumors resection,functional sinus surgery,correction of deviated nasal septum,low-temperature plasma hemostasis in patients with coronary heart disease after PCI were analyzed retrospectively.
RESULT:
Serious bleeding did not take place with the 12 cases during surgery, and surgery progressed smoothly; one of patients had heavy nosebleed after surgery, however her condition was stable when received active treatment. Follow-up 3 months to 2 years, nasal diseases of 12 patients recovered well and symptoms were relieved; cardiovascular events such as hemorrhage, thrombosis and so on did not occur.
CONCLUSION
Due to physiological function of the heart dysfunction in patients with coronary heart disease after PCI,they often accompany a number of trouble issues such as medical disorders, oral antiplatelet drugs, surgery affordability loss and increase surgical risk. Correct and effective perioperative treatments, strictly surgical indications are really necessary which can keep patients safe through perioperative period.

6.Isolation and identification of four isoflavones and one pterocarpan from BUYANGHUANWUTANG

Object To study the antithombotic portion and bioactive fraction of BUYANGHUANWUTANG (BHT), a decoction of astragalus in combination with peony and several other ingredients. Methods The EtOAc portion of BHT extract was repeatedly separated by chromatography and identified by means of spectral analysis. Results One pterocarpan, (6aR, 11aR) 9, 10-dimethoxypterocarpan-3-O-?-D-glucopyanoside (Ⅴ) and four isoflavones, formononetin (Ⅰ), calycosin (Ⅱ), formononetin-7-O-?-D-glucopyanoside (Ⅲ), and calycosin-7-O-?-D-glucopyranoside (Ⅳ) were identified. Conclusion All five compounds were isolated for the first time from BHT.

Objective: To study the chemical constituents of the stem bark of Albizia julibrissin Durazz. Methods: Chemical constituents were isolated by the repeated chromatography methods and their structures were identified by spectral analysis. Results: Five compounds were obtained as follows: 3 O [? D xylopyranosyl (1→2) ? D fucopyranosyl (1→6) ? D glucopyranosyl] 21 O (6S) 2 trans 2 hydroxy methyl 6 methyl 6 O ? D quinovopyranosyl 2,7 octadienoyl acacic acid 28 O ? D glucopyran osyl (1→3) [? L arabinofuranosyl (1→4)] ? L rhamnopyranosyl (1→2) ] ? D glucopyranosyl ester (1). acacigenin B (2), julibrotriterpenoidal lactone A (3), machaerinic acid lactone (4), acacic acid methylester (5). Conclusion: Compound 1 was isolated directly from this plant for the first time.
8.Clinical application evaluation of thrombus aspiration on emergency percutaneous coronary intervention of patients with ST-segment elevation type of myocardial infarction

Objective To investigate the influence of thrombus aspiration on ventricular remodeling of patients with ST?segment elevation type of myocardial infarction ( STEMI ) who underwent emergency percutaneous coronary intervention ( PCI) and its predicative value for patients′ prognosis. Methods Three hundred and eight patients who were diagnosed with STEMI line emergency PCI and hospitalized in the Cardiology Department of Jinghai Clinical College of Medical University of Tianjin from March 2011 to March 2014 were retrospective analyzed, including 152 patients received thrombus aspiration during primary PCI as thrombus aspiration group,and the remaining 156 patients without thrombus aspiration as control group. Indexes of myocardial perfusion levels such as ST?segment resolution 2 hours after PCI and TIMI myocardial perfusion grade were measured in all patients. All patients accepted echocardiography examinations to detect left ventricular end diastolic diameter(LVED) and left ventricular ejection fraction(LVEF) 10 days after PCI. The indexes differences of the two groups,secondary end points major cardiac adverse events(MACE) at 6 months were compared. Results There was no statistical difference between the ratio of two groups for ST segment fell more than 50%( 78. 9%( 120/152) vs. 71. 2%( 111/156) ,χ2=0. 428,P=0. 669) . But for ST segment fell more than 70%,the ratio of the thrombus aspiration group was significantly higher than that in the control group ( 73. 7%( 112/152 ) vs. 47. 4% ( 74/156 ) , χ2 = 4. 701, P = 0. 001 ) . Two groups of patients were treated by echocardiography 10 days after PCI,showed the LVED in the thrombus aspiration group was lower than that in the control group((50. 2±4. 7) mm vs. (51. 6±4. 6) mm,t=2. 642,P=0. 008),while the LVEF value was higher than that in the control group((56. 9±4. 9)% vs. (49. 4±4. 2)%,t=14. 434,P=0. 001). Recurrent angina pectoris decreased significantly in the thrombus aspiration group at 6 months ( 4. 61% ( 7/152 ) vs. 10. 90%(17/156),χ2=2. 056,P=0. 040). Conclusion STEMI patients adopt thrombus aspiration that undergoing emergency PCI,not only can improve myocardial reperfusion,especially microcirculation reperfusion, but also can improve the left ventricular systolic function and the prognosis of patients.

10.Effect of structured health education on quality of life and the end point of patients with coronary disease: a systematic review and Meta-analysis

Objective We sought to evaluate the effect of structured health education on quality of life and the end point of patients with coronary disease with evidence-based method.Methods The Cochrane Library,Cochrane Database of Systematic Reviews (CDSR),Medline (OVID),Pubmed and Web of Science were searched to collect the randomized controlled trials (RCT) which mentioned the structured health education to CHD patients,the quality of the RCTs was evaluated by Jadad Score,then effective data were extracted and analyzed by Meta-analysis with RevMan 5.2 software.Results A total of 12 eligible studies of RCT were included in the study with Jadad Score evaluations at B level or above.There were 20 476 cases in the intervention group,14 522 cases in the control group.Meta-analysis showed that the quality of life scores of the intervention group were significantly higher than those of the control group,weighted mean difference was 3.29,95% CI (0.49-6.09).No significant difference was found in cardiac morbidity outcomes,re-hospitalization and the mortality rate,relative risk,95% confidence interval were respectively 0.72(0.50-1.04),0.98(0.86-1.11),0.77(0.58-1.03).Conclusions The structured health education can improve the quality of life of patients with coronary disease,but has no obvious effect on the cardiac morbidity outcomes,re-hospitalization and mortality rate.We suggest transitional care to be carried out in future research to decrease the incidence rates of them.
