Objective To study effect of warfarin combined with lanatoside C on heart function indexes, its curative efficacy in elderly patients with dilated cardiomyopathy, and search for a better therapy for elderly patients with dilated cardiomyopathy.Methods 192 cases of patients with dilated cardiomyopathy were randomly divided into control group and treatment group, 96 cases in each group.Patients in control group were treated with warfarin, and treatment group were given warfarin combined with lanatoside C.After treatment, clinical curative effect evaluation, left ventricular ejection fraction (LVEF), early filling rate/advanced rate (E/A), early peak flow velocity (EPFV), atrium peak flow velocity (APFV), and cardiac indexes related to cardiac volume: left ventricular end systolic volume (LVESV), left ventricular end diastolic volume (LVEDV), left ventricular mass (LV), cardiothoracic ratio (CTR), and 6min walk test were observed and recorded in two groups.Results The total effective rate in treatment group (85.42%) was significantly higher than that of control group (68.75%, P<0.01).The LVEF, E/A, EPFV, APFV in two groups of patients were improved compared with those before treatment (P<0.01), and the improvement of the above indexes in treatment group were significantly better than those in control group (P<0.01).The LVESV, LVEDV, LV, and CTR were improved in two groups after treatment than before treatment (P<0.01), and the improvement of above indexes in treatment group were significantly better than control group after treatment(P<0.05).6min walking test results were improved in two groups after treatment than before treatment ( P<0.05 ) , and the treatment group was significantly better than control group (P<0.05).There were no significant differences between two groups in adverse reactions.Conclusion Warfarin combined with lanatoside C in the treatment of elderly patients with dilated cardiomyopathy is a safe and effective treatment method.

Objective To compare curative effect of chlorthalidone and bisoprolol in treatment of hypertension in the elderly man.Methods 2013 January to 2013 December in our hospital treatment of 60 elderly patients with hypertension were randomly divided into treatment group and control group, each of 30 cases.Treatment group was treated with chlorthalidone tablet, control group was treated with imported bisoprolol tablets.Observed and recorded the two groups before and after treatment the clinical curative effect, blood pressure, fasting blood glucose, uric acid, creatinine, urea nitrogen and adverse reaction.Results The total effective rate of the treatment group ( 90.00%) was significantly higher than that of the control group (73.33%), the difference was statistically significant (P<0.05); compared with before treatment, the treatment group before and after treatment systolic blood pressure and diastolic blood pressure were significantly decreased, the difference was statistically significant (P<0.05);the control group after treatment systolic blood pressure diastolic blood pressure, there was statistical significance between before and after treatment ( P<0.05 );compared with the control group, the treatment group after treatment systolic blood pressure was significantly better than the control group (P<0.05), while the two group of diastolic blood pressure had no significant difference in improving.The two groups before and after treatment, fasting blood glucose, uric acid, creatinine, urea nitrogen were no statistically significant difference.The treatment group and the control group had no obvious adverse reactions.Conclusion The curative effect of chlorthalidone treatment of hypertension in the elderly with good curative effect, few side effects, and beffer than bisoprolol.

Objective:To investigate the dynamic changes of intestinal flora in septic model mice.Methods:Forty-two male SPF C57BL/6 mice were selected, the sepsis model was reproduced by cecal ligation and puncture (CLP), and the experimental mice were divided into CLP 6-12 hours group ( n = 9) and 1, 2, 3 days group (all n = 10) and Sham group ( n = 3) according to the time points after modeling. Intestinal flora 16S rRNA sequencing was carried out within feces from the colonic lumen of mice, and the effective sequences were clustered to obtain an operational classification unit (OTU) for statistical analysis of biological information, including Alpha diversity analysis, species composition analysis, principal coordinate analysis (PCoA analysis) and species difference analysis (LEfSe analysis), the dynamic changes of intestinal flora after CLP modeling were analyzed. Results:Compared with Sham group, with the prolongation of modeling time, the Alpha diversity of intestinal flora decreased, showing a decrease in community richness index (3 days after CLP: Chao index 1 was 367.9±162.6 vs. 508.3±105.9, Ace index was 372.5±151.9 vs. 498.8±104.2), Shannon index decreased in community diversity index (3 days after CLP: 2.57±1.06 vs. 4.30±0.57, Simpson index increased (3 days after CLP: 0.26±0.19 vs. 0.04±0.03), suggesting that with the progress of CLP modeling time, the richness and diversity of intestinal flora decreased. According to species composition analysis, in OTU level, OTU 633 accounted for the highest proportion in CLP 1 day group (24.79%), OTU 1016 was dominant in CLP 2 days and 3 days groups, and the highest proportion was 61.75% in CLP 3 days group; in genus level, the abundance of norank_f_Muribaculaceae showed a slight increase and then a significant decrease with time, the abundance of Escherichia-Shigella increased significantly in CLP 2 days and 3 days groups, the abundance of Lactobacillus increased first and then decreased with time, the abundance of Bacteroides showed a trend of gradual increase with time. PCoA analysis suggested that CLP 6-12 hours group had a higher structural similarity with Sham group. The flora structure changed gradually with the time after modeling, and the change was significant after 3 days of CLP. LEfSe analysis indicated that the main components that caused the differences among the groups were g _norank_f_Muribaculaceae, g _Prevotellaceae_UCG-001, s_uncultured_Bacteroidales_bacterium_g_norank_f_Muribaculaceae, g_Parabacteroides, Escherichia-Shigella and OTU 1016. The differences in abundance of Escherichia-Shigella and OTU 1016 among the five groups ranked first in the genus level and OTU level respectively; the abundance of Escherichia-Shigella gradually increased from 0.01% (0%, 0.02%) in 6-12 hours after CLP to 44.79% (3.71%, 53.75%) in 3 days with time, and the abundance of OTU 1016 increased from 0.01% (0%, 0.02%) in 6-12 hours after CLP to 44.69% (3.66%, 53.64%) in 3 days with time. Conclusion:Intestinal dysbiosis occurred in all CLP model groups, the diversity of the flora gradually decreased with the progress of modeling time, Escherichia-Shigella gradually became the dominant bacteria.

Objective To find out the clinical characteristics and morbility factors of intravascular catheterrelated bloodstream infection(CRBSI). Methods Totally 21 patients who had CRBSI in neonatal intensive care unit were investigated retrospectively. Results The distribution of CRBSI was higher in very low birthweight preterm infants, gestational age among 28 ～34week, whose intravascular catheter remaining time were obove three weeks. Principal clinical presentation of CRBSI were poor feeding, unaocountable tachycardia, temperature instability, stressed hyperglycemia,refractoriness metabolic acidosis. The most common pathogens were coagulase-negative staphylococci (35.7%), Klebsiella pneumonia, bacilli ( 11.9% ) Staphylococcus aureus (9.5 % ), Pseudomonas aeruginosa( 7. 1% )and Enterobacter cloacae(7.1% ). Conclusions The clinical manifestations of CRBSI were concealment,and reducing the time of inserted central catherization and total parenteral nutrition, strengthening the nutrition of body would provide effective prevention of CRBSI.

Objective To explore the diagnosis, treatment and prognosis of acute perforation of gnstric cancer. Methods The clinical data of 24 patients with acute perforation of gastric cancer who had been admitted to Guangzhou General Hospital of PLA from July 1996 to December 2008 were retrospectively analyzed. Results Of all patients, 4 were treated by perforation repair, 2 by perforation repair combined with gastrojejunostomy, 11 by palliative subtotal gastrectomy, 2 by radical gastrectomy, and 5 by radical gastrectomy after perforation repair. The mean survival time of patients treated by perforation repair, perforation repair combined with gastro-jejunostomy, partial gastrectomy and radical gastrectomy were (4±5), (6±9), (12±7), and (25±9) months, respectively. Conclusions Early diagnosis and reasonable operation are the keys to decrease the morality and increase life quality for patients who suffered from acute perforation of gastric cancer. Different pathologic stages maybe an important factor in deciding the prognosis.

Objective To study the effect of acute lymphoblastic leukemia (ALL) children bone marrow mesenchymal stem cells (MSCs) on the resistance of K562cell atd mechanism in vitro.Method MSCs were obtained from AL children bone marrow after derivation, cultivation and identification.The coculture of MSCs and K562 and K562 suspension were established.Effects of MSCs on the growth of K562 cells were investigated in vivo.The two kinds of cells treated with different concentration of adriamycin (ADM) and the rate of apoptosis was evaluated by flow cytometry.Cell cycle was determined by flow cytometry.RT-PCR was used to detect Bcl-2 and Bax in K562 cells.Result Compared with the cell growth curve of K562 alone, the K562 cell co-cultured with MSCs grew slower and the exponential phase of growth was not obvious.The apoptosis index of the K562 cells co- clutured with MSCs was (9.19 ±0.53)% examined by flow cytometry, and that of the K562 cells alone was 4.00 ± 0.37% respectively( P ＜ 0.05 ).The percentage of cells at G0/G1 phase was (50.2 ± 2.26) % and that at S phase was (37.03 ± 3.50) % in the group of K562 alone, but those of the K562 cells co - cultured with MSCs were (80.95 ± 3.83) % and ( 17.40 ± 1.50)% respectively( P ＜0.05).The result of RT-PCR suggested expression of Bcl-2/Bax of the K562 cell co-cultured with MSCs was higher than K562 alone.Conclusion ALL children MSCs suppressed the growth of K562 cell in vitro.Adhesion made K562 depress sensitive to ADM.The mechanism was perhaps caused by adhesion with MSCs, K562 cell cycle was changed and related to Bcl-2 gene high level expression.
