Objective To explore the application value of aspirin and clopidogrel dual antiplatelet treatment combined with ABCD3-Ⅰ scores in treatment of transient ischemic attack (TIA).Methods Risk factors were evaluated and imaging was performed in 48 h in one hundred and twenty patients with first attack of TIA [magnetic resonance imaging (MRI),magnetic resonance angiography (MRA),diffusion weighted imaging (DWI) scan].According to the ABCD3-Ⅰ evaluation method,the patients were divided into low risk group,moderate risk group and high risk group.Single resistance group was given aspirin 100 mg/d; double resistance group was given aspirin 100 mg/d combined with clopidogrel 75 mg/d.Then the incidence of ischemic stroke in each group within 7 days was observed,and the effects of dual anti-platelet and aspirin monotherapy were compared.Results Within 7 d after TIA,low-risk single resistance group and low-risk double resistance group had no stroke; 9 cases (42.9％,9/21) had stroke in moderate-risk single resistance group,2 cases (2/19) had stroke in moderate-risk double resistance group,and there was significant difference (P ＜ 0.05) ; 9 cases (9/18) had stroke in high-risk single resistance group,3 cases (15.0％,3/20) had stroke in high-risk double resistance group,and there was significant difference (P ＜ 0.05).Three patients in single resistance group and 4 cases in double resistance group showed gastrointestinal adverse reactions such as nausea,acid reflux,and there was no significant difference between 2 groups (P ＞ 0.05).Each group during treatment showed no bleeding tendency,and liver and kidney function in patients was normal.Conclusion In patients of the moderate risk and high risk of ABCD3-Ⅰ scores,dual anti-platelet may have an advantage over single druge in the prevention of ischemic stroke.

Objective To investigate the high-risk factors of premenstrual acne to provide evidence for disease prevention and treatment.MethodsA total of 660 patients with premenstrual acne during February 2006 and October 2009 were retrospectively reviewed.Pillshbury method was used for diagnose and classification of premenstrual acne.Six hundred and sixty complete questionnaires were received for statistic analysis.Results Age and season showed no effect on moderate to sever premenstrual acne.However,spicy and fried food(x2=5.68),mental stress(x2=14.58),gastrointestinal disorders(x2=8.07),family history(x2=12.79),longer hairs around areolas(X2=11.88),and thyroid gland,adrenal gland or adnexal cysts(x2=9.72)had significantimpact on moderate to severpremenstrual acne(all P＜0.05).In addition.inflammatory papules on frontal partress were found in 67.4% of moderate premenstmal acne.ConclusionFood intake,mental health,gastrointestinal disorders,family history,longer hair around areolas,and thyroid gland,adrenal gland,adnexal cysts may be high-risk factors of premenstrual acne.

Objective To formulate genuine case mix plans using medical record information available in China. Methods An account was given of the research background of case mix studies, the current status of the studies and applications at home and abroad, the case mix model for measuring medical "output", data sources and statistical methods of classification. Results Case mix plans for army inpatients, civilian inpatients and outpatients were respectively formulated, and software for calculating the case mix index of a hospital which can run on Windows 95/98 computer systems was developed. Conclusion The theoretical, methodological and data conditions necessary for the formulation of case mix plans based on the first pages of medical records in China are now ripe and the case mix plans formulated on the basis of data from large samples can now be used for the control of medical costs, the assessment of a hospital's medical "output" and the estimation of the financial compensations for hospitals.

Objective To explore the value of ABCD3-Ⅰ score in predicting prognosis of transient ischemic attack (TIA).Methods A total of 106 TIA patients were evaluated according to ABCD2,ABCD3 and ABCD3-Ⅰ criteria.The occurrence of cerebral infarction within 7 d and 90 d was observed.The patients with ABCD3-Ⅰ 0-3 scores were as low risk group (36 cases),the patients with ABCD3-Ⅰ 4-7 scores were as moderate risk group (36 cases),the patients with ABCD3-Ⅰ 8-13 scores were as high risk group (34 cases).Results The occurrence of cerebral infarction within 7 d and 90 d in high risk group was lower than that in low risk group and moderate risk group [within 7 d:23.5％(8/34) vs.2.8％(1/36) and 5.6％ (2/36),within 90 d:38.2％ (13/34) vs.2.8％ (1/36) and 11.1％ (4/36)],and there was significant difference (P ＜0.01).Symptom duration time ≥10 min in ABCD3-Ⅰ score,concomitant diabetes,double TIA,ipsilateral carotid artery stenosis ≥50％,high sign in diffusion weighted imaging were the risk factors for the occurrence of cerebral infarction within 7 d and 90 d in TIA patients (P ＜ 0.05).The area under the receiveroperating characteristic curve for predicting the occurrence of cerebral infarction within 7 d and 90 d in TIA patients by ABCD3-Ⅰ score was higher than that by ABCD2 and ABCD3 score (0.914 vs.0.614 and 0.877,0.869 vs.0.633 and 0.773).Conclusion The ABCD3-Ⅰ score is a simple and effective way to judge the short-term prognosis of TIA and can help to identify the different risk levels,so it is helpful in stratification treatment.

Many medical journals have already digitalized their publishing procedure,but not achieved digital publishing yet.Utilizing digital technology,digital publishing can achieve the functions that paper media cannot,such as information storage,fast search,real-time publishing,individualized information and interaction with readers.These functions entail digitalization of medical journals,and involve a range of macro- and micro-modifications,including laws,industry policies,personnel training,and culture development.There is a long way to go for Chinese medical journals to enter the stage of digital publishing.

Objective To evaluate the efficacy of administration of ampicillin/sulbactam and imipenem/cilastatin in treatment of aspiration pneumonia in aged patients. Methods 26 aged patients was treated with aspira-tion pneumonia (2.25～3.00 g) through vessel injection twice perday,and another 26 patients was treated with imi-penem/cilastatin (0.5 g) through vessile injection every 6～8 hours. We evaluated the efficacy through assessing the temperature,the chest radiography,WBC and CRP,incidence of side effects,the time to cure and the clearance of bacteria. Results The cure rate was 84.6% (22/26) and 92.3% (24/26),respectively (χ2=0.19,P=0.66). The time to cure was 8.2±2.8 days and 7.5±1.6 days for the groups (t=1.107,P=0.274). Gastrointestinal side effects were observed in the group of ampicillin/sulbactarn;slight abnormality of liver function occurred in the group of imipenem/cilastatin ;all above adverse effects in two groups were rapidly and completely disappeared after therapy stopped. Conclusion Both ampicillin/sulbactam and imipenem/cilastatin are effective antibiotics to treat aspiration pneumonia in aged patients.

Objective To investigate the acoustic characteristics of snoring sound in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) and with simple snoring. Methods 22 patients with OSAHS and 15 with simple snoring were included in this study. Natural overnight snoring was digitally recorded and portable sleep mo-nitoring was performed simultaneously. 10 snores, which were the 1st snores after 10 cycles of obstructive apnea, from each patient in OSAHS group, and 10 snores from each patient in simple snoring group were analyzed in the time and frequency domains. Results The sound waves of snoring in the two groups exhibited different patterns both in the time and frequency domains. The snoring spectrum of patients with simple snoring showed distinct fun-damental- harmonic structures which were not clear in patients with OSAHS. The central frequency of the patients with OSAHS was higher, and 800 Hz power ratio was lower than those of the patients with simple snoring. In the OSAHS group, the central frequency of the patients with moderate-to-severe OSAHS was higher, and 800 Hz power ratio was lower than those of the patients with mild OSAHS. The differences of the two parameters were of statistical significance. Conclusion The snoring sounds in patients with OSAHS and with simple snoring have dif-ferent characteristics in time and frequency domains, indicating that it is feasible to research the OSAHS by way of snore monitoring and analyzing technique.

OBJECTIVE: To discuss the main influence factors of sound pressure level parameters in patients with simple snoring (SS) and obstructive sleep apnea hypopnea syndrom (OSAHS). METHOD: Eighty-four cases with snoring disease underwent polysomnography and simultaneously snoring sound pressure level recording. The correlations between AHI, age, BMI, abdomen circumference, neck circumference, the lowest oxygen saturation total apnea time and sound pressure level parameters: equivalent continuous sound level(LAeq)and maximum sound pressure level(L10) were analyzed. RESULT: LAeq was significantly correlated to AHI (P= 0. 000) and BMI (P= 0. 007), and the odd ratios of AHI and BMI were 5. 74,2. 09 respectively, but it was unrelated to age, abdomen circumference, neck circumference, the lowest oxygen saturation and total apnea time. A significantly association also existed between L10 and AHI(P=0. 000), BMI(P=0. 032), and the odd ratios were 4. 11 and 2. 33 respectively. Other factors had nothing to do with L10. CONCLUSION The main factors which affect the snoring sound pressure level parameters LAeq and L10 are AHI and BMI.

Objective To translate the English version of Infant Feeding & Nutrition Checklist For Congenital Heart Disease (IFNC:CHD) into Chinese,and to test the reliability and validity of the Chinese version of IFNC:CHD.Methods After obtaining authorization from the developer,the IFNC:CHD was translated and culturally adapted into Chinese version.The reliability and validity of the Chinese version of IFNC:CHD were tested in 168 patients.Results The Cronbach's α coefficient was 0.804.Two nurses used the scale to evaluate the consistency of the results,and the Kappa coefficient was 0.812.The content validity index of item ranged from 0.80 to 1.00,and the average index was 0.90 for the total scale.The correlations coefficients between each dimension score and the total score ranged from 0.339 to 0.474 (P＜0.01),and the correlations coefficients between each dimension score ranged from 0.198 to 0.861(P＜0.01,P＜0.05),Three factors were extracted from exploratory factor analysis,and the cumulative variance contribution rate was 72.66％.Conclusion The Chinese version of IFNC:CHD has been proved to have good reliability and validity.It can be used to assess infant feeding and nutritional risk of congenital heart disease in the Chinese settings.
