Objective To observe the evaluation of multi‐tumor marker protein chip in breast cancer with neoadjuvant chemo‐therapy ,and to find high saitivity tumor makers ,then evaluate the clinical significance .Methods Forty health examination persons (control group) and sixty‐four breast cancer patients(experimental group) were accepted with multi‐tumor marker protein chip in‐spection ,then we evaluated the markers of diagnosis .The experimental group patients were accepted four cycles preoperative chem‐otherapy with TAC plan ,and tested multi‐tumor marker protein chip three weeks after therapy .Then we evaluated the clinical sig‐nificance of the markers above‐mentioned .Results The positive rate of CA153 ,CA125 ,CEA and FT was 32% ,27% ,21% and 19% in the experimental group ,it indicated significant change compared with the control group (P<0 .05) .And it indicated signifi‐cant reducing change of CA153 ,CA125 and CEA in the experimental group (P<0 .05) after therapy .The sensitivity would be in‐creased when the four markers were tested combined .Conclusion CA153 ,CA125 ,CEA and FT could be used as assistance diagno‐sis index ;CA153 ,CA125and CEA could keep consistence to the curative effect of neoadjuvant chemotherapy and have clinical value .

Objective To investigate the expression of DNA methyltransferase 1 (DNMT1)mRNA in tissue of human transitional cell carcinoma of bladder(TCCB)and its clinical significance.Methods DNMT1 mRNA expression in 35 tumor tissues and 10 normal bladder tissues was measured by real-time PCR.Results Higher expression of DNMT1 transcripts was detected in tumor(3.25±0.74)than in normal tissues(1.53±0.44,P＜0.001).The expression level of DNMT1 mRNA in stage Tis-T1,T2-T4 was 3.14±0.67,3.31±0.84;in grade Ⅰ,Ⅱ,Ⅲ was 3.13±0.59,3.25±0.64,3.43±0.55;in patients older tham 40 years and younger than 40 years was 3.34±0.50.3.01±0.27;in male and female group was 3.31±0.42,3.01±0.20;in primary and recurrence group was 3.21±0.63,3.45±0.33.mRNA levels did not correlate with grade.stage and recurrence (P＞0.05).Higher levels of expression were associated with advanced age(P＜0.05).Male group had significant higher levels of expression than female group(P＜0.05).Conclusions Over-expres-sion of DNMT1 may play an important role in TCCB.DNMT1 can be an important therapeutic target in TCCB.

Objective To investigate the possible association between the CCND1 A870G polymorphism and susceptibility to prostate cancer(PCa)in a Chinese population.Methods The CCND1 A870G genotypes were determined by TaqMan assays in 245 PCa cases and age-matched 245 controis.Odds ratios(OR)for PCa and 95%confidence intervals(CI)from unconditional logistic regression models were used to evaluate relative risks.Age,a potential PCa risk factor,was included in the logistic regression models as a covariate in the multivariate analyses on genotype and PCa risk.Results The frequencies for G and A alleles were 0.476,0.524 in PCa cases,and 0.408 and 0.592 in controls,respectively.The G allele was marginally significantly associated with the presence of PCa (P=0.054)and had an increased risk for PCa(OR=1.31,95%CI=1.00-1.72)as compared to theA allele.Compared to AA homozygote,AG heterozygote had a 1.43-fold increased risk(95%CI=0.89-2.31,P=0.142),whereas GG homozygote had a significantly higher 2.02-fold increased risk (95%CI=1.07-3.80,P=0.029)of PCa(Armitage's trend test,P=0.029).The G allele(AG or GG genotype)was more frequently found in PCa patients with metastasis than those without metastasis(P=0.014).Conclusion The G allele of CCND1 A870G is associated with the presence of PCa and GG homozygote is potentially one of the genetic risk factors for PCa in Chinese population.The G allele may be associated with the progression of PCa metastasis.

Objective To discuss the influence of blood exchange transfusions on hemato- microenvironment of neonates with hyperbilirubinemian.Methods Blood exchange transfusion was performed in 35 neonates.Mean transfusion volume was (136.9±27.4) ml/kg.The speed of blood exchange transfusion was 92 ml/(kg·h).Blood bilirubin,blood electrolyte,blood routine and blood sugar levels were measured before,during and after the transfusion.Results Serum total bilirubin (TB) decreased by 53.78% after blood exchange,but there was no change in Na~+,Cl~-,and blood sugar (P＞0.05),and K~+ was remarkably decreased from (4.45±2.40)mmol/L to (3.87±0.52)mmol/L (t=21.979,P＜0.05).TB,Hb,PLT,WBC,Ca~(2+) were decreased[TB from(511.52±80.21) μmol/L to (236.41±66.54) μmol/L;Hb from (145.20±11.70) g/L to (128.66±2.54) g/L;PLT from (207.84±70.67)×10~9/L to (134.86±48.61)×10~9/L;WBC from (16.90±10.56)×10~9/L to (8.94±7.44)×10~9/L,and Ca~(2+) from (1.26±0.32) mmol/L to (0.99±0.22) mmol/L,P＜0.05 or ＜0.01].Conclusions Blood exchange transfusion can not influence Na~+,Cl~-,and blood sugar,but has remarkable effect on total bilirubin,Ca~(2+),Hb,K~+,PLT,and WBC.

Objective To observe the morphological damages of hyperbilirubinemia on rat central nervous system, and evaluate the therapeutic effects of different routes of basic fibroblast growth factor (bFGF) administration. Methods Totally 90 Wistar rats were inflicted to hyperbilirubinemia models and then divided randomly into 3 groups:untreated group (BG),treated-1 group (T_ 1 G), treated-2 group (T_ 2 G), and another 30 rats served as control group (CG). bFGF injection were given at 30 min, 1, 3, 5 and 7 d by abdominal cavity (T_ 1 G,) and cranial cavity (T_ 2 G). GAP-43 expression levels and the micro-morphological features were observed with immunohistochemical assay and transmission electron microscopy respectively at 2 nd, 8 st, 28 st and 60 st day. Objective To investigate the methods of steroid supplement following adrenalectomy in the patients with Cushing syndrome.Methods The methods and outcome of steroid supplement following adrenalectomy in 75 cases of Cushing syndrome were retrospectively reviewed.Results Supraphysiological doses of glucocorticoid was given during the perioperative period,and the dosage was reduced gradually.Prednisone with a dosage of 5 to 7.5 mg was given once a day on discharge.In the patients with adrenocortical adenoma,4 to 15 month of steroid supplement was required.The patients undergoing bilateral adrenalectomy required lifelong steroid supplementation.Steroid withdrawal syndrome occurred during steroid tapering in 8 patients with adrenocortical adenoma,2 of them had normal plasma cortisol level.Conclusion In the patients with Cushing syndrome,even those with normal plasma cortisol level might have steroid withdrawal syndrome episodes during steroid tapering.

Objective: To investigate the factors associated with CD4⁺ /CD8⁺ T lymphocyte ratio normalization in acquired immunodeficiency syndrome (AIDS) patients after antiretroviral therapy (ART). Methods: The data of 1 188 human immunodeficiency virus (HIV)/AIDS patients from the national ART reporting system in Yuxi City, Yunnan Province between January 1, 2006 and December 31, 2016 were retrospectively collected and analyzed. The rate of CD4⁺ /CD8⁺ T lymphocyte ratio normalization after ART was calculated by lifetable. Cox proportional hazard models were used to analyze the factors associated with CD4⁺ /CD8⁺ T lymphocyte normalization in AIDS patients after ART. The Wilcoxon rank sum test was used for comparison between groups. Results: The follow-up time was 3.8 (1.0-10.8) years. CD4⁺ /CD8⁺ T lymphocyte ratio normalization was documented in 95 patients with the rate of 1.89 per 100 person-years (95% confidence interval(CI) 1.52-2.27) after ART. The average time from ART to CD4⁺ /CD8⁺ T lymphocyte ratio normalized was 9.4 years. The cumulative normalization rate was 0.02 for the first year, 0.06 for the third year, 0.11 for the fifth year, 0.19 for the seventh year and 0.37 for the ninth year. By Cox proportional hazard models, the probability of CD4⁺ /CD8⁺ T lymphocyte ratio normalization in patients infected HIV by heterosexual contacts was 3.709 (95%CI 1.781-7.726) times higher than those by intravenous injection. The probability of CD4⁺ /CD8⁺ T lymphocyte ratio normalization in patients with baseline CD4⁺ T lymphocytes of 350-499 and more than 500 cell/μL groups were 2.792 (95%CI 1.196-6.519) and 3.832 (95%CI 1.648-8.913) times higher than those with baseline CD4⁺ T lymphocytes less than 200 cell/μL, respectively. The probability of normalization after ART in patients with higher baseline CD4⁺ /CD8⁺ T lymphocyte ratio was higher than those with baseline CD4⁺ /CD8⁺ T lymphocyte ratio≤ 0.20 (hazard ratio>1, all P<0.01). Conclusion The CD4⁺ /CD8⁺ T lymphocyte ratio normalization in AIDS patients after ART is associated with baseline CD4⁺ T lymphocyte counts, baseline CD4⁺ /CD8⁺ T lymphocyte ratio and HIV transmission mode.

The role of serum and glucocorticoid-induced kinase 1 (SGK1) pathway in the connective tissue growth factor (CTGF) expression was investigated in cultured human mesangial cells (HMCs) under high glucose. By using RT-PCR and Western blot, the effect of SGK1 on the CTGF expression in HMCs under high glucose was examined. Overexpression of active SGK1 in HMCs transfected with pIRES2-EGFP-S422D hSGK1 (SD) could increase the expression of phosphorylated SGK1 and CTGF as compared with HMCs groups transfected with pIRES2-EGFP (FP) under high glucose or normal glucose. Overexpression of inactive SGK1 in HMCs transfected with pIRES2-EGFP-K127N hSGK1 (KN) could decrease phosphorylated SGK1 and CTGF expression as compared with HMCs groups transfected with FP under high glucose. In conclusion, these results suggest that high glucose-induced CTGF expression is mediated through the active SGK1 in HMCs.

To investigate the expression and the role of three isoforms of Serum and Glucocorticoid-inducible Kinase (SGK) in experimental diabetic nephropathy (DN), 12 male C57BL/6 mice of 8-weeks-old were divided into two groups. Streptozotocin (STZ)-induced diabetic nephropathy and normal controls were analyzed at the end of the 4th week after the induction of diabetes. Renal hemodynamics and histological studies were performed. The expression of SGK1 mRNA, SGK2 mRNA and SGK3 mRNA of kidney cortex were measured by RT-PCR, and the cortical SGK1 protein was detected with Western blotting. Our results showed that the blood glucose, blood HbA1c, 24h urinary protein, creatinine clearance and the renal index were all increased in DN group. More extracellular matrix (ECM) accumulation was observed. The level of cortical SGK1 mRNA and protein were up-regulated in DN group in comparison with control group. SGK2 and SGK3 mRNA were elevated in DN mice. In DN, mRNA level of three SGK isoforms and SGK1 protein were increased significantly. It is concluded that SGKs may contribute to the early renal injury of DN.

OBJECTIVE:To observe clinical efficacy and safety of Shuxuetong injection combined with alprostadil in the treat-ment of acute cerebral infraction(ACI). METHODS:A total of 123 ACI patients selected from our hospital during Mar. 2014-Sept. 2016 selected as research subjects were divided into observation group (62 cases) and control group (61 cases) according to ran-dom number table. Control group was additionally given Alprostadil injection 2 mL+0.9% Sodium chloride injection 100 mL, ivgtt,qd,on the basis of routine treatment. Observation group was additionally given Shuxuetong injection 6 mL+0.9% Sodium chloride injection 250 mL,ivgtt,qd,on the basis of control group. Both groups were treated for 2 weeks. MPV,FIB content, hemorheology indexes,neurologic impairment degree score before and after treatment as well as clinical efficacy,the occurrence of ADR were compared between 2 groups. RESULTS:Before treatment,there was no statistical significance in MPV,FIB content, hemorheology indexes or neurologic impairment score between 2 groups (P>0.05). Compared with before treatment,MPV,FIB content,hemorheology indexes and modified Rankin scale of 2 groups were all decreased significantly;Barthel index and NIHSS scores were increased significantly,and each aspect of observation group was better than that of control group,with statistical sig-nificance(P<0.05). Total response rate of observation group was 95.16%,which was significantly higher than 85.25% of control group,with statistical significance(P<0.05). No severe ADR was found in 2 groups. CONCLUSIONS:Shuxuetong injection com-bined with alprostadil can significantly improve the neurological function of ACI patients,improve hypercoagulable state by reduc-ing MPV and FIB content,and have good therapeutic efficacy and safety.
