1.Application of high quality nursing service in perioperative treatment of patients with coronary heart disease

Objective The study was to find the application effect of high quality nursing service in perioperative treatment of patients with coronary heart disease.Methods Eighty patients were randomly divided into the comfort nursing group and the routine nursing group with 40 patients in each group.The comfort nursing group was given high quality nursing service,and the routine nursing group received the conventional care.The effect in two groups was compared after rehabilitation.The statistical software of SPSS 17.0 was used to analyze the data.Results The degree of anxiety was lower,the number of patients with complication was less,the degree of satisfaction was higher and the time in hospital was less in the comfort nursing group.Conclusions The comfortable nursing care should be used in peri-operative treatment of coronary heart disease,which can make patients keep the best mental state to accept and cooperate with the surgical treatment.It also can improve the care quality and patients' satisfaction degree and shorten the hospital stay.
2.Clinical Observation of Shuxuetong Injection Combined with Alprostadil in the Treatment of Acute Cere-bral Infarction

OBJECTIVE:To observe clinical efficacy and safety of Shuxuetong injection combined with alprostadil in the treat-ment of acute cerebral infraction(ACI). METHODS:A total of 123 ACI patients selected from our hospital during Mar. 2014-Sept. 2016 selected as research subjects were divided into observation group (62 cases) and control group (61 cases) according to ran-dom number table. Control group was additionally given Alprostadil injection 2 mL+0.9% Sodium chloride injection 100 mL, ivgtt,qd,on the basis of routine treatment. Observation group was additionally given Shuxuetong injection 6 mL+0.9% Sodium chloride injection 250 mL,ivgtt,qd,on the basis of control group. Both groups were treated for 2 weeks. MPV,FIB content, hemorheology indexes,neurologic impairment degree score before and after treatment as well as clinical efficacy,the occurrence of ADR were compared between 2 groups. RESULTS:Before treatment,there was no statistical significance in MPV,FIB content, hemorheology indexes or neurologic impairment score between 2 groups (P>0.05). Compared with before treatment,MPV,FIB content,hemorheology indexes and modified Rankin scale of 2 groups were all decreased significantly;Barthel index and NIHSS scores were increased significantly,and each aspect of observation group was better than that of control group,with statistical sig-nificance(P<0.05). Total response rate of observation group was 95.16%,which was significantly higher than 85.25% of control group,with statistical significance(P<0.05). No severe ADR was found in 2 groups. CONCLUSIONS:Shuxuetong injection com-bined with alprostadil can significantly improve the neurological function of ACI patients,improve hypercoagulable state by reduc-ing MPV and FIB content,and have good therapeutic efficacy and safety.
3.Changes of etiology and management of male urethral stricture in China: a multicentre evaluation

ObjectiveTo investigate the etiology and management of male urethral stricture at 8 medical centers in China during the period from 2004 to 2009 years,and to investigate whether there were any changes in etiology and management of urethral stricture with time change.MethodsThe database on 3455 male patients with urethral stricture who underwent treatment at 8 medical centers in China between January 2004 and December 2009 were prospectively collected.The databases were analyzed for possible cause of stricture and treatment techniques for urethral stricture,and for the changes in etiology and management with time change.ResultsThere were 3455 operations for urethral stricture during the study period.The main causes of urethral strictures were traumas in 1833 patients (53.05%),among which pelvic fractures were in 1327 (38.41%) and perineal trauma in 506 (14.65%).The second cause was iatrogenic causes in 1181 patients (34.18% ),among which transurethral operations or examinations were in 602 (17.42%),hypospadias surgery in 291 (8.42%) and urethral catheterization in 164 (4.75% ).Less common causes were urethritis in 201 patients (5.82%),lichen sclerosus in 149 (4.31%),undefined in 91 (2.63%).The treatments of urethral strictures were endourological surgery including internal urethrotomy and dilation and open urethroplasty including end-to-end urethroplasty and the substitude urethroplasty etc.The ratios of using various techniques in total number of patients were obviously different by time.The most application technique for treatment of urethral stricture was endourological surgery ( 709 ) during 2004 -2006 and occupied 52.67% in total number of patients.It was gradually decreased during 2007 -2009 (726) and only occupied 34.42% (P <0.01 ).Open urethroplasty gradually increased during 2007 -2009 ( 1243,58.94% ) compared with the first three years (563,41.83% ) (P < 0.01 ). Conclusions During the recent years there was an increase in the incidence of urethral stricture being trauma and iatrogenic causes.The main treatments of urethral strictures were endourological surgery and open urethroplasty.Endourological surgery was significantly decreased in total number of patients,while open urethroplasty were significantly increased during the late three years.
4.Clinical effect of edaravone and breviscapine injection and its influence on neurologic impairment and life quality of patients with ischemic stroke

Objective To explore clinical effect of edaravone and breviscapine injection and its influence on neurologic impairment and life quality of patients with ischemic stroke.Methods A total of 148 patients were randomly divided into observation group (74 cases) and control group (74 cases).Patients in observation group were treated with edaravone and breviscapine injection,while those in the control group were treated with edaravone injection.The clinical efficacy,neurologic impairment and quality of life were compared.Results After treatment,the total response rate was 94.59% in observation group,which was significantly higher than 81.08% in the control group (P <0.05).The NIHSS scores of observation group were significantly better than that of control group (P <0.05).The serum NSE,S-100β and MMP-9 levels of observation group after 7 d treatment were significantly lower than that of the control group (P < 0.05),and the serum MMP-9 level of observation group after 14 d was significantly lower than that of the control group (P < 0.05).The SF36 quality of life scores of observation group were significantly better than that of control group (P <0.05).Conclusion The treatment of edaravone and breviscapine for patients with ischemic stroke has an obvious curative effect,and it is worthy of further promotion in clinic.

6.Establishment of patient-derived esophageal squamous-cell carcinoma xenograft in mice and characteristics of signaling pathways related to pro-liferation in SCID mice

AIM:To establish and characterize the patient-derived esophageal squamous-cell carcinoma xeno-graft (PDECX) in mice.METHODS:The samples of human esophageal cancer were grafted into severe combined immu-nodeficient ( SCID) mice.The xenografts were transferred to SCID mice when the first passage of xenografts grew up .The growth of tumors in the first, second and third passages was observed .HE staining was performed.The expression of CK5/6, p63 and p40 in the patient samples , and the first and third passages of the xenografts were detected by immunohisto-chemical analysis.The expression of mTOR, p-mTOR, p70S6K, p-p70S6K, Akt1, p-Akt (Ser473), Erk1/2 and p-Erk1/2 were determined by Western blot .RESULTS:The PDECX was successfully established .The positive expression of CK5/6, p63 and p40 in the xenografts was consistent with that in the patients ’ samples.The levels of phosphorylated and total proteins of proliferation-related signaling pathways were different in the xenografts from different patients .CONCLU-SION:The PDECX model adequately reflects the tumal heterogeneity that is observed in the patients .
7.Non-contact Coculture Reveals a Comprehensive Response of Chondrocytes Induced by Mesenchymal Stem Cells Through Trophic Secretion

Coculture between mesenchymal stem cells (MSCs) and chondrocytes has significant implications in cartilage regeneration. However, a conclusive understanding remains elusive. Previously, we reported that rabbit bone marrow-derived MSCs (rbBMSCs) could downregulate the differentiated phenotype of rabbit articular chondrocytes (rbACs) in a non-contact coculture system for the first time. In the present study, a systemic investigation was performed to understand the biological characteristics of chondrocytes in coculture with MSCs. Firstly, cells (MSCs and chondrocytes) from different origins were cocultured in transwell system. Different chondrocytes, when cocultured with different MSCs respectively, consistently demonstrated stimulated proliferation, transformed morphology and declined glycosaminoglycan secretion of chondrocytes. Next, cell surface molecules and the global gene expression of rbACs were characterized. It was found that cocultured rbACs showed a distinct surface molecule profile and global gene expression compared to both dedifferentiated rbACs and rbBMSCs. In the end, cocultured rbACs were passaged and induced to undergo the chondrogenic redifferentiation. Better growth and chondrogenesis ability were confirmed compared with control cells without coculture. Together, chondrocytes display comprehensive changes in coculture with MSCs and the cocultured rbACs are beneficial for cartilage repair.

8.Anti-tumor effects of phytosphingosine on leukemia cells by inducing cell apoptosis

Objective To preliminarily investigate the anti-tumor effects of phytosphingosine(PHS)and the involvement of inducing apoptosis of leukemia cells.Methods Cellular model of leukemia was established in leukemia cell lines K562 and SUP-B15.CCK-8 assay and EdU assay were used to measure the viability and DNA synthesis of K562 and SUP-B15 cells.RNA-seq was carried out to verify the differentially expressed genes(DEGs)after PHS treatment.Gene Ontology(GO)enrichment and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analyses were applied to analyze the involved functions and signaling pathways.Comparative Toxicogenomics Database(CTD)and Discovery Studio software were employed to predict the underlying targets of PHS and molecular docking.Cell apoptosis was detected by flow cytometry,mitochondrial membrane potential was evaluated by JC-1 probe,and protein expression of key molecules was validated by Western blotting.Results PHS inhibited the proliferation of K562 and SUP-B15 cells in a time-and dose-dependent manner.The half-maximal inhibitory concentration(IC50)of K562 cells was 17.67 and 12.52 pmol/L for 24 and 48 h,respectively,and the IC50 value of SUP-B15 cells was 17.58 and 14.86 μmol/L for 24 and 48 h,respectively.PHS treatment at a dose of 20 μmol/L for 48 h resulted in significant inhibition of DNA synthesis.GO enrichment analysis of the K562 cells showed that PHS might be involved in positive regulation of apoptotic process,plasma membrane and its integral components,and protein kinase binding and activity.Reverse predictive analysis showed that BCL-2 protein was the most likely target of PHS.PHS significantly increased the apoptotic rate of leukemia cells(P<0.05)in a dose-dependent manner,reduced the mitochondrial membrane potential,and down-regulated BCL-2 level(P<0.05)and up-regulated the levels of Cleaved caspase-3 and Cleaved caspase-9(P<0.05).Conclusion PHS may inhibit the proliferation of leukemia cells by inducing mitochondria-dependent apoptosis,possibly through PHS and BCL-2 interaction.
9.Effects of biorhythm factors on development of acute kidney injury after cardiac surgery using cardiopulmonary bypass: a retrospective cohort study

Objective:To evaluate the effects of biorhythm factors on the occurrence of acute kidney injury (AKI) after cardiac surgery using cardiopulmonary bypass.Methods:This was a retrospective cohort study. Data from patients undergoing heart surgery involving cardiopulmonary bypass from June 2018 to December 2019 were collected and divided into 2 groups ( n=125 each) based on the time of anesthesia operation: morning rhythm group (group Ⅰ) and afternoon rhythm group (group Ⅱ). Anesthesia operation was performed from 8: 00 to 12: 00 in group Ⅰ. Anesthesia was performed from 14: 00 to 18: 00 in group Ⅱ. The occurrence of postoperative AKI and other postoperative complications (pulmonary infection, sepsis, cerebral infarction) was recorded. Results:Compared with group Ⅱ, the incidence of postoperative AKI was significantly increased, the relative risk was 3.2 (95% confidence interval 1.31-7.70), and no significant change was found in the incidence of pulmonary infection, sepsis and cerebral infarction in group Ⅰ ( P>0.05). Conclusions:Biorhythm factors affect the development of AKI after cardiac surgery using cardiopulmonary bypass, and performing surgeries in the afternoon rather than the morning helps reduce the risk of postoperative AKI.
10.Analysis of effects and postoperative clinicopathological prognosis factors of neoadjuvant chemotherapy in patients with stageⅠb2 andⅡa2 squamous cell carcinoma of the cervix

Objective To explore the effects and postoperative prognostic factors in cervical cancer patients received neoadjuvant chemotherapy combined with surgery and post-operative adjuvant therapy. Methods A total of 177 patients with cervical cancer at International Federation of Gynecology and Obstetrics (FIGO) stageⅠb2, Ⅱa2 who underwent neoadjuvant chemotherapy (NACT) followed by surgery with and without adjuvant therapy in Cancer Hospital, Chinese Academy of Medical Sciences were included. Univariate and multivariate analyses of 5-year overall survival (OS) rate and 5-year disease-free survival (DFS) rate were performed. Results Of 177 patients, 133 (75.1%) had stageⅠb2 and 44 (24.9%) hadⅡa2 cancers. After NACT,overall response rate was 63.3% (112/177) including 12 cases of complete response (CR), 100 of partial response (PR) and no progressive disease (PD) case. At a median follow-up of 59.2 months, the 5-year DFS rate was 73.6% and the 5-year OS rate was 86.8%. Univariate analysis revealed that lymph node metastasis, deep stromal invasion and tumor size after NACT significantly affected 5-year DFS rate (P<0.05). Lymph node metastasis, deep stromal invasion and tumor size after NACT significantly affected 5-year OS rate (P<0.05). The multivariate analysis showed that, stromal invasion (outer 1/3 or outer 1/2) was independent risk factor of 5-year DFS rate (P<0.05), and 5-year OS rate was significantly affected by tumor size >3 cm after NACT (P<0.05). Conclusions The effect of NACT in Ⅰb2, Ⅱa2 squamous carcinoma of the uterined cervix is confirmed. The independent risk factor for 5-year DFS rate in patients received NACT and hysterectomy is deep stromal invasion of the cervix. The presence of tumor size>3 cm after NACT adversely affect 5-year OS rate.
