1.Advantages of elastic intramedullary nail in the treatment of closed fracture of the limbs of preschool children

Objective To study the clinical effect of elastic intramedullary nail in the treatment of preschool children limb fractures.Methods Ninety-two 92 cases closed fracture of extremities of preschool children in Children ′s Hospital of Shenyang from July 2014 to February 2016 were selected,and randomly divided into control group and observation group,46 cases in each group.The control group were treated with open reduction and plate fixation treatment,observation group were treated with elastic intramedullary nail treatment.The surgical effect and prognosis index and the incidence of postoperative complications of two groups were compared.Results The operation time,the amount of bleeding,hospitalization time,fracture healing time and loading time of the observation group were significantly lower than the control group((63.57±3.12) min vs.(94.72±6.49) min,(52.43±11.35) ml vs.(134.26±23.50) ml,(6.10±1.44) d vs.(15.83±2.61) d,(8.34±1.13) weeks vs.(12.05±1.62) weeks,(10.85±2.71) weeks vs.(16.94±3.25) weeks,t=29.34,21.29,22.14,12.74,9.76,P<0.01).The incidence of complications in the observation group was 6.52%(3/46),significantly lower than the control group(19.54%(9/46),χ2=5.06,P=0.02).Conclusion The elastic intramedullary nail for treatment of fractures of extremities shaft closed preschool children can guarantee good clinical effect,and has little trauma high quality,healing of fracture,less postoperative complications and easy care,is a kind of efficient treatment technology,can be widely developed in clinic.

Objective To observe the effect of He's Santong method (Three ways to remove obstruction) of acupuncture for treatment of cervical spondylosis of nerve-root type.Methods The 52 patients with nerve-root type cervical spondylosis were randomly divided into two groups, 26 in each. The treatment group was treated by He's Santong method of acupuncture while the control group was administered Gentongping Granule, 2 weeks as one course. Five main flavonoids of Hebei Xiangju were studied using the Density Functional Theory (DFT) B3LYP method with 6-311 G (d) basis set.Their activities were analyzed based on molecular structure,bond dissociation energy (BDE),natural orbital charge distribution (NBO),bond order and the energy gap between HOMO and LUMO. The results showed that the existing of intra molecular hydrogen bond in B ring can improve the antioxidant activity of the flavonoids, at the same time, the hydroxyl groups on the glycosides do not have the activity of eliminating free radicals, but decrease the total molecular antioxidant activity. As a result, the antioxidant ability order of the five flavonoids compounds is luteolin< luteolin-7-O-glucoside< apigenin < acacetin < acacetin-7-O-glucose, which is agreement with the experimental conclusion reported in literature. The results showed that the DFT method can provide theoretical guidance for the selection of natural flavonoid antioxidants.
5.Diagnostic value of oblique axial scan for anterior talofibular ligament in ankle

Objective To clarify the diagnostic value of oblique axial scan parallel to the course of the anterior talofibular ligament in magnetic resonance imaging of anterior talofibular ligament in patients with chronic ankle instability. Methods 96 patients with ankle injury who were admitted into our hospital form June 2010 to March 2015 were examed with oblique axial scan beside the routine axial magnetic res-onance imaging examination. The grade of the anterior talofibular ligament injury and confirmed full-length views of the anterior talofibular lig-ament were analyzed by two musculoskeletal radiologists ( with more than 5 years experience) , and associated lesions were also checked. Re-sults The full-length view of the anterior talofibular ligament was checked in 69 patients (71. 8%) through the routine axial view and it was checked in 93 patients (96. 9%) through the oblique axial scan view. Anterior talofibular ligament injury was checked in 71 patients through the oblique axial scan, which was 19 patients more than that through the routine axial. Conclusion An additional oblique axial scan is use-ful in evaluation of anterior talofiibular ligament in patients with ankle injury.
6.Develepment and application of the inserted mouth gag

Objective To evaluate the effectiveness of mouth gag which can inserted into lavage tube during gastric lavage. Methods The Sixty- six cases of acute poisoning patients with gastric lavage according to random number table method were randomly divided into two groups, 33 cases in each group. Observation group used the inserted mouth gag, and the control group used the metal ones. Results The observation group were lower than the control group at the time required to insert the lavage tube [(2.23±0.19)s to (3.26±0.30)s] and the incidence of complication rate [18.2%(6/33) to 45.5%(15/33)], the oral cavity bleeding [6.0%(2/33) to 30.3%(10/33)], lavage tube displacement [0(0/33)to 24.2%(8/33)], lavage tube twisting[0(0/33) to 24.2%(8/33)], the difference was statistically significant (P<0.01), while higher in the rate of successful intubation[100.0%(33/33)] to [54.5%(18/33)], the difference was statistically significant, χ2=19.4,P<0.01. Conclusions The application of the mouth gag which can insert into gastric lavage improved the successful intubation rate, saved the rescue time, reduced the complication and improved the nursing quality.
7.Diagnosis of metastases encephaloma by using susceptibility weighted imaging and contrast-enhanced T1-weighted imaging of MR before and after radiotherapy

Objective To evaluate the value of susceptibility-weighted imaging (SWI)sequence in the diagnosis of metastatic en-cephaloma pre and after radiotherapy by comparing the appearance of the SWI and contrast-enhanced T1-weighted imaging(T1 WI). Methods Thirty-eight lung cancer patients with brain metastases underwent SWI and contrast-enhanced T1-weighted imaging re-spectively before and 2-3 months after radiotherapy.Evaluated the two methods by score(the score ranged from 0-3,0 represen-ted no showing,1 represented not showing clearly,2 represented could judging,3 represented showing obviously).Results Before radiotherapy,SWI detected 1 61 lesions of metastases and could obviously show tumor vascular in brain metastases (mean score 2.73±0.05).Contrast-enhanced T1 WI detected 1 61 lesions(mean score 1.93±0.04 ).SWI showed significantly higher score than enhanced T1 WI(P <0.05)through paired sample t test.After radiotherapy,SWI found 98 lesions (mean score 1.47±0.1 1 )and en-hanced T1 WI found 140 lesions (mean score 1.80±0.07),enhanced T1 WI had significantly higher score than SWI(P <0.05).Con-clusion SWI is superior to contrast-enhanced T1 WI to show the metastases before radiotherapy and can show tumor vascular in brain metastases obviously.SWI is inferior to enhanced T1 WI after radiotherapy.It has some value and may provide a new method for evaluating effects of radiotherapy.
8. Cloning and bioinformatics analysis of iridoid oxidase gene in Gentiana rigescens

Objective To obtain the key enzyme gene involving in the monoterpenoid biosynthesis pathway, an iridoid oxidase gene (GrIDO) and its promoter were cloned from the leaves of Gentiana rigescens, and its bioinformatics analysis were also performed. Methods The gene specific primers were designed according to the GrIDO gene of transcriptome in G. rigescens. The open reading frame (ORF) of GrIDO gene was cloned by RT-PCR method. The bioinformation of GrIDO gene was analyzed by online softwares. Meanwhile, the gene specific primers were designed according to the cloned GrIDO gene, and the promoter of GrIDO gene was amplified by PCR method. Its sequence analysis was also performed. Results The length of GrIDO gene ORF (GenBank accession number KP722034) was 1 557 bp, which encoded a protein with 518 amino acids. Its relative molecular weight was 58 920 with the theoretical isoelectric point of 8.40. GrIDO was the member of cytochrome P450 superfamily and may localize in chloroplast. GrIDO was a hydrophilic stable protein without signal peptide and composed of mainly α-helix (51.07%) and loops (42.69%). GrIDO protein had a high similarity with CrIDO of Catharanthus roseus (85.83%) and their genetic relationship was close. The cloned GrIDO promoter had a length of 720 bp (GenBank accession number KT428570), which possessed cis-regulatory elements TATA-box and CAAT-box, and had cis-acting elements involved in the abscisic acid and MeJA responsiveness, part of a light responsive element and MYB transcription factor binding site. Conclusion The expression of GrIDO gene were regulated by multifactors. The GrIDO gene and its promoter were cloned from G. rigescens. This study will lay foundations for the functional research of GrIDO gene in monoterpenoid biosynthesis pathway.
9. Cloning, sequence analysis, and prokaryotic expression of GrCYP450-17 gene in Gentiana rigescens

Objective: To obtain the key enzyme gene GrCYP450 involving in the terpenoid biosynthesis, a CYP450-17 gene was cloned from young leaves of Gentiana rigescens, and its sequence analysis and prokaryotic expression were performed. Methods: According to the GrCYP450-17 gene sequence of transcriptome of triennial G. rigescens, a pair of primers were designed, and the ORF (Open reading frame)of cDNA sequences was obtained by RT-PCR. Then TA cloning, sequencing, and sequence analysis were performed. Prokaryotic expression vector pGEX-4T-1-GrCYP450-17 was constructed and transformed into E. coli Rosetta (DE3) for the expression under the induction of Isopropyl β-D-1-Thiogalactopyranoside (IPTG.). Results: The ORF of GrCYP450-17 has a length of 1 545 bp coding for 514 amino acids. Sequence analysis showed that GrCYP450-17 was the member of CYP714 family. Results of phylogenic analysis showed that GrCYP450-17 was close to SlCYP450 of tomato. The SDS-PAGE results showed that the expressed proteins were consistent with the anticipated size. Conclusion: The GrCYP450-17 gene is successfully cloned, and the stable prokaryotic expression system is established. This study will provide a foundation for the further purification, structural and functional researches of GrCYP450-17 protein.
10. Cloning, sequence analysis, and prokaryotic expression of 1-deoxy-D-xylulose-5- phosphate reductoisomerase GrDXR gene in Gentiana rigescens

Objective: To clone the 1-deoxy-D-xylulose 5-phosphate reductoisomerase gene GrDXR which is the key enzyme involving in the terpenoid biosynthesis from young leaves of Gentiana rigescens, and to perform its sequence analysis and prokaryotic expression. Methods: According to the GrDXR gene sequence of G. rigescens transcriptome, a pair of primers were designed, and the ORF of cDNA sequence was obtained by RT-PCR. Then TA cloning, sequencing, and sequence analysis were performed. Prokaryotic expression vector pGEX-4T-1-GrDXR was constructed and transformed into Escherichia coli Rosetta (DE3) for expression under the induction of IPTG. Results: The ORF of GrDXR had a length of 1 425 bp coding for 474 amino acids. Sequence analysis showed that GrDXR was the member of DXR family. Results of phylogenic analysis showed that GrDXR was close to RvDXR, HbDXR, and CrDXR. The pGEX-4T-1-GrDXR recombinant plasmid was constructed and the stable prokaryotic expression system was obtained. The SDS-PAGE results displayed that the expressed proteins were consistent with the anticipated size. Conclusion: The GrDXR gene is successfully cloned, and the stable prokaryotic expression system is established. This study will provide a foundation for further purification, structural and functional research of GrDXR protein.
