Objective To assess the risk of nonylphenol in reclaimed wastewater used in city to damage the human endocrine system and to present the data for making the related water quality standard. Methods Based on the monitored results of nonylphenol in the reclaimed wastewater from a reclaimed water plant in Beijing and referenced the phenol toxic parameter from integrated risk information system of the Unite State Environmental Protection Agency and the nonylphenol tested toxicity data from related literature, the risk assessment of nonylphenol on human health was conducted using four step health risk assessments when the sewage was reclaimed as a different approach to use. Results The endocrine disruption risk of nonylphenol in reclaimed wastewater on human health was about in a range of 10-8-10-10, which could even be ignored. Conclusion The risk of nonylphenol is acceptable to human health when the reclaimed wastewater were used for municipal, agricultural irrigation, urban lake supplement, then, nonylphenol as a human health risk factor can be ignored when making the standard of nonylphenol in reclaimed water.

Based on the analysis of blended learning characteristics, a blended learning pro-cess model including preliminary analysis, learning activities design, blended evaluation was con-structed. Then, a blended learning case study was carried out by taking the Fourth Military Medical University Information Security course as an example. The implement methods of setting learning goal, selecting learning content, constructing learning environment, organizing learning activities and course evaluation, were described in detail. Lastly, from the effectiveness questionnaire, it could be shown that blended learning motivated the learners' enthusiasm significantly. In order to improve the learning performance, we proposed that blended learning activities must be flexible and adaptive, and integrate face-to-face learning with online learning effectively.

Objective To explore more suitable calculation method of the insulin dosage in insulin hypoglycemia-growth hormone stimulation test(insulin tolerance test, ITT). Methods Fifty-six subjects suspected of growth hormone deficiency were divided into primary and secondary onset groups. All the patients took oral glucose tolerance test and ITT. Homeostasis model of assessment for insulin resistance index ( HOMA-IR) and insulin sensitivity index ( ISI), area under insulin curve ( AUCINS ) and the area under glucose curve ( AUCPG ) were calculated. The insulin dosages during ITT between two groups were compared and the main factors influencing the insulin dosage were analyzed. Results There was no difference in the insulin dosage during ITT between primary and secondary groups. The actual dosage of insulin in this cohort study revealed a significant difference from the initial insulin dosage recommended by the guideline. Multiple linear regression analysis found that AUCINS and body mass index were the independent factors affecting the insulin dosage. Then the optimized coefficient of ITT ( γ) were found. Conclusion The insulin dosage used in our study was inconsistent with the guidelines-recommended ones. In order to make ITT more efficient and safer, a more optimized calculation method to improve the successful rate of insulin-induced hypoglycemia in ITT is proposed.

Objective To study the prevalence of metabolic syndrome (MS) in women with polycystic ovary syndrome (PCOS) ,and to explore the risk factors of MS in young women. Methods The prevalence of MS and its components were evaluated in 348 young women with PCOS and 113 control subjects without PCOS. Results According to the 2005 modified Adult Treatment Panel Ⅲ criteria,MS was found in 27.0% of PCOS women, being significantly higher than 10.6% in the controls(P<0.01). The rates of MS components, except for the triglyceride (TG) level, were all significantly higher in PCOS subjects than those in control subjects ( P < 0. 05 or P < 0.01). However, the difference disappeared after adjustment for age and body mass index ( BMI, P>0. 05). The results of logistic regression analysis revealed that homeostasis model assessment insulin resistance (HOMA-IR)and BMI were the independent predictors of MS( both P<0.01) ,and the presence of PCOS was not the independent risk factor for MS (P>0. 05). Conclusions Obesity and insulin resistance are the independent predictors of MS. PCOS alone does not increase the risk of MS prevalence.

ObjectiveTo compare the sensitivity and specificity of a new index of insulin resistance (IR) derived from plasma triglyceride and glucose with homeostasis model assessment for insulin resistance index ( HOMA-IR) and insulin sensitivity index from frequent sampling intravenous glucose tolerance test (FSIGT-ISI).Methods A total of 1024 subjects (240 normal control,335 with obesity,312 impaired glucose regulation, and 137 type 2 diabetes mellitus) were included in the present study.Standard oral glucose tolerance test, insulin release test, lipid profiles, and other biochemical markers were measured. Among them 540 subjects were selected to undertake FSIGT.TyG index is derived from plasma triglyceride and fasting glucose.TyG2 index is derived from plasma triglyceride and postprandial 2 plasma glucose.ResultsPearson correlation coefficient between TyG and HOMA-IR or FSIGT-ISI was 0.427 ( P<0.01 ) and -0.100 ( P=0.024 ),respectively, and that between TyG2 and HOMA-IR or FSIGT-ISI was 0.455 ( P<0.01 ) and - 0.162 ( P<0.01 ),respectively. The sensitivity and specificity of TyG index for diagnosis of IR was 68.5% and 63.5% compared with HOMA-IR,and 68.5% and49.5% compared with FSIGT-ISI.For TyG2 index, the slightly higher sensitivity ( 81.7% with HOMA-IR,75.7% with FSIGT-ISI),but lower specificity (51.5% with HOMA-IR,48.2% with FSIGT-ISI) were found.ConclusionsBothTyGandTyG2 indices could be used as a surrogate for assessing IR in Chinese subjects. However, considering its moderately high sensitivity but low specificity, these two indices are limited in the use of large-scale epidemiological screening.

To investigate the relationship between serum retinol-binding protein 4(RBP4) and gestational diabetes mellitus (GDM) in Chinese Han pregnant women.195 (23-42 years) pregnant women were recruited (July 2005 to December 2007) from the Department of Gynecology and Obstetric in Ruijin Hospital during their visiting for routine prenatal examination.99 subjects belonged to GDM group, and 96 belonged to the group with normal glucose tolerance (NGT).65 non-pregnant healthy women served as control.Serum RBP4 was measured using sandwich enzyme linked immunosorbent assay (ELISA).Pregnant women had higher level of serum RBP4 than that of non-pregnant control. The concentration of serum RBP4 was significantly increased in GDM group as compared with NGT group[(43.04±1.85 vs 33.84±2.17) rag/L,P<0.01].Multiple stepwise regression analysis showed that triglycerides and homeostasis assessment for insulin resistance (HOMA-IR) were independent variables of RBP4 (r2 =0.165) in pregnant women. The results suggest that serum RBP4 level is significantly increased in pregnant women.Women with GDM had even higher RBP4 level than that of normal pregnant women, and RBP4 levele was positively correlated with triglycerides and HOMA-IR.

Objectivc To observe the therapeutic effects of combined cytokines on hematopoietic injuries induced by 4.5 Gy60 Co γ-rays irradiation in beagles, and to provide experimental evidences for the clinical treatment of extremely severe myeloid acute radiation sickness(ARS).Methods 16 beagles were given 4.5 Gy60 Co γ-rays total body irradiation, and then randomly assigned into irradiation control group, supportive care group and cytokines group. In addition to supportive care, recombinant human granulocyte colony-stimulating factor (rhG-CSF),recombinant human interleukin-11(rhIL-11)and recombinant human interleukin-2(rhIL-2)were administered subcutaneouly to dogs in cytokines group.Peripheral blood hemogram was examined once every two days.Bone marrow and peripheral blood were collected to proceed colony cultivation 4 d pre-irradiation and 1 and 45 d post-irradiation.Conventional histopathological sections of sternum were prepared to observe the histomorphology changes. Results After irradiation, the population of all kinds of cells in peripheral blood declined sharply.WBC nadir Was elevated(1.04×109/L,but 0.28×109/L and 0.68×109/L for the irradiation control group and the supportive care group separately),the duration of thrombocytopenia was shortened (24 days, but 33 days for the supportive care groug) and red blood cell counts were maintained in the range of normal values after cytokincs treatment in combination. The colony forming efficiency of haemopoietic stem cells(HSCs)in bone marrow and peripheral blood decreased obviously 1 d post irradiation, but recovered to the level of that before irradiation 45 d post irradiation after supportive care and cytokines treatment.Hematopoietic cells disappeared in bone marrow of animals in irradiation control group, but hematopoietic functions were recovered after cytokines were administrated. Conclusions RhG-CSF.rhIL-11 and rhIL-2 used in combination could elevate WBC nadir, accelerate the recovery of leukocytes, platelets and red blood cells and promote the proliferation, differentiation and maturity of HSPCs left in the body after 4.5 Gy γ-rays total body irradiation, eventually restore the hematopoietic function.Hence, combination of rhG-CSF,rhIL-11 and rhIL-2 could serve as better therapeutic strategy to treat extremely severe myeloid ARS.
