3.Effect of different treatments on breast ductal carcinoma in situ

Objective To investigate the efficacy of different treatments for breast ductal carcinoma in situ. Methods From January 2010 to March 2015,a total of seventy-eight patients with intraductal carcinoma in Shanghai First People's Hospital Baoshan Branch were enrolled in the study. According to the different treatment methods,the patients were divided into the experimental group (33 cases) and control group (45 cases). The experimental group was given breast conserving surgery plus adjuvant radiotherapy,while the control group was treated with modified radical operation. The operative time, intraoperative blood loss, drainage volume and hospital stay were compared between the two groups. The survival and adverse events of the two groups were analyzed. Results The hospitalization time (( 8. 13 ± 3. 43) d), operation time (( 108. 56 ± 22. 27) min), intraoperative blood loss ((78. 72±30. 74) ml) and drainage ((259. 68±35. 95) ml) in observation group were significantly shorter than those in the control group ((15. 56±2. 73)d,(148. 62±23. 85) min,(169. 28±26. 33) ml,(553. 37 ± 45. 64) ml) The difference between the two groups was statistically significant ( t=12. 160, 9. 378,17. 304,38. 265,P<0. 05). Log-rank test showed that the good prognosis rate of two groups in 24 months was 90. 32% and 88. 64% respectively,the difference was not statistically significant ( x 2=1. 246,P>0. 05) . Conclusion Breast conserving surgery plus adjuvant radiotherapy is the first choice for the treatment of breast ductal carcinoma in situ.
4.Correlation between preoperative GPS and postoperative complications and prognosis of patients with colorectal cancer

Objective To investigate the value of GPS score and CEA in predicting the prognosis of pa-tients with colorectal cancer undergoing laparoscopic surgery. Methods 120 patients diagnosed as colorectal can-cer in our hospital were involved and their baseline information include Height,weight,history,complication, course of disease,tumor size,pathological type,tumor location,TNM stage,vascular tumor thrombus and lymph node metastasis were recorded. Then,all of the patients were followed-up 18 months and patients with favorable prognosis were defined as the favorable group while patients with unfavorable prognosis were defined as the unfavor-able group. Cox′s proportional hazard regression model analysis was applied to evaluate the influencing degree of those factors on the prognosis of the subjects. The factors in predicting prognosis were calculated by ROC curves. Results The poor prognosis rate of patients with colorectal cancer after operation treatments was 37.84%. Cox′s proportional hazard regression model analysis showed that CEA(P = 0.035),GPS score(P = 0.023)have influenc-es on the prognosis. Conclusion GPS scores and CEA may assess the prognosis of patients with colorectal cancer undergoing laparoscopic surgery,which is expected to be used as an indicator of predicting the prognosis of pa-tients with colorectal cancer undergoing laparoscopic surgery.
5.Prevalence of depressive symptoms among community-dwelling elderly with mild cognitive impairment in China: a Meta-analysis

Objective:To estimate the prevalence of depressive symptoms in community-dwelling elderly with mild cognitive impairment(MCI) in China by systemic review.Methods:PubMed, Web of Science, CNKI, VIP database and other databases were searched for cross-sectional studies on the prevalence of depressive symptoms in the elderly with MCI in China. The search period was from January 1, 2011 to October 1, 2021. The studies were screened according to predefined inclusion and exclusion criteria. Stata 12.0 software was used for statistical analysis, and finally the prevalence rate of depressive symptoms in the elderly with MCI in China was calculated.Results:A total of 2 036 relevant studies were retrieved, among which 14 met the inclusion criteria, involving 3 819 cases. Meta-analysis showed that 32.3%(30.9%-33.7%) of community-dwelling elderly with MCI had depressive symptoms. Subgroup analysis showed that the detection rate of depressive symptoms in studies published during 2016—2021 was higher than that during 2011—2015 (34.6% vs. 23.5%, χ2=11.64, P<0.001).However there were no significant differences in detection rate between genders, among studies using different depression assessment and MCI assessment tools, and among different geographic regions (all P>0.05). Conclusion:The study shows that the prevalence of depressive symptoms in community-dwelling elderly with MCI in China is high and it presents a rising trend, suggesting that attention should be paid to mental health of elderly in the community and the screening of depressive symptoms should be strengthened for those with MCI.

Objective:To systematically review research on the association between vitamin K and type 2 diabetes and diabetes-related biomarkers in humans, and evaluate the role of vitamin K in the prevention of type 2 diabetes.Methods:"Vitamin K", "type 2 diabetes" and related terms were searched in PubMed, EMBASE, China National Knowledge Infrastructure (CNKI) and Wanfang Med Online up to November 2018.Results:A total of 1 Chinese and 12 English articles were included. Among 6 observational studies, 5 of them showed that higher dietary vitamin K intake and plasma vitamin K level were associated with the decrease of the risk of type 2 diabetes. Among 6 clinical intervention studies, 5 of them indicated that the supplementation of vitamin K 1 or K2 could have positive influence on insulin metabolism. One Mendelian randomization study showed higher circulation vitamin K level might reduce the risk of type 2 diabetes. Conclusion:Vitamin K plays an important role in the prevention and control of type 2 diabetes, which may be related to the improvement of insulin metabolism and blood glucose level.

8.LNX1 inhibits the proliferation,invasion and migration of renal clear cell carcinoma cells by regulating TIAM1/ERK signaling pathway

Objective:To investigate the effect of ligand of numb-protein X1(LNX1)on the proliferation,invasion and migration of renal clear cell carcinoma cells and its underlying molecular mechanism. Methods:Gene Expression Profiling Interactive Analysis(GEPIA)database was used to analyze the mRNA expression level of LNX1 in renal clear cell carcinoma tissues and its relationship with the prognosis of patients with renal clear cell carcinoma.LNX1 gene specific shRNA(shLNX1)was delivered into renal clear cell carcinoma cell lines 786-O and ACHN by lentiviral infection,and flag-LNX1 plasmid was delivered into 786-O and ACHN cells by transient transfection.CCK-8 assay and colony formation assay were used to assess the effects of LNX1 silencing or overexpression on the proliferation of 786-O and ACHN cells.Transwell assay was used to evaluate the effects of LNX1 silencing or overexpression on the invasion and migration of 786-O and ACHN cells.Bioinformatics analysis was used to screen the downstream target genes of LNX1.Western blotting was used to examine the effects of LNX1 silencing or overexpression on the expression level of T-lymphoma invasion and metastasis-inducing protein 1(TIAM1)as well as the expression levels of total and phosphorylated ERK(phospho-ERK,p-ERK)in the ERK signaling pathway downstream of TIAM1 in 786-O and ACHN cells.786-O and ACHN cells overexpressing LNX1 were treated with proteasome inhibitor MG132,and the protein expression level of TIAM1 was analyzed by Western blotting.Finally,myc-TIAM1 recombinant plasmid was transfected into LNX1-overexpressing cells.Then,the expression levels of proteins in the ERK signaling pathway and the abilities of proliferation,invasion and migration of 786-O and ACHN cells were examined by Western blotting,colony formation assay and Transwell assay,respectively. Results:The mRNA expression level of LNX1 in renal clear cell carcinoma tissue was decreased(P<0.05)and was positively correlated with the survival time of patients with renal clear cell carcinoma(P<0.001).LNX1-silencing 786-O and ACHN cells and LNX1-overexpressing 786-O and ACHN cells were constructed successfully.After LNX1 silencing,the proliferation,invasion and migration of 786-O and ACHN cells were significantly enhanced(all P<0.05).After LNX1 overexpression,the abilities of proliferation,invasion and migration of 786-O and ACHN cells were significantly decreased(all P<0.05).Bioinformatics analysis identified TIAM1 as a potential target of LNX1.After silencing LNX1,the protein expression levels of TIAM1 and p-ERK were significantly increased(all P<0.05),while the expression level of ERK remained unchanged.After LNX1 overexpression,the protein expression levels of TIAM1 and p-ERKwere significantly decreased(all P<0.01),while the expression level of ERK was unchanged.Treatment with proteasome inhibitor MG132 increased the protein expression level of TIAM1 in LNX1-overexpressing 786-O and ACHN cells(P<0.01 and P<0.001).After LNX1-overexpressing cells were transfected with myc-TIAM1 plasmid,the protein expression level of p-ERK was increased,the abilities of cell proliferation,invasion and migration were enhanced(all P<0.05),and the expression level of ERK protein remained unchanged. Conclusion:LNX1 inhibits the proliferation,invasion and migration of renal clear cell carcinoma cells by degrading TIAM1 which further regulates the phosphorylation of ERK.
9.Comparison of diagnostic performance of Clear Cell Likelihood Score v1.0 and v2.0 for clear renal cell carcinoma.

OBJECTIVE:
To compare the performance of Clear Cell Likelihood Score (ccLS) v1.0 and v2.0 in diagnosing clear cell renal cell carcinoma (ccRCC) from small renal masses (SRM).
METHODS:
We retrospectively analyzed the clinical data and MR images of patients with pathologically confirmed solid SRM from the First Medical Center of the Chinese PLA General Hospital between January 1, 2018, and December 31, 2021, and from Beijing Friendship Hospital of Capital Medical University and Peking University First Hospital between January 1, 2019 and May 17, 2021. Six abdominal radiologists were trained for use of the ccLS algorithm and scored independently using ccLS v1.0 and ccLS v2.0. Random- effects logistic regression modeling was used to generate plot receiver operating characteristic curves (ROC) to evaluate the diagnostic performance of ccLS v1.0 and ccLS v2.0 for ccRCC, and the area under curve (AUC) of these two scoring systems were compared using the DeLong's test. Weighted Kappa test was used to evaluate the interobserver agreement of the ccLS score, and differences in the weighted Kappa coefficients was compared using the Gwet consistency coefficient.
RESULTS:
In total, 691 patients (491 males, 200 females; mean age, 54 ± 12 years) with 700 renal masses were included in this study. The pooled accuracy, sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of ccLS v1.0 for diagnosing ccRCC were 77.1%, 76.8%, 77.7%, 90.2%, and 55.7%, as compared with 80.9%, 79.3%, 85.1%, 93.4%, 60.6% with ccLS v2.0, respectively. The AUC of ccLS v2.0 was significantly higher than that of ccLS v1.0 for diagnosis of ccRCC (0.897 vs 0.859; P < 0.01). The interobserver agreement did not differ significantly between ccLS v1.0 and ccLS v2.0 (0.56 vs 0.60; P > 0.05).
CONCLUSION
ccLS v2.0 has better performance for diagnosing ccRCC than ccLS v1.0 and can be considered for use to assist radiologists with their routine diagnostic tasks.
