66 cases were treated with Yizhi(Intellingencebenefitted)Syrup. After treatment, the scores on behavioral appraisal markedly decreased with dramatic upgrading of learning results, marked lowering of positive soft nerve signs, the total effective rate being 84.8%. There wore also marked increase in the levels of urine NE, DA, 3-4(?)PAC, CAMP and(?)r.
2.Early and middle-term results after surgical treatment for slipped capital femoral epiphysis

Objective To evaluate the early and middle-term results of in-situ single-screw fixation and subtrochanteric osteotomy of femur with external fixator for slipped capital femoral epiphysis(SCFE).Methods From June 1998 to July 2008, eleven patients (12 hips) with SCFE were treated, including 7males (8 hips) and 4 females (4 hips) with an average age of 14.3 years (range, 9-18). The average BMI was 31.1 g/m2. According to the Southwick measurement, mild (<30°) in 6 cases (7 hips), moderate (30°-50°) in 2 cases (2 hips), and severe (>50°) in 3 cases (3 hips). All the mild and moderate cases were undergone insitu single screw fixation, and three severe cases were performed subtrochanteric osteotomy of femur with external fixator. All the cases were evaluated at 3 months, 6 months and every year postoperatively. Results All the cases were followed up for average 5.6 years (range, 2-12). The Harris hip score increased from mean 74.8 points preoperatively to 90.6 points postoperatively, with 10 hips excellent, 1 hip good, 1 hip fair and 0 hip poor. The excellent and good rate was 91.7% (11/12). The X-ray showed the epiphyseal closure in in-situ single screw fixation cases, and no further increase in epiphyseal-shaft angle. Partial reduction of slipped epiphysis with normal joint spaces was observed in subtrochanteric osteotomy of femur with external fixator cases. Conclusion In-situ single screw fixation provides a surgical treatment option to treat mild and moderate SCFE with satisfactory clinical outcome, and profit to the prevention further slip. Subtrochanteric osteotomy of femur with external fixator is suitable for severe SCFE with late deformity. The realignment procedure can correct deformity and postpone or avoid the occurrence of osteoarthritis.

Objective To explore the regular progressive pattern of nontraumatic osteonecrosis of the femoral head (ONFH) in order to establish the reliable and convenient new classification of ONFH.Methods The coronal section of the femoral head was divided into three pillars (medial,central and lateral).The mid-coronal section of the femoral head on MRI was selected.The China-Japan Friendship Hospital (CJFH)classification of ONFH was established according to the site of necrotic focus in three pillars.A total of 153hips with ONFH were classified according to CJFH classification and Japanese Investigation Committee (JIC)classification,respectively.The collapse rate was observed and compared between both classifications of ONFH.Results The CJFH classification for ONFH consists of 3 types:type A,the medial pillar was involved; type B,the medial and central pillars were involved; type C,the lateral pillar was involved.According to site of necrosis focus in the lateral pillar,the type C was divided into 3 types:C1,there pillars were involved but there still was some normal tissue in lateral pillar;,C2,partial central pillar and all lateral pillar were involved; C3,the whole femoral head was involved.The natural history of the ONFH showed the collapse rate of type C2 and C3 in CJFH classification (95.3%) was higher than that (72.3%) of type C2 in JIC classification.Conclusion The CJFH classification of ONFH based on three pillars is more sensitive than JIC classification in predicting collapse of the femoral head.Moreover,the CJFH classification is convenient to use.
4.Dresden technique versus open surgery for acute Achilles tendon ruptures

Objective To compare the outcomes of Dresden technique versus open surgery for acute Achilles tendon ruptures.Methods From January 2013 to June 2017,67 patients were operatively treated for acute Achilles tendon rupture at Department of Orthopaedics,China Japan Friendship Hospital.Of them,Dresden technique was used in 32(minimally invasive group) and open surgery in 35(open surgery group).The 2 groups were compared in terms of baseline characteristics,operation time,operative incision length,hospital stay,postoperative complications like incision infection and tendon re-rupture and the Achilles Tendon Total Rupture Score(ATRS).Results The 2 groups were compatible due to insignificant differences in the baseline characteristics(P>0.05).They were followed up for 18 months.The minimally invasive group had significant shorter operation time(40.0min),incision length(3.0cm) and hospital stay(5 d) than the open surgery group(42.5 min,10.0 cm and 6 d)(P<0.05).No sural nerve lesion was observed in either group.The rate of wound complications was 0% for the minimally invasive group and 8.6% for the open surgery group;the rate of re-rupture was 3.1% for the former and 2.9% for the latter;the ATRS at the final follow-up was 98.0 for the former and 98.5 for the latter.There were no significant differences between the 2 groups in the above comparisons(P>0.05).Conclusions Percutaneous suture of the Achilles tendon with the Dresden instruments is a safe,fast and standardized surgical procedure for acute Achilles tendon ruptures,leading to shorter operation time,incision length and hospital stay,better functional recovery and a tendency of decreased wound complications than the open surgery.
5.Lengths of intramedullary nails and treatment efficacy of osteoporotic unstable intertrochanteric fracture

Objective:To investigate the effects of lengths of intramedullary nails on the treatment efficacy of osteoporotic unstable intertrochanteric fracture.Methods:Retrospectively analyzed were the data of patients with osteoporotic unstable intertrochanteric fracture who had been admitted to Department of Orthopedics, China-Japan Friendship Hospital from January 2017 to December 2019. According to the lengths of intramedullary nails, the patients were divided into 2 groups. In the short nail group of 135 cases, there were 38 males and 97 females with an age of (82.2 ± 7.7) years and an intramedullary nail with a length of 170 mm was used. In the long nail group of 32 cases, there were 8 males and 24 females with an age of (81.5±7.1) years and an intramedullary nail longer than 300 mm was used. The amount of intraoperative hemoglobin drop value, operation time, Harris hip score at 6 months after surgery, incidence of complications and mortality were compared between the 2 groups.Results:There was no significant difference in the preoperative general data between the 2 groups, indicating comparability between groups ( P>0.05). There were no significant differences between the 2 groups in intraoperative hemoglobin drop value [(19.8±2.2) g/L versus (20.0±2.1) g/L], Harris hip score (72.0±3.0 versus 71.2±3.6), incidence of complications [3.0% (4/135) versus 6.2 (2/32)] or mortality [14.3% (19/135) versus 15.6% (5/32)] ( P>0.05). The long nail group needed significantly longer operation time than the short nail group [(84.6±5.6) min versus (92.0±7.2) min] ( P<0.05). Conclusions:In the treatment of osteoporotic unstable intertrochanteric fracture, short and long intramedullary nails show no significant difference in hemoglobin drop value, functional score, complications or mortality, indicating they can both lead to safe and reliable curative efficacy. However, long nails need longer operation time.
6.Periprosthetic Femoral Fractures after Total Knee Arthroplasty:Report of 18 Cases

Objective:To investigate the treatment methods and efficacy of periprosthetic femoral fractures after total knee ar-throplasty.Methods:A total of 21 patients with periprosthetic femoral fractures (Insall I or II)after total knee arthroplasty from 2007 Jul to 2012 Jun were enrolled.There were 18 patients were followed for 12-26 monthes,with an average time of 19.8 months.Among them,5 cases (Insall I 1,ⅡA 2,ⅡB 2)were treated conservatively,while the other 13 cases (Insall I 1,ⅡA 2,ⅡB 10)were treated with open reduction and internal fixation with a locking-plate.At 6 monthes after fracture,the range of motion,physical line of force were measured,and the knee function of all patients were assessed by hospital for special surgery knee score (HSS).Results:The average HSS score was 75.6 in 5 patients treated conservatively,and 85.08 in 13 patients un-dergone operation.Conclusions:Open reduction and fixation inside with locking-plate is curative for periprosthetic femoral frac-tures after total knee arthroplasty.

Objective:To investigate the effect of smoking on autophagy in alveolar macrophages (AMs) of silicosis patients.Methods:In December 2019, a random sampling method was used to select 42 male patients with silicosis (19 cases of stage II and 23 cases of stage III) who were treated with large volume whole lung lavage from August to December 2017 in the Beidaihe sanatorium. According to the different smoking index of the study subjects (smoking index=smoking cigarette consumptions per day×years of smoking) , we divided them into high (Smoking index>400) , medium (200≤smoking index≤400) , low (smoking index <200) and non-smoking group. The levels of autophagy related proteins LC3, Beclin1, p62 and apoptosis related protein Cleaved Caspase-3 were detected by Western blot. The effects of smoking on autophagy activity of AMs in silicosis were analyzed.Results:The ratio of autophagy related protein LC3 II/LC3 I, the expression of Beclin1, p62, and apoptosis related protein Cleaved Caspase-3 in the high smoking group were significantly higher than that of the middle, low smoking group and the non-smoking group ( P<0.05) . Conclusion:Smoking can aggravate the dysfunction of autophagic degradation in silicosis patients' AMs, which may accelerate the progress of silicosis through increasing apoptosis in AMs.

9.Influence of comprehensive rehabilitation on postoperative functions in elderly patients with femoral intertrochanteric fracture

Objective:To analyze the influence of comprehensive rehabilitation on postoperative functions in the elderly patients with femoral intertrochanteric fracture.Methods:From June 2015 to June 2019, 144 elderly patients were treated at Department of Orthopedics, China-Japan Friendship Hospital for femoral intertrochanteric fractures by internal fixation with proximal femoral nail anti-rotation (PFNA). Of them, 65 received comprehensive rehabilitation, including intervention measures for details, prevention and management of comorbidities and complications and active exercise of the injured limb, while 79 conventional rehabilitation. In the comprehensive rehabilitation group, there were 22 males and 43 females with an age of (76.6±6.7) years, and 18 cases of type 31-A1, 35 cases of type 31-A2 and 12 cases of type 31-A3 by the AO type. In the control group, there were 23 males and 56 females with an age of (75.2±7.0) years, and 25 cases of type 31-A1, 39 cases of type 31-A2 and 15 cases of type 31-A3 by the AO type. The 2 groups were compared in terms of visual analog scale (VAS), Harris hip score, Barthel index, postoperative ambulation time and complications at 6 months after operation.Results:The 2 groups were comparable because there was no significant difference between them in the preoperative or intraoperative general data ( P>0.05). The comprehensive rehabilitation group had a significantly lower VAS score (1.5±1.0), a significantly higher Harris hip score(83.9±5.4), a significantly higher Barthel index (81.6±5.4), significantly shorter postoperative ambulation time [(2.6±1.0) d], and a significantly lower incidence of complications [4.6% (3/65)] than the control group did [1.9±1.2, 80.2±7.9, 78.2±7.9, (3.2±1.4) d, 16.5%(13/79), respectively] (all P< 0.05). Conclusion:For elderly patients with femoral intertrochanteric fracture, comprehensive rehabilitation can significantly reduce complications, promote functional recovery of the hip, and speed up recovery of capabilities of daily living compared with conventional rehabilitation.
10.Efficacy Comparison between DHS and Intertan in the Treatment of Elderly Patients with Evans Type ⅠIntertrochanteric Fracture

Objective :To compare the clinical efficay between dynamic hip screw (DHS)and proximal femoral nail(Intertan) fixation after closed reduction for the treatment of elderly patients with Evans type Ⅰ intertrochanteric fracture .Methods :Among 70 elderly patients with intertrochanteric fracture (Evans Ⅰ ) undergoing surgical treatment ,30 cases (DHS Group) were treated by closed reduction and DHS internal fixation and the other 40 cases (Intertan Group) were treated by closed reduction and Intertan fixation .Results :All the patients were followed up for 6-24 (12 .0 ± 3 .0) months .All the fractures were healed and no complication such as failure of internal fixation or femoral head necrosis occurred . There was statistically significant difference between operation time the DHS group and the Intertan group with 40-90 (63 .6 ± 8 .4) min and 40-90 (55 .6 ± 9 .8) min in DHS Group and Intertan Group ,respectively .And the intraoperative blood loss were 100-200 (145 .4 ± 22 .5) mL and 100-200 (92 .4 ± 25 .5) mL in DHS Group and Intertan Group ,respectively .Furthermore ,the duration of stay were 6-16 (11 .2 ± 2 .8) days and 6-9 (7 .9 ± 1 .1) days in DHS Group and Intertan Group ,respectively .There were significant difference regarding operation time ,intraoperative blood loss ,and duration of stay between the two groups(P< 0 .05) .Wound healing were achieved by postoperative liquefaction wound care in 4 cases of DHS group .There was no significant difference regarding fracture healing time between the 2 .5-4 .0 (3 .4 ± 0 .8) months in DHS Group and the 2 .5-4 .0 (3 .2 ± 0 .7) months in Intertan Group(P> 0 .05) .There were statistically significant differences between the two groups regarding Harris function scores after operation ,which were 67-84 (73 .5 ± 5 .1) points in DHS Group and 70-86 (77 .9 ± 3 .7) points in Intertan Group , and function scores which were 23-40 (30 .8 ± 3 .2) points in DHS Group and 25-44 (34 .4 ± 2 .9) points in Intertan Group (P<0 .05) .There was no significant difference between two groups regarding pain scores ,which were 20-44 (34 .9 ± 4 .8) points in DHS Group and 20-44 (35 .8 ± 5 .0) points in Intertan Group ,and activity range scores ,which were 2-4 (3 .0 ± 0 .8) points in DHS Group and 2-4 (3 .0 ± 1 .0) points in Intertan Group (P> 0 .05) .Conclusions :The Intertan and DHS internal fixation are effective methods for the treatment of elderly patients with intertrochanteric fractures .Intertan showed less trauma ,shorter duration of stay ,and better recovery of hip function .
